Aquest estudi multicèntric, fase IV analitza l’eficàcia i la seguretat del tractament amb dosi ajustada d’EPOCH amb Rituximab en infusió continua, en pacients no tractats prèviament amb mal pronòstic diagnosticats de limfomes B de cèl.lula gran. Material i mètodes. S’inclogueren 81 pacients, 68 d’aquests diagnosticats de Limfoma B difús de cèl.lula gran, 7 de Limfoma fol.licular grau III i 6 de Limfoma B primari de mediastí. Eren pacients de mal pronòstic segons IPI≥2 o IPIae≥1, ECOG/PS 0-4, estadi II-IV (estadi I si malaltia voluminosa al diagnòstic). Resultats. La edat mitjana era de 52 anys (rang, 21-77 anys), 92,5% d’alt risc segons l’IPIae. Es va obtenir resposta completa (RC i RCi) en el 79% dels pacients, la supervivència global i la supervivència lliure d’esdeveniment, a la mediana del temps de seguiment (34,3 mesos) va ser de 68% i 63,2%, respectivament. La supervivència lliure de malaltia als 5 anys va ser de 72,2% pels pacients que van assolir RC/RCi. Cap dels factors de risc de l’IPI o IPIae, ni els índexs per ells mateixos van resultar amb associació estadísticament significativa a la OS o PFS. Només es va obtenir associació entre els nivells de ß2 microglobulina (&3,5mg/l) i la PFS i OS (p=0,075 i p=0,015, respectivament). Es van reportar 54 episodis de neutropènia febril (11,5% dels cicles); i es van morir 27 pacients, 15 (55,5%) degut a progressió de la malaltia i 9 (33,3%) degut a sepsi o infecció grau 4. Conclusions. Els resultats aconseguits amb la combinació de Rituximab i DA-EPOCH mostren uns resultats prometedors i amb nivells de toxicitat molt acceptables en els pacients d’alt risc diagnosticats de Limfomes B de cèl.lula gran no tractats prèviament.

Este artigo tenta dar uma visão geral sobre a epoché fenomenológica, relacionando-a ao problema da constituição do saber fenomenológico, e também da fundamentação da Ciência. A epoché, formulada assim de forma universal é considerada como uma alteração radical da atitude natural.

Working memory (WM) is not a unitary construct. There are distinct processes involved in encoding information, maintaining it on-line, and using it to guide responses. The anatomical configurations of these processes are more accurately analyzed as functionally connected networks than collections of individual regions. In the current study we analyzed event-related functional magnetic resonance imaging (fMRI) data from a Sternberg Item Recognition Paradigm WM task using a multivariate analysis method that allowed the linking of functional networks to temporally-separated WM epochs. The length of the delay epochs was varied to optimize isolation of the hemodynamic response (HDR) for each task epoch. All extracted functional networks displayed statistically significant sensitivity to delay length. Novel information extracted from these networks that was not apparent in the univariate analysis of these data included involvement of the hippocampus in encoding/probe, and decreases in BOLD signal in the superior temporal gyrus (STG), along with default-mode regions, during encoding/delay. The bilateral hippocampal activity during encoding/delay fits with theoretical models of WM in which memoranda held across the short term are activated long-term memory representations. The BOLD signal decreases in the STG were unexpected, and may reflect repetition suppression effects invoked by internal repetition of letter stimuli. Thus, analysis methods focusing on how network dynamics relate to experimental conditions allowed extraction of novel information not apparent in univariate analyses, and are particularly recommended for WM experiments for which task epochs cannot be randomized.

Superposed epoch studies have been carried out in order to determine the ionospheric response at mid-latitudes to southward turnings of the interplanetary magnetic field (IMF). This is compared with the geomagnetic response, as seen in the indices K p, AE and Dst. The solar wind, IMF and geomagnetic data used were hourly averages from the years 1967–1989 and thus cover a full 22-year cycle in the solar magnetic field. These data were divided into subsets, determined by the magnitudes of the southward turnings and the concomitant increase in solar wind pressure. The superposed epoch studies were carried out using the time of the southward turning as time zero. The response of the mid-latitude ionosphere is studied by looking at the F-layer critical frequencies, f o F2, from hourly soundings by the Slough ionosonde and their deviation from the monthly median values, δf o F2. For the southward turnings with a change in B z of δB z > 11.5 nT accompanied by a solar wind dynamic pressure P exceeding 5 nPa, the F region critical frequency, f o F2, shows a marked decrease, reaching a minimum value about 20 h after the southward turning. This recovers to pre-event values over the subsequent 24 h, on average. The Dst index shows the classic storm-time decrease to about −60 nT. Four days later, the index has still to fully recover and is at about −25 nT. Both the K p and AE indices show rises before the southward turnings, when the IMF is strongly northward but the solar wind dynamic pressure is enhanced. The average AE index does register a clear isolated pulse (averaging 650 nT for 2 h, compared with a background peak level of near 450 nT at these times) showing enhanced energy deposition at high latitudes in substorms but, like K p, remains somewhat enhanced for several days, even after the average IMF has returned to zero after 1 day. This AE background decays away over several days as the Dst index recovers, indicating that there is some contamination of the currents observed at the AE stations by the continuing enhanced equatorial ring current. For data averaged over all seasons, the critical frequencies are depressed at Slough by 1.3 MHz, which is close to the lower decile of the overall distribution of δf o Fl values. Taking 30-day periods around summer and winter solstice, the largest depression is 1.6 and 1.2 MHz, respectively. This seasonal dependence is confirmed by a similar study for a Southern Hemisphere station, Argentine Island, giving peak depressions of 1.8 MHz and 0.5 MHz for summer and winter. For the subset of turnings where δB z > 11.5 nT and P ≤ 5 nPa, the response of the geomagnetic indices is similar but smaller, while the change in δf o F2 has all but disappeared. This confirms that the energy deposited at high latitudes, which leads to the geomagnetic and ionospheric disturbances following a southward turning of the IMF, increases with the energy density (dynamic pressure) of the solar wind flow. The magnitude of all responses are shown to depend on δB z . At Slough, the peak depression always occurs when Slough rotates into the noon sector. The largest ionospheric response is for southward turnings seen between 15–21 UT.

A new diatom species, Thalassiosira praeoestrupii Dumont, Baldauf and Barron, is described. The first occurrence of T. praeoestrupii in coastal California diatom-bearing outcrops occurs between the last occurrence of Rouxia californica at 6.0 Ma, and the first occurrence of Thalassiosira oestrupii at 5.1 Ma. The latter two species have customarily been used to identify the Miocene/Pliocene boundary. Paleomagnetic studies at Santa Cruz, California, demonstrate that the first occurrence of T. praeoestrupii coincides with the top of magnetic polarity Chron 5, which closely approximates the Miocene/Pliocene Epoch boundary.

The formation and evolution of galaxy bulges is a greatly debated topic in modern astrophysics. An approach to address this issue is to look at the Galactic bulge, the closest to us. According to some theoretical models, our bulge built-up from the merger of substructures formed from the instability and fragmentation of a proto-disk in the early phases of Galactic evolution. We may have discovered the remnant of one of these substructures: the stellar system Terzan 5. Terzan 5 hosts two stellar populations with different iron abundances, thus suggesting it once was far more massive than today. Moreover, its peculiar chemistry resembles that observed only in the Galactic bulge. In this Thesis we perform a detailed photometric and spectroscopic analysis of this cluster to determine its formation and evolutionary histories. Form the photometric point of view we built a high-resolution differential reddening map in Terzan 5 direction and we measured relative proper motions to separate its member population from the contaminating field stars. This information represents the necessary work to measure the absolute ages of Terzan 5 populations via the Turn-off luminosity method. From the spectroscopic point of view we measured abundances for more than 600 stars belonging to Terzan 5 and its surroundings in order to build the largest field-decontaminated metallicity distribution for this system. We find that the metallicity distribution is extremely wide (more than 1 dex) and we discovered a third, metal-poor and alpha-enhanced population with average [Fe/H]=-0.8. The striking similarity between Terzan 5 and the bulge in terms of their chemical formation and evolution revealed by this Thesis suggests that Terzan 5 formed in situ with the bulge itself. In particular its metal-poor populations trace the early stages of the bulge formation, while its most metal-rich component contains crucial information on the bulge more recent evolution.

Abundant Fe-Mn carbonate concretions (mainly siderite, manganosiderite, and rhodochrosite) were found in the hemipelagic claystones of Site 603 on the eastern North American continental rise. They occur as nodules, micronodules, or carbonate-replaced burrow fills and layers at a subbottom depth of between ~ 120 (Pliocene) and 1160 m (Albian-Cenomanian). In general, the Fe-Mn carbonate concretions form from CO3- produced by the microbiological degradation of organic matter in the presence of abundant Fe + or Mn + and very low S- concentrations. However, there is also some evidence for diagenetic replacement of preexisting calcite by siderite. The carbon isotope composition of diagenetic Fe-Mn carbonate nodules is determined by CO2 reduction during methanogenesis. Carbonate nodules in Cretaceous sediments at sub-bottom depths of 1085 and 1160 m have distinctly lower d13C values (- 12.2 and - 12.9 per mil) than Neogene siderites, associated with abundant biogenic methane in the pore space (-8.9 to 1.7 per mil between 330 and 780 m depth). Since no isotopic zonation could be detected within individual nodules, we assume that the isotopic composition reflects more or less geochemical conditions at the present burial depth of the carbonate nodules. Carbonates did not precipitate within the zone of sulfate reduction (approximately 0.01 to 10 m), where all of the pyrite was formed. The oxygen isotope composition indicates precipitation from seawater-derived interstitial waters. The d18O values decrease with increasing burial depth from + 5.1 to - 1.2 per mil, suggesting successively higher temperatures during carbonate formation.

This chapter summarizes the principal results of drilling at Deep Sea Drilling Project (DSDP) Site 595, where the Ngendei Seismic Experiment and the emplacement of DARPA's Marine Seismic System (MSS) were carried out. Background and objectives for this work are presented in the introductory chapter to this volume. Interpretation of the seismic experiment and drilling results are presented in subsequent parts of this volume. The chapter also provides a detailed operational summary of the successful deployment of the MSS during Leg 91.

Originally, we had planned to piston core at Site 595 in order to meet the sedimentologic and biostratigraphic objectives outlined in the introductory chapter. However, consultation with our colleagues, Thomas Jordan and John Orcutt on board Melville, indicated that coring near the ocean bottom seismometer (OBS) array around Hole 595B could alter the programmed signal to noise ratio above which teleseisms trigger recording in the OBSs. They requested that we core no closer than about 8 km from three OBSs nearest Hole 595B, and selected a target for us about that distance to the west. Since a new beacon was required at this distance, a new site number, 596, was designated. Briefly, we planned to obtain oriented hydraulic piston cores to the top of the cherts, then core through the cherts using the extended core barrel (XCB) to basement. With improved recovery, we hoped to reach the sediment/basalt contact, and thus obtain a reliable biostratigraphic determination of the basement age. We planned to obtain at least one core in basement, perhaps more, with time permitting. We planned no geophysical program for the hole.

Twenty-four sediment samples from DSDP Holes 605 (Leg 93) and 613 (Leg 95) on the New Jersey continental rise were analyzed by pyrolysis-gas chromatography. Twelve of these samples were also analyzed by pyrolysis-gas chromatography/mass spectrometry. The degree of preservation of sediment organic matter, as determined by these techniques, helped to distinguish slumped sediments from sediments that have not moved from their original place of deposition. Total levels of pyrolyzable organic material, as determined from pyrolysis-gas chromatography, were low in sediments that were not slumped, indicating that the organic material is highly degraded. Nitrogen- and oxygen-containing compounds were the primary compounds detected by gas chromatography/mass spectrometry (GCMS) analysis of the pyrolyzate of non-slumped sediments. Smaller amounts of aromatic compounds and branched alkanes were also present in some of these samples. In contrast, slumped sediments showed larger amounts of pyrolyzable organic matter, as determined from pyrolysis-gas chromatography, and better preservation of alkyl chains in the sediment organic matter, as suggested by the presence of n-alkanes in GCMS analysis of the pyrolyzate. Better preservation of the organic matter in slumped sediments can be attributed to more moderate bioturbation by bottom-dwelling organisms at the original deposition site.
