Estudi observacional prospectiu obert sobre l’eficàcia del propranolol oral en 25 nens amb hemangiomes infantils que associen algun tipus de morbiditat. S’analitzen també els efectes secundaris, l’evolució de les lesions i les variables que influeixen en les recaigudes al finalitzar la medicació. L’estudi conclou que el propranolol és un fàrmac efectiu i ben tolerat en el tractament dels hemangiomes infantils. La duració del tractament és la única variable que demostra influir de forma estadísticament significativa en l’aparició de recidives al suspendre la medicació.

La vena safena interna es considera el conducte òptim per a la revascularització quirúrgica del sector infragenicular però fins a un 25% dels casos, aquesta no es troba disponible o bé no es apte per a bypass. L’objectiu de l’estudi és analitzar els resultats en quant a permeabilitat, taxa de salvament de l’extremitat i morbiditat associada dels procediments de revascularització infragenicular utilitzant les venes braquials (cefàlica i basílica) com a conducte arterial.

S'ha realitzat un estudi retrospectiu sobre 593 pacients dividits en tres grups d'edat (&70, 70-79 i& 80 anys), intervinguts de càncer de còlon per laparoscòpia entre 1998 i 2009. S’han analitzat totes les variables preoperatòries, la morbiditat associada a cada grup d’edat tant mèdica como quirúrgica i les dades intraoperatòries, per a obtenir la taxa de complicacions postoperatòries. Després dels resultats s'ha objectivat que l'índex de complicacions en el grup de majors de 80 anys és major a causa del gran nombre de factors de risc pel que fa als més joves.

L'anastomosi biliar en el trasplantament hepàtic és una de les causes més freqüents de morbiditat. En aquest treball s'ha analitzat l'evolució de les complicacions biliars després d'un trasplantament hepàtic en adults al llarg de 20 anys, comparant els primers 100 trasplantaments realitzats amb els darrers 200 entre 1985 i 2007. Resultats: la incidència de les complicacions biliars ha disminuït significativament al llarg dels anys, principalment a causa de l'abandonament de l'ús rutinari del tub de Kehr. La gravetat d'aquestes complicacions ha estat similar en ambdós grups.

Estudi transversal de pacients VIH en els que es va determinar colecalciferol (25-OH- Vit.D3) i PTH, excloint a pacients amb insuficiència renal, hepàtica i nivells plasmàtics anormals de calci i/o fósfor Es van incloure 566 pacients, amb una exposició a tenofovir del 56,4%. La prevalència de vitamina D insuficient va der del 71,2% i la deficiència del 39,6% . La PTH es va determinar en 228 casos, presentant nivells elevats 86 d’ells (37,7%). Els factors de risc ajustats de deficiència de vitamina D van ser, ésser de raza no blanca i la morbilitat psiquiàtrica, essent la lipoatròfia, un factor protector. Els factors de risc independents de nivells elevats de PTH van ser: Vitamina D&12 ng/ml: OR: 2,14 (IC95%: 1,19-3,82, p: 0,01) i l’ exposició a tenofovir: OR: 3,55 (IC95%: 1,62-7,7, p: 0,002).

INTRODUCCIÓ: nasofaringitis aguda (NFA) malaltia infecciosa amb elevada morbiditat-demanda en Atenció Primària. OBJECTIUS: adherència dels metges a la guia semfyc, conèixer patró epidemiològic i despesa en NFA. MATERIAL: Disseny descriptiu, anys 2009-2010. Càlcul mostral Epidat, 269 pacients. Mostreig aleatori simple. Tractament estadístic SPSS. RESULTATS: No prescripció d'antibiòtics 86,1% (p &0,05). Prevalença NFA 14%. 1 consulta mèdica (85,4%). Complicacions (2,2%). Paracetamol-AINEs (62,9%), genèrics (65%). Diferències entre prescripció antibiòtica i edat (p = 0,025) menor no prescripció a & 80años (p = 0,025). Cost mitjà total 69.6 euros (54,7-188,8; DT 22,8). CONCLUSIONS: bona adherència, alta prevalença, poques complicacions, elevada prescripció de fàrmacs simptomàtics. Cost considerable.

Existeix un gran nombre de grups virtuals dedicats a la maternitat (i a la paternitat), i alguns d'ells tenen un alt grau de participació. En el cas de la lactància materna, la manca de suport social i familiar, i la poca informació, o informació contradictòria que reben les mares, fins i tot, per part del seu pediatra, fa pensar que aquests grups poden ser un recurs a tenir en compte per tal d'obtenir aquesta expertesa i recolzament necessaris per a portar endavant amb èxit l'alletament dels fills. L'objectiu d'aquest treball és observar com funcionen alguns d'aquests grups, a través de l'observació participant, parant especial atenció a dos aspectes concrets: l'equilibri entre la funció informativa i la funció de suport, i el que he definit com a l'economia de la veritat.

This document summarizes the issues raised in a think-tank meeting held by professionals with expertise in pediatric Home Parenteral Nutrition. This nutritional technology enables patients to return home to their family and social environment, improves their quality of life and decreases health-care costs; however, it is complex and requires an experienced nutritional support team. Patient selection is normally made according to their underlying disease, the estimated duration of support and family and social characteristics. The patient''s family must agree to take on caregiver's responsibilities and should be able to perform treatment safely and effectively after receiving proper training from the nutritional support team. Close monitoring must be carried out to ensure tolerance and effectiveness of nutritional support, thereby avoiding complications. This nutritional treatment achieves, in most cases, recovery and intestinal adaptation in varying periods of time. In certain diseases, and when home parenteral nutrition becomes complicated, intestinal transplant may be recommendable, so referral to rehabilitation units and Intestinal Transplantation should be made early on.

BACKGROUND Cerebral oedema is associated with significant neurological damage in patients with traumatic brain injury. Bradykinin is an inflammatory mediator that may contribute to cerebral oedema by increasing the permeability of the blood-brain barrier. We evaluated the safety and effectiveness of the non-peptide bradykinin B2 receptor antagonist Anatibant in the treatment of patients with traumatic brain injury. During the course of the trial, funding was withdrawn by the sponsor. METHODS Adults with traumatic brain injury and a Glasgow Coma Scale score of 12 or less, who had a CT scan showing an intracranial abnormality consistent with trauma, and were within eight hours of their injury were randomly allocated to low, medium or high dose Anatibant or to placebo. Outcomes were Serious Adverse Events (SAE), mortality 15 days following injury and in-hospital morbidity assessed by the Glasgow Coma Scale (GCS), the Disability Rating Scale (DRS) and a modified version of the Oxford Handicap Scale (HIREOS). RESULTS 228 patients out of a planned sample size of 400 patients were randomised. The risk of experiencing one or more SAEs was 26.4% (43/163) in the combined Anatibant treated group, compared to 19.3% (11/57) in the placebo group (relative risk = 1.37; 95% CI 0.76 to 2.46). All cause mortality in the Anatibant treated group was 19% and in the placebo group 15.8% (relative risk 1.20, 95% CI 0.61 to 2.36). The mean GCS at discharge was 12.48 in the Anatibant treated group and 13.0 in the placebo group. Mean DRS was 11.18 Anatibant versus 9.73 placebo, and mean HIREOS was 3.94 Anatibant versus 3.54 placebo. The differences between the mean levels for GCS, DRS and HIREOS in the Anatibant and placebo groups, when adjusted for baseline GCS, showed a non-significant trend for worse outcomes in all three measures. CONCLUSION This trial did not reach the planned sample size of 400 patients and consequently, the study power to detect an increase in the risk of serious adverse events was reduced. This trial provides no reliable evidence of benefit or harm and a larger trial would be needed to establish safety and effectiveness. Introduction: The use of ultrasound in pediatric Primary Care improves both the quality and efficiency of care. Material and methods: This study indertook 250 ultrasounds over a 12 moth period from May 2009 to April 2010. Results: Te results found for each anatomical region were 45% abdominal, 25% cranial, 15% of the hip, 10% of parotid and thyroids and 5% of tender areas. From these, the most relevant pathology was Gastroesophageal Reflux, acoounting for 7.2% of all ultrasounds undertaken. Renal pyelectasia accounted for 1.6% cephalhematoma 1.6%, dysplasia of the hip 1.2%, acute parotitis 1.2%, adenopathy 1.2%, cervical angioma 0.4%, and abscessed adenopathy 0.4%. Conclusion: From this analysis, we conclude that ultrasound is a useful tool for the detection of abdominal and cranial pathologies and secondarily, for thyroid, hip and cervical pathologies.

Introduction: today, cultural changes may have changed the usual pattern of the mother as the accompanying person in the pediatric office. Objetives: descriptive analysis of the current situation in the pediatrics office. Material and methods: Las Delicias Health Center (health district of Malaga). Selected sample of 250 patients between the periods 1-15 July and 15-30 September, 2011. Results: in most cases (54.8%), the mother stands as the main accompanying person, appearing both parents in the consultation in 16.4% of cases. The figure of the grandparents is of special significance when the mother is an active worker. Most consultations were on-demand by appointment (82.4%), for acute symptoms, with symptoms lasting less than 3 days in most cases (59.2%). Discussion: the mother is presented as the main accompanying person in most cases. The father or grandparents will be present in front of social-cultural factors, mainly active employment status of the mother.

Objective: to determine the incidence of hand washing in parents as a preventive factor in the spread of viral respiratory infections in children under two years assigned to an urban pediatric basic health area affected by catarrhal symptoms during the influenza vaccination campaign. Material and methods: from 230 children under two years included in the basic health area attending the Well Child program, 51 consulted during the influenza vaccination campaign (October 2011) by catarrhal symptoms. Results: fifty-one children were included, 23 male and 28 female. From the clinical point of view, 33 cases were afebrile and only 18 had fever, runny nose 100%. The clinic diagnosis in 44 cases was nasopharyngitis, acute bronchitis in 5 cases and obstructive bronchiolitis in 2 cases. No family history in 19, however 32 cases had a family member with catarrhal symptoms. As for the performance of hand washing as a preventive measure, 34 cases said they did not comply with this measure, only 17 cases did, despite the recommendations. Conclusions: while recognizing the importance of hand washing as a preventive measure for respiratory infections and being included among the recommendations offered in the well child program, only 33% of the population of our study reported washing hands. This recommendation should also be included in consultations on demand during the influenza season to encourage compliance.

BACKGROUND Advanced heart failure (HF) is associated with high morbidity and mortality; it represents a major burden for the health system. Episodes of acute decompensation requiring frequent and prolonged hospitalizations account for most HF-related expenditure. Inotropic drugs are frequently used during hospitalization, but rarely in out-patients. The LAICA clinical trial aims to evaluate the effectiveness and safety of monthly levosimendan infusion in patients with advanced HF to reduce the incidence of hospital admissions for acute HF decompensation. METHODS The LAICA study is a multicenter, prospective, randomized, double-blind, placebo-controlled, parallel group trial. It aims to recruit 213 out-patients, randomized to receive either a 24-h infusion of levosimendan at 0.1 μg/kg/min dose, without a loading dose, every 30 days, or placebo. RESULTS The main objective is to assess the incidence of admission for acute HF worsening during 12 months. Secondarily, the trial will assess the effect of intermittent levosimendan on other variables, including the time in days from randomization to first admission for acute HF worsening, mortality and serious adverse events. CONCLUSIONS The LAICA trial results could allow confirmation of the usefulness of intermittent levosimendan infusion in reducing the rate of hospitalization for HF worsening in advanced HF outpatients.
