Because of the large variability in the pharmacokinetics of anti-HIV drugs, therapeutic drug monitoring in patients may contribute to optimize the overall efficacy and safety of antiretroviral therapy. An LC-MS/MS method for the simultaneous assay in plasma of the novel antiretroviral agents rilpivirine (RPV) and elvitegravir (EVG) has been developed to that endeavor. Plasma samples (100 μL) extraction is performed by protein precipitation with acetonitrile, and the supernatant is subsequently diluted 1:1 with 20-mM ammonium acetate/MeOH 50:50. After reverse-phase chromatography, quantification of RPV and EVG, using matrix-matched calibration samples, is performed by electrospray ionization-triple quadrupole mass spectrometry by selected reaction monitoring detection using the positive mode. The stable isotopic-labeled compounds RPV-(13) C6 and EVG-D6 were used as internal standards. The method was validated according to FDA recommendations, including assessment of extraction yield, matrix effects variability (<6.4%), as well as EVG and RPV short and long-term stability in plasma. Calibration curves were validated over the clinically relevant concentrations ranging from 5 to 2500 ng/ml for RPV and from 50 to 5000 ng/ml for EVG. The method is precise (inter-day CV%: 3-6.3%) and accurate (3.8-7.2%). Plasma samples were found to be stable (<15%) in all considered conditions (RT/48 h, +4°C/48 h, -20°C/3 months and 60°C/1 h). Selected metabolite profiles analysis in patients' samples revealed the presence of EVG glucuronide, that was well separated from parent EVG, allowing to exclude potential interferences through the in-source dissociation of glucuronide to parent drug. This new, rapid and robust LCMS/MS assay for the simultaneous quantification of plasma concentrations of these two major new anti-HIV drugs EVG and RPV offers an efficient analytical tool for clinical pharmacokinetics studies and routine therapeutic drug monitoring service. Dolido por la escasa repercusión y el nulo interés de la geografía (crítica) estadounidense ante los atentados de Al Qaeda en Madrid, el cuaderno de viaje de Margarita del Vez (participante en el 100 Congreso de la Association of American Geographers) reprocha no sólo que el conocimiento geográfico sigue siendo mercantilizado y puesto al servicio del poder, sino que, descontextualizado y desideologizado, a menudo se convierte en una nueva forma de «saber ignorante». Con la deconstrucción formal de la narrativa «científica», el texto pretende ser también una denuncia al hecho de que la institución académica, por guardar las formas, llegue al extremo de dar la espalda a la realidad.

Heavy traffic volumes frequently cause distress in asphalt pavements which were designed under accepted design methods and criteria. The distress appears in the form of rutting in the wheel tracks and rippling or shoving in areas where traffic accelerates or decelerates. Apparently accepted stability test methods alone do not always assure the desired service performance of asphaltic pavements under heavy traffic. The Bituminous Research Laboratory, Engineering Research Institute of Iowa State University undertook the development of a laboratory device by which the resistance of an asphalt paving mix to displacement under traffic might be evaluated, and also be used as a supplemental test to determine adequacy of design of the mix by stability procedures.

A study was undertaken by the Bituminous Research Laboratory of the Engineering Research Institute at Iowa State University, under the sponsorship of the Iowa Highway Research Board, project HR 100, to ascertain the effects of a number of characteristics and properties of asphaltic concrete mixes upon the service behavior of the mixes as evaluated by the Traffic Simulator and by field observations. The study included: Investigations of the relations, of gradation, fraction and resistance to wear of aggregates; of stability, cohesion, per cent voids and asphalt content: of a number of laboratory and field mixes to service behavior as indicated by the Traffic Simulator under various test conditions. Based upon the results of the tests and the relationships noted, tentative criteria for the Traffic Simulator test were devised, subject to verification by observations and measurements of field service behavior of the mixes.

La extracción del tercer molar inferior incluido provoca efectos secundarios como dolor, inflamación y trismo. Presentamos un estudio cuyo objetivo fue relacionar la edad, el sexo, el tabaco y la higiene oral con el postoperatorio de 100 de terceros molares inferiores incluidos. La edad, el sexo y el consumo de tabaco no influyeron significativamente en el postoperatorio. Sólo hallamos una correlación estadísticamente significativa entre la higiene oral y el dolor a las 6 horas de la exodoncia.

BACKGROUND AND PURPOSE: Patients with symptoms of semicircular canal dehiscence often undergo both CT and MR imaging. We assessed whether FIESTA can replace temporal bone CT in evaluating patients for SC dehiscence. MATERIALS AND METHODS: We retrospectively reviewed 112 consecutive patients (224 ears) with vestibulocochlear symptoms who underwent concurrent MR imaging and CT of the temporal bones between 2007 and 2009. MR imaging protocol included a FIESTA sequence covering the temporal bone (axial 0.8-mm section thickness, 0.4-mm spacing, coronal/oblique reformations; 41 patients at 1.5T, 71 patients at 3T). CT was performed on a 64-row multidetector row scanner (0.625-mm axial acquisition, with coronal/oblique reformations). Both ears of each patient were evaluated for dehiscence of the superior and posterior semicircular canals in consensual fashion by 2 neuroradiologists. Analysis of the FIESTA sequence and reformations was performed first for the MR imaging evaluation. CT evaluation was performed at least 2 weeks after the MR imaging review, resulting in a blinded comparison of CT with MR imaging. CT was used as the reference standard to evaluate the MR imaging results. RESULTS: For SSC dehiscence, MR imaging sensitivity was 100%, specificity was 96.5%, positive predictive value was 61.1%, and negative predictive value was 100% in comparison with CT. For PSC dehiscence, MR imaging sensitivity was 100%, specificity was 99.1%, positive predictive value was 33.3%, and negative predictive value was 100% in comparison with CT. CONCLUSIONS: MR imaging, with a sensitivity and negative predictive value of 100%, conclusively excludes SSC or PSC dehiscence. Negative findings on MR imaging preclude the need for CT to detect SC dehiscence. Only patients with positive findings on MR imaging should undergo CT evaluation.

Aquest estudi es basa en l'idea de que l'escurçament de la cadena musuclar posterior és una causa important del mal d'esquena en nens. És per aquest motiu que es planteja un tractament per a prevenir l'escurçament de la cadena muscular posterior i el mal d'esquena en forma de sessions de Stretching Global Actiu, mètode basat en la Reeducació Postural Global del mateix Ph.E. Souchard, que es posarà en pràctica en 3 escoles diferents de la comarca d'Osona. L'estudi té una durada de 3 cursos escolars (3r, 4t i 5è de primària) en el qual els nens seleccionats per al grup experimental (n=30) realitzaran classes de Stretching Global Actiu dues vegades a la setmana durant 3 mesos en cada curs escolar. Tres escoles més formaran el grup control (n=30) i s'espera que els nens que formen part del grup experimental millorin la flexibilitat de la cadena musuclar posterior (això es podrà observar a través dels resultats del test "sit and reach") i que no presentin mal d'esquena en el transcurs de 3 anys (durada de l'estudi).
