CIsH20N3Oa+.C1-.H2 O, M r = 395, orthorhombic, Pn21a, a = 7.710 (4), b = 11.455 (3), c -- 21.199 (3)/k, Z = 4, V = 1872.4/k 3, D m = 1.38, D C = 1.403 g cm -3, F(000) = 832, g(Cu Kct) = 20.94 cm -l. Intensities for 1641 reflections were measured on a Nonius CAD-4 diffractometer; of these, 1470 were significant. The structure was solved by direct methods and refined to an R index of 0.045 using a blockdiagonal least-squares procedure. The angle between the least-squares planes through the benzene rings is 125.0 (5) ° and the side chain is folded similarly to one of the independent molecules of imipramine hydrochloride.

Background Wandering represents a major problem in the management of patients with Alzheimer’s disease (AD). In this study we examined the utility of the Algase Wandering Scale (AWS), a newly developed psychometric instrument that asks caregivers to assess the likelihood of wandering behavior. Methods The AWS was administered to the caregivers of 40 AD patients and total and subscale scores were examined in relation to measures of mental and functional status, depressive symptoms and medication usage. Results AWS scores were comparable, though slightly lower, than those normative values previously published. Higher scores were associated with more severe dementia. The Negative Outcome subscale showed a significant increase in reported falls or injuries in association with anti-depressant use. Conclusions These data provide some construct validation for the AWS as a potentially useful scale to assess wandering behaviors in AD.

The title compound, C16H18N2O2, is an important precursor in the synthesis of 1,2,3,4-tetrahydropyrazinoindoles, which show excellent antihistamine, antihypertensive and central nervous system depressant properties. The carbethoxy group attached to C2 and the planar cyanoethyl group attached to N1 make dihedral angles of 11.0(4) and 75.0(3)degrees, respectively, with the mean plane of the indole ring, The C-C=N chain is linear with a bond angle of 179.3 (4)degrees.

The interactions of dextrin with biotite mica and galena have been investigated through adsorption, flotation, and electrokinetic measurements. The adsorption densities of dextrin onto mica continuously increase with increase of pH, while those onto galena show a maximum at pH 11.5. It is observed that the adsorption density of dextrin onto galena is quite high compared to that on mica. Both the adsorption isotherms exhibit Langmuirian behavior. Electrokinetic measurements portray conformational rearrangements of macromolecules with the loading, resulting in a shift of the shear plane, further away from the interface. Dissolution experiments indicate release of the lattice metal ions from mica and galena. Coprecipitation tests confirm polymer-metal ion interaction in the bulk solution. Dextrin does not exhibit any depressant action toward mica, whereas, with galena, the flotation recovery is decreased with an increase in pH beyond 9, in the presence of dextrin, complementing the adsorption results. Differential flotation results on a synthetic mixture of mica and galena show that mica can be selectively separated from galena using dextrin as a depressant for galena above pH 10. Possible mechanisms of interaction between dextrin and mica/galena are discussed.

Separation of Mussorie rock phosphate (P2O5 = 20%) from Uttar Pradesh, India, containing pyrite, calcite and other carbonaceous impurities by flotation has been successfully attempted to upgrade the phosphate values. Based on Hallimond cell flotation results of single and synthetic mineral mixtures of calcite and apatite using oleic acid and potassium phosphate, conditions were obtained for the separation of calcite from apatite which is considered to be the most difficult step in the beneficiation of calcareous phosphates. Further studies using 250 g of the mineral (−60 +150 and −150 mesh fractions, deslimed) in laboratory size Fagergren subaeration machine employed a stagewise flotation viz. carbonaceous materials using terpineol, pyrite using potassium-ethyl xanthate and calcite using oleic acid respectively. Separation was, however, found to be unsatisfactory in the absence of a depressant. Among starch, hydrofluosilicic acid and dipotassium hydrogen phosphate, which were tried as depressants for apatite in the final flotation stage, dipotassium hydrogen phosphate proved to be superior to others. However, the tests with the above fractions did not yield the required grade. This was possibly due to insufficient liberation of the phosphate mineral from the ore body and different experimental conditions due to scale up operations. Experiments conducted using −200 mesh deslimed fractions has yielded an acceptable grade of 27.6% P2O5 with a recovery of about 60%. The results have been explained in terms of the specific adsorption characteristics of phosphate ions on apatite and the liberation size of the mineral.

Spider venoms contain a plethora of insecticidal peptides that act on neuronal ion channels and receptors. Because of their high specificity, potency and stability, these peptides have attracted much attention as potential environmentally friendly insecticides. Although many insecticidal spider venom peptides have been isolated, the molecular target, mode of action and structure of only a small minority have been explored. Sf1a, a 46-residue peptide isolated from the venom of the tube-web spider Segesteria florentina, is insecticidal to a wide range of insects, but nontoxic to vertebrates. In order to investigate its structure and mode of action, we developed an efficient bacterial expression system for the production of Sf1a. We determined a high-resolution solution structure of Sf1a using multidimensional 3D/4D NMR spectroscopy. This revealed that Sf1a is a knottin peptide with an unusually large β-hairpin loop that accounts for a third of the peptide length. This loop is delimited by a fourth disulfide bond that is not commonly found in knottin peptides. We showed, through mutagenesis, that this large loop is functionally critical for insecticidal activity. Sf1a was further shown to be a selective inhibitor of insect voltage-gated sodium channels, consistent with its 'depressant' paralytic phenotype in insects. However, in contrast to the majority of spider-derived sodium channel toxins that function as gating modifiers via interaction with one or more of the voltage-sensor domains, Sf1a appears to act as a pore blocker.

Selective separation of pyrite from galena and quartz was achieved through microbiologically induced flotation in presence of Bacillus subtilis. B. subtilis functions as a depressant for pyrite while it promotes the flotation of galena and quartz. Bacterial extracellular protein (EP) was isolated and the protein profile of bacterial cells grown in presence and absence of minerals established.

Separation of molybdenite from chalcopyrite from the bulk concentrate of a low grade molybdenite ore from Chintamani, Kolar District, Karnataka, has been attempted in a modified Hallimond tube microflotation set-up. The flotation parameters studied are, sodium sulphide and potassium cyanide as depressants for chalcopyrite, pH, depressant concentration, steaming time and time of flotation. Selectivity index was used as a separation parameter. Steaming for 25 min followed by flotation for 7 min at 80 mg/l concentration of potassium cyanide and pH 9 gave the best results for separation of molybdenite. 19 ref.--AA

The interaction of guar gum with biotite mica has been investigated through adsorption, flotation and electrokinetic measurements. The adsorption densities of guar gum increase with increase of pH and the isotherms exhibit Langmuirian behaviour. Pretreatment of mica with a complexing agent such as EDTA results in a decrease in the adsorption density, highlighting the contribution of metal ions to the adsorption process. An increase in the surface face-to-edge ratio lends to an increase in the adsorption density. The flotation recoveries decrease as a function of pH, complementing the adsorption results. However, polymer depressant ability is reduced in the case of EDTA treated mica, consequent to reduction of metallic sites. Electrokinetic measurements portray conformational rearrangements of macromolecules with the loading, resulting in the shift of the shear plane, further away from the interface. Dissolution experiments indicate release of metal ions from mica, while co-precipitation tests confirm polymer-metal ion interaction in the bulk solution. Selective separation of haematite from alumina and silica/calcite was achieved through microbiologically induced flotation and flocculation in presence of Bacillus subtilis. Bacterial metabolites containing extracellular proteins were characterized from mineral-grown bacterial cell free extract. Bacteria can adhere to mineral surfaces and influence subsequent flotation of the minerals. Cells and metabolic products of bacteria were used in flotation, flocculation and adsorption studies on oxide minerals. Bacteria functions as a stronger depressant for haematite. Selective affinity of the bacterial cells towards the mineral surface was observed through adsorption studies. Bacterial byproduct like extracellular protein (EP) was isolated from bacteria. The protein profile of the EP of bacterial cells grown in presence and absence of minerals (haematite, corundum, quartz and calcite) was also studied. The role of such proteins in selective mineral separation was demonstrated through microbially induced selective flotation. This study has demonstrated the utility and amenability of microbially induced mineral beneficiation through the use of bacterially generated metabolic products and mineral-grown bacterial cells. Adsorption, electrokinetic, microflotation, and flocculation studies have been carried out on sphalerite and galena minerals using extracellular polysaccharides (ECP) isolated from Bacillus polymyxa. The adsorption density of ECP onto galena is found to be higher than that onto sphalerite. The adsorption of ECP onto sphalerite is found to increase from pH 3 to about pH 7, where a maximum is attained, and thereafter continuously decreases. With respect to galena, the adsorption density of ECP steadily increases with increased pH. The addition of ECP correspondingly reduces the negative electrophoretic mobilities of sphalerite and galena in absolute magnitude without shifting their isoelectric points. However, the magnitude of the reduction in the electrophoretic mobility values is found to be greater for galena compared to that for sphalerite. Microflotation tests show that galena is depressed while sphalerite is floated using ECP in the entire pH range investigated. Selective flotation tests on a synthetic mixture of galena and sphalerite corroborate that sphalerite could be floated from galena at pH 9-9.5 using ECP as a depressant for galena. Flocculation tests reveal that in the pH range 9-11, sphalerite is dispersed and galena is flocculated in the presence of ECP. Dissolution tests indicate release of the lattice metal ions from galena and sphalerite, while co-precipitation tests confirm chemical interaction between lead or zinc ions and ECP. Fourier transform infrared spectroscopic studies provide evidence in support of hydrogen bonding and chemical interaction for the adsorption of ECP onto galena/sphalerite surfaces. 1、KMBZ-009 改善高台应激所致认知障碍和应激相关的抑郁样行为及其相关机理 研究。 虽然适当的应激会提高动物的学习记忆功能,但过量的应激特别是无法逃避 的应激,往往导致依赖海马或前额叶的学习记忆功能受损,这与应激改变脑内应 激激素(皮质酮,皮质醇等)和神经递质的释放,影响突触传递和可塑性(包括 长时程增强和长时程抑制,LTP 和LTD)有关。一些疾病的发生、发展和恶化, 比如抑郁症(Depression)、创伤后应激障碍(PTSD),往往也和应激相关联,其 神经化学基础被证实与内分泌系统和单胺类(如五羟色,去甲肾上腺素,多巴胺) 神经递质系统的功能密切相关。遗憾的是,到目前为止还没有发现能治疗应激的 药物。本实验室过去的研究证实:KMBZ-009(申报新药时的名称为芬克罗酮,英 文名Phenchlobenpyrrone)——一种新的取代吡咯烷酮类化合物,通过调节细胞 内钙,改变脑内神经递质的释放,从而影响脑高级功能。KMBZ-009 对神经递质 释放影响是否能减轻应激导致的认知障碍及应激相关疾病的发生还没有进行研 究。本研究采用Morris 水迷宫、行为操作箱、绝望游泳、膜片钳和活体电生理 技术研究了KMBZ-009 对高台应激所致认知障碍和应激相关的抑郁样行为的影响 及其相关机理。 研究结果发现,高台应激或皮质酮注射造成大鼠空间记忆提取障碍,这与其 导致的海马CA1 区突触可塑性改变有关,而KMBZ-009 能成剂量依赖性地逆转应 激对空间记忆提取的损伤作用,这与它阻断应激或皮质酮异化的LTD 和恢复应激 或皮质酮损伤的LTP 密切相关。KMBZ-009 能部分地降低因应激而升高的血清皮 质酮含量,此外,KMBZ-009 对大鼠海马CA1 区锥体神经元的兴奋和抑制电流的 影响可能也参与了其对应激的调节作用。KMBZ-009 能显著增加海马CA1 区锥体 神经元上AMPA 受体介导的兴奋性突触后电流(EPSC)的幅度,但不影响其动力 学特性。NMDA 受体介导的EPSC 不受KMBZ-009 的影响;GABA 受体介导的抑制突 触后电流(IPSC)的幅度几乎不受KMBZ-009 的影响,而其受体动力学特性明显 被KMBZ-009 改变,表现为IPSC 恢复的时间显著延长。KMBZ-009 对CA1 区兴奋 抑制电流的调节作用,使大鼠海马细胞具有更强的维持细胞稳态的能力,从而避免应激导致神经元功能的损害。KMBZ-009 对抗应激对认知得损伤作用提示其可 能会减少动物的抑郁样行为,本实验结果发现,KMBZ-009 确实能明显减少小鼠 在强迫游泳(FST)中的不动时间,增加大鼠在72 秒低频差式强化(DRL-72s) 模型中的强化率,并降低其反应率。其机制是KMBZ-009 增加正常动物中枢神经 系统胞外NE 水平,激活alpha 和beta 肾上腺素受体,从而使得实验动物的抑郁 样行为明显减少。 2、KMBZ-009 减轻氧化应激对细胞活力、线粒体电位及海马LTP 的损伤作用。 前人的研究表明,氧自由基过多是导致老年痴呆患者和老年人神经细胞凋亡 与认知障碍的因素之一。KMBZ-009 和阿尼西坦是吡咯烷酮类化合物,研究显示 均具有促智作用。有报道指出阿尼西坦能减少神经胶质细胞在缺血缺氧时氧自由 基的生成,从而避免细胞受到氧应激损伤。本研究采用神经元原代培养和离体电 生理学方法,观察了KMBZ-009 和阿尼西坦对氧应激神经元的保护作用。结果发 现,KMBZ-009 和阿尼西坦均能保护氧应激神经元的线粒体的功能,对抗氧自由 基对神经元细胞活力的损伤,从而有效逆转了氧化应激对海马脑片CA1 区LTP 的 损伤作用。KMBZ-009 的作用效果比阿尼西坦的效果强10 倍。 3、银杏叶提取物及复方制剂改善老年大鼠空间学习记忆的突触可塑性机理。 有研究表明,银杏叶和三七叶提取物能调节神经系统的功能。本研究采用 Morris 水迷宫和活体电生理技术研究了银杏三七复方制剂及银杏叶提取物(以 标准银杏叶提取物——金纳多作为阳性对照药)改善老年大鼠空间学习记忆障碍 的突触可塑性机理研究。结果发现:老年大鼠空间学习记忆能力较差,高频诱导 不能在其海马CA1 区引发LTP,当长期服用金纳多或复方制剂一个月后,老年动 物的空间学习记忆功能得到明显改善,这可能与药物增强海马LTP 有密切关系。 复方制剂的作用效果与金纳多的效果相当。 4、悬尾应激损伤避暗作业学习行为的多巴胺D1 受体机制。 近年来的研究表明,DA 系统对应激非常敏感,应激改变PFC 内DA 的含量, 从而导致依赖于PFC 的工作记忆受损。但目前尚不知道应激对DA 系统的影响是 否涉及依赖杏仁核和海马的情绪学习记忆功能。因此,我们采用被动回避作业和 行为药理学的方法,初步探讨了此问题。结果发现:和对照组动物相比,随着悬 尾应激持续时间的增加(5min、10min、20min),动物在避暗作业作业重测试中的步入潜伏期明显缩短,当动物被悬尾应激后回到鼠笼中休息20min,其步入潜 伏期无明显变化;腹腔注射DA D1 受体拮抗剂SCH23390 呈剂量依赖性地缩短动 物的步入潜伏期,但SCH23390 腹腔注射和悬尾应激共同处理实验动物时,此种 D1 受体拮抗剂能有效逆转应激对步入潜伏期的影响;进一步的研究发现,应激 或D1 受体拮抗剂对痛觉感受的影响不是其改变动物步入潜伏期的主要因素。本 研究结果表明悬尾应激导致脑内多巴胺释放过度增加,杏仁核(可能还有海马及 相关神经回路)内的D1 受体被过度激活,从而导致小鼠在操作被动回避任务时 的记忆获得障碍。

Duchenne Muscular Dystrophy (DMD) is a fatal multi-system neuromuscular disease caused by loss of dystrophin. The loss of dystrophin from membranes of contractile muscle cells and the dysregulation of the DAPC, induces chronic inflammation due to tissue necrosis and eventual replacement with collagen which weakens muscular force and strength. Dystrophin deficiency may cause under-diagnosed features of DMD include mood disorders such as depression and anxiety and dysfunction of the gastrointestinal tract. The first study in the thesis examined mood in the dystrophin-deficient mdx mouse model of DMD and examined the effects of the tri-cyclic antidepressant, amitriptyline on behaviours. Amitriptyline had anti-depressant and anxiolytic effects in the mdx mice possibly through effects on stress factors such as corticotrophin-releasing factor (CRF). This antidepressant also reduced skeletal muscle inflammation and caused a reduction in circulating interleukin (IL)-6 levels. In the second and third studies, we specifically blocked IL-6 signalling and used Urocortin 2, CRFR2 agonist to investigate their potential as therapeutic targets in mdx mice pathophysiology. Isometric and isotonic contractile properties of the diaphragm, were compared in mdx mice treated with anti IL-6 receptor antibodies (anti IL-6R) and/or Urocortin 2. Deficits in force production, work and power detected in mdx mice were improved with treatment. In study three I investigated contractile properties in gastrointestinal smooth muscle. As compared to wild type mice, mdx mice had slower faecal transit times, shorter colons with thickened muscle layers and increased contractile activity in response to recombinant IL-6. Blocking IL-6 signalling resulted in an increase in colon length, normalised faecal output times and a reduction in IL-6-evoked contractile activity. The findings from these studies indicate that for both diaphragm and gastrointestinal function in a dystrophin-deficient model, targeting of IL-6 and CRFR2 signalling has beneficial therapeutic effects.

Introduction: Copayments for prescriptions are associated with decreased adherence to medicines resulting in increased health service utilisation, morbidity and mortality. In October 2010 a 50c copayment per prescription item was introduced on the General Medical Services (GMS) scheme in Ireland, the national public health insurance programme for low-income and older people. The copayment was increased to €1.50 per prescription item in January 2013. To date, the impact of these copayments on adherence to prescription medicines on the GMS scheme has not been assessed. Given that the GMS population comprises more than 40% of the Irish population, this presents an important public health problem. The aim of this thesis was to assess the impact of two prescription copayments, 50c and €1.50, on adherence to medicines.Methods: In Chapter 2 the published literature was systematically reviewed with meta-analysis to a) develop evidence on cost-sharing for prescriptions and adherence to medicines and b) develop evidence for an alternative policy option; removal of copayments. The core research question of this thesis was addressed by a large before and after longitudinal study, with comparator group, using the national pharmacy claims database. New users of essential and less-essential medicines were included in the study with sample sizes ranging from 7,007 to 136,111 individuals in different medication groups. Segmented regression was used with generalised estimating equations to allow for correlations between repeated monthly measurements of adherence. A qualitative study involving 24 individuals was conducted to assess patient attitudes towards the 50c copayment policy. The qualitative and quantitative findings were integrated in the discussion chapter of the thesis. The vast majority of the literature on this topic area is generated in North America, therefore a test of generalisability was carried out in Chapter 5 by comparing the impact of two similar copayment interventions on adherence, one in the U.S. and one in Ireland. The method used to measure adherence in Chapters 3 and 5 was validated in Chapter 6. Results: The systematic review with meta-analysis demonstrated an 11% (95% CI 1.09 to 1.14) increased odds of non-adherence when publicly insured populations were exposed to copayments. The second systematic review found moderate but variable improvements in adherence after removal/reduction of copayments in a general population. The core paper of this thesis found that both the 50c and €1.50 copayments on the GMS scheme were associated with larger reductions in adherence to less-essential medicines than essential medicines directly after the implementation of policies. An important exception to this pattern was observed; adherence to anti-depressant medications declined by a larger extent than adherence to other essential medicines after both copayments. The cross country comparison indicated that North American evidence on cost-sharing for prescriptions is not automatically generalisable to the Irish setting. Irish patients had greater immediate decreases of -5.3% (95% CI -6.9 to -3.7) and -2.8% (95% CI -4.9 to -0.7) in adherence to anti-hypertensives and anti-hyperlipidaemic medicines, respectively, directly after the policy changes, relative to their U.S. counterparts. In the long term, however, the U.S. and Irish populations had similar behaviours. The concordance study highlighted the possibility of a measurement bias occurring for the measurement of adherence to non-steroidal anti-inflammatory drugs in Chapter 3. Conclusions: This thesis has presented two reviews of international cost-sharing policies, an assessment of the generalisability of international evidence and both qualitative and quantitative examinations of cost-sharing policies for prescription medicines on the GMS scheme in Ireland. It was found that the introduction of a 50c copayment and its subsequent increase to €1.50 on the GMS scheme had a larger impact on adherence to less-essential medicines relative to essential medicines, with the exception of anti-depressant medications. This is in line with policy objectives to reduce moral hazard and is therefore demonstrative of the value of such policies. There are however some caveats. The copayment now stands at €2.50 per prescription item. The impact of this increase in copayment has yet to be assessed which is an obvious point for future research. Careful monitoring for adverse effects in socio-economically disadvantaged groups within the GMS population is also warranted. International evidence can be applied to the Irish setting to aid in future decision making in this area, but not without placing it in the local context first. Patients accepted the introduction of the 50c charge, however did voice concerns over a rising price. The challenge for policymakers is to find the ‘optimal copayment’ – whereby moral hazard is decreased, but access to essential chronic disease medicines that provide advantages at the population level is not deterred. This evidence presented in this thesis will be utilisable for future policy-making in Ireland.
