The Maillard or browning reaction between sugar and protein contributes to the increased chemical modification and cross-linking of long-lived tissue proteins in diabetes. To evaluate the role of glycation and oxidation in these reactions, we have studied the effects of oxidative and antioxidative conditions and various types of inhibitors on the reaction of glucose with rat tail tendon collagen in phosphate buffer at physiological pH and temperature. The chemical modifications of collagen that were measured included fructoselysine, the glycoxidation products N epsilon-(carboxymethyl)lysine and pentosidine and fluorescence. Collagen cross-linking was evaluated by analysis of cyanogen bromide peptides using sodium dodecyl sulfate-polyacrylamide gel electrophoresis and by changes in collagen solubilization on treatment with pepsin or sodium dodecylsulfate. Although glycation was unaffected, formation of glycoxidation products and cross-linking of collagen were inhibited by antioxidative conditions. The kinetics of formation of glycoxidation products proceeded with a short lag phase and were independent of the amount of Amadori adduct on the protein, suggesting that autoxidative degradation of glucose was a major contributor to glycoxidation and cross-linking reactions. Chelators, sulfhydryl compounds, antioxidants, and aminoguanidine also inhibited formation of glycoxidation products, generation of fluorescence, and cross-linking of collagen without significant effect on the extent of glycation of the protein. We conclude that autoxidation of glucose or Amadori compounds on protein plays a major role in the formation of glycoxidation products and cross-liking of collagen by glucose in vitro and that chelators, sulfhydryl compounds, antioxidants, and aminoguanidine act as uncouplers of glycation from subsequent glycoxidation and cross-linking reactions.

This paper describes a series of four-point bending tests that were conducted, under service loads and to failure, on unreinforced, reinforced and post-tensioned glulam timber beams, where the reinforcing tendon used was 12 mm diameter toughened steel bar. The research was designed to evaluate the benefits offered by including an active reinforcement in contrast to the passive reinforcement typically used within timber strengthening works, in addition to establishing the effect that bonding the reinforcing tendon has on the materials performance.

The laboratory investigations established that the flexural strength and stiffness increased for both the reinforced and post-tensioned timbers compared to the unreinforced beams. The flexural strength of the reinforced timber increased by 29.4%, while the stiffness increased by 28.1%. Timber that was post-tensioned with an unbonded steel tendon showed a flexural strength increase of 17.6% and an increase in stiffness of 8.1%. Post-tensioned beams with a bonded steel tendon showed increases in flexural strength and stiffness of 40.1% and 30% respectively.

This paper describes a series of four-point bending tests conducted, under service loads and to failure, on unreinforced, reinforced and post-tensioned glulam timber beams, where the reinforcing tendon used is 12 mm dia. basalt fibre-reinforced polymer. The research is designed to evaluate the benefits offered by including an active reinforcement in contrast to the passive reinforcement typically used within timber strengthening works, in addition to establishing the effect that bonding the reinforcing tendon has on the material's performance. Further experimental tests have also been developed to investigate the long-term implications of this research, with emphasis placed upon creep and loss of post-tensioning; however, this is ongoing and is not presented in this paper. The laboratory investigations establish that the flexural strength and stiffness increase for both the unbonded and bonded post-tensioned timbers compared to the unreinforced and reinforced beams. Timber that is post-tensioned with an unbonded basalt fibre-reinforced polymer tendon shows a flexural strength increase of 2ṡ8% and an increase in stiffness of 8ṡ7%. Post-tensioned beams with a bonded basalt fibre-reinforced polymer tendon show increases in flexural strength and stiffness of 15ṡ4% and 11ṡ5% respectively.

Improvements in the structural performance of glulam timber beams by the inclusion of reinforcing materials can improve both the service performance and ultimate capacity. In recent years research focusing on the addition of fibre reinforced polymers to strengthen members has yielded positive results. However, the FRP material is still a relatively expensive material and its full potential has not been realised in combination with structural timber. This paper describes a series of four-point bending tests that were conducted, under service and ultimate loads, on post-tensioned glulam timber beams where the reinforcing tendon used was 12 mm diameter Basalt Fibre Reinforced Polymer (BFRP). The research was designed to evaluate the additional benefits of including an active type of reinforcement, by post-tensioning the BFRP tendon, as opposed to the passive approach of simply reinforcing the timber beam.
From the laboratory investigations, it was established that there was a 16% increase in load carrying capacity, in addition to a 14% reduction in deflection under service loads when members containing the post-tensioned BFRP composite are compared with control timber specimens. Additionally a more favourable ductile failure mode was witnessed compared to the brittle failure of an unreinforced timber beam. The results support the assumption that by initially stressing the embedded FRP tendon the structural benefits experienced by the timber member increase in a number of ways, indicating that there is significant scope for this approach in practical applications.

Many types of non-invasive brain stimulation alter corticospinal excitability (CSE). Paired associative stimulation (PAS) has attracted particular attention as its effects ostensibly adhere to Hebbian principles of neural plasticity. In prototypical form, a single electrical stimulus is directed to a peripheral nerve in close temporal contiguity with transcranial magnetic stimulation delivered to the contralateral primary motor cortex (M1). Repeated pairing of the two discrete stimulus events (i.e. association) over an extended period either increases or decreases the excitability of corticospinal projections from M1, contingent on the interstimulus interval. We studied a novel form of associative stimulation, consisting of brief trains of peripheral afferent stimulation paired with short bursts of high frequency (≥80 Hz) transcranial alternating current stimulation (tACS) over contralateral M1. Elevations in the excitability of corticospinal projections to the forearm were observed for a range of tACS frequency (80, 140 and 250 Hz), current (1, 2 and 3 mA) and duration (500 and 1000 ms) parameters. The effects were at least as reliable as those brought about by PAS or transcranial direct current stimulation. When paired with tACS, muscle tendon vibration also induced elevations of CSE. No such changes were brought about by the tACS or peripheral afferent stimulation alone. In demonstrating that associative effects are expressed when the timing of the peripheral and cortical events is not precisely circumscribed, these findings suggest that multiple cellular pathways may contribute to a long term potentiation-type response. Their relative contributions will differ depending on the nature of the induction protocol that is used.

Enquadramento: A aplicação de crioterapia utiliza-se como coadjuvante na reeducação neuromuscular e tem como objetivo atingir o mais rápido possível a capacidade funcional, devido aos seus efeitos analgésicos e anti- inflamatórios, contudo, o seu efeito negativo ao nível da propriocepção tem vindo a ser questionado. Objetivo: Verificar se a aplicação de crioterapia no tendão quadricipital tem influência na força máxima, na sensação de força produzida e na funcionalidade do membro inferior. Metodologia: Tratou-se de um estudo experimental, do tipo laboratorial e com um desenho cruzado. Quarenta sujeitos foram distribuídos inicialmente e de forma aleatória em um grupo experimental e um grupo controlo, sendo que, após uma semana os elementos que pertenceram ao grupo experimental passaram para grupo controlo e vice-versa. O grupo experimental foi submetido a um programa com gelo dinâmico durante 10 minutos, avaliando-se a força máxima, sensação de força produzida e funcionalidade do membro, enquanto grupo controlo esteve em repouso o mesmo período de tempo. Resultados: Verificaram-se que não houve diferenças estatisticamente significativas relativas a todas variáveis estudadas (p>0,05). Conclusão: O uso da crioterapia na região mio-tendinosa do quadrícipete não provoca défices significativos ao nível das respostas sensório-motoras do joelho, e que à luz destes resultados, esta pode ser utilizada antes da realização de outras técnicas terapêuticas, sem qualquer risco adicional a nível motor.

Introdução: É reconhecida a importância do ligamento cruzado anterior (LCA) no funcionamento normal do joelho. Em caso de rotura ligamentar, nomeadamente em desportos com marcada solicitação dos movimentos de rotação do joelho, é justificada a necessidade de reconstrução do LCA na maioria dos casos. Objetivo (s): Avaliar a influência do tipo de enxerto na reconstrução do ligamento cruzado anterior na força muscular isocinética, assim como na funcionalidade e sintomas após 6 meses. Métodos: Estudo transversal analítico, constituído por 20 indivíduos voluntários do sexo masculino, que haviam sido submetidos a uma ligamentoplastia do cruzado anterior, pelo mesmo cirurgião, seguido de uma intervenção individualizada por um fisioterapeuta. Em 10 indivíduos, o procedimento cirúrgico foi realizado com enxerto do tendão rotuliano (grupo OTO), e nos restantes 10 com enxerto do semitendinoso e gracilis (grupo STG). Como forma de avaliar a Força Muscular Isocinética (Peak Torque, Trabalho Total Muscular, ratio Isquiotibiais/Quadricipite), foi utilizado o Dinamómetro Isocinético Biodex. A avaliação foi efectuada apenas aos 6 meses após o procedimento cirúrgico. Para observação da funcionalidade, amplitude de movimento e sintomas, utilizou-se o questionário International Knee Documentation Committee (IKDC). Resultados: Foi possível observar que entre os grupos apenas se observaram diferenças significativas no peak torque de extensão a 180º no membro não lesado (p=0,019). Contudo, foi observada uma tendência para o grupo OTO apresentar um maior défice no peak torque e trabalho total muscular em extensão. Comparativamente ao membro contra-lateral, o membro lesado apresentou valores significativamente inferiores na maioria das variáveis ( p < 0,05). Conclusão: Após 6 meses de pós-cirúrgico com reabilitação de fisioterapia, não foi possível apontar qual o enxerto que garante uma melhor recuperação da força muscular. Aos 6 meses, ambos os grupos ainda apresentaram limitações musculares, quando comparados com o lado contra-lateral. Relativamente ao rácio isquiotibiais/quadricípite, assim como no IKDC, não se observaram diferenças entre os dois tipos de enxertos.

Introduction: Calcific tendonitis of rotator cuff is observed on plainradiographs in 10% of adults, but remains asymptomatic in half thesecases. Sometimes, these calcifications induce acute flares withmassive inflammation similar to gout or CPPD crisis. Analgesics/anti-inflammatory medications are usually not sufficient to controlssymptoms in these situations. Local steroid infiltration with or withoutremoval of the calcific deposition with a needle aspiration may beuseful. A new approach could be IL-1 inhibitors. Indeed, basic calciumphosphate crystals are capable of stimulating the release of activeIL-1β in vitro. These crystals trigger IL-1β release, in an analogousmanner to MSU crystals in acute gout, suggesting that IL-1β blockademay be clinically useful.Case presentation: This report describes a 70-year old woman withacute rest pain of the right shoulder since 48 hours. On examination,we found massive limitations of active and passive movements. Thepatient evaluated, on the visual scale, her symptoms at 10/10 the nightand 5/10 the day. The radiography and showed a rounded, 8 mmcalcification in the subscapularis tendon. The ultrasound aspectrevealed a heterogeneous calcification partially non solid, surroundedby massive inflammation on Doppler. C-reactive protein anderythrocyte sedimentation rate were high (74 mg/ml, 54 mm/hour).The patient received subcutaneous injections of anakinra: 100 mgdaily for 3 days (D1-D3). We evaluated the patient in our consult at dayD1, D2, D3, D7, D16 and by phone at D70.This treatment rapidly relieved the inflammatory symptoms (within afew hours with no relapse). The mobility of the shoulder, the biologicsparameters improved and the size of the calcification as well thedegree of inflammation regressed on ultrasound after 3 days.Conclusion: This is the first report of a woman with an acute flareinduced by calcific tendonitis who received anakinra. IL-1 inhibitionmay be a therapeutic target in calcific tendonitis. To analyse thisresponse more precisely and elaborate definitive conclusions, aprospective pilot study is on-going in our ambulatory institute.

Purpose: First, to report ECU subsheath's normal MRI appearance and the findings in athletic injuries. Second, to determine the best MRI sequence for diagnosis. Methods and materials: Sixteen patients (13 males, 3 females, mean age 30.3 years) with ECU subsheath's athletic injuries sustained between January 2003 and June 2009 were retrospectively reviewed. Wrist MRI studies were performed on 1.5-T units and consisted of at least transverse T1 and STIR sequences in pronation, and FS Gd T1 in pronation and supination. Two radiologists assessed the following items, in consensus: injury type (A to C according to Inoue), ECU tendon stability, and associated lesions (ulnar head oedema, extensor retinaculum injury, ECU tendinosis and tenosynovitis). Then, each reader independently rated the sequences' diagnostic value: 0 = questionable, 1 = suggestive, 2 = certain. Follow-up studies were present in 8 patients. ECU subsheath's normal visibility (medial, central and lateral parts) was retrospectively evaluated in 30 consecutive control MRI studies. Results: FS Gd T1 sequences in supination (1.63) and pronation (1.59) were the most valuable for diagnosis, compared to STIR (1.22) and T1 (1). The study group included 9 type A, 1 type B and 6 type C injuries. There were trends towards diminution in pouches' size, signal intensity and enhancement in follow-up studies, along with tendon stabilization within the ulnar groove. In control studies, ECU subsheath's visibility in medial, central and lateral parts were noted in 66.7-80%, 63.3-80% and 30-50% respectively. Conclusion: ECU subsheath's athletic injuries are visible on 1.5-T MRI studies. FS Gd T1 sequences in supination and pronation are the most valuable.

Although tissue engineering and cell therapies are becoming realistic approaches for medical therapeutics, it is likely that musculoskeletal applications will be among the first to benefit on a large scale. Cell sources for tissue engineering and cell therapies for tendon pathologies are reviewed with an emphasis on small defect tendon injuries as seen in the hand which could adapt well to injectable cell administration. Specifically, cell sources including tenocytes, tendon sheath fibroblasts, bone marrow or adipose-derived stem cells, amniotic cells, placenta cells and platelet-derivatives have been proposed to enhance tendon regeneration. The associated advantages and disadvantages for these different strategies will be discussed and evolving regulatory requirements for cellular therapies will also be addressed. Human progenitor tenocytes, along with their clinical cell banking potential, will be presented as an alternative cell source solution. Similar cell banking techniques have already been described with other progenitor cell types in the 1950's for vaccine production, and these "old" cell types incite potentially interesting therapeutic options that could be improved with modern innovation for tendon regeneration and repair.

Objectlve:--This study examined the intraclass reliability· of different measures of the excitability of the Hoffmann reflex, derived from stimulus-response curves. The slope of the regression line of the H-reflex stimulus-response curve advocated by Funase et al. (1994) was also compared to the peak of the first derivative of the H-reflex stimulus-response curve (dHIdVmax), a new measure introduced in this investigation. A secondary purpose was to explore the possibility of mood as a covariate when measuring excitability of the H-reflex arc. Methods: The H-reflex amplitude at a stimulus intensity corresponding to 5% of the maximum M-wave (Mmax) is an established measure that was used as an additional basis of comparison. The H-reflex was elicited in the soleus for 24 subjects (12 males and 12 females) on five separate days. Vibration was applied to the Achilles tendon prior to stimulation to test the sensitivity of the measures on test day four. The means of five evoked potentials at each gradually increasing intensity, from below H-reflex threshold to above Mmax, were used to create both the H-reflex and M-wave stimulus response curves for each subject across test days. The mood of the subjects was assessed using the Subjective Exercise Experience Scale (SEES) prior to the stimulation protocol each day. Results: There was a modest decrease in all H-reflex measures from the first to third test day, but it was non-significant (P's>0.05). All measures of the H-reflex exhibited a profound reduction following vibration on test day four, and then returned to baseline levels on test day five (P's<0.05). The intraclass correlation coefficient (ICC) for H-reflex amplitude at 5% of Mmax was 0.85. The ICC for the slope of the regression line was 0.79 while it was 0.89 for dH/dVmax. Maximum M-wave amplitude had an ICC of 0.96 attesting to careful methodological controls. The SEES subscales of fatigue and psychological well-being remained unchanged IV across the five days. The psychological distress subscale (PO.05). Conclusions: The peak of the first derivative of the H-reflex stimulus-response curve (dH/dVmax) was shown to have comparable reliability and sensitivity to other more established measures of excitability. Psychological distress and the amplitude of the H-reflex at 5% Mmax follow similar trends across days, however there was no significant correlation between the two measures. Significance: The proposed method appears to be a more robust measure ofH-reflex excitability than the other methods tested. As such it would be an advantageous method to apply in clinical and investigative settings. Additionally, the results suggest that the relationship between psychological distress and H-reflex amplitude should be investigated further.

Les tendinites sont des lésions communes chez le cheval athlète, ayant un impact financier et sportif considérable. Les cellules souches mésenchymateuses (CSMs) de moelle osseuse (MO) sont empiriquement utilisées en clinique pour améliorer la guérison des affections myoarthrosquelettiques. Cependant, il est nécessaire de standardiser les protocoles d’isolement des CSMs équines et d’analyser leurs effets sur la guérison tendineuse pour ajuster leur dose. Les objectifs de cette étude étaient de comparer 3 méthodes d’isolement des CSMs équines et d’établir un modèle de guérison tendineuse minimal invasif pour analyser l’effet des CSMs sur cette guérison. Des CSMs de MO du sternum de juments étaient isolées par 3 protocoles couramment utilisés (adhérence au pétri (Classique) et 2 méthodes par gradient de densité (Percoll et Ficoll)). La viabilité des cellules après isolement, le rendement d’isolement, le nombre de CSMs obtenues après 14 jours de culture et leurs caractéristiques fonctionnelles (renouvellement et différentiation) étaient comparés entre les 3 protocoles. Les résultats suggéraient que le Percoll était le meilleur protocole en termes de rendement et de capacité de renouvellement des cellules. La différence n’était pas significative pour leur viabilité et leur capacité de différentiation. Un modèle de guérison tendineuse, consistant en une ténectomie du tendon extenseur latéral du doigt fut ensuite développé. Cependant, la grande variabilité interindividuelle de qualité de guérison dans le groupe pilote implique une ré-évaluation du modèle. Des études futures, avec des CSMs isolées par le Percoll dans de nouveaux modèles de guérison tendineuse devraient permettre de déterminer la dose adéquate de CSMs.

Dans l’hémiparésie consécutive à un AVC, une coactivation anormale des extenseurs du genou et de la cheville est souvent observée à la jambe atteinte lorsque la personne tente de bouger ou encore lors de la marche. Les mécanismes sous-jacents à cette coactivation sont mal compris. Bien que l’AVC entraîne une lésion supraspinale, des évidences démontrent le dysfonctionnement de certains circuits spinaux dans l’hémiparésie. Ce projet de doctorat visait à évaluer : 1) l’excitabilité des circuits spinaux intersegmentaires projetant des extenseurs du genou aux extenseurs de la cheville et 2) si un éventuel dysfonctionnement de ces circuits dans l’hémiparésie est associé à une coactivation anormale des extenseurs du genou et de la cheville lors de contractions statiques et au cours de la marche. La première étude compare la modulation de l’activité réflexe du soléaire suite à la stimulation du nerf fémoral entre des sujets hémiparétiques et sains. Une augmentation de la facilitation hétéronyme de courte latence et une diminution de l’inhibition ultérieure du réflexe H du soléaire ont été observées chez les sujets hémiparétiques. Ces résultats démontrent un dysfonctionnement des circuits intersegmentaires propriospinaux liant le quadriceps au soléaire suite à l’AVC. La deuxième étude démontre que ces changements dans la modulation hétéronyme des sujets hémiparétiques, évaluée au moyen de la méthode complexe basée sur l’activité réflexe du soléaire, sont similaires à ceux observés lorsque la modulation est évaluée en utilisant une méthode plus simple, soit celle de l’activité volontaire du soléaire. De plus, la modulation hétéronyme évaluée par les deux méthodes est corrélée avec l’atteinte motrice à la jambe parétique. La troisième étude a permis de quantifier une augmentation de la coactivation entre les extenseurs du genou et de la cheville lors de contractions volontaires statiques chez des personnes hémiparétiques par rapport à des personnes saines. De plus, le niveau accru de la coactivation involontaire des extenseurs de la cheville lors de l’activation volontaire des extenseurs du genou s’avère corrélé avec la modulation intersegmentaire du côté parétique. La quatrième étude a utilisé un indice temporel, soit l’intervalle entre les pics d’activation électromyographique (PAI), et un indice d’amplitude de coactivation (CAI) pour quantifier une augmentation de la coactivation entre les extenseurs du genou et de la cheville lors de la marche chez des personnes hémiparétiques par rapport à des personnes saines. Ces indices sont corrélés, pour certains groupes musculaires, avec la modulation intersegmentaire modifiée du côté parétique. Finalement, des résultats préliminaires montrent que la vibration mécanique du tendon rotulien (80 Hz) réduit la facilitation intersegmentaire accrue des sujets hémiparétiques. Ce projet doctoral a permis de mettre en lumière un dysfonctionnement de circuits spinaux liant le quadriceps et le soléaire dans l’hémiparésie consécutive à un AVC. Ce changement dans les mécanismes neurophysiologiques de la moelle épinière est corrélé avec des changements fonctionnels. Ainsi, ce dysfonctionnement pourrait contribuer à la coactivation involontaire entre les extenseurs du genou et de la cheville qui fait partie intégrante de la synergie pathologique en extension souvent rencontrée à la jambe parétique lors d’efforts en statique et pendant la marche. Finalement, une étude préliminaire suggère que la vibration mécanique serait une modalité sensorielle prometteuse pour réguler l’hyperexcitabilité des circuits spinaux qui contribuerait aux atteintes motrices chez les personnes hémiparétiques.
