Briefing 10 - Lessons from experience This document, commissioned by Public Health England, and written by the UCL Institute of Health Equity, sets out 12 points to consider when taking action locally on the social determinants of health. It is intended as a source of information on approaches to consider when devising local programmes and strategies to reduce health inequalities. It complements the other briefings and evidence reviews in this series, which provide more detail on action on specific social determinant areas, such as employment and early years interventions, including information on impacts and cost effectiveness where available. The 12 steps are divided across three parts. The first part sets out four strategies that help prioritise action on health equity. The next steps are principles of effective action on the social determinants of the health, presented in the second part. Finally, the steps in part three outline ways of ensuring that measures to increase health equity are sustainable and have impact over the long term. The briefing is available to download above. This document is part of a series. Ireland's future economic growth and competitiveness will increasingly depend on the extent to which it can support high value knowledge based industries. Mathematics is essential for disciplines such as science, technology, engineering and finance, but it also promotes the ability to think rationally, analyse and solve problems, and process data clearly and accurately. In a globalised competitive economy it is important that Ireland moves beyond being “average” at mathematics towards the promotion of advanced levels of skills, creativity and innovation. We urgently need to improve attainment levels in mathematics generally and to encourage more students to take Higher Level Mathematics. In addition, mathematics is an essential life skill for citizenship and economic and social participation in an increasingly complex world.

The high level of protection elicited in rodents and primates by the radiation-attenuated schistosome vaccine gives hope that a human vaccine relying on equivalent mechanisms is feasible. In humans, a vaccine would be undoubtedly administered to previously or currently infected individuals. We have therefore used the olive baboon to investigate whether vaccine-induced immunity is compromised by a schistosome infection. We showed that neither a preceding infection, terminated by chemotherapy, nor an ongoing chronic infection affected the level of protection. Whilst IgM responses to vaccination or infection were short-lived, IgG responses rose with each successive exposure to the vaccine. Such a rise was obscured by responses to egg deposition in already-infected animals. In human trials it would be necessary to use indirect estimates of infection intensity to determine vaccine efficacy. Using worm burden as the definitive criterion, we demonstrated that the surrogate measures, fecal eggs, and circulating antigens, consistently overestimated protection. Regression analysis of the surrogate parameters on worm burden revealed that the principal reason for overestimation was the threshold sensitivity of the assays. If we extrapolate our findings to human schistosomiasis mansoni, it is clear that more sensitive indirect measures of infection intensity are required for future vaccine trials.

Triatomines have been important model organisms for behavioural research. Diverse reports about triatomine host search, pheromone communication in the sexual, shelter and alarm contexts, daily cycles of activity, refuge choice and behavioural plasticity have been published in the last two decades. In recent times, a variety of molecular genetics techniques has allowed researchers to investigate elaborate and complex questions about the genetic bases of the physiology of insects. This, together with the current characterisation of the genome sequence of Rhodnius prolixus allows the resurgence of this excellent insect physiology model in the omics era. In the present revision, we suggest that studying the molecular basis of behaviour and sensory ecology in triatomines will promote a deeper understanding of fundamental aspects of insect and, particularly, vector biology. This will allow uncovering unknown features of essential insect physiology questions for a hemimetabolous model organism, promoting more robust comparative studies of insect sensory function and cognition.

Plasmodium vivax infects human erythrocytes through a major pathway that requires interaction between an apical parasite protein, the Duffy binding protein (PvDBP) and its receptor on reticulocytes, the Duffy antigen/receptor for chemokines (DARC). The importance of the interaction between PvDBP (region II, DBPII) and DARC to P. vivax infection has motivated our malaria research group at Oswaldo Cruz Foundation (state of Minas Gerais, Brazil) to conduct a number of immunoepidemiological studies to characterise the naturally acquired immunity to PvDBP in populations living in the Amazon rainforest. In this review, we provide an update on the immunology and molecular epidemiology of PvDBP in the Brazilian Amazon - an area of markedly unstable malaria transmission - and compare it with data from other parts of Latin America, as well as Asia and Oceania.

The purpose of this paper is to reflect on the possibilities and challenges of Community Development Banks (CDBs) as an innovative method of socioeconomic management of microcredit for poor populations. To this end, we will discuss the case of Banco Palmas in Conjunto Palmeiras in the city of Fortaleza, in the northeastern state of Ceará, as an empirical case study. The analyses presented here are based on information obtained from Banco Palmas between late 2011 and early 2012. In addition, previous studies by other researchers on the bank and other studies on CDBs were important. The primary data collected at Banco Palmas came from documents made available by the bank, such as reports and mappings. The analyses describe some of the characteristics of the granting of microcredit and allow one to situate it in the universe of microfinance and solidarity finance. They also show the significant growth of local consumption, mostly through the use of the Palmas social currency. The Banco Palmas experience, aside from influencing national public policies of solidarity finance, initiated a CDBs network that encourages the replication of these experiences throughout the country.

Buts de la revue: Les Médulloblastomes sont des tumeurs rares chez l'adulte. Le traitement habituel comprend une radiothérapie de tout l'axe cranio-spinal avec ou sans chimiothérapie. Beaucoup d'efforts sont actuellement entrepris pour mieux comprendre la biologie tumorale, afin de mieux stratifier les patients en différents groupes à risques et de les traiter en fonction. Cette revue discute les nouveaux facteurs de risques cliniques et moléculaires qui peuvent aider à optimiser le traitement des patients adultes avec des médulloblastomes. Découvertes récentes: Jusqu'à présent les patients étaient divisés en groupes à bas risque ou à haut risque sur la base de facteurs cliniques (âge, maladie résiduelle après chirurgie, dissémination dans le système nerveux central et l'histologie). Cette classification devrait être complétée par des facteurs pronostics moléculaires. Le profilage de l'expression des gènes a permis d'identifier six sous-groupes moléculaires de médulloblastomes. Le WNT sous-groupe montre une activation des gènes de la voie de signalisation WNT/wingless avec des mutations fréquentes du gène CNNTB1, une perte du chromosome 6 et une accumulation de β-catenine nucléaire. Ce sous-groupe est rencontré le plus souvent chez les enfants avec des médulloblastomes avec une histologie classique. Ils ont un bon pronostic. Une activation de la voie de signalisation du sonic hedgehog montre des mutations fréquentes des gènes PTCH et SUFU, une perte du 9q et une positivité pour GLI1 et SFRP1 et est rencontré plus fréquemment chez les enfants de moins de 3 ans et chez les adultes. Ce sous-groupe est souvent associé à une histologie de type desmoplastique. D'autre sous-groupes sont moins bien délimités et présentent des caractéristiques qui se chevauchent. Cependant une amplification MYC/MYCN, un gain du 17p et une histologie de type grandes cellules/anaplasique sont des facteurs de mauvais pronostic. Résumé: Des nouveaux sous-groupes moléculaires vont dorénavant aider à mieux adapter les traitements aux différents groupes de risque et permettront à développer de nouvelles thérapies ciblées. Des études prospectives et si possibles randomisées devraient être effectuées comprenant une stratification dans des sous-groupes moléculaires, afin d'identifier au mieux le meilleur traitement pour chaque groupe à risque.
