Diabetes mellitus is associated with multi-organ system dysfunction including the cardiovascular and autonomic nervous system. Although it is well documented that post-infarct patients are at higher risk of sudden cardiac death, diabetes adds an additional risk associated with autonomic neuropathy. However it is not known how the presence of diabetes in post-infarct patients affects cardiac rhythm. The majority of HRV algorithms for determining cardiac inter-beat interval changes describe only beat-to-beat variation determined over the whole heart rate recording and therefore do not consider the ability of a heart beat to influence a train of succeeding beats nor whether or how the temporal dynamics of the inter-beat intervals changes. This study used Poincaré Plot derived features and incorporated increased lag intervals to compare post-infarct patients with no history of prior infarct with or without diabetes and found that for the nondiabetic post-infarct patients only increased lag of short term correlation (SD1) predicted mortality, whereas in the diabetic post-infarct group only long-term correlations (SD2) significantly predicted mortality at a follow-up period of eight years. Temporal dynamics measured as a complex correlation measure (CCM) was also a significant predictor of mortality only in the diabetic post-infarct cohort. This study highlights the different pathophysiological progression and risk profile associated with presence of diabetes in a post-infarct patient population at eight year follow-up.

Insight into instabilities of fiber laser regimes leading to complex self-pulsing operations is an opportunity to unlock the high power and dynamic operation tunability of lasers. Though many models have been suggested, there is no complete covering of self-pulsing complexity observed experimentally. Here, I further generalized our previous vector model of erbium-doped fiber laser and, for the first time, to the best of my knowledge, map tunability of complex vector self-pulsing on Poincare sphere (limit cycles and double scroll polarization attractors) for laser parameters, e.g., power, ellipticity of the pump wave, and in-cavity birefringence. Analysis validated by extensive numerical simulations demonstrates good correspondence to the experimental results on complex self-pulsing regimes obtained by many authors during the last 20 years.

The heart rate asymmetry (HRA) is a disproportionate distribution of heart rate signal. The current study was designed to assess the changes in HRA in experimental conditions using Poincaré plot during parasympathetic blockade (atropine infusion) and parasympathetic enhancement (scopolamine administration). After atropine infusion, the heart rate variability in 5 out of 8 subjects was found asymmetric. In contrast, all 8 subjects were found to be asymmetric during scopolamine administration. The physiological relevance of HRA was demonstrated by showing correlation with standard frequency domain parameters during all phases of the experiment. The deviation of asymmetry index (GI ( p )) from symmetric range was further analyzed, which was maximum during scopolamine administration and minimum during atropine infusion. These findings suggest that parasympathetic block reduces the prevalence of HRA, and has significant correlation of GI ( p ) with frequency domain features of HRV analysis.
