QUESTIONS UNDER STUDY: We wished to investigate the prevalence of delirium in patients upon admission to nursing homes and whether or not the previous place of residence predicts delirium. METHODS: The Resident Assessment Instrument Minimum Data Set (RAI-MDS) and the Nursing Home Confusion Assessment Method (NHCAM) were used to determine whether the previous place of residence (community, nursing home, acute care, psychiatric, rehabilitation hospital) predicted the prevalence of sub-syndromal or full delirium in nursing home residents in three Swiss cantons (n = 11745). RESULTS: 39.7% had sub-syndromal and 6.5% had full delirium. Lower cognitive performance and increased depressive symptoms were significant predictors of higher NHCAM values independent of previous residence. Age, civil status, continence, newly introduced drugs, and basic activities of daily living were predictors in some resident groups. The variance of NHCAM scores explained varied between 25.1% and 32.3% depending on previous residence. CONCLUSIONS: Sub-syndromal and full delirium are common upon nursing home admission. Increased dependence and depression are consistently associated with higher NHCAM scores. Patients from psychiatric settings have an increased risk of delirium. Although factors associated with delirium depend on a patient's previous residence, all patients must be carefully screened for sub-syndromal and full delirium.

Background: One third of hospitalized older-adults who developed a delirium are discharged prior to complete resolution of their symptoms. Others may develop symptoms shortly after their hospital discharge or an acute illness. Therefore, strategies for early detection and prevention of delirium at home must be created and implemented. Aims: The aim of the pilot study was two-fold. First, to develop and test the effectiveness of a nursing intervention to detect and prevent delirium among older- adults who were recently hospitalized or had an acute illness. Second, we assessed the feasibility and acceptability of this nursing intervention strategy with this specific population. Methods: A total of 114 patients age 65 and older were recruited in a home health service to participate between February and November 2012. Participants were randomized into an experimental group (n=56) or a control group (n=58). In addition to the control group which only receives standard home care, nursing interventions tailored to detect/prevent delirium were delivered to the experimental group at 5 time points following discharge (at 48 hours, 72 hours, 7 days, 14 days, and 21 days). Socio-demographic characteristics, body mass index, medications, comorbidities, delirium symptoms (Confusion Assessment Method), cognitive impairment (Mini- Mental State Examination) and functional status (Activities of Daily Living) were assessed at study entry (TT). Outcomes of delirium symptoms, cognitive impairment and functional status were assessed after one month (T2). Descriptive and bivariate methods were used to analyse the data. Results: The two groups were similar at baseline. At one month following discharge no statistical differences were observed between groups in terms of symptoms of delirium (p= 0.085), cognitive impairment (p= 0.151) and functional status (p= 0.235). However in the nursing intervention group, significant improvements in cognitive functioning (p= 0,005) and functional status (p= 0,000) as well as decreased delirium symptoms (p=0,003) were observed. The nursing intervention strategy was feasible and well received by the participants. Conclusion: Nursing intervention strategy to detect/prevent delirium appears to be effective but a larger clinical study is needed to confirm these preliminary findings. - Introduction : Un tiers des personnes âgées hospitalisées développent un état confusionnel aigu (ECA) et quittent l'hôpital sans que les symptômes ne soient résolus. D'autres peuvent développer des symptômes d'ECA à domicile après une hospitalisation ou une maladie aiguë. Pour ces raisons, des stratégies de détection et prévention précoces d'ECA doivent être développées, implantées et évaluées. But : Cette étude pilote avait pour but de développer et tester les effets d'une stratégie d'interventions infirmières pour détecter et prévenir l'ECA chez des personnes âgées à domicile après une hospitalisation ou une maladie récente. Dans un deuxième temps, la faisabilité et l'acceptabilité de l'implantation de cette stratégie auprès de cette population spécifique ainsi que de l'étude ont été évaluées. Méthode : Au total 114 personnes 65 ans) ont été recrutées entre février et novembre 2012. Les participants ont été randomisés, soit dans le groupe expérimental (GE, n=56), soit dans le groupe témoin (GT, n=58). En complément des soins usuels, une stratégie d'interventions de détection/prévention d'ECA a été dispensée au GE à 48 heures, 72 heures, 7 jours, 14 jours et 21 jours après le retour à domicile ou une maladie récente. Des données sociodémographiques et de santé (Indice de Masse Corporelle, relevé de la médication, comorbidités), la présence de symptômes d'ECA (Confusion Assessment Method), de troubles cognitifs (Mini évaluation de l'état mental) et de déficit fonctionnel (Activités de la vie quotidienne et instrumentales) ont été évalués à l'entrée de l'étude (T,). L'effet de la stratégie d'interventions a été mesuré sur le nombre de symptômes d'ECA, du déficit/état cognitif (Mini évaluation de l'état mental) et du déficit/état fonctionnel (Activités de la vie quotidienne) après un mois (T2). Des analyses descriptives et bivariées ont été effectuées. Résultats : Les deux groupes étaient équivalents au début de l'étude. Aucune différence significative n'a été retrouvée après un mois entre le GE et le GT par rapport au nombre de symptômes d'ECA (p= 0,085), au déficit cognitif (p= 0,151) et fonctionnel (p= 0,235). Toutefois, une amélioration significative a été observée dans le GE par rapport aux symptômes d'ECA (p= 0,003), aux déficits cognitifs (p= 0,005) et fonctionnels (p= 0,000) à un mois. La stratégie d'interventions s'avère faisable et a été bien acceptée par les participants. Conclusion : La stratégie d'interventions infirmières de détection/prévention d'ECA à domicile semble prometteuse, mais des études cliniques à large échelle sont nécessaires pour confirmer ces résultats préliminaires.

Summary Due to their conic shape and the reduction of area with increasing elevation, mountain ecosystems were early identified as potentially very sensitive to global warming. Moreover, mountain systems may experience unprecedented rates of warming during the next century, two or three times higher than that records of the 20th century. In this context, species distribution models (SDM) have become important tools for rapid assessment of the impact of accelerated land use and climate change on the distribution plant species. In my study, I developed and tested new predictor variables for species distribution models (SDM), specific to current and future geographic projections of plant species in a mountain system, using the Western Swiss Alps as model region. Since meso- and micro-topography are relevant to explain geographic patterns of plant species in mountain environments, I assessed the effect of scale on predictor variables and geographic projections of SDM. I also developed a methodological framework of space-for-time evaluation to test the robustness of SDM when projected in a future changing climate. Finally, I used a cellular automaton to run dynamic simulations of plant migration under climate change in a mountain landscape, including realistic distance of seed dispersal. Results of future projections for the 21st century were also discussed in perspective of vegetation changes monitored during the 20th century. Overall, I showed in this study that, based on the most severe A1 climate change scenario and realistic dispersal simulations of plant dispersal, species extinctions in the Western Swiss Alps could affect nearly one third (28.5%) of the 284 species modeled by 2100. With the less severe 61 scenario, only 4.6% of species are predicted to become extinct. However, even with B1, 54% (153 species) may still loose more than 80% of their initial surface. Results of monitoring of past vegetation changes suggested that plant species can react quickly to the warmer conditions as far as competition is low However, in subalpine grasslands, competition of already present species is probably important and limit establishment of newly arrived species. Results from future simulations also showed that heavy extinctions of alpine plants may start already in 2040, but the latest in 2080. My study also highlighted the importance of fine scale and regional. assessments of climate change impact on mountain vegetation, using more direct predictor variables. Indeed, predictions at the continental scale may fail to predict local refugees or local extinctions, as well as loss of connectivity between local populations. On the other hand, migrations of low-elevation species to higher altitude may be difficult to predict at the local scale. Résumé La forme conique des montagnes ainsi que la diminution de surface dans les hautes altitudes sont reconnues pour exposer plus sensiblement les écosystèmes de montagne au réchauffement global. En outre, les systèmes de montagne seront sans doute soumis durant le 21ème siècle à un réchauffement deux à trois fois plus rapide que celui mesuré durant le 20ème siècle. Dans ce contexte, les modèles prédictifs de distribution géographique de la végétation se sont imposés comme des outils puissants pour de rapides évaluations de l'impact des changements climatiques et de la transformation du paysage par l'homme sur la végétation. Dans mon étude, j'ai développé de nouvelles variables prédictives pour les modèles de distribution, spécifiques à la projection géographique présente et future des plantes dans un système de montagne, en utilisant les Préalpes vaudoises comme zone d'échantillonnage. La méso- et la microtopographie étant particulièrement adaptées pour expliquer les patrons de distribution géographique des plantes dans un environnement montagneux, j'ai testé les effets d'échelle sur les variables prédictives et sur les projections des modèles de distribution. J'ai aussi développé un cadre méthodologique pour tester la robustesse potentielle des modèles lors de projections pour le futur. Finalement, j'ai utilisé un automate cellulaire pour simuler de manière dynamique la migration future des plantes dans le paysage et dans quatre scénarios de changement climatique pour le 21ème siècle. J'ai intégré dans ces simulations des mécanismes et des distances plus réalistes de dispersion de graines. J'ai pu montrer, avec les simulations les plus réalistes, que près du tiers des 284 espèces considérées (28.5%) pourraient être menacées d'extinction en 2100 dans le cas du plus sévère scénario de changement climatique A1. Pour le moins sévère des scénarios B1, seulement 4.6% des espèces sont menacées d'extinctions, mais 54% (153 espèces) risquent de perdre plus 80% de leur habitat initial. Les résultats de monitoring des changements de végétation dans le passé montrent que les plantes peuvent réagir rapidement au réchauffement climatique si la compétition est faible. Dans les prairies subalpines, les espèces déjà présentes limitent certainement l'arrivée de nouvelles espèces par effet de compétition. Les résultats de simulation pour le futur prédisent le début d'extinctions massives dans les Préalpes à partir de 2040, au plus tard en 2080. Mon travail démontre aussi l'importance d'études régionales à échelle fine pour évaluer l'impact des changements climatiques sur la végétation, en intégrant des variables plus directes. En effet, les études à échelle continentale ne tiennent pas compte des micro-refuges, des extinctions locales ni des pertes de connectivité entre populations locales. Malgré cela, la migration des plantes de basses altitudes reste difficile à prédire à l'échelle locale sans modélisation plus globale.

A rapid biological method for the determination of the bioavailability of naphthalene was developed and its value as an alternative to extraction-based chemical approaches demonstrated. Genetically engineered whole-cell biosensors are used to determine bioavailable naphthalene and their responses compared with results from Tenax extraction and chemical analysis. Results show a 1:1 correlation between biosensor results and chemical analyses for naphthalene-contaminated model materials and sediments, but the biosensor assay is much faster. This work demonstrates that biosensor technology can perform as well as standard chemical methods, though with some advantages including the inherent biological relevance of the response, rapid response time, and potential for field deployment. A survey of results from this work and the literature shows that bioavailability under non-equilibrium conditions nonetheless correlates well with K(oc) or K(d). A rationale is provided wherein chemical resistance is speculated to be operative.

The reason for this study is to propose a new quantitative approach on how to assess the quality of Open Access University Institutional Repositories. The results of this new approach are tested in the Spanish University Repositories. The assessment method is based in a binary codification of a proposal of features that objectively describes the repositories. The purposes of this method are assessing the quality and an almost automatically system for updating the data of the characteristics. First of all a database was created with the 38 Spanish institutional repositories. The variables of analysis are presented and explained either if they are coming from bibliography or are a set of new variables. Among the characteristics analyzed are the features of the software, the services of the repository, the features of the information system, the Internet visibility and the licenses of use. Results from Spanish universities ARE provided as a practical example of the assessment and for having a picture of the state of the development of the open access movement in Spain.

OBJECTIVE: - Clinical observations and a review of the literature led us to hypothesize that certain personality and character traits could provide improved understanding, and thus improved prevention, of suicidal behaviour among young women with eating disorders. METHOD: - The clinical group consisted of 152 women aged between 18 and 24 years, with DSM-IV anorexia nervosa/restrictive type (AN-R = 66), anorexia nervosa/purging type (AN-P = 37), bulimia nervosa/non-purging type (BN-NP = 9), or bulimia nervosa/purging type (BN-P = 40). The control group consisted of 140 subjects. The assessment measures were the Minnesota Multiphasic Personality Inventory-second version (MMPI-2) scales and subscales, the Beck Depression Inventory (BDI) used to control for current depressive symptoms, plus a specific questionnaire concerning suicide attempts. RESULTS: - Suicide attempts were most frequent in subjects with purging behaviour (30.0% for BN-P and 29.7% for AN-P). Those attempting suicide among subjects with eating disorders were mostly students (67.8%). For women with AN-R the scales for 'Depression' and 'Antisocial practices' represented significant suicidal risk, for women with AN-P the scales for 'Hysteria', 'Psychopathic deviate', 'Shyness/Self-consciousness', 'Antisocial Practices', 'Obsessiveness' and 'Low self-esteem' were risk indicators and for women with BN-P the 'Psychasthenia', 'Anger' and 'Fears' scales were risk indicators. CONCLUSION: - This study provides interesting results concerning the personality traits of young women with both eating disorders and suicidal behaviour. Students and those with purging behaviour are most at risk. Young women should be given more attention with regard to the risk of suicide attempts if they: (a). have AN-R with a tendency to self-punishment and antisocial conduct, (b). have AN-P with multiple physical complaints, are not at ease in social situations and have antisocial behaviour, or (c). if they have BN-P and tend to be easily angered with obsessive behaviour and phobic worries. The MMPI-2 is an interesting assessment method for the study of traits indicating a risk of suicidal behaviour in young subjects, after controlling for current depressive pathology.

Firefighters work contains numerous hazards. In rescue missions there are always hazards, which are unavoidable. Risk assessments and near-miss –situation investigation increases firefighters risk awareness and decreases accidents. This thesis concerns occupational risk management in fire department. This thesis introduces methods for identifying occupational hazards and risk assessment. In this thesis is introduced risk assessment method developed whit fire departments. This thesis also concerns a model of fire departments safety management system. In addition thesis contains a short introduction of how occupational health care cold be used and what kind of information system would support fire departments daily occupational safety activities.

AIMS: Estimating the effect of a nursing intervention in home-dwelling older adults on the occurrence and course of delirium and concomitant cognitive and functional impairment. METHODS: A randomized clinical pilot trial using a before/after design was conducted with older patients discharged from hospital who had a medical prescription to receive home care. A total of 51 patients were randomized into the experimental group (EG) and 52 patients into the control group (CG). Besides usual home care, nursing interventions were offered by a geriatric nurse specialist to the EG at 48 h, 72 h, 7 days, 14 days, and 21 days after discharge. All patients were monitored for symptoms of delirium using the Confusion Assessment Method. Cognitive and functional statuses were measured with the Mini-Mental State Examination and the Katz and Lawton Index. RESULTS: No statistical differences with regard to symptoms of delirium (p = 0.085), cognitive impairment (p = 0.151), and functional status (p = 0.235) were found between the EG and CG at study entry and at 1 month. After adjustment, statistical differences were found in favor of the EG for symptoms of delirium (p = 0.046), cognitive impairment (p = 0.015), and functional status (p = 0.033). CONCLUSION: Nursing interventions to detect delirium at home are feasible and accepted. The nursing interventions produced a promising effect to improve delirium.

Yhteisten työpaikkojen turvallisuuteen vaikuttaa keskeisten työympäristön vaaratekijöiden lisäksi oleellisesti tilaajan ja toimittajan välinen tiedonkulku. Ratkaisu yhteisten työpaikkojen turvallisuuden hallintaan on kokonaisuuden hyvä hallinta eli toimintojen yhteensovittaminen. Tämän työn tarkoituksena oli etsiä matkatyöntekijöiden turvallisuutta vaarantavia tekijöitä ja ratkaisuja niiden poistamiseksi Laroxilla. Epäkohtien määrittelemiseksi haastateltiin työntekijöitä ja tehtiin riskien arviointi Sosiaali- ja terveysministeriön menetelmällä ”Riskien arviointi työpaikalla”. Riskien arvioinnin tuloksia hyödynnettiin myös luotaessa turvallisuusohjeita matkatyöntekijöille ja suunniteltaessa turvallisuusohjeista pidettävän koulutuksen sisältöä. Turvallisuusohjeiden yksi tärkeimmistä osioista on proseduuri, jonka mukaan asiakkaiden tehtaiden turvallisuuteen liittyviä tietoja aletaan jatkossa kerätä ja tallentaa.

Background Chronic obstructive pulmonary disease (COPD) is a heterogeneous disease whose assessment and management have traditionally been based on the severity of airflow limitation (forced expiratory volume in 1 s (FEV1)). Yet, it is now clear that FEV1 alone cannot describe the complexity of the disease. In fact, the recently released Global Initiative for Chronic Obstructive Lung Disease (GOLD), 2011 revision has proposed a new combined assessment method using three variables (symptoms, airflow limitation and exacerbations). Methods Here, we go one step further and propose that in the near future physicians will need a"control panel" for the assessment and optimal management of individual patients with complex diseases, including COPD, that provides a path towards personalised medicine. Results We propose that such a"COPD control panel" should include at least three different domains of the disease: severity, activity and impact. Each of these domains presents information on different"elements" of the disease with potential prognostic value and/or with specific therapeutic requirements. All this information can be easily incorporated into an"app" for daily use in clinical practice. Conclusion We recognise that this preliminary proposal needs debate, validation and evolution (eg, including"omics" and molecular imaging information in the future), but we hope that it may stimulate debate and research in the field.

BACKGROUND: Delirium and frailty - both potentially reversible geriatric syndromes - are seldom studied together, although they often occur jointly in older patients discharged from hospitals. This study aimed to explore the relationship between delirium and frailty in older adults discharged from hospitals. METHODS: Of the 221 patients aged >65 years, who were invited to participate, only 114 gave their consent to participate in this study. Delirium was assessed using the confusion assessment method, in which patients were classified dichotomously as delirious or nondelirious according to its algorithm. Frailty was assessed using the Edmonton Frailty Scale, which classifies patients dichotomously as frail or nonfrail. In addition to the sociodemographic characteristics, covariates such as scores from the Mini-Mental State Examination, Instrumental Activities of Daily Living scale, and Cumulative Illness Rating Scale for Geriatrics and details regarding polymedication were collected. A multidimensional linear regression model was used for analysis. RESULTS: Almost 20% of participants had delirium (n=22), and 76.3% were classified as frail (n=87); 31.5% of the variance in the delirium score was explained by frailty (R (2)=0.315). Age; polymedication; scores of the Confusion Assessment Method (CAM), instrumental activities of daily living, and Cumulative Illness Rating Scale for Geriatrics; and frailty increased the predictability of the variance of delirium by 32% to 64% (R (2)=0.64). CONCLUSION: Frailty is strongly related to delirium in older patients after discharge from the hospital.

BACKGROUND: Delirium is an acute cognitive impairment among older hospitalized patients. It can persist until discharge and for months after that. Despite proof that evidence-based nursing interventions are effective in preventing delirium in acute hospitals, interventions among home-dwelling older patients is lacking. The aim was to assess feasibility and acceptability of a nursing intervention designed to detect and reduce delirium in older adults after discharge from hospital. METHODS: Randomized clinical pilot trial with a before/after design was used. One hundred and three older adults were recruited in a home healthcare service in French-speaking Switzerland and randomized into an experimental group (EG, n = 51) and a control group (CG, n = 52). The CG received usual homecare. The EG received usual homecare plus five additional nursing interventions at 48 and 72 h and at 7, 14 and 21 days after discharge. These interventions were tailored for detecting and reducing delirium and were conducted by a geriatric clinical nurse (GCN). All patients were monitored at the start of the study (M1) and throughout the month for symptoms of delirium (M2). This was documented in patients' records after usual homecare using the Confusion Assessment Method (CAM). At one month (M2), symptoms of delirium were measured using the CAM, cognitive status was measured using the Mini-Mental State Examination (MMSE), and functional status was measured using Katz and Lawton Index of activities of daily living (ADL/IADL). At the end of the study, participants in the EG and homecare nurses were interviewed about the acceptability of the nursing interventions and the study itself. RESULTS: Feasibility and acceptability indicators reported excellent results. Recruitment, retention, randomization, and other procedures were efficient, although some potentially issues were identified. Participants and nurses considered organizational procedures, data collection, intervention content, the dose-effect of the interventions, and methodology all to be feasible. Duration, patient adherence and fidelity were judged acceptable. Nurses, participants and informal caregivers were satisfied with the relevance and safety of the interventions. CONCLUSIONS: Nursing interventions to detect/improve delirium at home are feasible and acceptable. These results confirm that developing a large-scale randomized controlled trial would be appropriate. TRIAL REGESTRATION: ISRCTN registry no: 16103589 - 19 February 2016.

Objective The objective of the present study was to evaluate current radiographic parameters designed to investigate adenoid hypertrophy and nasopharyngeal obstruction, and to present an alternative radiographic assessment method. Materials and Methods In order to do so, children (4 to14 years old) who presented with nasal obstruction or oral breathing complaints were submitted to cavum radiographic examination. One hundred and twenty records were evaluated according to quantitative radiographic parameters, and data were correlated with a gold-standard videonasopharyngoscopic study, in relation to the percentage of choanal obstruction. Subsequently, a regression analysis was performed in order to create an original model so the percentage of the choanal obstruction could be predicted. Results The quantitative parameters demonstrated moderate, if not weak correlation with the real percentage of choanal obstruction. The regression model (110.119*A/N) demonstrated a satisfactory ability to “predict” the actual percentage of choanal obstruction. Conclusion Since current adenoid quantitative radiographic parameters present limitations, the model presented by the present study might be considered as an alternative assessment method in cases where videonasopharyngoscopic evaluation is unavailable.

A simple and rapid conductometric method for captopril determination using copper(II) sulphate solution as titrant was developed. The method was based on the chemical reaction between captopril and Cu(II) ions yielding a precipitate. The conductance of the solution was monitored as a function of the added volume of titrant. The method was applied with success for captopril determination in three pharmaceutical formulations. The relative standard deviation for six successive measurements was smaller than 0.5%. Recovery values from three samples, ranging from 97.7 to 103%, were obtained.

This paper describes a simple and rapid spectrophotometric method for quantitative determination of sildenafil citrate based on its reaction with p-chloranil accelerated by hydrogen peroxide, producing a stable purple compound (λmax= 535 nm). In the absence of peroxide this reaction is very slow. The experimental conditions were optimized by using response surface methodologies. Beer's law is obeyed in a concentration range of 8.52 x 10-5 - 1.70 x 10-3 mol L-1 (r = 0.999). The detection limit was 1.96 x 10-5 mol L-1. The method was successfully applied for the determination of sildenafil citrate in medicines with good accuracy and precision.
