Introduction.- Knowledge of predictors of an unfavourable outcome, e.g. non-return to work after an injury enables to identify patients at risk and to target interventions for modifiable predictors. It has been recently shown that INTERMED; a tool to measure biopsychosocial complexity in four domains (biologic, psychologic, social and care, with a score between 0-60 points) can be useful in this context. The aim of this study was to set up a predictive model for non-return to work using INTERMED in patients in vocational rehabilitation after orthopaedic injury.Patients and methods.- In this longitudinal prospective study, the cohort consisted of 2156 consecutively included inpatients with orthopaedic trauma attending a rehabilitation hospital after a work, traffic or sport related injury. Two years after discharge, a questionnaire regarding return to work was sent (1502 returned their questionnaires). In addition to INTERMED, 18 predictors known at baseline of the rehabilitation were selected based on previous research. A multivariable logistic regression was performed.Results.- In the multivariate model, not-returning to work at 2 years was significantly predicted by the INTERMED: odds-ratio (OR) 1.08 (95% confidence interval, CI [1.06; 1.11]) for a one point increase in scale; by qualified work-status before the injury OR = 0.74, CI (0.54; 0.99), by using French as preferred language OR = 0.60, CI (0.45; 0.80), by upper-extremity injury OR = 1.37, CI (1.03; 1.81), by higher education (> 9 years) OR = 0.74, CI (0.55; 1.00), and by a 10 year increase in age OR = 1.15, CI (1.02; 1.29). The area under the receiver-operator-characteristics curve (ROC)-curve was 0.733 for the full model (INTERMED plus 18 variables).Discussion.- These results confirm that the total score of the INTERMED is a significant predictor for return to work. The full model with 18 predictors combined with the total score of INTERMED has good predictive value. However, the number of variables (19) to measure is high for the use as screening tool in a clinic.

Introduction.- The model presented in part I (19 predictors) had good predictive values for non-return to work 2 years after vocational rehabilitation for orthopaedic trauma. However, the number of predictors is high for the detection of patients at risk in a clinic. For example, the INTERMED for itself consists of 20 questions and needs 20 minutes to be filled in. For this reason, the aim of this study was to compare the predictive value of different models for the prediction of non-return to work.Patients and methods.- In this longitudinal prospective study, the cohort consisted of 2156 included inpatients with orthopaedic trauma attending a rehabilitation hospital after a work, traffic, sport or leisure related injury. Two years after discharge, 1502 patients returned a questionnaire regarding return to work. We compared the area under the receiver-operator-characteristics curve (ROC) between different models: INTERMED total score, the 4 partial INTERMED scores, the items of the most predictive partial score; with or without confounders.Results.- The ROC for the total score of the INTERMED plus the five confounders of the of the part one (qualified work, speaking French, lesion of upper extremity, education and age) was 0.72. The sole partial INTERMED score to predict return to work was the social sub score. The ROC for the five items of the latter sub score of the INTERMED was 0.69. The ROC for the five items of the social subscale of the INTERMED combined with five predictors was 0.73. This was significantly better than the use of only the five items from INTERMED alone (delta 0.034; 95% CI 0.017 to .050). The model presented in part I (INTERMED total score plus 18 predictors) was not significantly better than the five items INTERMED social score plus five confounders.Discussion.- The use of a model with ten variables (INTERMED social five items plus five confounders) has good predictive value to detect patients not returning to work after vocational rehabilitation after orthopaedic trauma. The parsimony of this model facilitates its use in a clinic for the detection of patients at risk.

The objective of this work was to identify factors associated with the 56-day non-return rate (56-NRR) in dairy herds in the Galician region, Spain, and to estimate it for individual Holstein bulls. The experiment was carried out in herds originated from North-West Spain, from September 2008 to August 2009. Data of the 76,440 first inseminations performed during this period were gathered. Candidate factors were tested for their association with the 56-NRR by using a logistic model (binomial). Afterwards, 37 sires with a minimum of 150 first performed inseminations were individually evaluated. Logistic models were also estimated for each bull, and predicted individual 56-NRR rate values were calculated as a solution for the model parameters. Logistic regression found four major factors associated with 56-NRR in lactating cows: age at insemination, days from calving to insemination, milk production level at the time of insemination, and herd size. First-service conception rate, when a particular sire was used, was higher for heifers (0.71) than for lactating cows (0.52). Non-return rates were highly variable among bulls. Asignificant part of the herd-level variation of 56-NRR of Holstein cattle seems attributable to the service sire. High correlation level between observed and predicted 56-NRR was found.

BACKGROUND: Workers with persistent disabilities after orthopaedic trauma may need occupational rehabilitation. Despite various risk profiles for non-return-to-work (non-RTW), there is no available predictive model. Moreover, injured workers may have various origins (immigrant workers), which may either affect their return to work or their eligibility for research purposes. The aim of this study was to develop and validate a predictive model that estimates the likelihood of non-RTW after occupational rehabilitation using predictors which do not rely on the worker's background. METHODS: Prospective cohort study (3177 participants, native (51%) and immigrant workers (49%)) with two samples: a) Development sample with patients from 2004 to 2007 with Full and Reduced Models, b) External validation of the Reduced Model with patients from 2008 to March 2010. We collected patients' data and biopsychosocial complexity with an observer rated interview (INTERMED). Non-RTW was assessed two years after discharge from the rehabilitation. Discrimination was assessed by the area under the receiver operating curve (AUC) and calibration was evaluated with a calibration plot. The model was reduced with random forests. RESULTS: At 2 years, the non-RTW status was known for 2462 patients (77.5% of the total sample). The prevalence of non-RTW was 50%. The full model (36 items) and the reduced model (19 items) had acceptable discrimination performance (AUC 0.75, 95% CI 0.72 to 0.78 and 0.74, 95% CI 0.71 to 0.76, respectively) and good calibration. For the validation model, the discrimination performance was acceptable (AUC 0.73; 95% CI 0.70 to 0.77) and calibration was also adequate. CONCLUSIONS: Non-RTW may be predicted with a simple model constructed with variables independent of the patient's education and language fluency. This model is useful for all kinds of trauma in order to adjust for case mix and it is applicable to vulnerable populations like immigrant workers.

A novel multichannel carrier-suppressed return-to-zero (CSRZ) to non-return-to-zero (NRZ) format conversion scheme based on a single custom-designed fiber Bragg grating (FBG) with comb spectra is proposed. The spectral response of each channel is designed according to the algebraic difference between the CSRZ and NRZ spectra outlines. The tailored group delays are introduced to minimize the maximum refractive index modulation. Numerical results show that four-channel 200-GHz-spaced CSRZ signals at 40 Gbits/s can be converted into NRZ signals with high Q-factor and wide-range robustness. It is shown that our proposed FBG is robust to deviations of bandwidth and central wavelength detuning. Another important merit of this scheme is that the pattern effects are efficiently reduced owing to the well-designed spectra response.

Notre recherche a pour objet la migration pour études et plus précisément l’expérience de retour et du non-retour des diplômés guinéens formés au Canada. Elle repose sur une démarche qualitative. Prenant appui sur des matériaux discursifs issus d’entrevues individuelles dans deux sites (en Guinée et au Canada), auprès d’un échantillon de trente et un diplômés, notre étude s’est attachée, dans un premier temps, à faire ressortir les motivations des étudiants à « retourner au pays » à la fin de leurs études au Canada ou, à l’inverse, à demeurer dans le pays d’accueil. Dans une seconde phase, la recherche s'est intéressée au devenir des enquêtés après leurs études et en particulier à l’insertion ou réinsertion sociale, familiale et professionnelle des diplômés retournés en Guinée ou installés au Canada. Enfin, notre étude examine les perspectives d’avenir de l’ensemble de nos répondants; leurs satisfactions et frustrations après le retour en Guinée ou l’installation au Canada, le bilan qu’ils tirent de leur expérience de migration pour études mais aussi les perspectives de retour éventuelles et les liens que ceux qui sont restés entretiennent avec leur pays d’origine. Pour les diplômés retournés en Guinée, l’analyse des données montre que les « perspectives d’emploi et de promotion » ont joué un rôle central dans leur décision de rentrer. Ils sont également nombreux a déclaré avoir choisi le retour pour des raisons familiales. Certains justifient aussi leur retour par « la volonté de servir le pays ». Si l’insertion familiale a été facile pour la plupart des diplômés retournés en Guinée, la réinsertion sociale, le retour à des pratiques sociales et à un environnement précaire « qu’ils avaient oublié » semble en revanche avoir été moins aisé. Sur le plan professionnel, le séjour canadien est perçu comme ayant eu un impact très positif sur leur carrière. Les diplômes canadiens semblent très valorisés en Guinée et les réseaux canadiens que les diplômés ont pu établir lors de leur séjour sont aussi très utiles. La possibilité de trouver un emploi décent au Canada domine également le discours des répondants qui ont choisi de demeurer au pays d’accueil après leur formation. Les répondants ayant choisi de demeurer au Canada évoquent également fréquemment l’idée de « sacrifier » leur retour au profit de « l’avenir » des enfants. La politique de l’immigration canadienne par « l’incitation » de demeurer au Canada après les études ont aussi influencé certains diplômés dans leurs décisions de ne pas retourner. Même s’ils évoquent fréquemment l’emploi pour justifier leur installation au Canada, nos répondants restés au Canada ne trouvent pas facilement un travail qui correspond à leur formation et doivent souvent se contenter de « petits boulots ». Plusieurs pointent du doigt le «bilinguisme» et la «discrimination» en milieu de travail comme obstacles majeurs. Enfin, pour bon nombre d’entre eux le « retour au pays » est une perspective jamais écartée, la plupart n’ont jamais coupé le lien avec leur pays d’origine et plusieurs tiennent à faire connaître la culture guinéenne à leurs enfants.

La operación de mega minería que abarca CERREJON se desarrolla bajo lo más altos estándares de seguridad y calidad, con el compromiso de entregar al mercado un excelente producto y evitando el deterioro del entorno. Uno de los puntos más fuertes de la compañía se haya en la integración de los procesos productivos entre la mina de carbón, el ferrocarril y el puerto, logrando que la operación sea eficiente y que garantice niveles altos de óptimos resultados y que los porcentajes de falla no existan o cada vez sean menores en el proceso de explotación y transporte del carbón de Colombia hacia el mundo. El departamento de MATERIALES es el punto de origen para garantizar que se desarrolle la operación, dado que el departamento tiene la gran labor de adquirir y entregar los bienes y servicios requeridos por la Compañía al más bajo costo total evaluado, en el menor tiempo posible y bajo el marco de la legislación Colombiana en el proceso de nacionalización, con un alto énfasis en el desarrollo de relaciones fuertes y sinérgicas entre todos los eslabones de la cadena. Todo el proceso del departamento se enmarca dentro de un ciclo que debe encaminarse a ser cada vez más efectivo y eficiente; de allí que se busquen opciones de mejoramiento para afinar los procesos. Para que sea posible y factible manejar una mega operación como esta se requiere afinar al máximo la red que se establece en su cadena de abastecimiento, buscando lograr un flujo de producto, información y fondos que garanticen disponibilidad del producto, para así generar una rentabilidad alta a la compañía, y controlar los costos por la operación. Dado que la cadena de suministro es susceptible a mejoras, gracias a la interacción con los proveedores, colaboradores y clientes; se pretende sacar el mejor provecho de esto, a través del análisis de la cadena actual de CERREJON; presentando una opción de mejora en uno de los eslabones del proceso productivo, esta opción ha sido contemplada desde años anteriores, pero en esta ocasión gracias a la integración de afinamientos en los sistemas de información, y la participación activa de los proveedores, se encuentra una opción viable de la eliminación de un reproceso, que garantiza eficiencia y efectividad en la agilización del ciclo de producción en CERREJON. A través de la ejecución del proyecto de reforma del conteo, se presenta la oportunidad de mejoramiento en la cadena del departamento; el proceso de reconteo en la mina, realizado posteriormente al conteo inicial en Puerto Bolívar, de los materiales que llegan importados vía marítima. El proceso de afinamiento del recibo documental en el consolidador de carga y los mayores proveedores de entrega directa (HITACHI y MCA) al transportador, genera la opción del uso de terminales portátiles, que de la mano con los ajustes documentales, permitirán que la carga sea contada y separada por localización, para enviarla vía tren a LMN, reduciendo tiempo de entrega al cliente final, los costos de remanejo al volver a contar, y los costos asociados a las entregas no oportunas de devolución de contenedores.

A remarkable cervid bone accumulation occurs at a single passage (named Cervid Passage; CP) at Lapa Nova, a maze cave in eastern Brazil. CP lies away from cave entrances, is a typical pitfall passage and contains bone remains of at least 121 cervids, besides few bats, peccaries and rodents remains. There is no evidence of water (or sediment) flow at the site and in general bones lack post depositional alterations and display anatomical proximity, suggesting that the majority of the remains found inside CP (mainly cervids) are due to animals that after entering the cave got trapped in the site. Observations suggest that two entrances could have provided access to cervids (and the few other animals, besides bats), either by falling inside the cave or by entering by their own free will. Once inside the cave, the maze pattern would make route finding difficult, and of all passage intersections, only the one leading to CP would result in a non-return situation, starving the animal to death. Radiocarbon dates suggest that animal entrapment occurred during at least 5 thousand years, during the Holocene. The reasons why mainly cervids were found are unknown but they are probably related to the biology of this group coupled with the fact that caves provide several specific taphonomic processes that may account for a strong bias in bone accumulation. Indeed, the frequent occurrence of Cervidae in both the fossil and sub-fossil record in Brazilian caves may be related to an overall high faunal abundance or may suggest that these animals were especially prone to enter caves, perhaps in search of nutrients (as cave saltpetre) or water.

We present a simultaneous optical signal-to-noise ratio (OSNR) and differential group delay (DGD) monitoring method based on degree of polarization (DOP) measurements in optical communications systems. For the first time in the literature (to our best knowledge), the proposed scheme is demonstrated to be able to independently and simultaneously extract OSNR and DGD values from the DOP measurements. This is possible because the OSNR is related to maximum DOP, while DGD is related to the ratio between the maximum and minimum values of DOP. We experimentally measured OSNR and DGD in the ranges from 10 to 30 dB and 0 to 90 ps for a 10 Gb/s non-return-to-zero signal. A theoretical analysis of DOP accuracy needed to measure low values of DGD and high OSNRs is carried out, showing that current polarimeter technology is capable of yielding an OSNR measurement within 1 dB accuracy, for OSNR values up to 34 dB, while DGD error is limited to 1.5% for DGD values above 10 ps. For the first time to our knowledge, the technique was demonstrated to accurately measure first-order polarization mode dispersion (PMD) in the presence of a high value of second-order PMD (as high as 2071 ps(2)). BACKGROUND: Health risk appraisal is a promising method for health promotion and prevention in older persons. The Health Risk Appraisal for the Elderly (HRA-E) developed in the U.S. has unique features but has not been tested outside the United States. METHODS: Based on the original HRA-E, we developed a scientifically updated and regionally adapted multilingual Health Risk Appraisal for Older Persons (HRA-O) instrument consisting of a self-administered questionnaire and software-generated feed-back reports. We evaluated the practicability and performance of the questionnaire in non-disabled community-dwelling older persons in London (U.K.) (N = 1090), Hamburg (Germany) (N = 804), and Solothurn (Switzerland) (N = 748) in a sub-sample of an international randomised controlled study. RESULTS: Over eighty percent of invited older persons returned the self-administered HRA-O questionnaire. Fair or poor self-perceived health status and older age were correlated with higher rates of non-return of the questionnaire. Older participants and those with lower educational levels reported more difficulty in completing the HRA-O questionnaire as compared to younger and higher educated persons. However, even among older participants and those with low educational level, more than 80% rated the questionnaire as easy to complete. Prevalence rates of risks for functional decline or problems were between 2% and 91% for the 19 HRA-O domains. Participants' intention to change health behaviour suggested that for some risk factors participants were in a pre-contemplation phase, having no short- or medium-term plans for change. Many participants perceived their health behaviour or preventative care uptake as optimal, despite indications of deficits according to the HRA-O based evaluation. CONCLUSION: The HRA-O questionnaire was highly accepted by a broad range of community-dwelling non-disabled persons. It identified a high number of risks and problems, and provided information on participants' intention to change health behaviour.

Advanced optical modulation format polarization-division multiplexed quadrature phase shift keying (PDM-QPSK) has become a key ingredient in the design of 100 and 200-Gb/s dense wavelength-division multiplexed (DWDM) networks. The performance of this format varies according to the shape of the pulses employed by the optical carrier: non-return to zero (NRZ), return to zero (RZ) or carrier-suppressed return to zero (CSRZ). In this paper we analyze the tolerance of PDM-QPSK to linear and nonlinear optical impairments: amplified spontaneous emission (ASE) noise, crosstalk, distortion by optical filtering, chromatic dispersion (CD), polarization mode dispersion (PMD) and fiber Kerr nonlinearities. RZ formats with a low duty cycle value reduce pulse-to-pulse interaction obtaining a higher tolerance to CD, PMD and intrachannel nonlinearities.

This study determined the relationship between two measures of field fertility of I I high-use Australian artificial insemination (AI) dairy bulls and thirty standard laboratory assessments of spermatozoal post-thaw viability. The two measures of field fertility used, conception rates (cCR) and non-return rates (cNRR), were both corrected for all major non-bull variables. Sperm viability assessments were conducted on semen collected within the same season as that used to derive the field fertility estimates. These assessments measured sperm concentration, motility, morphology and membrane integrity at thawing, after 2 h incubation and after the swim-up sperm selection procedure. Derivations of these measures and in vitro embryo fertilizing and developmental capacity were also determined. The Genstat Statistical Package [Genstat 5 Release 4.2 Reference Manual, VSN International, Oxford, 20001 was used to conduct an analysis of variance on the viability parameters across semen straws and bulls, and to calculate the strength of correlation between each semen parameter, cNRR and cCR in a correlation matrix. Step forward multiple regression identified the combination of semen parameters that were most highly correlated with cCR and with cNRR. The sperm parameters identified as being most predictive of cCR were the percentage of morphologically normal sperm immediately post-thaw (zeroNorm), the number of morphologically normal sperm after the swim-up procedure (nSuNorm), and the rate of zygote cleavage in vitro (Clv); the predictive equation formed by these parameters accounted for 70% of variance. The predictive equation produced for cNRR contained the variables zeroNorm, the proportion of membrane intact sperm after 2 h incubation at 37 degreesC (twoMem) and Clv and accounted for 76.5% of the variation. ZeroNorm was found to be consistent across straws and semen batches within-bull and the sperm parameter with the strongest individual predictive capacity for both cCR (P = 0.1) and cNRR (P = 0.001). Post-thaw sperm parameters can be used to predict field fertility of Australian dairy sires; the calculated predictive equations are particularly useful for identifying and monitoring bulls of very high and very low potential fertility within a group. We introduce a novel transmission technique of periodic in-line all-optical format conversion between return-to-zero and non-return-to-zero-like aimed at delaying the accumulation of format-specific impairments. A particular realization of this approach using in-line normal dispersion fibre-enhanced nonlinear optical loop mirrors at 40Gbit/s data rate is presented. © 2004 Optical Society of America.

Return-to-Zero (RZ) and Non-Return-to-Zero (NRZ) Differential Phase Shift Keyed (DPSK) systems require cheap and optimal transmitters for widespread implementation. The authors report on a gain switched Discrete Mode (DM) laser that can be employed as a cost efficient transmitter in a 10.7 Gb/s RZ DPSK system and compare its performance to that of a gain switched Distributed Feed-Back (DFB) laser. Experimental results show that the gain switched DM laser readily provides error free performance and a receiver sensitivity of -33.1 dBm in the 10.7 Gbit/s RZ DPSK system. The standard DFB laser on the other hand displays an error floor at 10(-1) in the same RZ DPSK system. The difference in performance, between the two types of gain switched transmitters, is analysed by investigating their linewidths. We also demonstrate, for the first time, the generation of a highly coherent gain switched pulse train which displays a spectral comb of approximately 13 sidebands spaced by the 10.7 GHz modulation frequency. The filtered side-bands are then employed as narrow linewidth Continuous Wave (CW) sources in a 10.7 Gb/s NRZ DPSK system.

We present a phase locking scheme that enables the demonstration of a practical dual pump degenerate phase sensitive amplifier for 10 Gbit/s non-return to zero amplitude shift keying signals. The scheme makes use of cascaded Mach Zehnder modulators for creating the pump frequencies as well as of injection locking for extracting the signal carrier and synchronizing the local lasers. An in depth optimization study has been performed, based on measured error rate performance, and the main degradation factors have been identified.
