PURPOSE: To evaluate an experimental animal model to study the abdominal tissue activity considering its interaction with a polypropylene mesh, through the use of one of the optical phenomena of light Laser, the biospeckle. METHODS: Fifty Wistar male rats were divided into four groups: Group 1: ten animals not submitted to surgery; Group 2: ten animals submitted to surgery without polypropylene mesh; Group 3: 20 animals submitted to surgery followed by the mesh placement; Group 4: (sham) with ten animals. None of the animals presented post surgical complications being submitted to the optical tests at the 20th postoperative day. RESULTS: The analysis from the biospeckle tests, comparing the medians and standard deviations with T Student test, indicated that no significative difference was observed on the abdominal wall tissue activity in the four groups considered, with and without polypropylene mesh prosthesis implantation. CONCLUSION: The animal model is viable and the biospeckle open ways for a great number of experiments to be developed in evaluating tissue activity.

Objective: The aim of this study was to investigate the efficacy of an infrared GaAlAs laser operating with a wavelength of 830 nm in the postsurgical scarring process after inguinal-hernia surgery. Background: Low-level laser therapy (LLLT) has been shown to be beneficial in the tissue-repair process, as previously demonstrated in tissue culture and animal experiments. However, there is lack of studies on the effects of LLLT on postsurgical scarring of incisions in humans using an infrared 830-nm GaAlAs laser. Method: Twenty-eight patients who underwent surgery for inguinal hernias were randomly divided into an experimental group (G1) and a control group (G2). G1 received LLLT, with the first application performed 24 h after surgery and then on days 3, 5, and 7. The incisions were irradiated with an 830-nm diode laser operating with a continuous power output of 40 mW, a spot-size aperture of 0.08 cm(2) for 26 s, energy per point of 1.04 J, and an energy density of 13 J/cm(2). Ten points per scar were irradiated. Six months after surgery, both groups were reevaluated using the Vancouver Scar Scale (VSS), the Visual Analog Scale, and measurement of the scar thickness. Results: G1 showed significantly better results in the VSS totals (2.14 +/- 1.51) compared with G2 (4.85 +/- 1.87); in the thickness measurements (0.11 cm) compared with G2 (0.19 cm); and in the malleability (0.14) compared with G2 (1.07). The pain score was also around 50% higher in G2. Conclusion: Infra-red LLLT (830 nm) applied after inguinal-hernia surgery was effective in preventing the formation of keloids. In addition, LLLT resulted in better scar appearance and quality 6 mo postsurgery.

Background: We aimed to compare plasma concentrations of carbon dioxide (CO(2)) in dogs that underwent intra- and preperitoneal CO(2) insufflation. Materials and Methods: Thirty dogs were studied. Ten formed a control group, 10 underwent intraperitoneal CO(2) insufflation, and 10 underwent preperitoneal CO(2) insufflation. General anesthesia with controlled ventilation was standardized for all dogs. After stabilizing the anesthesia, blood samples were collected at predetermined times and were sent for immediate gasometric analysis. Analysis of variance was used for comparing variables. Results: The plasma CO(2) concentration in the intraperitoneal insufflation group increased significantly more than in the preperitoneal insufflation group and was significantly greater than in the control group (P < 0.05). The pH values in the intraperitoneal group were lower than in the preperitoneal group (P < 0.05). Conclusion: The data from this study suggest that a greater plasma concentration of CO(2) is achieved by insufflation at constant pressure into the intraperitoneal space than into the preperitoneal space.

Obstruction of the fetal trachea is a potent stimulus for fetal lung growth and may have therapeutic potential in human fetuses with lung hypoplasia. However, the effects of increased lung expansion on lung development near midgestation, which is the preferred timing for fetal intervention, have not been well studied. Our aim was to determine the effects of increased lung expansion on lung development at 75-90 d of gestation in fetal sheep. In three groups of fetuses (n = 4 for each), the trachea was occluded for either 10 [10-d tracheal occlusion (TO) group] or 15 d (15-d TO group) or left intact (control fetuses). TO for both 10 and 15 d caused fetal hydrops, resulting in significantly increased fetal body weights. Both periods of TO significantly increased total lung DNA contents from 99.8 +/- 10.1 to 246.0 +/- 5.3 and 246.9 +/- 48.7 mg in 10- and 15-d TO fetuses, respectively. TO for 10 and 15 d also increased airspace diameter, although the percentage of lung occupied by airspace was not increased in 10-d TO fetuses due to large increases in interairway distances; this resulted from a large increase in mesenchymal tissue. Our aim was to determine whether antenatal corticosteroids improve perinatal adaptation of the pulmonary circulation in lambs with lung hypoplasia (LH). LH was induced in 12 ovine fetuses between 105 and 140 days gestation (term similar to 147 days); in 6 of these the ewe was given a single dose of betamethasone (11.4 mg im) 24 hr before delivery (LH + B). All lambs, including a control group (n = 6), were delivered at similar to 140 days and ventilated for 2 hr during which arterial pressures, pulmonary blood flow (PBF), and ventilating pressure and flow were recorded. During ventilation, respiratory system compliance was lower in both LH + B and LH groups than in controls. Pulmonary vascular resistance (PVR) was lower in LH + B lambs than in LH lambs and similar to controls; PBF was reduced in LH lambs but was restored to control levels by betamethasone. The mean density of small arteries of LH + B lambs was similar to that of LH lambs (P = 0.06) and lower than in controls; the thickness of the media of small pulmonary arteries from LH + B lambs was similar to that in LH lambs and thicker than in controls. VEGF mRNA levels were not different between groups. PDGF mRNA levels in LH + B lambs were higher than in LH lambs; a similar trend (P = 0.06) was seen for PECAM-1. SP-C mRNA levels were greater in both LH and LH + B lambs than in controls. Effects of betamethasone were greater on indices of pulmonary circulation than ventilation. We conclude that a single dose of maternal betamethasone 24 hr prior to birth has significant favorable effects on the postnatal adaptation of the pulmonary circulation in lambs with LH.

A 74 year old patient, EW, with dorsolateral frontal cortical compression due to hyperostosis frontalis interna, in the absence of the Morgagni or Stewart-Morel syndromes, is described. In addition to conventional neuropsychological measures EW was administered one nonspatial and two spatial self ordered working memory tasks, as well as a standard measure of fluid intelligence or g. She showed impaired performance on all three self ordered working memory tasks compared with a normal control group of 10 subjects matched for age, education, sex, and IQ. By contrast, her performance on the fluid intelligence test was comparable with that of the controls. It is concluded that the compression of dorsolateral frontal cortex accompanying hyperostosis frontalis interna may produce selective cognitive impairment.

Background. Abdominal hernias are a common disease among cirrhotic patients, because of malnutrition and persistently high intra-abdominal pressure due to ascites. When tense ascites is present, life-threatening complications are likely to occur. In such cases, the morbidity and mortality rates are high. Objective. We describe 3 cirrhotic patients with rare complicated hernias that needed surgical repair. We discuss optimal timing for surgical approaches and the necessity of ascites control before surgery, as well as the technical details of the procedures. Method. Review of hospital charts of selected rare cases of herniae in cirrhotic patients. Conclusion. Elective surgical approaches can treat even uncommon hernias in cirrhotic patients with good results.

We investigated the effect of transcutaneous electrical nerve stimulation (TENS) for inguinal herniorrhaphy postoperative pain control in a prospective, randomized, double-blinded, placebo-controlled study. Forty patients undergoing unilateral inguinal herniorrhaphy with an epidural anesthetic technique were randomly allocated to receive either active TENS or placebo TENS. Postoperative pain was evaluated using a standard 10-point numeric rating scale (NRS). Analgesic requirements were also recorded. TENS (100 Hz, strong but comfortable sensory intensity) was applied for 30 minutes through 4 electrodes placed around the incision twice, 2 and 4 hours after surgery. Pain was assessed before and after each application of TENS and 8 and 24 hours after surgery. In the group treated with active TENS, pain intensity was significantly lower 2 hours (P = .028), 4 hours (P = .022), 8 hours (P = .006), and 24 hours (P = .001) after the surgery when compared with the group that received placebo TENS. Active TENS also decreased analgesic requirements in the postoperative period when compared with placebo TENS (P = .001). TENS is thus beneficial for postoperative pain relief, after inguinal herniorrhaphy; it has no observable side effects, and the pain-reducing effect continued for at least 24 hours. Consequently, the routine use of TENS after inguinal herniorrhaphy is recommended. Perspective: This study presents the hypoalgesic effect of high-frequency TENS for postoperative pain after inguinal herniorrhaphy. This may reinforce findings from basic science showing an opioid-like effect provided by TENS, given that high-frequency TENS has been shown to activate delta-opioid receptors. Introdução: neste estudo foi avaliado um caso de lombociatalgia com hérnia discal lombar através de exame clínico confirmado por ressonância magnética. Foi efectuado um tratamento com terapia manual durante 4 semanas até à abolição dos sintomas e recuperação total da função. Objectivos: realizar um tratamento de terapia manual num paciente com lombociatalgia com hérnia discal lombar num período de 1 mês. Métodos: foi realizado um estudo num paciente de 40 anos do sexo masculino que apresentava um quadro de lombociatalgia com hérnia discal lombar. Para a avaliação foram utilizados testes palpatórios com movimento (teste de Mitchell lombar, teste de Gillet sacroilíaco); observação de assimetrias posturais; teste de mobilidade activa em pé; EVA para avaliação da dor; testes de compressão lombar na posição de sentado; testes de condução neurológica; testes neurodinâmicos. Os testes foram aplicados no início e final das 4 primeiras sessões de tratamento com terapia manual, com intervalo de uma semana e um follow-up realizado uma semana após a 4ª sessão. Resultados: após o tratamento de 4 sessões, verificou-se a ausência de postura antálgica. A dor diminuiu de 5/10 para 0/10 (EVA) nos movimentos de flexão, extensão, rotação esquerda e inclinação esquerda do segmento lombar, em pé, com amplitudes normais de mobilidade. A dor diminuiu 3/10 para 0/10 (EVA) no teste de compressão lombar sentado e de 6/10 para 0/10 (EVA) para o teste de compressão lombar sentado com inclinação esquerda. O teste de Mitchell lombar inicial (FRS L5; NSR lombar) ficou negativo assim como o teste de Gillet sacroilíaco; teste de SLR ; SLR + neck flection ; testes de condução de L5 que se encontrava positivo com hipostesia no dermátomo de L5 e grau 3+ de força nos dorsiflexores à esquerda ficou negativo para a hipostesia e grau 5 de força nos dorsiflexores. Conclusão: neste caso o conjunto de técnicas de terapia manual aplicadas juntamente com exercícios terapêuticos, concelhos posturais e de estilos de vida ajudaram o paciente a recuperar totalmente a função e ver abolida a dor.

As anomalias congénitas do diafragma (ACD) dão habitualmente sintomatologia no período neonatal; embora 5 a 30% se posssam manter sob uma forma latente e virem a ter uma apresentação tardia. Os A.A. efectuam uma revisão retrospectiva das ACD internadas na UCIP do H.D.E. num período de 41 meses (4 hérnias de Bochdalek, 2 eventrações, 1 hérnia transhiatal e 1 hérnia de Morgagni). Em nenhum dos casos houve sintomatologia neonatal sugestiva, tendo a idade media do diagnóstico sido de 6.6 meses (minimo 1 mes - maximo 18 meses). Em 75% dos casos (6) havia sintomatologia prévia recorrente inespecífica (3 do aparelho respiratório, 2 do digestivo e 1 de ambos), tendo 3 crianças (37.5%) má evolução ponderal. Todos foram sujeitos a cirurgia, com boa evolução em 7 casos (87.5%). Verificou-se 1 óbito (a criança mais jovem), caso em que a uma eventração gigante se associava uma hipoplasia pulmonar grave.

A hérnia de Spiegel (HS) é uma entidade clínica infrequente, sendo particularmente rara na criançaa. Embora possa associar-se a fatores adquiridos, na criança a maioria dos casos parece dever-se a alterações congénitas. A HS associa-se muitas vezes com outros defeitos congénitos, sendo a associação mais frequente a criptroquidia ipsilateral, observando-se com elevada frequência a presença do testículo no saco herniário e não se identificando gubernáculo ou canal inguinal. Dado o risco de encarceramento e estrangulamento do conteúdo da HS, o seu diagnóstico precoce e tratamento atempado são essenciais. O tratamento consiste na herniorrafia e orquidopexia extra-dartos, que deverá ser realizada através de um túnel subcutâneo caso não se identifique canal inguinal. A frequência entre HS e criptorquidia homolateral sugere que a normal presença do testículo seja sempre pesquisada aquando do diagnóstico de HS, e que esta associação deve ser tida em consideração em casos de criptorquidia com testículo não palpável no seu habitual trajeto. Neste artigo descrevemos um caso clínico de um lactente com HS associada a criptorquidia, que foi submetido a herniorrafia e orquidopexia extradartos com criação de um trajeto inguinal para o testículo, abaixo do transverso do abdómen.

RESUMO: A operação de Nissen, por laparoscopia, é considerada a cirurgia antirefluxo mais adequada por ser a que melhor replica a fisiologia normal da válvula gastresofágica na maioria dos doentes com sintomas típicos de doença do refluxo gastresofágico (DRGE). São critérios técnicos o encerramento seguro dos pilares do diafragma e a criação de fundoplicatura completa (360 graus), curta (inferior a dois centímetros), lassa e sem tensão – desiderando para o qual a laqueação proximal dos vasos curtos gástricos é crucial. Realizei a operação de Nissen, por laparoscopia, em sessenta mulheres e quarenta homens com DRGE, sem mortalidade operatória, no Serviço de Cirurgia 6 do Hospital dos Capuchos, CHLC, EPE. Os cem doentes apresentavam média etária de 46 anos e queixas, com tempo de evolução entre 1 e 43 anos, de pirose (90%), regurgitação (80%), azia (73%), epigastralgias (54%). A endoscopia alta revelou esofagite de grau Savary-Miller 0-I (62%), II (23%), III (8%), IV (7%); hérnia de deslizamento (71%), hérnia paraesofágica (8%), sem hérnia (21%); a pHmetria de 24h diagnosticou padrão misto (38%), levantado (20%), deitado (20%), inconclusiva (22%) e a manometria diagnosticou EEI hipotónico (35%), peristálise esofágica normal (88%), hipomotilidade ligeira (5%) e foi omissa (7%). Hérnia hiatal, esofagite grave, ineficácia do controlo sintomático com inibidor da bomba de protões e desejo de descontinuidade terapêutica constituíram as indicações para tratamento cirúrgico. Por celioscopia, efetuei laqueação dos vasos curtos gástricos (70%), cruroplastia e fundoplicatura total (seda 2/0), curta (dimensão média 1,5-2 cm), lassa, sem tensão e sem calibração intraoperatória do esófago. A fundoplicatura de Nissen laparoscópica mostrou-se segura e eficaz no tratamento da DRGE. A sua idoneidade foi ainda comprovada pela normalização da pHmetria de 24 horas e da manometria pós-operatórias, com significado estatístico, num grupo de catorze voluntários assintomáticos. Em catamnese com recuo médio 30,7 meses 94% dos indivíduos persistem assintomáticos. Interrogando-me acerca das repercussões desta operação sobre a microcirculação do fundo gástrico coloquei, como premissa, a possibilidade de na operação de Nissen a laqueação dos vasos curtos poder induzir modificação no diâmetro arteriolar da parede do fundo gástrico. Para pesquisar a influência da laqueação dos vasos curtos gástricos e da fundoplicatura total sobre o calibre arteriolar da parede do estômago no cárdia, no fundo e na região dos vasos curtos gástricos, idealizei um Projeto de investigação experimental em cobaias. O Projeto foi desenvolvido no Centro de Investigação do Departamento de Anatomia da FCM-UNL. Para a sua realização obtive autorização da Comissão Científica e Pedagógica da FCM-UNL, requeri a acreditação como investigador à Direção Geral de Veterinária e, por recorrer à utilização de animais, submeti-o à Comissão de Ética da FCM-UNL, que o aprovou por unanimidade. Para limitar o número de animais utilizados ao mínimo necessário, calculei, por método estatístico, a quantidade de cobaias necessárias. Subdividindo-as num grupo de ensaio (GE), onde realizei a operação de Nissen, e num grupo de controlo (GC), onde apenas procedi a tração gástrica, defini e apliquei protocolos de anestesia, de cirurgia e de eutanásia, segundo os princípios dos 3R – Replacement, Reduction, Refinement da técnica de experimentação humana de Russell e Burch (1959) – uma estrutura ética amplamente aceite para a realização de experimentação científica humanizada com animais. A utilização das técnicas de estudo angiomorfológico permitiu-me analisar e descrever a anatomia normal, a vascularização arterial macroscópica, a microangioarquitetura, por microscopia eletrónica de varrimento de moldes de corrosão vascular, e a histologia da parede do estômago da cobaia. Procedi, também, à definição dos critérios morfológicos que considerei suscetíveis de validação deste modelo animal para o estudo proposto. Por razões académicas, foi necessário abreviar o Projeto encurtando, em cerca de dois anos, o prazo disponível para conclusão do estudo. Apreciando-o com o Gabinete de Análise Epidemiológica e Estatística do Centro de Investigação do CHLC, EPE, optou-se, perante a escassez de elementos após já terem sido recrutados 46 animais, por uma amostra, suplementar, de dimensão de conveniência de oito cobaias (quatro em cada grupo), condicionada pelo limite temporal universitário e pelo respeito pela dignidade dos animais. Neste subgrupo procedi, por microscopia eletrónica de varrimento, à medição dos calibres arteriolares nos moldes vasculares do cárdia, do fundo e da zona dos vasos curtos gástricos tanto no GC como no GE efetuando 469 medições no primeiro e 461 no último. Os dados foram enviados ao Centro de Investigação do CHLC, EPE que procedeu à sua análise estatística (ANOVA). A referida análise revelou que as arteríolas do plexo mucoso e as do plexo submucoso do cárdia, do fundo e da região dos vasos curtos gástricos, mostraram aumento de calibre no GE. O aumento foi, estatisticamente, significativo por ser superior a 50% do calibre do GC. Nos vasos curtos, a diferença foi mais pequena, mas persistiu sendo, estatisticamente, significativa. Os vasos retos dilataram na base, na sua emergência do plexo seroso, apenas no fundo gástrico. Na cobaia a operação de Nissen – fundoplicatura total com laqueação dos vasos curtos gástricos –, provocou vasodilatação arteriolar do fundo gástrico. Considero que essa vasodilatação constituiu acomodação à modificação introduzida e infiro que o mesmo possa acontecer no ser humano. Admito, assim, que também ocorra vasodilatação no ser humano, na sequência da laqueação dos vasos curtos gástricos, pela analogia microvascular entre as duas espécies e que essa vasodilatação corresponda, igualmente, a um mecanismo de adaptação arteriolar visando, por exemplo, suprir a perda incorrida pela laqueação. A associação experimental entre laqueação dos vasos curtos gástricos e realização de fundoplicatura total, que exerce aumento inerente de pressão sobre a JEG, não só não provocou défice da microcirculação do esófago distal ou do estômago proximal como desencadeou um mecanismo de vasodilatação fúndica que reforça o conceito de segurança da operação de Nissen para tratamento da DRGE. -------------- ABSTRACT: The laparoscopic Nissen operation is considered to be the most appropriate antirefluxsurgery because it suitably replicates the standard physiology of the gastroesophageal valve in most patients with typical symptoms of gastroesophageal reflux disease (GERD). The technical criteria includes the safe shutdown of the diafragmatic crura(cruroplasty) and the creation of a complete fundoplication (360 degrees), short (lesser than two inches), floppy and without tension – a goal for which the proximal ligation of the gastric short vessels is crucial. The laparoscopic Nissen operation was performed in sixty women and forty men with GERD, without any operative mortality, at the Surgical Department of the Hospital dos Capuchos, CHLC, EPE. The one hundred patients, averaged 46 years old, complained of heartburn (90%), regurgitation (80%) and upper abdominal pain (54 %). The endoscopy process revealed Savary-Miller esophagitis of grade 0-I (62%), II (23%), III (8%), IV (7%), sliding hernia (71%), paraesophageal hernia (8%) or no herniation (21%). The pHmetry/24h diagnosed mixed pattern (38%), raised (20%), lying (20%) or inconclusive (22%). The manometry diagnosed hypotensive LES (35%), normal esophageal peristalsis (88%), mild hypomotility (5%) and was absent (7%). Hiatal hernia, severe esophagitis, ineffective symptomatic control with proton pump inhibitor and request for treatment discontinuation were the signs for surgical action. A laparoscopic ligation of short gastric vessels (70%), cruroplasty and fundoplication (silk 2/0), short (average size 1.5–2 cm) and floppy, without tension and without intraoperative calibration of the esophagus were thus performed. The laparoscopic Nissen fundoplication behaved safe and effective in treating GERD. In a group of 14 asymptomatic volunteers its reputation was confirmed with statistical significance by normalization of postoperative pHmetry/24h and manometry. 94% of the individuals remained asymptomatic up to 30.7 months (average) in the follow-up. Interrogating myself about the impact of this operation on the microcirculation of the gastric fundus I put premised on the possibility of the ligation of the short gastric vessels in the Nissen procedure can induce changes in the arteriolar diameter in the Wall of the gastric fundus. To explore the influence of ligation of the short gastric vessels and the fundoplication at the arteriolar caliber of the cardia, the fundus and the region of the short vessels of the gastric wall, I designed a project of experimental research in guinea pigs with two interdependent components: one veterinary and another technical where I applied angiomorphological studies. The project was developed at the Research Centre of the Department of Anatomy FCMUNL. For its accomplishment I got permission from the Scientific and Pedagogical Committee of the FCM-UNL, I requested for accreditation as a researcher at the General Directorate of Veterinary and, by resorting to the use of animals I submitted it to the Ethics Committee of the FCM-UNL, which approved it unanimously. The guinea pigs were divided into two experimental groups: an experimental group (EG), in which the Nissen procedure was performed and a control group (CG) in which only a gastric traction was done. Protocols of anesthesia, surgery and euthanasia were applied according to the 3Rs – Replacement, Reduction, Refinement of the technique of human experimentation of Burch and Russell (1959) – a widely accepted ethical framework for conducting scientific experiments using animals humanely. Using histological and angiomorphological techniques, I performed the analysis and the description of the normal, macro and microvascular, anatomy of the guinea pig stomach and I defined the morphological criteria that I considered susceptible for validation of this animal model for the proposed study. By means of scanning electron microscopy I measured the arteriolar calibers of the vascular casts of the cardia, of the fundus and of the short gastric vessels in both CG and EG, making 469 measurements in the former and 461 in the latter. The data were sent to the Research Center of the CHLC which conducted the statistical analysis (ANOVA). The data were sent to the Centre for Research of the CHLC, EPE which proceeded to statistical analysis (ANOVA). This analysis revealed that the arterioles plexus of the mucosal and submucosal plexus of the cardia, fundus and region of the short gastric vessels, showed increased caliber in EG. The increase was statistically significant for being greater than 50% CG gauge. In the short gastric vessels, the difference was smaller, but persisted and statistically significant. Straight vessels were dilated at the base, on its emergence of the plexus serous only in the fundus. In the guinea pig, the Nissen procedure - complete fundoplication with ligation of the short gastric vessels - caused arteriolar vasodilation on the gastric fundus. I believe that this vasodilation constituted some accommodation to the modification introduced and infer that the same might happen in humans. I admit therefore that vasodilation also occurs in humans following the ligation of the short gastric vessels by microvascular analogy between the two species and that this vasodilation corresponds also to na adaptation mechanism arteriolar, for example, to compensate the loss incurred by ligation. The association of experimental ligation of the short gastric vessels with conducting complete fundoplication, which exerts increased pressure on the EGJ, not only did not cause a microcirculation deficit of the distal esophagus or proximal stomach as triggered a mechanism of fundic vasodilation which reinforces the security concept of the Nissen procedure for treatment of GERD.

RESUMO: O envelhecimento da população é um fenómeno à escala mundial, com tendência a aumentar, sendo cada vez mais os idosos e mais velhos. Esta propensão é notória nos utentes que recorrem aos cuidados de saúde. À medida que se envelhece maior vai sen-do a vulnerabilidade e riscos para a saúde, bem como a prevalência para doenças croni-cas, que se traduz num aumento de hospitalizações e consumo. As complicações ocorri-das em idosos aquando submetidos a intervenções cirúrgicas estão descritas na literatu-ra, havendo uma escassez de publicações nacionais relacionadas com o tema. O presente estudo teve por objectivo caracterizar idosos submetidos a correcção cirúrgi-ca de hérnias da parede abdominal, os procedimentos e as complicações ocorridas. O estudo, transversal e de natureza quantitativa, foi realizado no Centro Hospitalar de Lisboa Norte – Hospital Pulido Valente, e teve como critério de inclusão os indivíduos com 65 ou mais anos submetidos a correcção cirúrgica de hérnias da parede abdominal, realizadas em 2009-2010, que tiveram um período de recobro na Unidade de Cuidados Anestésicos Pós-operatórios. Para a obtenção dos dados, foram analisados os processos clínicos do Arquivo Clínico da Instituição e registada informação sobre variáveis de caracterização sócio demográfi-ca, de saúde pré, intra e pós-operatória imediata e descritivas da cirurgia. Foram avaliadas as situações de 164 indivíduos, 78.7% eram homens e 21.3% mulheres e a média de idades era de 72.4 anos e desvio padrão 5.46. Em termos de hábitos de Vida, foi possível constatar um predomínio de baixo consumo de tabaco (13.4%) e álcool (6.7%). O valor médio de co-morbilidades foi de 2.6 e desvio padrão de 1.34 sendo a maior prevalência de 3-4 co-morbilidades com 48.8%. Salienta-se que todas as mulheres da amostra apresentaram doenças. As patologias mais prevalentes são as do foro cardiovascular: Hipertensão Arterial (75.0%), Dislipidémia (35.4%) e Insuficiência Venosa Periférica (14.6%), de forma menos expressiva, mas igualmente relevante a Diabetes Mellitus (23.8%). Nos mais idosos [75 ou mais], verificou-se uma expressivi-dade Hipertrofia benigna da próstata (18.2%). Das cirurgias ocorridas predominou a correcção cirúrgica de Hérnia Inguinal.Das complicações identificadas prevaleceu a dor (97.6%). A hipotermia (34.8%), HTA (24.4%) e bradicárdia (14.6%) encontram-se entre as complicações mais determinantes. Verificaram-se diferenças com significado estatístico entre os dois grupos etários consi-derados nas complicações bradicárdia (p=0.021) e a saturação de oxigénio <90% (p = 0.006), hemorragia (p=0.036) e retenção urinária (p = 0.038). Neste estudo constata-se que a cirúrgica de hérnias da parede abdominal em idosos é uma realidade prevalente. Dos factores que possam estar associados a propensão para complicações pós-cirúrgicas, a idade e a presença de co-morbilidade controlada não revelaram ter influência. Também os tempos de cirurgia e de anestesia não revelaram influências no surgimento de complicações. A monitorização dos sinais vitais deve ser constante, desde o período pré-operatório imediato de forma a permitir aos profissionais de saúde identificar uma propensão para vulnerabilidade, com vantagem em ser apre-sentado sob a forma de score. Este manifestou diferenças significativas a nível dos dois grupos etários considerados (p=0.001). Este score, vai possibilitar sinalizar quais os idosos com maior susceptibilidade de ocorrência de complicações.------------------ABSTRACT:Population aging has become a worldwide phenomenon with tendency to increase. This is particularly evident due to the amount of elderly that attend the medical services. As one gets older, more vulnerable and more risks to one’s health. Also chronic diseas-es are more likely to exist which lead to more hospitalizations and increase of consuma-bles. The complications after a surgery in older people are well documented in international scientific studies, however national studies are still lacking. The present study aims to characterize elderly patients after abdominal hernia surgical correction, the procedures and complications detected. This transversal and quantitative nature like study, was held in the Centro Hospitalar Lisboa-Norte – Hospital Pulido Valente, and as inclusive criteria where patients with more than 65 or more years old, who undergone abdominal hernia surgical correction surgery in 2009-2010 and had the post-operation on the Unidade de Cuidados Anestésicos Pós-Operatorios. Data from the clinical files from the archive of the hospital where analyzed. Information was taken regarding social demographic variables; before, during and immediately after surgery variables and also descriptive of the surgery itself. Were evaluated 164 individuals, 78, 7% of which were men and 21, 3% women with an average age of 72, 4 years and standard deviation 5.46. Life habits analyzed detected low consumption of tobacco (13, 4%) and alcohol (6, 7%). The average of co-morblity was 2, 6 and standard deviation 1.34. The biggest prevalence of 3-4 co-morbilities was 48, 8%. Important to notice that, all women from the study presented illnesses. The illnesses more frequent were cardiovascular: High Blood Pressure (75%), Dyslipidemia (35, 4%), Deep Venous Insufficiency (14, 6%), less significant but also relevant, Diabetes Melli-tus (23, 8%). In the older (75 or more), was noticed Benign Prostatic (18, 2%). In the surgeries verified, the abdominal hernia correction was more frequent. From all thecomplications identified, pain was 97, 6%, Hypothermia 34, 8%, high Blood Pressure 24, 4% and Bradycardia 14, 6%, were higher prevalence. Was seen differences in the statistic meaning of the two groups in the Bradycardia com-plications (p=0.021), and the oxygen saturation < 90% (p=0.006), bleeding (p=0,036) and urine retention (p=0,038). In the present study it is evident that the abdominal hernia surgical correction in older population is a common procedure. Of all the factors that might be related with post-operatory complications, age and the presence of controlled co-morbility didn’t reveal relevant. Also, surgical times and duration of anesthesia didn´t seem to be related to complications incidence. Monitoring the vital signs must be constant since the immedi-ate post-operatory to ensure that the professionals can identify any possible vulnerabil-ity, presenting it in a score like way. The score has shown meaningful differences be-tween the two age groups analyzed (p=0,001). This score allows identifying which pa-tients are more likely to suffer complications.

The authors report the magnetic resonance imaging diagnostic features in two cases with respectively lumbar epidural hematoma and cavernous hemangioma of the lumbar epidural space. Enhanced MRI T1-weighted scans show a hyperintense signal rim surrounding the vascular lesion. Non-enhanced T2-weighted scans showed hyperintense signal.
