Pesquisa qualitativa, de caráter descritivo-exploratório, realizada em uma clínica de hemodiálise, no interior do Paraná. A coleta de dados ocorreu com pacientes que se encontravam em tratamento hemodialítico, no período entre setembro e novembro de 2003, totalizando quinze sujeitos. O objetivo consistiu em conhecer situações significativas para o paciente renal crônico vivenciadas no espaço de hemodiálise. A coleta de dados ocorreu por meio de entrevistas semi-estruturadas e o material foi submetido à análise de conteúdo. Quatro categorias principais apontam os resultados do estudo: início do tratamento; possibilidade de vida; estar com o(s) outro(s) e organização do ambiente físico. Nos relatos, sobressaíram dificuldades associadas ao processo de comunicação, sendo que a falta de informações e o desconhecimento de aspectos que envolvem o tratamento resultam em um fator de insatisfação, no cotidiano vivenciado na clínica. O tratamento hemodialítico é apreendido como uma possibilidade de continuar vivendo, mas esta condição é iluminada e sustentada pela esperança de acessar o transplante renal, como única forma de libertação do próprio tratamento. Ainda ficou patente a atribuição de importância à família, à relação com os demais usuários e com os membros da equipe de saúde, como forma de contribuir para o processo de recuperação e de adaptação, em meio às adversidades impostas pela doença, mas também àquelas requeridas pelo tratamento. Aspectos relacionados à apresentação e estética do ambiente, como limpeza, temperatura e conforto das instalações, emergiram nas falas como fatores que também interferem na satisfação dos pacientes, amenizando o sofrimento e colaborando para o enfrentamento de condições adversas a que estão submetidos.

Exploratory descriptive study, prospective with quantitative approach, performed in the nefrology outpatient service from the Onofre Lopes University Hospital (HUOL), Natal/RN, aiming to identify aspects that concern Quality of Life (QOL) on transplanted and hemodialysis patients, as measured by WHOQOL-bref and describe the aspects that differentiate QL between the two groups as measured by the same instrument. The population was of 62 renal patients in hemodialysis and 58 transplantd, with data collected from february to march 2006. The instrument used was WHOQOL-bref, translated to and validated for Portuguese by the multicentric group of the World Health Organization (WHO) in Brazil. The results show predominance of the masculine gender both in the post-transplant group (55,17%), as well as in the hemodialysis group (51,61%); the age strip of 28 to 43 prevailed, among which were 53,45% transplanted and 48,99% hemodialysis patients; 79,93% of the hemodialysis patients and 62,07% of the transplanted were provenient from ouside the capital of the state; from the hemodialysis population, 59,68% were married, while among the transplanted 48,28% were single; 58,06% of the hemodialisys patients had 1 to 3 children, while the majority of the transplanted, 44,83%, had no children; regarding scholarity level on both groups, there was a predominance of incomplete fundamental school, representing 62,9% of the hemodialysis patients and 46,6% of the transplanted; regarding work status, 98,39% of those in hemodialysis informed not to be working, and the same applied to the transplanted, with 75,86%; regarding treatment time, most patients of both populations were in the 1- to 4-year interval, of which were 62,9% of the hemodialisys patients and 53,45% of the transplanted. The analysis of WHOQOL-bref reliability through Crobach s Alpha coefficient had a value of 0,8816, demonstrating good internal consistency for the instrument. Regarding description of QOL tests, the average scores of the post-transplant were (Q-1) 18,14 e (Q-2) 18,69, and 12,3 (Q-1) and 11,29 (Q-2) for the group in hemodialisys. Regarding the aspects that differentiate QOL on the two groups observed through average scores from the Domains: Physical, 15,91 for the post-transplant group and 12,71 for the hemodialisys group; Psychological, 16,75 for the post-transplant group and 14,84 for the hemodialisys group; Social Relations, 17,79 on the post-transplant group and 16,58 on the hemodialisys group; Environment, 14,16 on the post-transplant group and 12,38 on the hemodialisys group. On every evaluated item, the post-transplant group achieved higher scores when compared to the hemodialisys group. The diference in QOL for both populations was significant on all items evaluated with a p<0,005

O presente trabalho trata-se de estudo transversal e quantitativo que analisou o conhecimento dos profissionais de enfermagem sobre Eventos Adversos (EA) em uma unidade de hemodiálise de um hospital de ensino. A coleta dos dados ocorreu de fevereiro a abril de 2011, a partir de entrevistas com 25 profissionais. A análise dos dados identificou 517 relatos de 32 tipos, sendo os mais citados: cateter obstruído, retirada acidental da agulha e coagulação do sistema extracorpóreo. As causas relacionadas ao paciente foram mencionadas em 42,8% dos relatos. As principais condutas foram implementação/alteração de protocolos e educação continuada, sendo a última a principal sugestão para a prevenção. Os resultados podem contribuir para uma análise crítica sobre a qualidade do cuidado em unidades de hemodiálise, gerando o desenvolvimento de ações que auxiliem a promoção da segurança dos pacientes.

The nurses in the hemodialysis has an important role in the nursing process implementation, in the context of a theoretical referential. Among the nursing theories, highlights the Roy´s adaptations model, who considers a person as an holistic adaptive system that aims to adapt customers to different living conditions. Thus, it is believed that the Roy´s nursing process will guide nursing care to patients on dialysis. Therefore, the study aimed to analyze the nursing diagnosis present in patients with chronic kidney disease on hemodialysis based on the theoretical model of Roy and NANDA-International. Descriptive and cros-sectional study, performed at a dialysis center in a city in northeastern Brazil. Sample of 178 patients and consecutive sampling by convenience. Data collection ocurred from October/2011 until February/2012, through interview and physical examination forms. Data analysis was initiated by clinical reasoning, diagnosis judgment and similarity relation. Then, the data were entered into SPSS program, 16.0 version, generating descriptive statistics. The project was approved by the Ethics Research Committee (protocol nº 115/11) with a Presentation Certificate for Ethics Appreciation (in 0139.0.051.000-111) and was funded by the Universal edict MCT / CNPq 14/2010. The results revealed that most patients were male (52.2%), married (62.9%) and residents in the Natal´s metropolitan region (54.5%). The mean age was 46.6 years and the years of study, 8,5. Regarding nursing diagnosis obtained an average of 6.6, especially: Risk of Infection (100%), excessive fluid volume (99.4%) and hypothermia (61.8%). On the other hand the adaptive problems average was 6.4, and the most common: intracellular fluid retention (99.4%); Hyperkalemia (64.6%); Hypothermia (61.8%) and edema (53.9%). Were established 20 similarity relations between the NANDA-International nursing diagnosis and adaptive problems of Roy, namely: risk of falls / injury risk and potential for injury, impaired physical mobility and walking mobility and / or restricted coordination, dressing self-care deficit and loss of self-care ability; hypothermia and hypothermia; impaired skin integrity and impaired skin integrity; excessive fluid volume and intracellular fluid retention / Hyperkalemia / Hypocalcemia / edema; imbalanced nutrition: less than body requirements and Nutrition less than the body's needs; constipation and constipation, acute pain and acute pain, chronic pain and chronic pain, sensorial perception disturbed: visual, tactile and auditory disabilities and a primary sense: sight, hearing and tactile; sleep deprivation and insomnia, fatigue and intolerance to activities; ineffective self health and fails in the role; sexual dysfunction and sexual dysfunction; situational low self-esteem and low self-esteem, and diarrhea and diarrhea. We conclude that there is similarity between the typologies and was required a model´s analysis, because they present different ways to establish the nursing diagnosis. Moreover, the nursing process use, under the context of a theory and a classification system, subsidizes the care and contributes to the strengthening of nursing science

Trasnversal study, with the objective of evaluating the accuracy of clinical indicators of nursing diagnosis excessive fluid volume in patients undergoing hemodialysis. The study occurred in two stages, the first consisted of the evaluation of the diagnostic indicators in study; and the second, the diagnostic inference conducted by nurse diagnosticians. The first stage occurred from december 2012 to april 2013, in a University Hospital and a Hemodialysis Clinic in Northeastern of Brazil, with a sample of 100 chronic renal failure patients on hemodialysis. The data were selected through an interview form and a physical examination, organized into spreadsheets and analyzed as to the presence or absence of the indicators of diagnosis excessive fluid volume. In the second step, the spreadsheets were sent to three nurses diagnosticians, who judged the presence or absence of diagnosis in the clientele searched. This step was conducted from july to september 2013. For analysis of the data, we used descriptive and inferential statistics. In the descriptive analysis, we used measures of central tendency and dispersion. In inferential analysis, we used the tests Chi- square, Fisher and prevalence ratios. The accuracy of the clinical indicators pertaining to the diagnosis were measured as to the specificity, sensitivity, predictive values, likelihood ratios and Diagnostic Odds Ratio. Also developed a logistic regression. The results were organized in tables and discussed with literature. This study was approved by the Ethics Committee in Research of the Federal University of Rio Grande do Norte, with Presentation Certificate for Ethics Appreciation nº 08696212.7.0000.5537. The results revealed that the diagnosis studied was present in 82% of patients. The characteristics with prevalence above 50 % that stood out were: azotemia, decreased hematocrit, electrolyte imbalance, intake exceeds output, anxiety, edema, decreased hemoglobin, oliguria and blood pressure changes. Eight defining characteristics were presented statistically significant association with the nursing diagnosis investigated: pulmonary congestion, intake exceeds output, electrolytes imbalance, jugular vein distension, edema, weight gain over short period of time, agitation and adventitious breath sounds. Among these, the 10 characteristics which showed higher prevalence ratios were: edema and weight gain over short period of time. The features with the highest sensitivity were edema, electrolytes imbalance and intake exceeds output and the standing out with greater specificity were: anasarca, weight gain over short period of time, change in respiratory pattern, adventitious breath sounds, pulmonary congestion, agitation and jugular vein distension. The indicators jugular vein distension, electrolytes imbalance, intake exceeds output, increased central venous pressure and edema, together, were identified in the logistic regression model as the most significant predictors. It is concluded that the identification of accurate clinical indicators allow a good prediction of the nursing diagnosis of excessive fluid volume in patients undergoing hemodialysis in order to assist the nurse in the inference process, which will contribute to the success of patient care. In addition, nurses will consider for diagnostic inference not only his clinical experience, but also scientific evidence of the occurrence of excessive fluid volume, contributing to the control of volemia in these patients

The study aimed to analyze the nursing diagnoses of the nutrition domain from NANDA International in patients undergoing hemodialysis. This is a transversal study conducted in a university hospital in northeastern Brazil, with 50 hemodialysis patients. The data collection instrument was an interview form and a physical examination, in digital format, applied between the months of December 2013 to May 2014. Data analysis was divided into two stages. In the first, defining characteristics, related factors and risk factors were judged as to their presence by the researcher, according to the data collected. In the second stage, based on data from the first, diagnostic inference by experts was held. The results were organized in tables and analyzed using descriptive and inferential statistics for the diagnoses that showed higher frequencies than 50%. The project was approved by the Ethics Committee responsible for the research institution (protocol number 392 535), with Certificate Presentation to Ethics Assessment 18710613.4.00005537 number. The results indicate a median of 7 (± 1.51) nursing diagnoses of the nutrition domain per patient. Six diagnoses with greater frequency than 50% were identified, namely: Risk for electrolyte imbalance, Risk for unstable blood glucose level, Excess fluid volume, Readiness for enhanced fluid balance, Readiness for enhanced nutrition and Risk for deficient fluid volume. The defining characteristics, related and risk factors presented an average of 34.78 (± 6.86), 15.50 (± 3.40) and a median of 4 (± 1.93), respectively, and 11 of these components had statistically significant association with the respective diagnoses. Were identified associations between adventitious breath sounds, edema and pulmonary congestion with the diagnosis Excess fluid volume; Expressed desire to increase fluid balance with the nursing diagnosis Readiness for enhanced fluid balance; It feeds regularly, Attitude to food consistent with the health goals, Consume adequate food, expresses knowledge about healthy food choices, expresses desire to improve nutrition, expresses knowledge about liquid healthier choices and following appropriate standard supply with diagnosis Readiness for enhanced nutrition. It is concluded that the diagnosis of the nutrition domain related to electrolyte problems are prevalent in customer submitted to hemodialysis. The identification of these diagnoses contributes to the development of a plan of care targeted to the needs of these clients, providing better quality of life and advance in the practice of care

The concept analysis process carefully examines the description and uses of a word or term, enabling the standardization of language, in addition to providing representation to the profession, and facilitate the work of taxonomies. The aim of the study was to analyze the concept of nursing diagnosis ineffective self-health in patients undergoing hemodialysis. Study concept analysis, based on Walker and Avant model and operationalized through integrative literature review. The databases searched were: SCOPUS, CINAHL, PUBMED, LILACS and COCHRANE, with descriptors: Selfmanagement, Adherence and Hemodialysis. The inclusion criteria were: articles published in the last five years, complete articles are available free in selected databases; articles available in Portuguese, English or Spanish; and articles that address the self-concept of health, the antecedents and the consequent. And Exclusion: editorials, letters to the editor, theses and dissertations. The survey of the articles occurred in the months from January to March 2014. The initial sample of 16785 articles, with 11748 in PUBMED, 4767 in Scopus, 174 in CINAHL, the Cochrane 70 and 26 in LILACS. After applying the criteria, 76 articles were selected, 19 in CINAHL, 18 in PUBMED, 30 in Scopus, and 9 in LILACS. In analyzing the data, given that the concept was sought in the literature was self-health, was held interpretation to ineffective self-health diagnosis through the transposition in the denial of the attributes, antecedents and consequences identified. It is noteworthy that the terms identified in the literature as defining characteristics and related factors of the diagnosis under study were added to the survey, not even the transposition into opposite term is possible. The results show that the concept developed for the inefficient self-health diagnosis was: the patient's inability to control habits and achieve the negotiated with professionals therapeutic targets, resulting in health complications. 33 antecedents relating to social, psychological and therapeutic aspects and 16 consequential, involving physiological, social, psychological and therapeutic aspects were identified. Thus, it is concluded that the ineffective self-health concept is broad and involves individual patient factors and the therapeutic relationship between patient and professionals. It is believed that the study contributed to the improvement of diagnosis in renal clientele, besides being an important base for the growth of the scientific body of nursing, subsidizing the development of own technology area

Methodological study, in order to validate the content of the nursing diagnosis protection ineffective in patients undergoing hemodialysis. The research took place in two stages, namely: concept analysis and content analysis of the instrument by experts. T he first step was operationalized through an integrative review of the databaes LILACS, CINAHL, PubMed, Scopus and Cochrane, with the key words protection and hemodialysis, in October and November 2013. The sample consisted of 32 articles, which were analyz ed by a c areful reading to identify the sections that correspond ed to the defining attributes , antecedents and consequences of protection in patients undergoing hemodialysis. T he interpretation for the diagnosis of effective protection was made by transpos ing the components of the diagnosis (definition, defining characteristics and related factors) to the denial form . In the second stage, we elaborated an instrument with the components of the nursing diagnostic s studied and proceeded to the analysis conduct ed in April 2014 by 22 specialists in nephrology and in the terminology of the NANDA International, selected by means of th e Lattes Platform . We used the binomial test to assess the proportion of experts who rated each item as appropriate, considering a si gnificance level of 5%. The project was approved by the Ethics Committee of the institution responsible for the research, an opinion on num b er 387 837 and CAAE 18486413.0.0000.5537. The results show that the proposal for the nursing diagnosis of ineffectiv e protection in patients undergoing hemodialysis is: definition - the same as that presented in the NANDA International Taxonomy II, location - domain safety / protection and class injury. Related factors are: Absence of routine vaccines; Non - adherence to care related to vascular access; Non - adherence to infection control measures; Non - adherence to prescribed diet; Non - adherence to drug therapy; Presence of comorbidities; Drug abuse; Immune disorders; Extremes of age; Abnormal blood profiles; Drugs that red uce immunity; and side effects and adverse treatment - related. The defining characteristics are: Presence of invading the bloodstream; Nutritional disorders; Increase in the number of hospitalizations; Uncontrolled dry weight; Infected vascular access; Vasc ular inadequate access; Increased blood pressure; fever; Bleeding disorder; Disability immunity; fatigue; weakness; itching; and maladaptive response to stress. It follows that the identification of the defining attributes, antecedents and consequences inc reased the wealth of vocabulary, allowing the construction of theoretical and empirical definitions for a broader understanding of the concept protection. Furthermore, the study contributed to the enrichment of nursing specific body of knowledge, as well a s in the direction of nursing care for patients undergoing hemodialysis.

OBJETIVO: Relatar o processo de investigação da contaminação da água e a conseqüente avaliação do surto ocorrido no serviço de hemodiálise. MÉTODOS: Em setembro de 2000, 16 pacientes sob terapia de hemodiálise de um hospital em Minas Gerais apresentaram reações hemolíticas compatíveis a sintomas de intoxicação por cloro e cloramina em água. Foi feita a medição das concentrações de cloro e cloramina em amostras coletadas em diversos pontos do sistema de tratamento e distribuição de água do serviço inspecionado. A identificação dos casos ocorridos durante o período de estudo foi feita pela revisão das anotações de prontuários dos pacientes. Foi feita a revisão dos procedimentos da equipe técnica, médica e de enfermagem por meio de entrevistas. RESULTADOS: A taxa de sintomas foi significativamente alta (p<0,028) durante o período epidêmico em relação ao período pré-epidêmico. Os pacientes afetados fizeram hemodiálise com água tratada por osmose reversa e usaram capilares dialisadores reprocessados manualmente. A água e os capilares apresentaram, durante o período epidêmico, concentrações residuais acima dos padrões desejáveis impostos pela Portaria 82 do Ministério da Saúde/GM, que exige o máximo de 0,5 mg/l para cloro e 0,1 mg/l para cloramina. Risco relativo de 2,58 (1,06 a 6,28) caracteriza elevadas concentrações de cloro livre e cloramina quanto à apresentação dos sintomas nos expostos. CONCLUSÃO: A vigilância dos procedimentos é necessária para que a água utilizada no processo de hemodiálise atenda aos padrões mínimos recomendados.

Os autores deste artigo desenvolvem algumas reflexões sobre o aspecto da comunicação, na prática do profissional enfermeiro junto ao paciente renal crônico, em programa de hemodiálise, cujo tratamento propicia uma relação prolongada e, muitas vezes, mais profunda. Analisa e exemplifica a necessidade de usarmos, de f orma consciente, alguns modos de comunicação verbal e o não-verbal para que possamos estabelecer uma interação terapêutica para o cuidar desses pacientes.

Este artigo tem como objetivo mostrar a utilização da história oral de vida como referencial metodológico para obtenção de dados qualitativos, buscando compreender o significado da hemodiálise e o impacto desta modalidade terapêutica na vida de pacientes do serviço de Hemodiálise do Hospital Universitário da Universidade de São Paulo.

Realizado estudo prospectivo em um grupo de 21 pacientes portadores de insuficiência renal crônica que apresentavam hipotensão arterial no decorrer da hemodiálise. Avaliada a pressão arterial durante duas sessões com dialisato a 35(9)C e duas a 37°C, observou-se que as pressões sistólica e diastólica, nas temperaturas estudadas, mostraram diferenças estatisticamente significativas quando comparadas aos valores iniciais pré-diálise, queda progressiva das pressões com prevalência de episódios hipotensivos na 3(5) e 4(5) horas de tratamento em ambas temperaturas, diminuição de 7,69% das hipotensões com dialisato a 35ºC e importante queixa de sensação de frio, tornando o tratamento desconfortável.

Utilizando a técnica de Tábuas de Vida de Seguimento o presente estudo estimou a sobrevida de pacientes submetidos à hemodiálise, segundo grupo etário, de 1997 a 2000, em Ribeirão Preto-SP A partir das probabilidades de sobrevivência estimou também, os gastos por paciente que inicie o tratamento. Cerca de 60% dos pacientes seguidos continuavam vivos ao final dos quatros anos de estudo, apontando para uma sobrevivência elevada. Os resultados mostram ainda que o crescimento dos gastos com hemodiálise deve-se ao aumento da sobrevida e não especificamente a uma ampliação da demanda pela terapia.

Submeter-se à Hemodiálise quatro horas por dia, três vezes por semana, pode configurar-se uma experiência carregada de tédio, além de causar desconforto durante sua realização. É comum os pacientes relatarem que o tempo parece se arrastar ou durar mais. O objetivo deste estudo foi conhecer a influência de dois diferentes ritmos musicais nos estados subjetivos e na percepção tempo-sensorial de pacientes adultos submetidos à hemodiálise, uma vez que a literatura sobre essa temática é escassa. O estudo foi realizado em um hospital privado, com 43 pacientes em hemodiálise, que se submeteram a duas sessões de improvisação musical em um teclado, nas quais os estados subjetivos e a percepção temporal foram avaliados pré e pós-intervenção. Mais de 80% dos pacientes sentiu o tempo passar mais rápido após as intervenções em ambos os ritmos. No entanto, o ritmo influenciou o tipo de experiência emocional que os pacientes apresentaram.
