Les Inuits sont le plus petit groupe autochtone au Canada. Les femmes inuites présentent des risques beaucoup plus élevés d’issues de grossesse défavorables que leurs homologues non autochtones. Quelques études régionales font état d’une mortalité fœtale et infantile bien plus importante chez les Inuits canadiens par rapport aux populations non autochtones. Des facteurs de risque tant au niveau individuel que communautaire peuvent affecter les issues de grossesse inuites. Les relations entre les caractéristiques communautaires et les issues de grossesse inuites sont peu connues. La compréhension des effets des facteurs de risque au niveau communautaire peut être hautement importante pour le développement de programmes de promotion de la santé maternelle et infantile efficaces, destinés à améliorer les issues de grossesse dans les communautés inuites. Dans une étude de cohorte de naissance reposant sur les codes postaux et basée sur les fichiers jumelés des mortinaissances/naissances vivantes/mortalité infantile, pour toutes les naissances survenues au Québec de 1991 à 2000, nous avons évalué les effets des caractéristiques communautaires sur les issues de grossesse inuites. Lorsque cela est approprié et réalisable, des données sur les issues de grossesse d’un autre groupe autochtone majeur, les Premières Nations, sont aussi présentées. Nous avons tout d'abord évalué les disparités et les tendances temporelles dans les issues de grossesse et la mortalité infantile aux niveaux individuel et communautaire chez les Premières Nations et les Inuits par rapport à d'autres populations au Québec. Puis nous avons étudié les tendances temporelles dans les issues de grossesse pour les Inuits, les Premières Nations et les populations non autochtones dans les régions rurales et du nord du Québec. Les travaux concernant les différences entre milieu rural et urbain dans les issues de grossesse chez les peuples autochtones sont limités et contradictoires, c’est pourquoi nous avons examiné les issues de grossesse dans les groupes dont la langue maternelle des femmes est l’inuktitut, une langue les Premières Nations ou le français (langue majoritairement parlée au Québec), en fonction de la résidence rurale ou urbaine au Québec. Finalement, puisqu'il y avait un manque de données sur la sécurité des soins de maternité menés par des sages-femmes dans les communautés éloignées ou autochtones, nous avons examiné les issues de grossesse en fonction du principal type de fournisseur de soins au cours de l'accouchement dans deux groupes de communautés inuites éloignées. Nous avons trouvé d’importantes et persistantes disparités dans la mortalité fœtale et infantile parmi les Premières Nations et les Inuits comparativement à d'autres populations au Québec en se basant sur des évaluations au niveau individuel ou communautaire. Une hausse déconcertante de certains indicateurs de mortalité pour les naissances de femmes dont la langue maternelle est une langue des Premières Nations et l’inuktitut, et pour les femmes résidant dans des communautés peuplées principalement par des individus des Premières Nations et Inuits a été observée, ce qui contraste avec quelques améliorations pour les naissances de femmes dont la langue maternelle est une langue non autochtone et pour les femmes résidant dans des communautés principalement habitées par des personnes non autochtones en zone rurale ou dans le nord du Québec. La vie dans les régions urbaines n'est pas associée à de meilleures issues de grossesse pour les Inuits et les Premières Nations au Québec, malgré la couverture d'assurance maladie universelle. Les risques de mortalité périnatale étaient quelque peu, mais non significativement plus élevés dans les communautés de la Baie d'Hudson où les soins de maternité sont prodigués par des sages-femmes, en comparaison des communautés de la Baie d'Ungava où les soins de maternité sont dispensés par des médecins. Nos résultats sont peu concluants, bien que les résultats excluant les naissances extrêmement prématurées soient plus rassurants concernant la sécurité des soins de maternité dirigés par des sages-femmes dans les communautés autochtones éloignées. Nos résultats indiquent fortement le besoin d’améliorer les conditions socio-économiques, les soins périnataux et infantiles pour les Inuits et les peuples des Premières Nations, et ce quel que soit l’endroit où ils vivent (en zone éloignée au Nord, en milieu rural ou urbain). De nouvelles données de surveillance de routine sont nécessaires pour évaluer la sécurité et améliorer la qualité des soins de maternité fournis par les sages-femmes au Nunavik.

La période périnatale est particulièrement propice pour que les futures et nouvelles mères consommatrices de substances psychoactives et leur partenaire entrent en relation d’aide avec des intervenants médicaux et psychosociaux. Pour ces mères et ces pères, l’arrivée d’un poupon dans leur vie est une source de motivation pour entreprendre une démarche d’aide basée sur la confiance. Pourtant, plusieurs études soulignent différentes contraintes qui rendent difficile l’entrée en relation d’aide de ces parents avec des intervenants. Ce mémoire à donc pour objectif d’explorer, à partir du point de vue de mères consommatrices et de leur partenaire, la façon dont se vivent leurs expériences relationnelles avec des intervenants médicaux et psychosociaux au cours de la période périnatale, tout en identifiant des éléments qui ont influencé leurs rapports. Nous avons fait une analyse secondaire du discours de vingt mères consommatrices et de leur partenaire, recrutés au moment de la naissance dans le cadre d’une recherche de Morissette, Devault et Rondeau (2006-2010). Les résultats obtenus indiquent que les parents semblent généralement satisfaits de leurs relations avec les intervenants médicaux et psychosociaux qu’ils ont rencontrés à cette période, même s’ils rapportent vivre des peurs importantes qui nuisent à la relation d’aide. Nos résultats révèlent également plusieurs éléments qui influencent les relations relevant autant des contextes d’intervention que des intervenants eux-mêmes et de leurs rôles. Nous constatons d’après les résultats qu’en dépit de certains aspects qui nuisent aux relations avec des intervenants, le cumul des éléments qui les favorisent semble susciter une satisfaction des mères consommatrices et de leur partenaire. Dans les situations où plusieurs éléments nuisent à la relation d’aide, il se créer une dynamique de méfiance réciproque entre ces parents et les intervenants. Néanmoins, comme les relations ne sont pas immuables, il est possible de transformer les relations difficiles pour les rendre plus satisfaisantes pour les mères consommatrices, leur partenaire et les intervenants médicaux et psychosociaux.

Le rôle que jouent les services orthophoniques dans l’ajustement des proches de personnes aphasiques ainsi que le contexte dans lequel les orthophonistes mettent en place des interventions auprès de ces proches ne sont actuellement pas connus. La présente thèse a donc pour but de comprendre de quelle manière les relations entre les orthophonistes et les proches de personnes aphasiques, développées en contexte de réadaptation, s’inscrivent dans la trajectoire dynamique des proches et dans la pratique des orthophonistes. Une approche qualitative par théorisation ancrée a été utilisée dans quatre études pour analyser les entrevues effectuées auprès de proches de personnes aphasiques et d’orthophonistes. Dans l’étude 1, les entrevues menées à trois reprises dans la première année suivant l’accident vasculaire-cérébral (AVC), et ce, auprès de quatre filles dont la mère est aphasique, ont été analysées. Un modèle théorique représentant la relation mère-fille a été développé. Ce modèle illustre que les perceptions de fragilité, de difficultés et de compétence, qu’ont les filles à l’égard de leur mère, les amènent à adopter des comportements de protection ou de confiance, ce qui génère des réactions de satisfaction ou d’insatisfaction chez la mère, renforçant alors les perceptions initiales des filles. Quatre patterns relationnels peuvent donc coexister au sein d’une même dyade. L’aphasie complexifierait cet ajustement relationnel. Dans l’étude 2, les entrevues effectuées à trois reprises durant la première année suivant l’AVC, auprès d’une fille dont la mère est sévèrement aphasique, ont été analysées. Un modèle théorique représentant l’expérience d’aider a été élaboré. Selon ce modèle, percevoir des difficultés chez sa mère et ressentir que leur relation antérieure est menacée a déclenché le processus d’aide chez la fille. Parallèlement, la reconnaissance de la compétence de sa mère a motivé la fille à offrir de l’aide visant à rendre sa mère heureuse et à favoriser son indépendance. Ce type d’aide a contribué à augmenter l’indépendance de sa mère, à retrouver une relation satisfaisante avec celle-ci et à s’adapter à l’aphasie. Dans l’étude 3, les entrevues de 12 proches de personnes aphasiques ont été analysées. Un modèle théorique représentant l’expérience de l’aphasie et de la réadaptation post-AVC a été développé et illustre que les proches sont centrés sur la personne aphasique et participent à la réadaptation dans le rôle d’aidant. Cette disposition influence alors leurs attentes envers la réadaptation, leurs interactions avec les professionnels, dont les orthophonistes, et leur appréciation de la réadaptation. Dans l’étude 4, les entrevues effectuées auprès de huit orthophonistes travaillant en réadaptation ont été analysées. Un modèle théorique illustrant le processus d’intervention des orthophonistes auprès des proches de personnes aphasiques a été construit. Pour les orthophonistes, le travail avec les proches est majoritairement perçu comme un ajout positif, mais exigeant, à leur pratique de base centrée sur la personne aphasique. Une satisfaction professionnelle peut en découler, mais des idéaux non-atteints peuvent persister. La relation proche-orthophoniste serait donc principalement axée sur le rôle d’aidant que joue le proche, et ce, en raison de leur expérience respective. Un agrandissement du territoire de rencontre entre les orthophonistes et les proches pourrait soutenir les proches dans les ajustements relationnels induits par l’AVC avec aphasie ainsi que permettre aux orthophonistes d’atteindre leurs idéaux.

The aim of this study was to assess which behavioral characteristics of the newborn infant are associated with an increased risk of postnatal depression (PND) in the mother. A total of 497 mothers from a prospective cohort study were recruited during the last trimester of pregnancy. Infants were evaluated at 3 days with the Brazelton Neonatal Behavioral Scale. Maternal PND was assessed at 6 weeks postpartum with the Edinburgh Postnatal Depression Scale. Behavioral characteristics of the infant predicted the occurrence of PND, independent of other risk factors for PND: The lower the infants' orientation performance, the higher the risk that the mother would present with PND 6 weeks after delivery. As orientation capacities play a key role in the interactional skills developed between mothers and their infants, an infant who is difficult to engage in interaction may contribute to the risk of PND.

The present study is an interpretation of the two myths copied in the Old Babylonian period in which the Sumerian mother goddess is one of the main actors. The first myth is commonly called “Enki and Ninḫursaĝa”, and the second “Enki and Ninmaḫ”. The theoretical point of departure is that myths have society as their referents, i.e. they are “talking about” society, and that this is done in an ideological way. This study aims at investigating on the one hand which contexts in the Mesopotamian society each section of the myths refers to, and on the other hand which ideological aspects that the myths express in terms of power relations. The myths are contextualized in relation to their historical and social setting. If the myth for example deals with working men, male work in the area during the relevant period is discussed. The same method of contextualization is used regarding marriage, geographical points of reference and so on. Also constellations of mythical ideas are contextualized, through comparison with similar constellations in other Mesopotamian myths. Besides the method of contextualization, the power relations in the myths are investigated. According to this latter method, the categories at issue, their ranking, as well as their changed ranking, are noted. The topics of the myths are issues important for the kingship and the country, such as irrigation, trade, health and healing, birth, collective work, artisanry and rivalry. All these aspects are used in order to express what the power relations between the goddess Ninḫursaĝa/Ninmaḫ and the god Enki look like. The relations are negotiated and recalibrated, which leads to the goddess getting a lowered status. Part of the negotiations and recalibrations is gender behavior, which is related to historical developments in society. The present work points to the function of these myths as tools of recalibrating not only deities, but also men and women in society.

Introduction: Studies have shown that having a preterm infant may cause stress and powerlessness for parents. It is important to support parents around the feeding situation, and that the Neonatal Intensive Care Unit (NICU) has appropriate space and place to help the family to bond to each other. For the healthcare professionals it is important to promote skin-to-skin contact and breastfeeding; particularly for preterm infants. There are many studies on parent’s experiences of NICUs and a few studies on parent’s experiences of feeding their infant in the NICU. Objective: The objective of this study was to explore parents experiences of feeding their infant in the NICU. Design: The study was conducted using an ethnographic design. Results: A global theme of ‘The journey in feeding’ was developed from four organising themes: ‘Ways of infant feeding’; ‘Environmental influences’; ‘Relationships’ and ‘Emotional factors’. These themes illustrate the challenges mothers reported with different methods of feeding. The environment had a big impact on parent’s experiences of infant feeding. Some mothers felt that breastfeeding seemed unnatural because their infant was so tiny but breastfeeding and skin-to-skin contact helped them to bond to their infant. The mothers thought it was difficult to keep up with the milk production by only pumping. Routines were not inviting parents to find their own rhythm. They also felt stressed about the weighing. Healthcare professionals had positive and negative influences on the parents. Conclusions: This study demonstrates that while all parents expressed the wish to breastfeed, their ‘journey in feeding’ was highly influenced by method of feeding, environmental, relational and emotional factors. The general focus upon routines and assessing milk intake generated anxiety and reduced relationality. Midwives and neonatal nurses need to ensure that they emphasise and support the relational aspects of parenting and avoid over-emphasising milk intake and associated progress of the infant

Death due to childhood cancer reflects an early outcome of life, which can cause a strong repercussion in the mother s existence - figure to whom the greatest part of responsibilities during the child s illness is commonly allocated. The aim of this study is to understand the experience of mothers who have lost a kid as a consequence of childhood cancer, approaching the personal senses of this fact. Following a qualitative research design, with an exploratory and comprehensive approach, the study used the narrative method, which was obtained from a semi-structured interview, as the data generation procedure. The research counted on the participation of three adult mothers who had lost their kids because of childhood cancer, after - at least - a six-month period of oncologic treatment. The proposal of analysis follows the parameters of the phenomenological method and the data are based on Martin Heidegger s existential analytic. The results were structured into three thematic axes: previous History, child illness and its repercussions; The network of support and care; Loss and after loss: facing and signifying. It was possible to comprehend that the emergence of cancer in childhood promotes, since the diagnosis, a disruption of everyday meanings, accentuating the fragile condition of human life. In this specific circumstance of childhood illness, all the participants restricted their possibilities of being-in-the-world, dedicating exclusively to the practice of maternity. Concerning their relationship with their children in treatment, the narratives unveiled, in a convergent manner, the existence of care in a substitutive mode. In the network of support - primarily constituted by family, the health team and the support institutions - the relations were marked by proximity and detachment movements. With the child s death, mothers began to live a way of being-with the absent child , ensuring the continuity of the relationship with the dead infant. From the results exposed above, we can understand the motherly mourn as a singular experience in constant resignification, in which the subjective time overlaps the cronological time. The increment of anguish, resulting from the mother s confrontation to the question of finitude, mobilizes a process of change in their way of being-in-the-world, promoting an openness to new possibilities in their lives. Singular attention to the mother, during the process of illness and child loss, turns out to be fundamental
Social support and infant malnutrition: a case-control study in an urban area of Southeastern Brazil

The relationship between malnutrition and social support was first suggested in the mid-1990s. Despite its plausibility, no empirical studies aimed at obtaining evidence of this association could be located. The goal of the present study was to investigate such evidence. A case-control study was carried out including 101 malnourished children (weight-for-age National Center for Health Statistics/WHO 5th percentile) aged 12-23 months, who were compared with 200 well-nourished children with regard to exposure to a series of factors related to their social support system. Univariate and multiple logistic regressions were carried out, odds ratios being adjusted for per capita family income, mother's schooling, and number of children. The presence of an interaction between income and social support variables was also tested. Absence of a partner living with the mother increased risk of malnutrition (odds ratio 2.4 (95 % CI 1.19, 4.89)), even after adjustment for per capita family income, mother's schooling, and number of children. The lack of economic support during adverse situations accounted for a very high risk of malnutrition (odds ratio 10.1 (95 % CI 3.48, 29.13)) among low-income children, but had no effect on children of higher-income families. Results indicate that receiving economic support is an efficient risk modulator for malnutrition among low-income children. In addition, it was shown that the absence of a partner living with the mother is an important risk factor for malnutrition, with an effect independent from per capita family income, mother's schooling, and number of children.

Objectives: To investigate the relationships between socio-demographic factors, infant feeding habits, oral hygiene and the prevalence and patterns of caries in Brazilian 3-4-year-olds. Methods: Systematic random sampling was used to select children enrolled in municipal nurseries in Araraquara, Brazil, in 1998. Clinical examinations were carried out by one examiner using dmft and dmfs indices and WHO criteria. Questionnaires for information related to the socio-demographic background, oral hygiene and dietary history of the children were completed by their mothers. Results: Caries was seen in 46% of the children; 17% of them had the more extensive pattern involving molars and incisors. Social class, mother's education, and age at which breast-feeding terminated showed statistically significant associations with caries. Feeding bottles with added sugars were still being given to 80% of the children. When the significant variables were taken into account only age at which breast-feeding terminated showed a significant relationship to the pattern of disease. Children who were never breast-fed or were breast-fed beyond the age of 24 months had a higher prevalence of the more extensive pattern of caries. Conclusions: The association between the length of time a mother breast-feeds and extensive caries should be a consideration in any local infant feeding policies or health promotion strategies. The duration for appropriate breast- or bottle-feeding should be emphasised.

The aim of the present study was to determine the fluoride concentration in some brands of mother's milk substitutes and evaluate the possibility of developing dental fluorosis by consuming these products. The products, all powdered, were divided into 3 groups: infant formulae (group I, n = 7), milk-based (group M, n = 8) and soy-based (group 8, n = 3). Samples from 3 cans of different batches of each brand were reconstituted in deionized water and analyzed using the specific electrode method, after hexamethyldisiloxane (HMDS) facilitated diffusion. The fluoride content (mg F/L) of the products ranged from 0.044 to 0.326 (I), 0.014 to 0.045 (M) and 0.253 to 0.702 (S). There was significant difference in the fluoride content of cans from distinct batches (p < 0.05) in most of the brands. The reconstitution of all products in water with optimal fluoride concentration for consumption during the mineralization phase of the primary teeth could result in daily fluoride intake above 0.07 mg F/kg body weight/day. Therefore, the consumption of these products, especially when reconstituted with optimally fluoridated water, could increase the risk of developing dental fluorosis.

A teoria do apego tem considerado a relação mãe-criança como um determinante do desenvolvimento, sendo que sua qualidade tem sido relacionada com a sensibilidade do cuidador, e conseqüentemente com a qualidade das relações com seus próprios cuidadores. Recentemente, tem se relacionado a sensibilidade materna com vários fatores, incluindo classe social e educação. Este trabalho teve como objetivo investigar as variáveis que influenciam a sensibilidade materna na situação de banho. Foram filmados 60 banhos dados por mães de classe baixa e classe média. As díades foram constituídas por mães que tinham entre 18 a 40 anos de idade, e por crianças de zero a um ano. Encontraram-se menores freqüências de comportamentos sensíveis entre mães de classe baixa do que entre mães de classe média, que possuíam mais escolaridade, mais idade e tinham com quem dividir os cuidados infantis. Estes resultados sugerem que a sensibilidade materna é um fenômeno relacionado com variáveis socioculturais.

This study focused on the risk factors in mother-child relationship that predispose babies to the development of dental caries. A prospective cohort study with 80 mother-child pairs was conducted. The mothers responded at 12, 18 and 30 months after their children's birth, to questions about variables related to diet, sucking habits, and oral care. Children were clinically examined to verify caries lesions (white spot lesions or cavitation). Data were analysed using Chi squared or Fisher's exact tests. The significance level was set at 5 %. Of the total, 3.75 % showed cavitated lesions after 18 months; 6.25 and 45 % had spot white lesions, respectively, at 18 and 30 months. The cariogenic diet was high at 12 (63.75 %) and 30 (88.75 %) months. Good oral hygiene was present in a minority of children at 12 months (46.25 %), but increased at 30 months (65 %), helping to prevent cavities and white spot lesions over this period (p = 0.0005). The variables of the blocks sucking habits and diet were not associated with caries. The lack of oral care in children was a risk factor for dental caries development.

Introduction - The Centro de Orientacao ao Adolescente of Campinas (Southeastern Brazil) maintains a program to qualify economically disadvantaged adolescent students aged 15 to 18 years to enter the labor market. Objective - To understand life projects of teenagers who became mothers while participating in the program, in the period from 2003 to 2008, aiming to find the place of professional life in their life trajectory before and after motherhood. Method - Eight young mothers were interviewed, and a qualitative methodology was applied to the analysis of the interviews. Results - The trajectories of study and work were discontinued or adapted due to motherhood. Four young mothers completed high school and none had entered university. Three did not return to work and the rest had diverse work experiences. The reported difficulties to enter the labor market were: inadequate instruments to support the children's care, low income, lack of work experience, presence of small children and little educational background. Final Considerations - Teenage motherhood did not indicate the exclusion of educational or work projects but indicated adjustments and the need for a family and social support network. It was noted the need for public policies targeted at the inclusion of youths in the labor market and at support services such as nurseries. Also, the need for a change in gender relations was demonstrated, with greater equality of rights as a precondition for the inclusion of women, especially those who are mothers, in the labor market.

Objective: To evaluate cases of mother-to-child transmission of HIV-1 at multiple sites in Latin America and the Caribbean in terms of missed opportunities for prevention. Methods: Pregnant women infected with HIV-1 were eligible for inclusion if they were enrolled in either the NISDI Perinatal or LILAC protocols by October 20, 2009, and had delivered a live infant with known HIV-1 infection status after March 1, 2006. Results: Of 711 eligible mothers, 10 delivered infants infected with HIV-1. The transmission rate was 1.4% (95% CI, 0.7-2.6). Timing of transmission was in utero or intrapartum (n = 5), intrapartum (n = 2), intrapartum or early postnatal (n = 1), and unknown (n = 2). Possible missed opportunities for prevention included poor control of maternal viral load during pregnancy; late initiation of antiretrovirals during pregnancy; lack of cesarean delivery before labor and before rupture of membranes; late diagnosis of HIV-1 infection; lack of intrapartum antiretrovirals; and incomplete avoidance of breastfeeding. Conclusion: Early knowledge of HIV-1 infection status (ideally before or in early pregnancy) would aid timely initiation of antiretroviral treatment and strategies designed to prevent mother-to-child transmission. Use of antiretrovirals must be appropriately monitored in terms of adherence and drug resistance. If feasible, breastfeeding should be completely avoided. OBJECTIVE: To understand the typical actions of the mother during the pregnancy of her teenage daughter. METHODS: Qualitative study, based on the theoretical-methodological framework of social phenomenology of Alfred Schütz. The data were collected in 2009, and the subjects were nine mothers of adolescent primigravidae. RESULTS: The mother of the pregnant adolescent is typified as one that reacts with surprise and disappointment to being notified of the pregnancy and who, subsequently, conforms to the new reality. In reflecting on her own experience of an adolescent mother, she has expectations to support her daughter during the pregnancy and to offer support, so that the course of her life is not impaired as a result of pregnancy. CONCLUSION: Considering the experience and expectations of the mother of the pregnant adolescent, this study could give subsidies to the planning and execution of the care for this binomial, decreasing the distance between the demands made by it and the practice of health professionals.
