Introduction: Postoperative endoscopic recurrence (PER) is the initial event after intestinal resection in Crohn’s disease (CD), and after a few years most patients present with progressive symptoms and complications related to the disease. The identification of risk factors for PER can help in the optimization of postoperative therapy and contribute to its prevention. Methods: Retrospective, longitudinal, multicenter, observational study involving patients with CD who underwent ileocolic resections. The patients were allocated into two groups according to the presence of PER and the variables of interest were analyzed to identify the associated factors for recurrence. Results: Eighty-five patients were included in the study. The mean period of the first postoperative colonoscopy was 12.8 (3–120) months and PER was observed in 28 patients (32.9%). There was no statistical difference in relation to gender, mean age, duration of CD, family history, previous intestinal resections, smoking, Montreal classification, blood transfusion, residual CD, surgical technique, postoperative complications, presence of granulomas at histology, specimen extension and use of postoperative biological therapy. The preoperative use of corticosteroids was the only variable that showed a significant difference between the groups in univariate analysis, being more common in patients with PER (42.8% vs. 21%; p = 0.044). Conclusions: PER was observed in 32.9% of the patients. The preoperative use of corticosteroids was the only risk factor associated with PER in this observational analysis.

Rationale and aim: This paper has the object to present the impact of nuts' and seeds' injuries withdrawing data from the Susy Safe registry, highlighting that as for other foreign bodies the main item efficiently and substantially susceptible to changes to decrease the accidents' rates is the education of adults and children, that can be shared with parents both from pediatricians and general practitioners. Indeed labeling and age related warnings have also a fundamental relevance in prevention. Methods: The present study draws its data from the Susy Safe registry. Details on injuries are entered in the Susy Safe Web-registry through a standardized case report form, that includes information regarding: children age and gender, features of the object, circumstances of injury (presence of parents and activity) and hospitalization's details (lasting, complications and removal details). Cases are prospectively collected using the Susy Safe system from 06/2005; moreover, also information regarding past consecutive cases available in each centre adhering to the project have been entered in the Susy Safe registry. Results: Nuts and seeds are one of the most common food item retrieved in foreign bodies injuries in children. In Susy Safe registry they represent the 38% in food group, and almost the 10% in general cases. Trachea, bronchi and lungs were the main location of FB's retrieval, showing an incidence of 68%. Hospitalization occurred in 83% of cases, showing the major frequency for foreign bodies located in trachea. This location was also the principal site of complications, with a frequency of 68%. There were no significant associations between these outcomes and the age class of the children. The most common complications seen (22.4%) was bronchitis, followed by pneumonia (19.7%). Adult presence was recorded as positive in 71.2% of cases, showing an association (p value 0.009) between the adult supervision and the hospitalization outcome. On the contrary there was a non significant association between adult presence and the occurrence of complications. In 80.7% of cases, the incident happened while the child was eating. Among those cases, 88.6% interested trachea, lungs and bronchi. Conclusions: Food-related aspiration injuries are common events for young children, particularly under 4 years of age, and may lead to severe complication. There is a need to study in more depth specific characteristics of foreign bodies associated with increased hazard, such as size, shape, hardness or firmness, lubricity, pliability and elasticity, in order to better identify risky foods, and more precisely described the pathogenetic pathway. Parents are not adequately conscious and aware toward this risk; therefore, the number and severity of the injuries could be reduced by educating parents and children. Information about food safety should be included in all visits to pediatricians in order to make parents able to understand, select, and identify key characteristics of hazardous foods and better control the hazard level of various foods. In the next Swiss National HIV and Sexually Transmitted Infection (STI) Strategy 2011-2017, STI control will be integrated with HIV prevention. Information is needed which will improve the targeting of professional education. The objective of this study was to describe the clinical specialities and settings to which patients with bacterial STI present in Switzerland.

To study the association between cannabis use and frequent sexual risk behavior, we tested the hypothesis of a situational influence of cannabis use in sexual encounters using a combination of global association study and event-level analysis and examined possible mediator variables, including the personality trait of hedonism/risk preference, psychosocial stress, and HIV-related beliefs, using mediation models. The results of a computer-assisted telephone interview of a random sample of 2790 heterosexual men and women aged 16-24 years showed that risky sexual behavior was more frequent in cannabis-using men and women than in non-using persons. The results did not support a situational effect of cannabis intoxication on sexual risk behavior. The more frequent sexual risk behavior among cannabis users was mediated by decreased intentions to use HIV protection, by lower HIV-self-efficacy, and higher risk preference/hedonism. Only among women psychosocial stress was a partial mediator. The findings show that HIV prevention programs for cannabis-using young adults should emphasize the role of person variables instead of situation variables.

OBJECTIVES: To investigate the contribution of a real-time PCR assay for the detection of Treponema pallidum in various biological specimens with the secondary objective of comparing its value according to HIV status. METHODS: Prospective cohort of incident syphilis cases from three Swiss hospitals (Geneva and Bern University Hospitals, Outpatient Clinic for Dermatology of Triemli, Zurich) diagnosed between January 2006 and September 2008. A case-control study was nested into the cohort. Biological specimens (blood, lesion swab or urine) were taken at diagnosis (as clinical information) and analysed by real-time PCR using the T pallidum 47 kDa gene. RESULTS: 126 specimens were collected from 74 patients with primary (n = 26), secondary (n = 40) and latent (n = 8) syphilis. Among primary syphilis, sensitivity was 80% in lesion swabs, 28% in whole blood, 55% in serum and 29% in urine, whereas among secondary syphilis, it was 20%, 36%, 47% and 44%, respectively. Among secondary syphilis, plasma and cerebrospinal fluid were also tested and provided a sensitivity of 100% and 50%, respectively. The global sensitivity of T pallidum by PCR (irrespective of the compartment tested) was 65% during primary, 53% during secondary and null during latent syphilis. No difference regarding serology or PCR results was observed among HIV-infected patients. Specificity was 100%. CONCLUSIONS: Syphilis PCR provides better sensitivity in lesion swabs from primary syphilis and displays only moderate sensitivity in blood from primary and secondary syphilis. HIV status did not modify the internal validity of PCR for the diagnosis of primary or secondary syphilis.

Chlamydia trachomatis infection (chlamydia) is the most common notifiable bacterial sexually transmitted infection (STI) worldwide. In the United States of America (USA) in 2009, 1,244,180 cases of chlamydia were reported to the Centers for Disease Control and Prevention (CDC), the largest number of cases ever reported to CDC for any notifiable disease [1]. It has been estimated, from population prevalence surveys, that approximately 2 % of sexually active adults aged 18–44 years old in the UK [2] and 2.2 % (CI, 1.8–2.8 %) of the US population aged 14–39 years [3] are infected with chlamydia. This level of prevalence in the USA translates into an estimated 2,291,000 (95 % confidence interval, CI, 1,857,000–2,838,000) chlamydia infections each year [3]. Globally, the World Health Organization (WHO) estimates that there are about 92 million new cases of chlamydia each year [4].

BACKGROUND The treatment and outcomes of patients with human immunodeficiency virus (HIV)-associated Hodgkin lymphoma (HL) continue to evolve. The International Prognostic Score (IPS) is used to predict the survival of patients with advanced-stage HL, but it has not been validated in patients with HIV infection. METHODS This was a multi-institutional, retrospective study of 229 patients with HIV-associated, advanced-stage, classical HL who received doxorubicin, bleomycin, vinblastine, and dacarbazine (ABVD) plus combination antiretroviral therapy. Their clinical characteristics were presented descriptively, and multivariate analyses were performed to identify the factors that were predictive of response and prognostic of progression-free survival (PFS) and overall survival (OS). RESULTS The overall and complete response rates to ABVD in patients with HIV-associated HL were 91% and 83%, respectively. After a median follow-up of 5 years, the 5-year PFS and OS rates were 69% and 78%, respectively. In multivariate analyses, there was a trend toward an IPS score >3 as an adverse factor for PFS (hazard ratio [HR], 1.49; P=.15) and OS (HR, 1.84; P=.06). A cluster of differentiation 4 (CD4)-positive (T-helper) cell count <200 cells/μL was associated independently with both PFS (HR, 2.60; P=.002) and OS (HR, 2.04; P=.04). The CD4-positive cell count was associated with an increased incidence of death from other causes (HR, 2.64; P=.04) but not with death from HL-related causes (HR, 1.55; P=.32). CONCLUSIONS The current results indicate excellent response and survival rates in patients with HIV-associated, advanced-stage, classical HL who receive ABVD and combination antiretroviral therapy as well as the prognostic value of the CD4-positive cell count at the time of lymphoma diagnosis for PFS and OS. The scale-up of antiretrovirals (ARVs) to treat HIV/AIDS in Africa has been rapid over the last five years. Botswana was the first African nation to roll out a comprehensive ARV program, where ARVs are available to all citizens who qualify. Excellent adherence to these ARVs is necessary to maintain HIV suppression and on-going health of all individuals taking them. Children rely almost entirely on their caregivers for the administration of these medications, and very little research has been done to examine the factors which affect both adherence and disclosure to the child of their HIV status. ^ Methods. This cross-sectional study used multiple methods to examine adherence, disclosure, and stigma across various dimensions of the child and caregiver's lives, including 30 caregiver questionnaires, interviewer-administered 3-day adherence recalls, pharmacy pill counts, and chart reviews. Fifty in-depth interviews were conducted with caregivers, male caregivers, teenagers, and health care providers. ^ Results. Perceived family stigma was found to be a predictor of excellent adherence. After controlling for age, children who live with their mothers were significantly less likely to know their HIV status than children living with any other relative (OR=0.403, p=0.014). Children who have a grandmother living in the household or taking care of them each day are significantly more likely to have optimal adherence than children who don't have grandmother involvement in their daily lives. ^ Discussion. Visible illness plays an intermediary role between adherence and perceived family stigma: Caregivers know that ARVs suppress physical manifestations of HIV, and in an effort to avoid unnecessary disclosure of the child's status to family members, therefore have children with excellent adherence. Grandmothers play a vital role in supporting the care and treatment of children in Botswana. ^

Gasoline is perhaps one of the most familiar substances to people in our society. Benzene is a component of gasoline and it is hemotoxic and carcinogenic at high concentrations. With increased governmental regulation of gasoline and benzene exposure, the hazardous effects of benzene on public health are very limited. As long as environmental conditions meet OSHA regulation standards, most individuals are not aware of the hazardous effect of benzene. However, recent studies have found that less than 1ppm benzene exposure which is below OSHA exposure regulatory standard may cause hematological damage. This paper reviews the relationship among gasoline-related work, daily life gasoline exposure, public health and new updated research in this field. We searched for relevant publications from 1966 through 2006 in three databases: Ovid Medline, PubMed and TOXNET. This review is a means toward educating people about exposure to benzene in different environments. Although government has developed good monitoring and prevention methods to decrease the harm from gasoline, we are aware that the problem is still there. The primary public health message is the benzene may cause hematological effects even at or below the U.S occupational standard of 1ppm.^

The preterm birth rate has been increasing over time in the United States, causing a large social and individual financial burden. Though the cause of preterm birth is now known, risk factors such as a previous preterm birth and a short cervical length have been identified as possible predictors. There are many contributing social and behavioral factors that play a role was well as medical problems that occur before and during pregnancy. Though there have been prevention methods identified, such as prenatal care, tocolytic therapy and cervical cerclage, none of these methods have shown to definitively prevent preterm birth over a long period of time. 17 alpha hydroxyprogesterone has been recognized as a possible prevention method for women at high risk for preterm birth. Three out of the five studies assessed in this review showed a significant reduction in preterm birth with administration of progesterone, both for women with a previous preterm birth and with a short cervical length. Currently there is no standard of care for those at high risk for preterm birth. More large clinical trials need to be conducted to determine if this progesterone for the prevention of preterm birth is effective. ^

Desde 1995, el derecho internacional se ha integrado en el ordenamiento jurídico español en lo referente a seguridad y salud. Sin embargo, la situación de la seguridad en los aprovechamientos forestales no ha mejorado notablemente, puesto que se detectan frecuentes casos de personal trabajando sin equipo de protección e incumpliendo las instrucciones de trabajo seguro, y son abundantes las referencias a una elevada accidentalidad. Para evaluar estos aspectos, primeramente se han comparado y valorado los datos de accidentalidad de los taladores de una empresa de referencia con los de los sectores agrario y de la construcción a través de las estadísticas nacionales y con los datos depurados del sector forestal a partir de la base de datos de declaración electrónica de accidentes de la C.A. de Castilla y León. En segundo lugar, se han identificado las deficiencias en materia de prevención, diseñando para ello una encuesta que se ha llevado a cabo en 35 aprovechamientos de Castilla y León. Posteriormente, se ha verificado que la reducción de la accidentalidad en la empresa de referencia se debe a la aplicación del plan de prevención. Se han caracterizado los riesgos en los que habían incurrido los taladores y se han descrito las medidas adoptadas para la reducción de los accidentes en la empresa estudiada, lo que ha conducido a una serie de recomendaciones para reducir la accidentalidad. Por otro lado se han analizado las causas de los picos de accidentalidad detectados. Como conclusión, se puede reducir sensiblemente la accidentalidad sólo si la empresa integra la política de prevención como un objetivo más de la gestión empresarial, en todos los niveles de su organización, acompañando las herramientas técnicas con formación temprana y motivación adecuada. From 1995, Spanish safety legislation has been adapted to the Internationa! framework. However, operationally the situation is not so good, at least in the wood harvesting activity. Some forest workers are still found not wearing the compulsory safety equipment, working without respecting safety rules and references to high accidents rates are frequent. To assess those aspects, firstly some comparisons have been made between the accident records in a reference company and the accident indexes at national and regional level for building and farming activities. Secondly, using a purpose-designed questionnaire applied to 35 logging worksites in Castilla y Leon Region, deficiencies in prevention have been identified. The relation between the accident reduction and the prevention methods used by the reference studied company has been confirmed. Also the main causes and peaks of the accidents have been analyzed. Finally, some recommendations to reduce the risk of accidents in harvesting operations are provided. The main conclusion confirms that the possibility of reducing the accident risk is conditioned to the commitment of all the staff of the timber harvesting company, at every level of responsibility, to search for excellence in safety as one more entrepreneurial goal. This fact should be accompanied by the staff early training and the adequate motivation means. La aparición de Internet y los sistemas informáticos supuso un antes y un después en el modo que las personas emplearían para acceder a los sistemas de información. El crecimiento exponencial seguido en los años posteriores ha llevado este hecho hasta la situación actual, donde prácticamente todos los ámbitos del día a día se encuentran reflejados en la Red. Por otro lado, a la par que la sociedad se desplazaba al ciberespacio, también comenzaban a hacerlo aquellos que buscaban obtener un rendimiento delictivo de los nuevos medios y herramientas que se ponían a su disposición. Avanzando a pasos agigantados en el desarrollo de técnicas y métodos para vulnerar unos sistemas de seguridad, aún muy inmaduros, los llamados ciberdelincuentes tomaban ventaja sobre las autoridades y su escasa preparación para abordar este nuevo problema. Poco a poco, y con el paso de los años, esta distancia ha ido reduciéndose, y pese a que aún queda mucho trabajo por hacer, y que el crecimiento de los índices de ciberdelincuencia, junto con la evolución y aparición de nuevas técnicas, sigue a un ritmo desenfrenado, los gobiernos y las empresas han tomado consciencia de la gravedad de este problema y han comenzado a poner sobre la mesa grandes esfuerzos e inversiones con el fin de mejorar sus armas de lucha y métodos de prevención para combatirla. Este Proyecto de Fin de Carrera dedica sus objetivos a la investigación y comprensión de todos estos puntos, desarrollando una visión específica de cada uno de ellos y buscando la intención final de establecer las bases suficientes que permitan abordar con la efectividad requerida el trabajo necesario para la persecución y eliminación del problema. ABSTRACT. The emergence of Internet and computer systems marked a before and after in the way that people access information systems. The continued exponential growth in the following years has taken this fact to the current situation, where virtually all areas of everyday life are reflected in the Net. On the other hand, meanwhile society moved into cyberspace, the same began to do those seeking to obtain a criminal performance of new media and tools at their disposal. Making great strides in the development of techniques and methods to undermine security systems, still very immature, so called cybercriminals took advantage over the authorities and their lack of preparation to deal with this new problem. Gradually, and over the years, this distance has been declining, and although there is still much work to do and the growth rates of cybercrime, along with the evolution and emergence of new techniques, keep increasing at a furious pace, governments and companies have become aware of the seriousness of this problem and have begun to put on the table great efforts and investments in order to upgrade their weapons to fight against this kind of crimes and prevention methods to combat it. This Thesis End of Grade Project focuses its objectives on the research and understanding of all these points, developing a specific vision of each of them and looking for the ultimate intention of establishing a sufficient basis by which to manage with the required effectiveness the type of work needed for the persecution and elimination of the problem.

Aim. The aim of this study is to assess the role of progesterone in preterm birth prevention. Methods. A MEDLINE search (from 1966 to the present; date of last search January 2005) was performed - using the key words progesterone, pregnancy, preterm birth, preterm labor, and randomized, controlled trial - in order to identify randomized, controlled trials in which progesterone (either intramuscular or vaginal administration) was compared with placebo or no treatment. Data were extracted and a meta-analysis was performed. Results. Seven randomized, controlled trials were identified. Women who received progesterone were statistically significantly less likely to give birth before 37 weeks (seven studies, 1020 women, RR = 0.58, 95% CI = 0.48-0.70), to have an infant with birth weight of

Background: fall-related hip fractures are one of the most common causes of disability and mortality in older age. The study aimed to quantify the relationship between lifestyle behaviours and the risk of fall-related hip fracture in community-dwelling older people. The purpose was to contribute evidence for the promotion of healthy ageing as a population-based intervention for falls injury prevention. Methods: a case-control study was conducted with 387 participants, with a case-control ratio of 1:2. Incident cases of fall-related hip fracture in people aged 65 and over were recruited from six hospital sites in Brisbane, Australia, in 2003-04. Community-based controls, matched by age, sex and postcode, were recruited via electoral roll sampling. A questionnaire designed to assess lifestyle risk factors, identified as determinants of healthy ageing, was administered at face-to-face interviews. Results: behavioural factors which had a significant independent protective effect on the risk of hip fracture included never smoking [adjusted odds ratio (AOR): 0.33 (0.12-0.88)], moderate alcohol consumption in mid- and older age [AOR: 0.49 (0.25-0.95)], not losing weight between mid- and older age [AOR: 0.36 (0.20-0.65)], playing sport in older age [AOR: 0.49 (0.29-0.83)] and practising a greater number of preventive medical care [AOR: 0.54 (0.32-0.94)] and self-health behaviours [AOR: 0.56 (0.33-0.94)]. Conclusion: with universal exposures, clear associations and modifiable behavioural factors, this study has contributed evidence to reduce the major public health burden of fall-related hip fractures using readily implemented population-based healthy ageing strategies.
