Dyspnea is a common and disabling symptom of respiratory and heart diseases, which is growing in incidence. During hospital admission, breathlessness is under-diagnosed and under-treated, although there are treatments available for controlling the symptom. We have developed a tailored implementation strategy directed to medical staff to promote the application of these pharmacological and non-pharmacological tools in dealing with dyspnea. The primary aim is to decrease the rate of patients that do not receive an adequate relief of dyspnea. This is a four-stage quasi-experimental study. The intervention consists in two teaching talks that will be taught in Cardiology and Respiratory Medicine Departments. The contents will be prepared by Palliative Care specialists, based on available tools for management of dyspnea and patients' needs. A cross-sectional study of dyspnea in hospitalized patients will be performed before and after the intervention to ascertain an improvement in dyspnea intensity due to changes in medical practices. The last phase consists in the creation of consensus protocols for dyspnea management based in our experience. The results of this study are expected to be of great value and may change clinical practice in the near future and promote a changing for the better of dyspnea care.

A população portuguesa encontra-se num processo de envelhecimento, e tem de enfrentar esta nova realidade demográfica. Vários determinantes nomeadamente biológicos, psicológicos e sociais, foram já identificados. O aumento da prevalência de doenças crónicas e incapacitantes, como consequência directa do envelhecimento humano, tem aumentado a procura de respostas na área da saúde e ao nível social. A Rede Nacional de Cuidados Continuados Integrados (REDE) surge como resposta a esta nova problemática. Através da implementação de Unidades de Cuidados Continuados Integrados (UCCI), a REDE é uma alternativa aos internamentos nos hospitais de pessoas em situação de dependência e com necessidade de cuidados diferenciados mas sem necessidade de cuidados típicos de uma unidade de agudos. As UCCI apresentam um modelo de intervenção multidisciplinar que contempla as vertentes da saúde, social e económica dos utentes, composta por equipas multidisciplinares. Devido às especificidades da população idosa e/ou dependente, a intervenção especializada dos profissionais das UCCI, deve ser cuidadosamente preparada com base no treino e especialização, aspectos centrais do sucesso da REDE.

Lichen planus is a common mucocutaneous disease. However, lichen planus with concomitant oral and genital involvement is a rare condition that requires multidisciplinary intervention for appropriate diagnosis. In this study, we reported two cases of oro-genital lichen planus in a man and a woman. Analyses of oral and genital lesions were clinically and histopathologically compatible with lichen planus. This description highlights the importance of patients presenting oral lichen planus be routinely required to undergo further medical examination in order to investigate putative genital mucosal involvement. These lesions have the potential to progress to neoplasia. Therefore, medical and dentistry professionals should be aware of this condition. (c) 2007 Elsevier B.V. All fights reserved.

Obesity is defined as excess body fat. It is considered today a serious public health problem and has reached high proportions in childhood and adolescence. Among factors highlight the lifestyle, poor diet and little physical activity for their development. Childhood obesity is not treated can become permanent in adulthood increases the risk of several diseases such as cardiovascular, metabolic and other disorders such as depression, low self-esteem among others. It's extreme need multidisciplinary intervention to prevent it, with guidelines on healthy eating habits and regular physical activity. Treatment is necessary in a multidisciplinary team of doctors, nutritionists, physical educators, psychologists and family that has a decisive weight in the treatment of obese children. The media could give more space for the disclosure of childhood obesity, so that the population is aware of this epidemic in a more serious understand its risks and health effects. This bibliographic study has character and aims to seek the causes of obesity, its consequences in adulthood and possible forms of intervention

Introducción: Por la creciente prevalencia de la obesidad en la infancia y/o adolescencia, deben existir suficientes programas de intervención para combatirla. Objetivo: identificar y comparar los distintos tipos de programas de intervención en niños y adolescentes, para el tratamiento del sobrepeso y la obesidad en España y en otros países. Método: se realizó una búsqueda con las palabras clave “Obesity” AND “Diet” AND “intervention” en tres bases de datos documentales: Pubmed, EBSCO, Scielo. Criterios de inclusión: edad de la muestra (2-17 años, basado en el estudio Enkid), que el programa de intervención incluyera dieta, ensayos clínicos publicados entre el enero de 2.010 y abril de 2014, cuya fuente sean revistas científicas indexadas. Resultados: se han analizado 490 artículos, de los cuales sólo fueron seleccionados 13 artículos. El 85 % de los programas intervención son aplicados fuera de España; sólo el 23 % fueron realizadas en ambiente escolar; el 70% de las intervenciones fueron llevadas a cabo en niños menores de 12 años; el 62% de las investigaciones destacan el uso de la intervención multidisciplinar (dieta, ejercicio y en ocasiones, cambios de conducta); y se muestra una oscilación en la duración de estas intervenciones entre 3 y 24 meses. Discusión: se han encontrado pocas intervenciones a nivel nacional, en participantes obesos y se diferencia dos tipos de intervenciones, a corto y largo plazo. Conclusiones: se recomienda promover programas de intervención de pérdida de peso en España, sobre todo en centros docentes. El uso exclusivo de dieta, ha resultado efectiva a corto plazo. Y a largo plazo, la intervención multidisciplinar obtiene mejores resultados de pérdida, sobre todo de masa grasa.

A principios del siglo XX el yacimiento arqueológico de Empúries abrió sus puertas a las visitas. Desde aquellos momentos hasta nuestros días numerosos yacimientos de la zona están preparados para ser visitados. Estas actuaciones, en muchas ocasiones, se encuentran relacionadas con su uso como elementos de atracción turística. En este trabajo se analizan las características del uso de estos yacimientos por parte de sus visitantes y se hace un especial hincapié en el interés de situar estas visitas en el contexto más general de las estancias turísticas o las salidas excursionistas.

Surveyed 45 therapists who had participated in a family intervention for schizophrenia training program to examine the difficulties they had encountered, their recall of the intervention strategies, and the extent that they thought the approach had become integrated in their everyday work. Between 6 mo and 3 yrs after the family training, Ss reported the number of families they had systematically treated, and the difficulties they had encountered. Allowance of time to undertake the intervention, afterhours scheduling, and illness or holidays presented particular difficulties. Only 4% reported that their knowledge of behavioral techniques was a problem, but in a written test most therapists did not display minimum recall of the material of cognitive therapy, social skills training, or behavioral strategies. The study demonstrated significant problems in disseminating cognitive-behavioral approaches to multidisciplinary settings.

Background: Cardiac patients with diabetes are at higher readmission rates (22%) compared to only 6% for those patients without diabetes. Evidence shows benefits of peer support and using information technology to improve chronic illness and achieve better health outcomes. However limited evidence suggests that cardiac or diabetes self-management programs incorporating peer supporters (patients with similar conditions) or telephone and text-messaging, have improved health outcomes and reduce health care utilisations. A multidisciplinary research team approach is crucial to accommodate the complex aspects of delivering intervention programs for these at-risk patients. However, challenges such as the inconsistency in significance of key concepts across research fields, as well as practical and operational issues within different contexts are often experienced. Aims: To develop an effective multidisciplinary team approach to deliver a peer support based cardiac-diabetes self-management program incorporating the preparation of lay personnel to provide telephone and text-messaging follow up support. Methods: The approach was used for a multidisciplinary project using randomised controlled trial. Results: The findings from multidisciplinary team approach reveal the feasibility of a Peer support based cardiac-diabetes self-management program.

Introduction Patients post sepsis syndromes have a poor quality of life and a high rate of recurring illness or mortality. Follow-up clinics have been instituted for patients postgeneral intensive care but evidence is sparse, and there has been no clinic specifically for survivors of sepsis. The aim of this trial is to investigate if targeted screening and appropriate intervention to these patients can result in an improved quality of life (Short Form 36 health survey (SF36V.2)), decreased mortality in the first 12 months, decreased readmission to hospital and/or decreased use of health resources. Methods and analysis 204 patients postsepsis syndromes will be randomised to one of the two groups. The intervention group will attend an outpatient clinic two monthly for 6 months and receive screening and targeted intervention. The usual care group will remain under the care of their physician. To analyse the results, a baseline comparison will be carried out between each group. Generalised estimating equations will compare the SF36 domain scores between groups and across time points. Mortality will be compared between groups using a Cox proportional hazards (time until death) analysis. Time to first readmission will be compared between groups by a survival analysis. Healthcare costs will be compared between groups using a generalised linear model. Economic (health resource) evaluation will be a within-trial incremental cost utility analysis with a societal perspective. Ethics and dissemination Ethical approval has been granted by the Royal Brisbane and Women’s Hospital Human Research Ethics Committee (HREC; HREC/13/QRBW/17), The University of Queensland HREC (2013000543), Griffith University (RHS/08/14/HREC) and the Australian Government Department of Health (26/2013). The results of this study will be submitted to peer-reviewed intensive care journals and presented at national and international intensive care and/or rehabilitation conferences.

Childhood obesity and physical inactivity are increasing dramatically worldwide. Children of low socioeconomic status and/or children of migrant background are especially at risk. In general, the overall effectiveness of school-based programs on health-related outcomes has been disappointing. A special gap exists for younger children and in high risk groups. This paper describes the rationale, design, curriculum, and evaluation of a multicenter preschool randomized intervention study conducted in areas with a high migrant population in two out of 26 Swiss cantons. Twenty preschool classes in the German (canton St. Gallen) and another 20 in the French (canton Vaud) part of Switzerland were separately selected and randomized to an intervention and a control arm by the use of opaque envelopes. The multidisciplinary lifestyle intervention aimed to increase physical activity and sleep duration, to reinforce healthy nutrition and eating behaviour, and to reduce media use. According to the ecological model, it included children, their parents and the teachers. The regular teachers performed the majority of the intervention and were supported by a local health promoter. The intervention included physical activity lessons, adaptation of the built infrastructure; promotion of regional extracurricular physical activity; playful lessons about nutrition, media use and sleep, funny homework cards and information materials for teachers and parents. It lasted one school year. Baseline and post-intervention evaluations were performed in both arms. Primary outcome measures included BMI and aerobic fitness (20 m shuttle run test). Secondary outcomes included total (skinfolds, bioelectrical impedance) and central (waist circumference) body fat, motor abilities (obstacle course, static and dynamic balance), physical activity and sleep duration (accelerometry and questionnaires), nutritional behaviour and food intake, media use, quality of life and signs of hyperactivity (questionnaires), attention and spatial working memory ability (two validated tests). Researchers were blinded to group allocation. The purpose of this paper is to outline the design of a school-based multicenter cluster randomized, controlled trial aiming to reduce body mass index and to increase aerobic fitness in preschool children in culturally different parts of Switzerland with a high migrant population.
