L’ecografia è la metodica diagnostica più utilizzata come screening e follow-up nei pazienti epatopatici con o senza lesioni focali e questo grazie alle sue peculiari caratteristiche, che sono date dall’essere real-time, maneggevole, priva di radiazioni ionizzanti e con bassi costi. Tuttavia tale metodica se confrontata con la TC o la RMN, può avere importanti limiti, quali l’impossibilità di visualizzare piccole lesioni localizzate in aree anatomicamente “difficili” o in pazienti obesi, che sono già state identificate con altre tecniche, come la TC o la RMN. Per superare queste limitazioni sono stati introdotti dei sistemi di “fusione d’immagine” che consentono di sincronizzare in tempo reale una metodica real time con bassa risoluzione spaziale come l’ecografia ed una statica ad alta risoluzione come la TC o la RMN. Ciò si ottiene creando attorno al paziente un piccolo campo elettromagnetico costituito da un generatore e da un rilevatore applicato al trasduttore ecografico ed introducendo in un computer abbinato all’ecografo il “volume rendering” dell’addome del paziente ottenuto mediante TC multistrato o RM. Il preciso “ appaiamento spaziale “ delle due metodiche si ottiene individuando in entrambe lo stesso piano assiale di riferimento e almeno 3-4 punti anatomici interni. Tale sistema di fusione d’immagine potrebbe essere molto utile in campo epatologico nella diagnostica non invasiva del piccolo epatocarcinoma, che secondo le ultime linee guida, nei noduli di dimensioni fra 1 e 2 cm, richiede una concordanza nel comportamento contrastografico della lesione in almeno due tecniche d’immagine. Lo scopo del nostro lavoro è stato pertanto quello di valutare, in pazienti epatopatici, il contributo che tale sistema può dare nell’identificazione e caratterizzazione di lesioni inferiori a 20 mm, che erano già state identificate alla TC o alla RMN come noduli sospetti per HCC, ma che non erano stati visualizzati in ecografia convenzionale. L’eventuale re-identificazione con l’ecografia convenzionale dei noduli sospetti per essere HCC, può permettere di evitare, alla luce dei criteri diagnostici non invasivi un’ ulteriore tecnica d’immagine ed eventualmente la biopsia. Pazienti e Metodi: 17 pazienti cirrotici (12 Maschi; 5 Femmine), con età media di 68.9 +/- 6.2 (SD) anni, in cui la TC e la RMN con mezzo di contrasto avevano identificato 20 nuove lesioni focali epatiche, inferiori a 20 mm (13,6 +/- 3,6 mm), sospette per essere epatocarcinomi (HCC), ma non identificate all’ecografia basale (eseguita in cieco rispetto alla TC o alla RMN) sono stati sottoposti ad ecografia senza e con mezzo di contrasto, focalizzata su una zona bersaglio identificata tramite il sistema di fusione d’immagini, che visualizza simultaneamente le immagini della TC e della RMN ricostruite in modalità bidimensionale ( 2D), tridimensionale ( 3 D) e real-time. La diagnosi finale era stata stabilita attraverso la presenza di una concordanza diagnostica, secondo le linee guida internazionali o attraverso un follow-up nei casi di discordanza. Risultati: Una diagnosi non invasiva di HCC è stata raggiunta in 15/20 lesioni, inizialmente sospettate di essere HCC. Il sistema di fusione ha identificato e mostrato un comportamento contrastografico tipico in 12/15 noduli di HCC ( 80%) mentre 3/15 HCC (20%) non sono stati identificati con il sistema di fusione d’immagine. Le rimanenti 5/20 lesioni non sono state visualizzate attraverso i sistemi di fusione d’immagine ed infine giudicate come falsi positivi della TC e della RMN, poiché sono scomparse nei successivi mesi di follow-up e rispettivamente dopo tre, sei, nove, dodici e quindici mesi. Conclusioni: I nostri risultati preliminari mostrano che la combinazione del sistema di fusione dell’immagine associata all’ecografia senza e con mezzo di contrasto (CEUS), migliora il potenziale dell’ecografia nell’identificazione e caratterizzazione dell’HCC su fegato cirrotico, permettendo il raggiungimento di una diagnosi, secondo criteri non invasivi e slatentizzazndo casi di falsi positivi della TC e della RMN.

1230 year 11 and 12 college students, modal age 16 and 17, in three colleges in Bombay, India, were studied on sexual behaviors or risk of sexual behaviors, beliefs about sex, HIV/STD knowledge, perceived norms regarding sexual behaviors, and the relationships between social skills/anxieties in HIV/STD prevention and actual and anticipated sexual behaviors. A quantitative questionnaire examining HIV/STD risk behaviors, knowledge, attitudes and beliefs, and the AIDS Social Assertiveness Scale (ASAS) were administered to these 1230 college students. Data indicated that 8% of males and 1% of females had had sexual experience, but over one third were not sure at all of being able to abstain from sexual activity with either steady or casual partners. Perceived norms were slanted toward sexual abstinence for the majority of the sample. Knowledge of protective effects of condoms was high, although half of those who had had sex did not use condoms. Logistic regression showed knowledge was higher among males, those who believed it was OK to have sex with a steady partner and that they should not wait until they were older, those who believed that condoms should be used even if the partner is known, and those who believed it was acceptable to have multiple partners. Gender differences in sexual activity and beliefs about sexual activity showed males were less likely to believe in abstaining from sexual activity. The 5 scales of the ASAS were scored and compared on ANOVA on: those who had had sexual experience (HS), those who anticipated being unable to refuse sex (AS), and those who did not anticipate problems in refusing sex (DS). Those in the AS group had greater anxieties about refusing sexual or other risk behaviors than HS and DS groups. There were greater anxieties about dealing with condoms in the AS and DS groups compared with the HS group. Confiding sexual or HIV/STD-related problems to significant others was more anxiety-provoking for the AS group compared with the HS group, and the AS group were more anxious about interactions with people with HIV. Factor analysis produced the same 5 factors as those found in previous studies. Of these, condom interactions and confiding in significant others were most anxiety provoking, and condom interactions most variable based on demographic and attitudinal factors.^ This age group is appropriate for HIV/STD reduction education given the low rate of sexual activity but despite knowledge of the importance of condom use, social skills to apply this knowledge are lacking. Social skills training in sexual negotiations, condom negotiations, and confiding HIV/STD-related concerns to significant others should reduce the risks of Indian college students having unwanted or unprotected sex. ^

This cross-sectional study was undertaken to evaluate the impact in terms of HIV/STD knowledge and sexual behavior that the City of Houston HIV/STD prevention program in HISD high schools has had on students who have participated in it by comparing them with their peers who have not, based on self reports. The study further evaluated the program cost-effectiveness for averting future HIV infections by computing Cost-Utility Ratios based on reported sexual behavior. ^ Mixed results were obtained, indicating a statistically significant difference in knowledge with the intervention group having scored higher (p-value 0.001) but not for any of the behaviors assessed. The knowledge score outcome's overall p-value after adjusting for each stratifying variable (age, grade, gender and ethnicity) was statistically significant. The Odds Ratio of intervention group participants aged 15 years or more scoring 70% or higher was 1.86 times; that of intervention group female participants was 2.29 times; and that of intervention group Black/African American participants was 2.47 times relative to their comparison group counterparts. The knowledge score results remained statistically significant in the logistic regression model, which controlled for age, grade level, gender and ethnicity. The Odds Ratio in this case was 1.74. ^ Three scenarios based on the difference in the risk of HIV infection between the intervention and comparison group were used for computation of Cost-Utility Ratios: Base, worst and best-case scenario. The best-case scenario yielded cost-effective results for male participants and cost-saving results for female participants when using ethnicity-adjusted HIV prevalence. The scenario remained cost-effective for female participants when using the unadjusted HIV prevalence. ^ The challenge to the program is to devise approaches that can enhance benefits for male participants. If it is a threshold problem implying that male participants require more intensive programs for behavioral change, then programs should first be piloted among boys before being implemented across the board. If it is a reflection of gender differences, then we might have to go back to the drawing board and engage boys in focus group discussions that will help formulate more effective programs. Gender-blind approaches currently in vogue do not seem to be working. ^

Objective. To explore (1) the association between "club drug" use and unprotected anal intercourse (UAI) and (2) the association between binge drug use and UAI among HIV seronegative men who have sex with men (MSM) seeking HIV/STD testing at a local clinic in Houston. ^ Study design. A sub-sample of 297 HIV seronegative MSM from a cross-sectional study of drug and sexual behavior in Houston was conducted in 2006. Patients who were seeking HIV/STD testing at a local MSM-identified STD clinic were recruited for an anonymous computer-assisted interview. Analysis of identified secondary data consisted of self-reported information about demographic characteristics, use of drugs, and sexual behaviors. ^ Results. With new and casual sex partners, there was a strong and statistically significant association between use of "club drugs" and UAI. No association between binge drug use and UAI was evident. Men aware of HIV seropositivity or unaware of the HIV serostatus of their primary partner were less likely to report UAI. ^ Conclusion. These data suggest that in the Houston area, HIV-negative MSM club drug users, particularly multiple drug users, are at higher risk of UAI than comparable MSMs who do not use club drugs. Episode-level data regarding binge use of these and other drugs, and UAI should be collected in future studies to explore their relationship. The 'new partner' category should be added to sex partner types to measure sex and drug use behaviors in future studies.^ Keywords. The aim of this study was to determine the relationship between condom use and emotional intimacy. The study was a gonorrhea case-comparison ('Cases and Places') study with the samples being drawn from the public health clinics (cases) and select bars/nightclubs (places) of Houston, TX (N=215). Data were collected by questionnaires administered on a laptop computer. The majority of respondents were African-American (97.7%), female (69.3%) and either had high school or GED education (72.6%). Condom use with last sexual partner was analyzed along with intimacy with that partner assessed on a 3-point scale. Analysis showed that higher intimacy was related to greater condom use which was significant in males (χ2=7.85, p=.00) but not females (χ2=1.46, p=.15). These data were opposite to previous studies which showed an inverse relationship between condom use and emotional intimacy. We hypothesize that in a high-risk environment, people make more effort to protect those they feel closer to. These data suggest a need to further explore the complex relationship between emotional intimacy and condom use.^

Syftet med denna studie är att förstå hur ungdomar, som tillfrågats informellt av lärare att utöva socialt stöd till en utsatt skolkamrat, upplever situationen och vilken innebörd socialt stöd har för dem. Genom tolkande fenomenologisk metod har kvalitativa intervjuer med sex ungdomar i åldern 15-18 år genomförts och analyserats. Resultatet visade tre huvudteman i upplevelsen. Dubbla känslor beskriver en kluvenhet inför rollen med såväl positiva som negativa upplevelser. De upplevde förståelse av stödet som en arbetssituation genom att tala om rollen som ett uppdrag, beskriva den utsatte som annorlunda än de själva och markera stödets avgränsning till skoltiden. I informanternas berättelser om stödets utformning framkom deras syn på innebörden av socialt stöd, som att få med den utsatte i gemenskapen. De uttryckte även en önskan att fler elever ska involveras i stödarbetet. Slutsatsen är att rollen kan innebära en utsatt position och att barn och ungdomar, som utövar informellt socialt stöd till en skolkamrat på uppdrag av lärare, behöver uppmärksammas och vikten av stöttning till dem belysas samt beforskas ytterligare. Studien bidrar med ny kunskap om upplevelser av att utöva informellt stöd i skolan.
