Parmi les neuroleptiques disponibles, l'amisulpride est de plus en plus prescrit. De par son affinité sélective dose-dépendante aux récepteurs dopaminergiques, il est possible de moduler les effets de cette substance montrant des propriétés antidépressives à faible dose et agissant comme un puissant antipsychotique à forte dose. Toutefois, l'amisulpride induit une prolongation dose-dépendante de l'intervalle QT ce qui peut potentialiser la survenue d'arythmies ventriculaires, souvent fatales lorsque sa consommation est concomitante à celles d'autres médicaments ou stupéfiants allongeant l'intervalle QT. Cependant, la littérature scientifique demeure peu documentée en ce qui concerne la détermination de concentrations sanguines thérapeutiques, toxiques et létales. Après avoir présenté les propriétés pharmacodynamiques et pharmacocinétiques de l'amisulpride, nous nous proposons de définir plus précisément les seuils sanguins de concentrations thérapeutiques, toxiques et létales pour cette molécule illustrée par la revue des cas mortels impliquant l'amisulpride en Suisse romande depuis 2005. Among the available neuroleptic drugs, amisulpride is increasingly prescribed. Thanks to its dose-dependent selective affinity for dopamine receptors, it is possible to modulate the effects of this substance showing antidepressant properties at low dose and acting as a powerful antipsychotic at high dose. However, amisulpride induces a dose-dependent prolongation of the QT interval which may potentiate the occurrence of ventricular arrhythmias, often fatal when its consumption is concomitant with those of other drugs that prolong the QT interval. However, the scientific literature remains poorly documented with regard to the determination of therapeutic, toxic and lethal blood concentrations. After presenting the pharmacodynamic and pharmacokinetic properties of amisulpride, we propose to define more precisely the blood levels of therapeutic, toxic and lethal concentrations for this molecule illustrated by a review of the fatal cases involving amisulpride in Western Switzerland since 2005.

El estudio analiza, a partir de una prueba experimental, los problemas de accesibilidad que actualmente presenta la información científica en formato digital, más concretamente los artículos científicos en soporte digital que actualmente se pueden consultar a través de los portales de acceso a las revistas electrónicas y en los repositorios abiertos ("open access") de literatura científica. El estudio se centra en los aspectos de facilidad de uso del contenido de los documentos, sin entrar en la observación de los distintos sistemas de recuperación.Se han analizado los dos formatos más utilizados para la publicación de artículos científicos en soporte digital: HTML y PDF, estudiando la variabilidad interna que representa la presencia de sumarios o de tablas de datos internas o vinculadas.Esta investigación se ha realizado a partir del trabajo experimental con dos colectivos: sujetos ciegos y sujetos no ciegos. El primer grupo se ha concretado en 30 sujetos ciegos que han colaborado de forma voluntaria y que han sido contactados gracias a la mediación de la Fundación ONCE. El segundo grupo se ha concretado en 30 profesores del Departamento de Biblioteconomía y Documentación de la Universidad de Barcelona.

Background: Due to complains of respiratory symptoms of some employees a pharmaceutical company asked in 2008 the occupational medical department of the Institute for Work and Health in Lausanne to evaluate the health status of their workers exposed to Mesalazine powder, which is the active agent of a drug used for the treatment of bowels inflammation. Therefore we examined the 21 workers exposed to Mesalazine powder. Method: After a visit of the pharmaceutical company in order to investigate the Mesalazine powder production, we performed an individual medical evaluation of the 21 workers. Our medical protocol was based on the safety data sheet of Mesalazine, the data found in the scientific literature and the «Compendium Suisse des Médicaments» and covered upper and lower respiratory tract as well as skin and eyes. Results: Sixty two percent (62%) of the exposed employees had symptoms of skin, eyes and throat irritation. Three employees reported respiratory symptoms such as dyspnoea, cough and expiratory wheezing, which appeared during the working hours. The Peak Flow series performed at the workplace was lowered in the three employees with lower respiratory tract symptoms. None of the three had consulted a physician, even though the symptoms had been present since some months. The pneumological evaluation confirmed for all three cases the asthma diagnoses. Conclusion: It is known that patients who are treated with drugs including Mesalazine can develop adverse health effect such as asthma. However occupational asthma in workers exposed to Mesalazine powder inhalation is until now not described in the literature. Immunologic investigations in order to know if the occupational asthma caused by Mesalazine is of allergic or mechanical irritation nature are still ongoing. Concerning the three workers with asthma, inability to work with Mesalazine was pronounced. Furthermore, the SUVA recognized the three patients with asthma as occupational respiratory diseases. Following our results and recommendations, the company undertook some measures to reduce the exposure to Mesalazine. A new health evaluation of the employees in the Mesalazine production is hence planned in 2009. As each year new causes of occupational asthma are described, the possible work relation of new asthma onset has to be carefully investigated as the consequences for the patient e.g. removal from exposure and for the exposed co-workers are of substantial importance.

BACKGROUND: Management of ischemic stroke in the presence of aneurysmal brain disease is controversial. Recent retrospective evidence suggests that in selected patients, intravenous thrombolysis (IVT) remains a safe approach for reperfusion. METHODS: We document a case of post-thrombolysis aneurysmal rupture. Supported by additional scientific literature we postulate that acute aneurysmal thrombosis leading to stroke in the culprit artery may be an ominous sign of rupture and should be considered separately from fortuitously discovered distant aneurysmal disease. RESULTS: A 71-year-old female presented with an acute right middle cerebral artery stroke syndrome. IVT allowed vessel reperfusion and revealed a previously concealed, juxtaposed non-giant M1 segment saccular aneurysm. Secondary aneurysmal rupture ensued. The aneurysm was secured by surgical clipping. Postoperative course was uneventful. CONCLUSIONS: This case shows that despite reports of thrombolysis safety in the presence of brain aneurysms, thrombolysis remains potentially hazardous and hints toward an increased risk when the stroke arises on the parent vessel itself.

Over the past few decades, Fourier transform infrared (FTIR) spectroscopy coupled to microscopy has been recognized as an emerging and potentially powerful tool in cancer research and diagnosis. For this purpose, histological analyses performed by pathologists are mostly carried out on biopsied tissue that undergoes the formalin-fixation and paraffin-embedding (FFPE) procedure. This processing method ensures an optimal and permanent preservation of the samples, making FFPE-archived tissue an extremely valuable source for retrospective studies. Nevertheless, as highlighted by previous studies, this fixation procedure significantly changes the principal constituents of cells, resulting in important effects on their infrared (IR) spectrum. Despite the chemical and spectral influence of FFPE processing, some studies demonstrate that FTIR imaging allows precise identification of the different cell types present in biopsied tissue, indicating that the FFPE process preserves spectral differences between distinct cell types. In this study, we investigated whether this is also the case for closely related cell lines. We analyzed spectra from 8 cancerous epithelial cell lines: 4 breast cancer cell lines and 4 melanoma cell lines. For each cell line, we harvested cells at subconfluence and divided them into two sets. We first tested the "original" capability of FTIR imaging to identify these closely related cell lines on cells just dried on BaF2 slides. We then repeated the test after submitting the cells to the FFPE procedure. Our results show that the IR spectra of FFPE processed cancerous cell lines undergo small but significant changes due to the treatment. The spectral modifications were interpreted as a potential decrease in the phospholipid content and protein denaturation, in line with the scientific literature on the topic. Nevertheless, unsupervised analyses showed that spectral proximities and distances between closely related cell lines were mostly, but not entirely, conserved after FFPE processing. Finally, PLS-DA statistical analyses highlighted that closely related cell lines are still successfully identified and efficiently distinguished by FTIR spectroscopy after FFPE treatment. This last result paves the way towards identification and characterization of cellular subtypes on FFPE tissue sections by FTIR imaging, indicating that this analysis technique could become a potential useful tool in cancer research.

The legislatives evolutions imply an important recourse to the psychiatric expertise in order to evaluate the potential dangerousness of a subject. However, in spite of the development of techniques and tools for this evaluation, the dangerousness assessment of a subject is in practice extremely complex and discussed in the scientific literature. The evolution of the concept of dangerousness to the risk assessment involved a technicisation of this evaluation which should not make forget the limits of these tools and the need for restoring the subject, the meaning and the clinic in this evaluation.

Introduction: Beryllium (Be) is increasingly used in various industrial applications. Occupational exposure to Be may lead to chronic beryllium disease (CBD), a pulmonary granulomatous disorder closely similar to sarcoidosis, which develop in 1 to 15% of exposed workers. Although Switzerland is one of the major Be importers worldwide, little information is available about occurrence of exposure and the number of workers exposed in this country. Objectives: 1) evaluate the number of workers potentially exposed to Be in Switzerland; 2) construct a screening tool to allow potential Be exposure detection in a clinical setting. Methods: After identification of industrial sectors involving beryllium exposure based on expert reports and scientific literature, an estimation of the number of workers employed in these relevant industries was made using data from the Swiss federal population census and registries of economic activities. A second analysis was performed to estimate the fraction of workers really exposed to Be in each industrial sector. This adjustment was made according to the results of a French survey (INRS, Institut National de Recherche et de Sécurité) conducted by questionnaire addressed to 4500 companies in relevant industries on their use of beryllium and other issues such as percentage of employees really exposed. These realistic data were used to develop a self-administrated screening questionnaire allowed to identify patients with possible Be exposure. Results: In Switzerland, the number of workers employed in industries using Be was nearly 150 000. The estimated number of workers exposed to beryllium in these industries ranged from 2000 to 4000. Relevant sectors were: microengineering, precision turning, watchmaking and metal waste treatment and recycling. The validation of the self-administrated questionnaire containing a list of jobs and leisure activities associated with potential Be exposure is in progress within the framework of a national study. Conclusions: The number of workers potentially exposed to Be in Switzerland is rather high compared to estimations for other industrialized countries and might constitute an underestimated occupational health problem. Undetected Be exposure in patients with sarcoidosis may occur and result in misdiagnosis. Once validated, the self-administrated questionnaire could be used by clinicians to screen for Be exposure in patients with granulomatous lung disorders.

The main goal of this observational and descriptive study is to evaluate whether the diagnosis axis of a nursing interface terminology meets the content validity criterion of being nursing-phenomena oriented. Nursing diagnosis concepts were analyzed in terms of presence in the nursing literature, type of articles published and areas of disciplinary interest. The search strategy was conducted in three databases with limits in relation to period and languages. The final analysis included 287 nursing diagnosis concepts. The results showed that most of the concepts were identified in the scientific literature, with a homogeneous distribution of types of designs. Most of these concepts (87.7%) were studied from two or more areas of disciplinary interest. Validity studies on disciplinary controlled vocabularies may contribute to demonstrate the nursing influence on patients" outcomes.

The present work deals with quantifying group characteristics. Specifically, dyadic measures of interpersonal perceptions were used to forecast group performance. 46 groups of students, 24 of four and 22 of five people, were studied in a real educational assignment context and marks were gathered as an indicator of group performance. Our results show that dyadic measures of interpersonal perceptions account for final marks. By means of linear regression analysis 85% and 85.6% of group performance was respectively explained for group sizes equal to four and five. Results found in the scientific literature based on the individualistic approach are no larger than 18%. The results of the present study support the utility of dyadic approaches for predicting group performance in social contexts.

Resumen: El artículo analiza los problemas de accesibilidad que actualmente presentan los artículos científicos en soporte digital. El estudio se centra en los aspectos de facilidad de uso del contenido de los documentos digitales según la forma en que se publiquen, sin entrar en el estudio de los distintos sistemas de recuperación. Se analizan los dos formatos más utilizados para la publicación de artículos científicos en soporte digital: HTML y PDF, estudiando el desempeño lector en relación a la presencia de sumarios o de tablas internas o vinculadas. El estudio se ha realizado con dos colectivos: 30 sujetos ciegos, usuarios de Jaws, contactados gracias a la mediación de la Fundación ONCE, y 30 sujetos no ciegos, profesores del Departamento de Biblioteconomía y Documentación de la Universidad de Barcelona. El estudio muestra que la localización de los datos contenidos en tablas se ve facilitada en documentos HTML por la inclusión de un sumario que vincule con la tabla, así como la inclusión de tablas completas en el cuerpo del documento HTML facilita la actividad lectora por parte de los usuarios ciegos. A nivel metodológico la presente investigación aporta dos novedades relevantes respecto a la literatura existente en los estudios de usabilidad con ciegos: estudia la usabilidad del formato PDF y es un test de usabilidad cuantitativo; este último hecho dificulta su comparación con la mayoría de artículos publicados. Abstract: This paper analyses the problems of accessibility posed by scientific articles published in digital format, focusing on the ease of use of their content with respect to the form in which they are published (irrespective of the recovery system). The two most widely used formats for the publication of scientific articles in digital format, HTML and PDF, are analysed, examining reader performance in relation to the presence of contents lists or internal or linked tables. The study involved two groups: 30 blind subjects, all JAWS users, contacted through the ONCE Foundation, and 30 sighted subjects, lecturers in the Department of Librarianship and Documentation of the University of Barcelona. The results shows the location of data in tables is easier in HTML documents through the inclusion of a contents list linked to these tables. Further, the inclusion of complete tables in the body of HTML document facilitates the reading activity of blind users. At the methodological level, this work reports two novelties with respect to the existing literature on usability by blind people: it examines the usability of the PDF format, and discusses a quantitative usability test. The latter hinders comparison with the majority of published articles.

BACKGROUND: The annotation of protein post-translational modifications (PTMs) is an important task of UniProtKB curators and, with continuing improvements in experimental methodology, an ever greater number of articles are being published on this topic. To help curators cope with this growing body of information we have developed a system which extracts information from the scientific literature for the most frequently annotated PTMs in UniProtKB. RESULTS: The procedure uses a pattern-matching and rule-based approach to extract sentences with information on the type and site of modification. A ranked list of protein candidates for the modification is also provided. For PTM extraction, precision varies from 57% to 94%, and recall from 75% to 95%, according to the type of modification. The procedure was used to track new publications on PTMs and to recover potential supporting evidence for phosphorylation sites annotated based on the results of large scale proteomics experiments. CONCLUSIONS: The information retrieval and extraction method we have developed in this study forms the basis of a simple tool for the manual curation of protein post-translational modifications in UniProtKB/Swiss-Prot. Our work demonstrates that even simple text-mining tools can be effectively adapted for database curation tasks, providing that a thorough understanding of the working process and requirements are first obtained. A fatality due to ingestion of a reversible inhibitor of monoamine-oxidase A (MAO-A) is reported. Moclobemide is generally considered as a safe drug far less toxic than tricyclic anti-depressants. However, severe intoxications may result from interactions with other drugs and food such as selective serotonin reuptake inhibitors (SSRIs), anti-Parkinsonians of the MAOI-type (e.g. selegiline) or tyramine from ripe cheese or other sources. In the present case, high levels of moclobemide were measured in peripheral blood exceeding toxic values reported so far in the scientific literature. The body fluid concentrations of moclobemide were of 498 mg/l in peripheral whole blood, 96.3 mg/l in urine while an amount of approximately 33 g could be recovered from gastric contents. The other xenobiotics were considered of little toxicological relevance. The victim (male, 48-year-old) had a past history of depression and committed one suicide attempt 2 years before death. Autopsy revealed no evidence of significant natural disease or injury. It was concluded that the manner of death was suicide and that the unique cause of death was massive ingestion of moclobemide.

Total knee arthroplasty (TKA) remains the preferred treatment for patients with invalidating osteoarthritis. It allows these patients to find once again a satisfactory quality of life. These results are reflected by the increasing number of interventions performed each year and in always younger patients. The objective of this review of the scientific literature was to determine the long-term quantitatively and qualitatively results of TKA and determine the parameters which could influence the long-term outcome. There does not seem to be any clear arguments in favor of one type of implant or surgical technique. One also notices that TKA in females and older patients seem to have longer lifespan, estimated to approximately 15 years. The most frequent complication remains aseptic loosening followed by infections.
