Respiratory therapy has historically been considered the primary role of the physiotherapist in neonatal intensive care in Australia. In 2001 a survey was undertaken of all level three neonatal intensive care units in Australia to determine the role of the physiotherapist and of respiratory therapy in clinical practice. It appears that respiratory therapy is provided infrequently, with the number of infants treated per month ranging from 0 to 10 in 15 of the 20 units who provide respiratory therapy, regardless of therapist availability. The median number of respiratory treatments per month during the week was three, and on weekends it was one. Respiratory therapy was carried out by physiotherapists and nurses in 54.6% of units, by physiotherapists only in 36.4% of units, and by nurses only in the remaining 9% of units surveyed. There was also a diminution of the role of respiratory therapy in the extubation of premature infants. A review of the literature shows that overall the use of respiratory therapy reflects current evidence. The question remains whether it is possible to maintain the competency of staff and justify the cost of training in the current healthcare economic climate. It seems probable that the future role of physiotherapists in neonatal intensive care unit may be in the facilitation of optimal neurological development of surviving very low birth weight infants.

PRINCIPLES: Respiratory care is universally recognised as useful, but its indications and practice vary markedly. In order to improve the appropriateness of respiratory care in our hospital, we developed evidence-based local guidelines in a collaborative effort involving physiotherapists, physicians and health service researchers. METHODS: Recommendations were developed using the standardised RAND appropriateness method. A literature search was conducted based on terms associated with guidelines and with respiratory care. A working group prepared proposals for recommendations which were then independently rated by a multidisciplinary expert panel. All recommendations were then discussed in common and indications for procedures were rated confidentially a second time by the experts. The recommendations were then formulated on the basis of the level of evidence in the literature and on the consensus among these experts. RESULTS: Recommendations were formulated for the following procedures: non-invasive ventilation, continuous positive airway pressure, intermittent positive pressure breathing, intrapulmonary percussive ventilation, mechanical insufflation-exsufflation, incentive spirometry, positive expiratory pressure, nasotracheal suctioning and non-instrumental airway clearance techniques. Each recommendation referred to a particular medical condition and was assigned to a hierarchical category based on the quality of the evidence from the literature supporting the recommendation and on the consensus among the experts. CONCLUSION: Despite a marked heterogeneity of scientific evidence, the method used allowed us to develop commonly agreed local guidelines for respiratory care. In addition, this work fostered a closer relationship between physiotherapists and physicians in our institution.

Nine children surviving severe adult respiratory distress syndrome were studied 0.9 to 4.2 years after the acute illness. They had received artificial ventilation for a mean of 9.4 days, with an Fio2 greater than 0.5 during a mean time of 34 hours and maximal positive end expiratory pressure levels in the range of 8 to 20 cm H2O. Three children had recurrent respiratory symptoms (moderate exertional dyspnea and cough), and two had evidence of fibrosis on chest radiographs. All patients had abnormal lung function; the most prominent findings were ventilation inequalities, as judged by real-time moment ratio analysis of multibreath nitrogen washout curves (abnormal in eight of nine patients) and hypoxemia (seven of nine). Lung volumes were less abnormal; one patient had restrictive and two had obstructive disease. A significant correlation between intensive care measures (Fio2 greater than 0.5 in hours and peak inspiratory plateau pressure) and lung function abnormalities (moment ratio analysis and hypoxemia) was found. A possibly increased susceptibility of the pediatric age group to the primary insult or respiratory therapy of adult respiratory distress syndrome is suggested.

INTRODUCTION: Respiratory therapy is a keystone of the treatment for cystic fibrosis (CF) lung disease, but it is time consuming. OBJECTIVES: We aimed to assess the total time spent on respiratory therapy, including chest physiotherapy (CPT) and physical activity (PA), as well as inhalation therapy (IT) and maintenance of materials (MM) to rationalise and optimise treatment. METHODS: A cross-sectional prospective study in a paediatric CF cohort. A questionnaire was developed to look at the time spent on respiratory care over 3 months. Enrolled in this study are all CF patients aged from 6 to 16 years (the exclusion criterion was lung transplantation). RESULTS: Of the 40 enrolled patients, 22 participated (13 boys and 9 girls), with a mean age of 11 years. The patients spent approximately 19.46 h per week (standard deviation ± 7.53, 8.00-35.25 h) on therapy: CPT (30.58%), IT (15.11%), PA (50%) and MM (4.32%), without statistical significance between sexes. CONCLUSION: In our cohort, CF patients spent an average of nearly 20 h a week in respiratory therapy, within a wide range of between 8 h to almost 36 h a week. PA consumes almost half of the time. Physicians have to take into consideration the burden of the treatment, to optimise the therapy.

Background: Acute viral bronchiolitis is a respiratory disease with high morbidity that affects newborn in the first two years of life. Its treatment with physiotherapy has been highlighted as an important tool, however, there is no consensus regarding its effects on patients improvement. We aimed to evaluate the physiological parameters before and after the procedure respiratory therapy in newborn with acute viral bronchiolitis. Method: This was a cross sectional observational study in 30 newborns with acute viral bronchiolitis and indicated for physiotherapy care in a hospitalized Urgency and Emergency Unit. It was collected the clinical data of newborn through evaluation form, and we measured heart rate (HR), oxygen saturation (SpO2) and respiratory rate (RR). We measured the variables before physiotherapy treatment, 3, 6 and 9 minutes after the physiotherapy treatment. Results: There has been no change in HR, however, we observed a decrease in RR at 6 and 9 min compared to 3 min and increase in SpO2 at 3, 6 and 9 min compared to before physiotherapy. Conclusion: Respiratory physiotherapy may be an effective therapy for the treatment of newborn with Acute Viral Bronchitis.

Background: Advances in information technology have been widely used in teaching health care professionals. The use of multimedia resources may be important for clinical learning and we are not aware of previous reports using such technology in respiratory physical therapy education. Objectives: Our approach was to evaluate a conventional bronchial hygiene techniques (BHTs) course with an interactive online environment, including multimedia resources. Methods: Previous developed audiovisual support material comprised: physiology, physiopathology and BHTs, accessible to students through the Internet in conjunction with BHTs classes. Two groups of students were compared and both attended regular classes: the on-line group (n=8) received access to online resources, while the control group (n=8) received conventional written material. Student's performance was evaluated before and after the course. Results: A preliminary test (score 0 to 10) was applied before the beginning of the course, showing that the initial knowledge of both groups was comparable [online, 6.75 (SD=0.88) vs. control, 6.125 (SD=1.35); p>0.05]. Two weeks after the end of the course, a second test showed that the online group performed significantly better than the control group [respectively, 7.75 (SD=1.28) vs. 5.93 (SD=0.72); p>0.05]. Conclusions: The use of a multimedia online resource had a positive impact on student's learning in respiratory therapy field in which instrumental and manual resources are often used and can be explored using this technology.

Six full-term newborn infants are described who suffered from severe adult respiratory distress syndrome (ARDS). The triggering event was intrauterine/perinatal asphyxia in five, and group B streptococcal (GBS) septicemia in three. All had severe respiratory distress/failure and were ventilated mechanically with high concentrations of inspired oxygen and positive end-expiratory pressure. Radiography of the chest showed dense bilateral consolidation with air bronchograms and reduced lung volume. Persistent pulmonary hypertension (PPH) was documented in all cases. The coincidence of ARDS and PPH rendered respiratory management extremely difficult. For this reason high-frequency ventilation was instituted in all patients in order to improve CO2 elimination and induce respiratory alkalosis. Acute complications of respiratory therapy were encountered in five patients (pneumothorax, pulmonary interstitial emphysema, pneumopericardium). Three infants died (irreversible septic shock, progressive severe hypoxemia, and sudden cardiac arrest) after 17, 80, and 175 h of life. Histologic examination of the lungs was possible in all fatal cases and revealed typical changes of acute to subacute stages of ARDS. Three infants survived, the mean time of mechanical respiratory support being 703 h. Two patients were still dependent on oxygen after 1 month of life, and all survivors had increased interstitial markings and increased lung volumes on their chest roentgenograms at this time.

RESUMO: A fisioterapia caminha inevitavelmente para a especialização em determinadas áreas de intervenção (WCPT, 2007), nomeadamente na da fisioterapia respiratória. Este facto ocorre, por um lado, por se tratar de uma área de intervenção da fisioterapia com um papel específico e de carácter especializado e, por outro lado, devido à enorme procura deste tipo de serviços, causada também pelo aumento da prevalência de doenças respiratórias crónicas, que se tem vindo a verificar e que se prevê que se continue a verificar (OMS, 2005), e ainda à enorme transversalidade deste tipo de intervenção. A formação pós-graduada em fisioterapia respiratória, com o objectivo de fornecer aos formandos competências teórico-práticas que os apetreche para uma prática baseada na evidência, parece ser fundamental, mas a formação de adultos acarreta um compromisso com os princípios da aprendizagem específicos desta população. Os modelos de formação nesta área específica deverão incluir metodologias e conteúdos que cumpram objectivos reais, baseados não totalmente em curricula pré-estabelecidos, mas nas necessidades e expectativas de quem vive esta realidade, para além de se sustentarem nos referenciais teóricos. Assim, este estudo pretende avaliar a eficácia de um programa de formação pósgraduada em fisioterapia respiratória, baseado nas necessidades e expectativas dos sujeitos que estão envolvidos neste processo, e sustentado nos referenciais teóricos que se conhecem sob a forma de “standards”, “guidelines” ou recomendações. Os nossos objectivos específicos foram: a) Aferir quais as características que são importantes para garantir a eficácia de metodologia de um programa de formação pós-graduada na área da fisioterapia respiratória, ou seja, a “dinâmica” do processo. b) Aferir quais os conteúdos base que deverão fazer parte dessa formação e, c) Aferir se há vantagens distintas para formandos que já detenham experiência clínica e para recém-licenciados, e que frequentem o referido curso, ou seja, aferir o público-alvo. O desenho metodológico considerado para atingir os objectivos a que se propõe este estudo teve como base uma abordagem essencialmente qualitativa, mas recorreu a preciosas ferramentas quantitativas. Trata-se assim de uma abordagem de carácter misto. A nossa amostra, de conveniência, foi constituída pelos 22 formandos que se matricularam no único curso de pós-graduação em fisioterapia respiratória, que neste momento existe no nosso país. A recolha de dados obedeceu a uma estratégia comum nas abordagens qualitativas e que incluiu a “triangulação” de várias fontes e de vários métodos de recolha, a fim de permitir articular diferentes pontos de vista sobre um mesmo objecto. Assim, após a análise de referenciais teóricos considerados relevantes, recorreu-se à opinião de diferentes sujeitos, através de painéis, e de questionários, (questionários de opinião, questionários de caracterização, questionários de avaliação de conhecimentos e atitudes, à entrada e no final) bem como à avaliação de comportamentos, competências e atitudes, observados em ambiente simulado e real. A filosofia de formação que esteve subjacente ao nosso programa foi um constante compromisso entre a teoria, a prática e a clínica, em que o fenómeno de “transfert” foi potencializado por algumas características como o “fio condutor” entre os conteúdos, a presença de utentes reais nas aulas, os workshops temáticos e os estágios profissionais, a apresentação e discussão de estudos de caso com profissionais de referência, e os períodos de pausa entre seminários, em que estão facilitadas a reflexão, aplicação e experimentação de novas competências. Estas características estiveram na base dessa estreita relação entre a teoria, a prática e a clínica. Os critérios de selecção do corpo docente, capazes de funcionar como fontes de informação credível e actual e, sobretudo, capazes de funcionar como modelos profissionais, foram cruciais. Também o conhecimento das expectativas iniciais dos formandos e a monitorização facilitaram o processo de aprendizagem e o ajuste do programa ao longo do período de formação, assim como as avaliações formativas frequentes. Acreditamos que estas características do nosso programa foram fundamentais para a eficácia do mesmo, e que através delas obtivemos resultados positivos, em que se destacam a obtenção dos melhores resultados situados, sobretudo, nos níveis de complexidade relativos à resolução de problemas, e a valorização das competências ao do domínio afectivo. Corroborámos a teoria de que pessoas com formações e /ou experiências diferentes agem de maneiras diferentes, e aprendem de maneiras diferentes, e concluímos que os resultados nos estadios de aprendizagem e nas expectativas e necessidades colmatadas são generalizadamente positivos, o que torna qualquer dos grupos estudados, um público-alvo deste tipo de formação, embora possam ser apontadas algumas vantagens distintas conforme a experiência profissional de cada um, sendo os formandos com mais experiência, aqueles que mais evoluem em todos os domínios de aprendizagem. O aumento da empregabilidade dos formandos, directa ou indirectamente relacionado com a frequência do curso, parece ter sido um resultado positivo e inesperado. A possível caducidade do programa, o acompanhamento do grupo após o término do curso, o peso que os métodos de avaliação podem ter tido nos resultados, são algumas questões que ficam por esclarecer, e que merecem estudos futuros. Apesar disto, julgamos poder afirmar que este modelo de formação foi eficaz.

INTRODUCTION Hemodynamic resuscitation should be aimed at achieving not only adequate cardiac output but also sufficient mean arterial pressure (MAP) to guarantee adequate tissue perfusion pressure. Since the arterial pressure response to volume expansion (VE) depends on arterial tone, knowing whether a patient is preload-dependent provides only a partial solution to the problem. The objective of this study was to assess the ability of a functional evaluation of arterial tone by dynamic arterial elastance (Ea(dyn)), defined as the pulse pressure variation (PPV) to stroke volume variation (SVV) ratio, to predict the hemodynamic response in MAP to fluid administration in hypotensive, preload-dependent patients with acute circulatory failure. METHODS We performed a prospective clinical study in an adult medical/surgical intensive care unit in a tertiary care teaching hospital, including 25 patients with controlled mechanical ventilation who were monitored with the Vigileo(®) monitor, for whom the decision to give fluids was made because of the presence of acute circulatory failure, including arterial hypotension (MAP ≤65 mmHg or systolic arterial pressure <90 mmHg) and preserved preload responsiveness condition, defined as a SVV value ≥10%. RESULTS Before fluid infusion, Ea(dyn) was significantly different between MAP responders (MAP increase ≥15% after VE) and MAP nonresponders. VE-induced increases in MAP were strongly correlated with baseline Ea(dyn) (r(2) = 0.83; P < 0.0001). The only predictor of MAP increase was Ea(dyn) (area under the curve, 0.986 ± 0.02; 95% confidence interval (CI), 0.84-1). A baseline Ea(dyn) value >0.89 predicted a MAP increase after fluid administration with a sensitivity of 93.75% (95% CI, 69.8%-99.8%) and a specificity of 100% (95% CI, 66.4%-100%). CONCLUSIONS Functional assessment of arterial tone by Ea(dyn), measured as the PVV to SVV ratio, predicted arterial pressure response after volume loading in hypotensive, preload-dependent patients under controlled mechanical ventilation.
