When well indicated, the orthodontic surgical approach is the ideal treatment mean for Skeletal Class III adult patients. To improve facial esthetic results from orthognatic surgery, the leveling and alignment of maxillary dental arch must be achieved with minimal inclination and projection or even retro-inclination of anterior upper teeth. During a pre-surgical phase of 12 months, headgear bilateral force of 150 g/F was applied to the upper molars of a 22 years old male compliant patient with Class III skeletal malocclusion, to provide an upper teeth control of mesial tipping and projection during alignment and leveling. The ideal occlusal parameters required for surgical procedure were achieved without dental extractions permitting a total treatment period of 37 months. The outcomes remained stable over 3 years follow up after the removal of the appliance. The results indicate that, although headgear use depends greatly on patient compliance, when well indicated it is an interesting alternativetopromote dentaldecompensationon pre-surgical period, in order to allow surgical correction of skeletal Class III malocclusion.

The study presents the construction process of research methodology "Training in SUS Humanization: effects evaluation of training processes from institutional supporters on health productionin Rio Grande do Sul, Santa Catarina and São Paulo territories." There was a search for developing an appropriate evaluative practice to the training processes, a methodology that instead of evaluating on something, assessed along with the supporters who attended the training-intervention, a participatory methodology.Therefore, the constitution of the Research Interest Group was an eminent tool. Trained supporters comprised the research team to expand participatory possibilities of a large and dispersed group, producing interferences in the investigative process conduction, described and analyzed in the study.At the same time, their experiences interfered in the understandings they had until then about the intervention-training experiences and effects on their daily lives, after almost four years.Thus, the methodological approach was intrinsically linked to the construction of a subjectivity differentiated plan and necessarily collective, which shifted the position of supporters involved from mere data suppliers to a lateralityposition in relation to other actors.The trial afforded by participatory strategies allowed researchers and supporters to interfere and compose the evaluation scenario with remarkable performances throughout the investigative process.The survey configuration was like a bet on a given methodological architecture that, in seeking to overcome evaluator-evaluated logic produced information for (retro) feedingthe intervention triggered by it. In the formative dimension, it also went through working processes analyzed by supporters rescuing the indissoluble characteristic that health activities mobilize among intervening, training and reviewing.

Chordomas are tumors derived from cells that are remnants of the notochord, particularly from its proximal and distal extremes, they are mainly midline and represent approximately 1% of all malignant bone tumors and 0.1 to 0.2% of intracranial neoplasms. Chordomas involving the sellar region are rare. Herein, we describe a 57-year-old male patient presenting with a history of retro-orbital headache, progressive loss of vision, and clinical features of hypopituitarism, for over 2 months. During evaluation, the CT scan revealed a large contrast-enhancing intrasellar tumor with a 3.6-cm largest diameter. The patient underwent transsphenoidal partial resection of the tumor, and histological examination was consistent with the diagnosis of chondroid chordoma. Although chordomas are rare, they may be considered to constitute a differential diagnostic of pituitary adenomas, especially if a calcified intrasellar tumor with bone erosion is diagnosed.

The yerba mate (Ilex paraguariensis) is composed of bioactive that interfere with lipid metabolism. The objective was to evaluate if daily consumption of mate tea (MT) change the lipid deposits and dyslipidemia caused by excessive consumption of sucrose. Thirty male Wistar rats (40 days old) were divided into four groups: Group C - free access to commercial chow and deionized water; S - free access to commercial food, water and sucrose solution 30% (w/v) in water; MTS and deionized - free access to commercial feed solution, water, 30% sucrose (w/v) and treated with daily infusion of MT (soluble mate Leão Júnior®) via orogastric tube at a dose of 100 mg/kg/mc for 8 weeks. After the trial period the lipid profile was evaluated by the following parameters: a) direct weighing of the retroperitoneal (RP) and epididymal (EPI) adipose tissues; b) determination of plasma total cholesterol, triglycerides and HDL cholesterol concentration. The MT promoted the reduction of 1.4 times in both tissues RP and EPI in MTS group compared to group S. Treatment with MT decreased 2.7 times triglyceride in the MTS group compared with the group S. The sucrose consumption did not alter the plasma cholesterol concentration, however the consumption of MT significantly reduced total cholesterol circulating. The HDL cholesterol concentration, in the MTS group, showed higher concentration than in group S (1.3 times). MT prevents in young male rats the increase of lipid deposits and dyslipidemia caused by excessive consumption of sucrose.

Monomers based on plant oil derivatives bearing furan heterocycles appended through thiol-ene click chemistry were prepared and, subsequently, polymerized via a second type of click reaction, i. e. the Diels-Alder (DA) polycondensation between furan and maleimide complementary moieties. Two basic approaches were considered for these DA polymerizations, namely (i) the use of monomers with two terminal furan rings in conjunction with bismaleimides (AA + BB systems) and (ii) the use of a protected AB monomer incorporating both furan and maleimide end groups. This study clearly showed that both strategies were successful, albeit with different outcomes, in terms of the nature of the ensuing products. The application of the retro-DA reaction to these polymers confirmed their thermoreversible character, i. e. the clean-cut return to their respective starting monomers, opening the way to original macromolecular materials with interesting applications, like mendability and recyclability.

The synthesis of a functionalized 1-oxo-1,2,3,4-tetrahydroisoquinoline-3-carboxylic acid has been performed in 10 steps from the readily available dimedone. Only three purifications by flash chromatography are required through the whole sequence. The key step is the reaction between a dimedone derivative and a chlorotetrolic ester, that gives a tetrasubstituted benzene ring (through a Diels-Alder/retro- Diels-Alder process) bearing the substituents in the suitable positions for further functionalization. Despite new methods and combined strategies, conventional cancer chemotherapy still lacks specificity and induces drug resistance. Gene therapy can offer the potential to obtain the success in the clinical treatment of cancer and this can be achieved by replacing mutated tumour suppressor genes, inhibiting gene transcription, introducing new genes encoding for therapeutic products, or specifically silencing any given target gene. Concerning gene silencing, attention has recently shifted onto the RNA interference (RNAi) phenomenon. Gene silencing mediated by RNAi machinery is based on short RNA molecules, small interfering RNAs (siRNAs) and microRNAs (miRNAs), that are fully o partially homologous to the mRNA of the genes being silenced, respectively. On one hand, synthetic siRNAs appear as an important research tool to understand the function of a gene and the prospect of using siRNAs as potent and specific inhibitors of any target gene provides a new therapeutical approach for many untreatable diseases, particularly cancer. On the other hand, the discovery of the gene regulatory pathways mediated by miRNAs, offered to the research community new important perspectives for the comprehension of the physiological and, above all, the pathological mechanisms underlying the gene regulation. Indeed, changes in miRNAs expression have been identified in several types of neoplasia and it has also been proposed that the overexpression of genes in cancer cells may be due to the disruption of a control network in which relevant miRNA are implicated. For these reasons, I focused my research on a possible link between RNAi and the enzyme cyclooxygenase-2 (COX-2) in the field of colorectal cancer (CRC), since it has been established that the transition adenoma-adenocarcinoma and the progression of CRC depend on aberrant constitutive expression of COX-2 gene. In fact, overexpressed COX-2 is involved in the block of apoptosis, the stimulation of tumor-angiogenesis and promotes cell invasion, tumour growth and metastatization. On the basis of data reported in the literature, the first aim of my research was to develop an innovative and effective tool, based on the RNAi mechanism, able to silence strongly and specifically COX-2 expression in human colorectal cancer cell lines. In this study, I firstly show that an siRNA sequence directed against COX-2 mRNA (siCOX-2), potently downregulated COX-2 gene expression in human umbilical vein endothelial cells (HUVEC) and inhibited PMA-induced angiogenesis in vitro in a specific, non-toxic manner. Moreover, I found that the insertion of a specific cassette carrying anti-COX-2 shRNA sequence (shCOX-2, the precursor of siCOX-2 previously tested) into a viral vector (pSUPER.retro) greatly increased silencing potency in a colon cancer cell line (HT-29) without activating any interferon response. Phenotypically, COX-2 deficient HT-29 cells showed a significant impairment of their in vitro malignant behaviour. Thus, results reported here indicate an easy-to-use, powerful and high selective virus-based method to knockdown COX-2 gene in a stable and long-lasting manner, in colon cancer cells. Furthermore, they open up the possibility of an in vivo application of this anti-COX-2 retroviral vector, as therapeutic agent for human cancers overexpressing COX-2. In order to improve the tumour selectivity, pSUPER.retro vector was modified for the shCOX-2 expression cassette. The aim was to obtain a strong, specific transcription of shCOX-2 followed by COX-2 silencing mediated by siCOX-2 only in cancer cells. For this reason, H1 promoter in basic pSUPER.retro vector [pS(H1)] was substituted with the human Cox-2 promoter [pS(COX2)] and with a promoter containing repeated copies of the TCF binding element (TBE) [pS(TBE)]. These promoters were choosen because they are partculary activated in colon cancer cells. COX-2 was effectively silenced in HT-29 and HCA-7 colon cancer cells by using enhanced pS(COX2) and pS(TBE) vectors. In particular, an higher siCOX-2 production followed by a stronger inhibition of Cox-2 gene were achieved by using pS(TBE) vector, that represents not only the most effective, but also the most specific system to downregulate COX-2 in colon cancer cells. Because of the many limits that a retroviral therapy could have in a possible in vivo treatment of CRC, the next goal was to render the enhanced RNAi-mediate COX-2 silencing more suitable for this kind of application. Xiang and et al. (2006) demonstrated that it is possible to induce RNAi in mammalian cells after infection with engineered E. Coli strains expressing Inv and HlyA genes, which encode for two bacterial factors needed for successful transfer of shRNA in mammalian cells. This system, called “trans-kingdom” RNAi (tkRNAi) could represent an optimal approach for the treatment of colorectal cancer, since E. Coli in normally resident in human intestinal flora and could easily vehicled to the tumor tissue. For this reason, I tested the improved COX-2 silencing mediated by pS(COX2) and pS(TBE) vectors by using tkRNAi system. Results obtained in HT-29 and HCA-7 cell lines were in high agreement with data previously collected after the transfection of pS(COX2) and pS(TBE) vectors in the same cell lines. These findings suggest that tkRNAi system for COX-2 silencing, in particular mediated by pS(TBE) vector, could represent a promising tool for the treatment of colorectal cancer. Flanking the studies addressed to the setting-up of a RNAi-mediated therapeutical strategy, I proposed to get ahead with the comprehension of new molecular basis of human colorectal cancer. In particular, it is known that components of the miRNA/RNAi pathway may be altered during the progressive development of colorectal cancer (CRC), and it has been already demonstrated that some miRNAs work as tumor suppressors or oncomiRs in colon cancer. Thus, my hypothesis was that overexpressed COX-2 protein in colon cancer could be the result of decreased levels of one or more tumor suppressor miRNAs. In this thesis, I clearly show an inverse correlation between COX-2 expression and the human miR- 101(1) levels in colon cancer cell lines, tissues and metastases. I also demonstrate that the in vitro modulating of miR-101(1) expression in colon cancer cell lines leads to significant variations in COX-2 expression, and this phenomenon is based on a direct interaction between miR-101(1) and COX-2 mRNA. Moreover, I started to investigate miR-101(1) regulation in the hypoxic environment since adaptation to hypoxia is critical for tumor cell growth and survival and it is known that COX-2 can be induced directly by hypoxia-inducible factor 1 (HIF-1). Surprisingly, I observed that COX-2 overexpression induced by hypoxia is always coupled to a significant decrease of miR-101(1) levels in colon cancer cell lines, suggesting that miR-101(1) regulation could be involved in the adaption of cancer cells to the hypoxic environment that strongly characterize CRC tissues.

Untersuchungen zur stereokontrollierten Synthese von 3-Mercaptolysinderivaten: 3-Mercaptolysin und Peptide mit einer 3-Mercaptolysin-Einheit sind als Liganden für Nukleardiagnostika in der Kontrastmittelforschung von großem Interesse. Für das Screening Gewebe-selektiver Diagnostika sollten Mercaptolysinderivate, die sich für den Einbau in Peptide eignen, stereokontrolliert aufgebaut werden. Als Grundlage wurde im Rahmen dieser Arbeit eine sehr effiziente Synthese von Methoxymethyl-(4-oxo-butyl)-carbaminsäure-tert-butylester aus Pent-4-en-1-ol entwickelt. Nach Olefinierung des Aldehyds konnten Auxiliar-substituierte 6-Amino-hexensäure-derivate in guten Ausbeuten erhalten werden. Diese bildeten die Startmaterialien für auxiliargesteuerte Aziridinierungen mit anschließender Ringöffnung durch Schwefel-Nukleophile. Zudem wurden Azidierungen an Auxiliar-bewehrten Substraten, Michael-Additionen von Schwefel-Nukleophilen an Dehydroaminosäuren und viele weitere Reaktionen untersucht. Es galt dabei auf patentrechtlich geschützte Reaktionen zu verzichten, weil die Produkte ggf. in großem Maßstab kommerziell genutzt werden sollen. Dabei konnten alle vier stereoisomeren 2-Acetylamino-6-(tert-butoxycarbonyl-methoxymethyl-amino)-3-(4-methoxy-benzylsulfanyl)-hexansäurementhylester in guter Ausbeute synthetisiert werden. Nach Herstellung größerer Mengen der entsprechenden am Schwefelatom ungeschützten N-Fmoc-Aminosäurederivate sollen alle vier Stereoisomere in Peptide eingebaut und auf ihre Eignung als Liganden in Nukleardiagnostika untersucht werden. Untersuchungen zur Synthese neuartiger Ansa-Steroide: Über die Synthese von Ansa-Seco-Steroiden mittels eine Kaskade von intermolekularer Diels-Alder-Reaktion und anschließender Retro-Diels-Alder-Reaktion an 5,6,7,8-Tetradehydrosteroiden wurde erstmals 1986 von E. Winterfeldt et al. berichtet. Die damit eröffnete Möglichkeit eines völlig neuen Zugangs zu pharmakologisch interessanten Makrolid-Substraten konnte aber bislang nicht effektiv genutzt werden, weil insbesondere im Zusammenhang mit der Diels-Alder-Reaktion erhebliche präparative Probleme auftraten: Brauchbare Reaktivität nur bei sehr wenigen Dienophilen, Regioselektivitätsprobleme, etc. Hier galt es zu untersuchen, inwiefern sich diese Probleme durch die intramolekulare Reaktionsführung der Cycloaddition unterdrücken lassen können. Für die intramolekulare Diels-Alder-Reaktion zwischen der 5,7-Dien-Einheit des Steroids und einer an das Substrat gebundenen Dienophil-Einheit ist die Möglichkeit einer günstigen Anordnung der beiden Reaktanden-Gruppen entscheidend. Dafür wurden umfangreiche Untersuchungen zur alpha-konfigurierten Anbindung eines Dienophils in die 3-Position an 3-Hydroxy-5,6,7,8-Tetradehydrosteroiden durchgeführt: Mitsunobu-Reaktionen, Oxidations-Reduktions-Sequenzen, Oxidations-Ketalisierungs-Sequenzen, etc. Hierbei wurden zahlreiche neue Steroide synthetisiert. Es gelang jedoch nicht, 5,6,7,8-Tetradehydrosteroide mit axial in Position 3 eingebundenen Dienophilen in präparativ nutzbaren Mengen zu synthetisieren. Bei der Untersuchung intermolekularer Diels-Alder-Reaktionen zwischen Ergosterol und Brommaleinsäureanhydrid wurde vorzugsweise die Bildung der Addukte des 7,8,14,15-Tetradehydroisomers des Ergosterols beobachtet.

La sindrome di Noonan (SN) è una patologia a trasmissione autosomica dominante caratterizzata da bassa statura, difetti cardiaci congeniti, dismorfia facciale. In letteratura sono stati pubblicati pochi case reports riguardanti le condizioni orali-facciali in pazienti affetti da SN. Obiettivo. Individuare patologie di pertinenza ortopedico-ortodontica caratteristiche della sindrome utilizzando un campione di pazienti con diagnosi di SN. Metodi. Un gruppo di 10 pazienti affetti da SN è stato sottoposto a esame obiettivo extraorale ed intraorale, ortopantomografia, teleradiografia latero-laterale, impronte delle arcate dentarie. Le misurazioni sulle TLL sono state effettuate sulla base dell'analisi MBT; i valori palatali provengono dai modelli di studio dell’arcata superiore. È stata utilizzato il test t-Student per mettere a confronto il gruppo di studio e il gruppo di controllo riguardo le misure cefalometriche e i valori palatali. Risultati. Nel gruppo di studio sono state rilevate anomalie di numero (un dente deciduo soprannumerario e una agenesia di un dente permanente). Il test t-Student rivela differenze statisticamente significative per 7 variabili cefalometriche su 13 e per 2 variabili palatali. Conclusioni. Basandosi su questo studio è possibile concludere che i pazienti con SN mostrano II classe scheletrica di tipo mandibolare, crescita iperdivergente, tendenza al morso aperto scheletrico, palatoversione degli incisivi superiori, palato stretto. Questi risultati possono fornire informazioni utili sia per la diagnosi di SN sia per la pianificazione del corretto trattamento ortodontico.

Dalla collaborazione fra il Comune di Ravenna ed ENI ha preso origine il progetto “RIGED – Ra” ossia il “Progetto di ripristino e gestione delle dune costiere ravennati”. Nell’ambito di tale attività sperimentale si è voluto effettuare una caratterizzazione dell’idrologia di una limitata, ma rappresentativa, porzione dell’acquifero freatico costiero situata in un cordone di dune posto nella Pineta di Lido di Classe, a sud di Foce Bevano. Lo studio si pone di essere rappresentativo per le caratteristiche idrogeologiche delle dune costiere adriatiche nella zona di Ravenna. A tale fine è stato valutato l’andamento di alcuni parametri chimico-fisici delle acque sotterranee; inoltre, è stata monitorata mensilmente la profondità della tavola d’acqua (water table - WT). Questi monitoraggi hanno permesso di descrivere la distribuzione delle acque dolci e di quelle salate nonché la loro dinamica stagionale. Infine, è stata eseguita un’analisi idro-geochimica con l’intento di valutare la tipologia delle acque presenti nell’area in esame e la loro eventuale variazione stagionale. Per la raccolta dei campioni è stata sfruttata l’innovativa metodologia a minifiltri utilizzata da alcuni anni nel nord dell’Europa, in modo particolare in Olanda. Questa tecnica ha due caratteristiche peculiari: i tempi di campionamento vengono ridotti notevolmente ed, inoltre, permette un’ottima precisione e rappresentatività delle acque di falda a diverse profondità poiché si effettua un campionamento ogni 0,50 m. L’unico limite riscontrato, al quale vi è comunque rimedio, è il fatto che la loro posizione risulti fissa per cui, qualora vi siano delle fluttuazioni dell’acquifero al di sopra del minifiltro più superficiale, queste non vengono identificate. È consigliato quindi utilizzare questo metodo di campionamento poiché risulta essere più performante rispetto ad altri (ad esempio al sistema che sfrutta lo straddle packers SolinstTM ) scegliendo tra due diverse strategie per rimediare al suo limite: si aggiungono minifiltri superficiali che nel periodo estivo si trovano nella zona vadosa dell’acquifero oppure si accompagna sempre il campionamento con una trivellata che permetta il campionamento del top della falda. Per quanto concerne la freatimetria il campionamento mensile (6 mesi) ha mostrato come tutta l’area di studio sia un sistema molto suscettibile all’andamento delle precipitazioni soprattutto per la fascia di duna prossima alla costa in cui la scarsa vegetazione e la presenza di sedimento molto ben cernito con una porosità efficace molto elevata facilitano la ricarica dell’acquifero da parte di acque dolci. Inoltre, sul cordone dunoso l’acquifero si trova sempre al di sopra del livello medio mare anche nel periodo estivo. Per questa caratteristica, nel caso l’acquifero venisse ricaricato artificialmente con acque dolci (Managed Aquifer Recharge), potrebbe costituire un efficace sistema di contrasto all’intrusione salina. Lo spessore d’acqua dolce, comunque, è molto variabile proprio in funzione della stagionalità delle precipitazioni. Nell’area retro-dunale, invece, nel periodo estivo l’acquifero freatico è quasi totalmente al di sotto del livello marino; ciò probabilmente è dovuto al fatto che, oltre ai livelli topografici prossimi al livello medio mare, vi è una foltissima vegetazione molto giovane, ricresciuta dopo un imponente incendio avvenuto circa 10 anni fa, la quale esercita una notevole evapotraspirazione. È importante sottolineare come durante la stagione autunnale, con l’incremento delle precipitazioni la tavola d’acqua anche in quest’area raggiunga livelli superiori a quello del mare. Dal monitoraggio dei parametri chimico – fisici, in particolare dal valore dell’Eh, risulta che nel periodo estivo l’acquifero è un sistema estremamente statico in cui la mancanza di apporti superficiali di acque dolci e di flussi sotterranei lo rende un ambiente fortemente anossico e riducente. Con l’arrivo delle precipitazioni la situazione cambia radicalmente, poiché l’acquifero diventa ossidante o lievemente riducente. Dalle analisi geochimiche, risulta che le acque sotterranee presenti hanno una composizione esclusivamente cloruro sodica in entrambe le stagioni monitorate; l’unica eccezione sono i campioni derivanti dal top della falda raccolti in gennaio, nei quali la composizione si è modificata in quanto, il catione più abbondante rimane il sodio ma non si ha una dominanza di un particolare anione. Tale cambiamento è causato da fenomeni di addolcimento, rilevati dall’indice BEX, che sono causati all’arrivo delle acque dolci meteoriche. In generale, si può concludere che la ricarica superficiale e la variazione stagionale della freatimetria non sono tali da determinare un processo di dolcificazione in tutto l’acquifero dato che, nelle zone più profonde, si rivela la presenza permanente di acque a salinità molto superiore a 10 g/L. La maggior ricarica superficiale per infiltrazione diretta nelle stagioni a più elevata piovosità non è quindi in grado di approfondire l’interfaccia acqua dolce-acqua salata e può solamente causare una limitata diluizione delle acque di falda superficiali.

I studied the effects exerted by the modifications on structures and biological activities of the compounds so obtained. I prepared peptide analogues containing unusual amino acids such as halogenated, alkylated (S)- or (R)-tryptophans, useful for the synthesis of mimetics of the endogenous opioid peptide endomorphin-1, or 2-oxo-1,3-oxazolidine-4-carboxylic acids, utilized as pseudo-prolines having a clear all-trans configuration of the preceding peptide bond. The latter gave access to a series of constrained peptidomimetics with potential interest in medicinal chemistry and in the field of the foldamers. In particular, I have dedicated much efforts to the preparation of cyclopentapeptides containing D-configured, alfa-, or beta-aminoacids, and also of cyclotetrapeptides including the retro-inverso modification. The conformational analyses confirmed that these cyclic compounds can be utilized as rigid scaffolds mimicking gamma- or beta-turns, allowing to generate new molecular and 3D diversity. Much work has been dedicated to the structural analysis in solution and in the receptor-bound state, fundamental for giving a rationale to the experimentally determined bioactivity, as well as for predicting the activity of virtual compounds (in silico pre-screen). The conformational analyses in solution has been done mostly by NMR (2D gCosy, Roesy, VT, molecular dynamics, etc.). A special section is dedicated to the prediction of plausible poses of the ligands when bound to the receptors by Molecular Docking. This computational method proved to be a powerful tool for the investigation of ligand-receptor interactions, and for the design of selective agonists and antagonists. Another practical use of cyclic peptidomimetics was the synthesis and biological evaluation of cyclic analogues of endomorphin-1 lacking in a protonable amino group. The studies revealed that a inverse type II beta-turn on D-Trp-Phe constituted the bioactive conformation.
