Reluctance has been expressed about treating chronic hepatitis C in active intravenous (IV) drug users (IDUs), and this is found in both international guidelines and routine clinical practice. However, the medical literature provides no evidence for an unequivocal treatment deferral of this risk group. We retrospectively analyzed the direct effect of IV drug use on treatment outcome in 500 chronic hepatitis C patients enrolled in the Swiss Hepatitis C Cohort Study. Patients were eligible for the study if they had their serum hepatitis C virus (HCV) RNA tested 6 months after the end of treatment and at least one visit during the antiviral therapy, documenting the drug use status. Five hundred patients fulfilled the inclusion criteria (199 were IDU and 301 controls). A minimum exposure to 80% of the scheduled cumulative dose of antivirals was reached in 66.0% of IDU and 60.5% of controls (P = NS). The overall sustained virological response (SVR) rate was 63.6%. Active IDU reached a SVR of 69.3%, statistically not significantly different from controls (59.8%). A multivariate analysis for treatment success showed no significant negative influence of active IV drug use. In conclusion, our study shows no relevant direct influence of IV drugs on the efficacy of anti-HCV therapy among adherent patients.

The ratio of cystatin C (cysC) to creatinine (crea) is regarded as a marker of glomerular filtration quality associated with cardiovascular morbidities. We sought to determine reference intervals for serum cysC-crea ratio in seniors. Furthermore, we sought to determine whether other low-molecular weight molecules exhibit a similar behavior in individuals with altered glomerular filtration quality. Finally, we investigated associations with adverse outcomes. A total of 1382 subjectively healthy Swiss volunteers aged 60 years or older were enrolled in the study. Reference intervals were calculated according to Clinical & Laboratory Standards Institute (CLSI) guideline EP28-A3c. After a baseline exam, a 4-year follow-up survey recorded information about overall morbidity and mortality. The cysC-crea ratio (mean 0.0124 ± 0.0026 mg/μmol) was significantly higher in women and increased progressively with age. Other associated factors were hemoglobin A1c, mean arterial pressure, and C-reactive protein (P < 0.05 for all). Participants exhibiting shrunken pore syndrome had significantly higher ratios of 3.5-66.5 kDa molecules (brain natriuretic peptide, parathyroid hormone, β2-microglobulin, cystatin C, retinol-binding protein, thyroid-stimulating hormone, α1-acid glycoprotein, lipase, amylase, prealbumin, and albumin) and creatinine. There was no such difference in the ratios of very low-molecular weight molecules (urea, uric acid) to creatinine or in the ratios of molecules larger than 66.5 kDa (transferrin, haptoglobin) to creatinine. The cysC-crea ratio was significantly predictive of mortality and subjective overall morbidity at follow-up in logistic regression models adjusting for several factors. The cysC-crea ratio exhibits age- and sex-specific reference intervals in seniors. In conclusion, the cysC-crea ratio may indicate the relative retention of biologically active low-molecular weight compounds and can independently predict the risk for overall mortality and morbidity in the elderly.

During the analysis of "glaucony" recovered during Leg 66, off Mexico, we reviewed the data on previously studied glaucony layers in active margin areas. We found the depth of Leg 66 glaucony sediments to be significantly greater than conventionally assumed appropriate to their genesis (100-500 m). Accordingly, we hypothesize their occurrence at unusual depth to be due to (1) transport of shallow sediments and redeposition at greater depths, (2) margin subsidence, or (3) genesis at greater depth than is generally assumed. For the area off Mexico, we reject (1). (2) has already been verified in Japan and is possible as an explanation for the present phenomenon without excluding (3), which we investigate in this chapter.

Las i¡genes visuales y de color en Petronio contribuyen notablemente a esa configuración, mientras que un estudio del léxico correspondiente, a partir en especial de las ediciones humanísticas de la obra, pueden sin duda arrojar luz sobre aspectos textuales aún no resueltos en esa obra maestra de la literatura latina

We investigated the solvent-extractable hydrocarbons, ketones, alcohols, and carboxylic acids of two Quaternary sediments from the Middle America Trench (Sections 487-2-3 and 491-1-5). These lipids are derived from a mixed input of autochthonous and allochthonous materials, with minor contributions from thermally mature sources. Their compositions are typical of those of immature Quaternary marine sediments, and their lipid distributions show many similarities to those of Japan Trench sediments.

Sediment accumulation rates, computed using agesediment thickness curves obtained from DSDP cores, are rarely corrected for compaction or bedding attitude to better approximate true sediment accumulation rates (c.f. van Andel et al., 1975; Davies et al., 1977; and Whitman and Davies, 1979). Variations with depth in either of these factors can hinder interpreting relative rates of sedimentary processes associated with a particular depositional environment. This problem becomes particularly relevant for convergent margin sediments, which often display variable bedding attitudes and pronounced changes in porosity, bulk density, and other parameters related to the compaction process at shallow depth. These rapid shallow changes render correlation of sedimentation rates within a single transect of holes very difficult. Two techniques have been applied to data collected from a transect of holes along the southwestern Mexico continental margin, DSDP Leg 66 (Fig. 1), to correct sediment accumulation rates for variations in compaction and bedding attitude. These corrections should help resolve true fluctuations in accumulation rates and their implications regarding convergent margin processes.

This publication presents results of microbiological and biogeochemical studies in the White Sea. Material was obtained during a series of expeditions in 1999-2002. The studies were carried out in the open part of the White Sea, in the Onega, Dvina and Kandalaksha Bays, as well as in the intertidal zone of the Kandalaksha Bay. Quantitative characteristics of activity of microbial processes in waters and bottom sediments of the White Sea were obtained. The total number of bacteria was equal to 150000-800000 cells/ml, and intensity of dark CO2 assimilation was equal to 0.9-17 µg C/l/day. Bacterial sulfate reduction was equal to 3-150 mg S/m**2/day, and methane formation and oxidation was equal to 13-6840 and 20-14650 µl CH4/m**2/day, respectively. Extremely high values of intensity of all principal microbial processes were found in intertidal sediments rich in organic matter: under decomposing macrophytes, in local pits at the lower intertidal boundary, and in the mouth of a freshwater brook. Average hydrogen sulfide production in highly productive intertidal sediments was 1950-4300 mg S/m**2/day, methane production was 0.5-8.7 ml CH4/m**2/day, and intensity of methane oxidation was up to 17.5 ml CH4/m**2/day. Calculations performed with account for areas occupied by microlandscapes of increased productivity showed that diurnal production of H2S and CH4 per 1 km**2 of the intertidal zone (August) was estimated as 60.8-202 kg S/km**2/day and 192-300 l CH4/km**2/day, respectively.
