O câncer bucal e de laringe representam um problema crescente de saúde pública no Brasil. O tabagismo e o álcool são considerados os principais causadores do câncer bucal e de laringe, porém uma parte da população desenvolve a doença sem estar exposta a estes fatores de risco, sugerindo a existência de outras causas como: predisposição genética, alteração de genes supressores tumorais, dieta e agentes virais, em particular o Papilomavírus humano (HPV) e o vírus Epstein-Barr (EBV). Este estudo teve como proposição verificar a prevalência do HPV e do EBV na mucosa oral normal e no câncer bucal e de laringe, e quais os tipos mais prevalentes nestas duas situações. Para este estudo foram estabelecidos dois grupos: um composto por 70 espécimes emblocados em parafina, com diagnóstico confirmado de câncer bucal e laringe e outro com 166 indivíduos sem presença de lesões na cavidade bucal. A análise laboratorial para detecção viral do HPV e a detecção e tipagem do EBV (EBV 1 ou tipo 2) foram realizadas através da técnica de PCR (Reação em Cadeia de Polimerase) convencional. Já as tipagens das amostras positivas para o HPV (tipos 6, 11, 16, 18, 33, 35, 38, 52 e 58) foram realizadas por PCR em tempo real, utilizando sondas específicas para cada tipo. A prevalência do HPV e EBV encontrada nas neoplasias orais e de laringe foi de 78,6% para HPV e de 84,3% para EBV e de 24,1% e 45,8% para HPV e EBV, respectivamente, em indivíduos sem lesões orais. Os tipos mais prevalentes de HPV foram HPV 58 (50,9%), HPV 6 (9,1%) e HPV 16 (9,1%) nas neoplasias e HPV 18 (12,5%), HPV 6 (7,5%) e HPV 58 (2,5%) no grupo com ausência de lesões. O EBV 2 foi mais prevalente tanto nas lesões neoplásicas quanto nos indivíduos sem lesões, com frequência de 94,9% e 82,9%, respectivamente. Não houve associação da infecção por HPV e EBV com o sexo, sendo a prevalência semelhante para homens e mulheres. Foi observada associação entre as prevalências de HPV e EBV e suas co-infecções com o grupo que desenvolveu câncer. A prevalência de infecção por HPV e EBV e a razão de chances na ocorrência do câncer foi de 8,86 (p<0,0001) nos indivíduos infectados pelo HPV e de 4,08 (p=0,0004) nos infectados pelo EBV. O valor probabilístico estimado para prevalência de HPV e EBV e co-infecção e a ocorrência de câncer, demonstrou que o indivíduo infectado pelos dois vírus tem 65,72% de probabilidade de desenvolver câncer, enquanto o infectado pelo HPV tem 31,94% e o infectado pelo EBV 17,79%. Os resultados encontrados neste estudo permitem sugerir que os agentes virais (HPV e EBV) são fatores de risco importantes para o desenvolvimento da carcinogênese, sendo o HPV mais efetivo que o EBV no desencadeamento da doença.

Indivíduos imunocomprometidos possuem maior risco de desenvolver linfomas associados ao EBV. A detecção desse vírus em sangue periférico e a determinação de sua carga viral podem ter importância na evolução clínica de indivíduos portadores do HIV. Foram avaliadas 156 amostras de pacientes HIVpositivos pela reação em cadeia da polimerase em tempo real (qPCR) para detecção e quantificação da carga viral do EBV. 123/156 (78,8%) casos apresentaram carga viral detectável para o EBV, sendo que a carga viral média foi de 6,9x10-3 cópias de EBV/célula. Foi detectada elevada carga viral do EBV em indivíduos com falha terapêutica ou sem HAART (p =0,0076), em coinfectados pelos EBVs 1 e 2 (p=0,0205), em pacientes com altas cargas de HIV (rho=0,27614, p=0,0005) e longos períodos de infecção pelo HIV (rho= 0,24164, p =0,0026) e os que apresentavam altos níveis de linfócitos T CD8 + (rho=0,19286, p =0,0159). A amplificação do gene EBNA-2 para realização da tipagem viral foi possível em 95/123 (77,2%) amostras, das quais 72 (75,8%) revelaram infecção pelo EBV-1, 9 (9,5%) pelo EBV-2 e 14 (14,7%) apresentavam coinfecção entre os EBVs 1 e 2. Esses dados estão de acordo com a literatura visto que o tipo 1 é predominante em países ocidentais e 70,0% da coorte era composta por indivíduos caucasianos e heterossexuais. A maioria dos pacientes que apresentaram coinfecção pelos EBVs 1 e 2 tiveram contagem de linfócitos T CD4 + entre 200 e 499 células/μL de sangue segundo classificação CDC (p =0,0272). Quanto a analise do gene BNLF-1, a amplificação foi possível em 99/123 (80,5%). Desses 50/99 (50,5%) apresentavam a deleção de 30pb no gene, enquanto 49/99 (49,5%) não a possuíam. The Epstein-Barr virus (EBV) is the etiological agent of oral hairy leukoplakia (OHL), an oral lesion with important diagnostic and prognostic value in acquired immunodeficiency disease syndrome. The two EBV genotypes, EBV-1 and EBV-2, can be distinguished by divergent gene sequences encoding the EBNA-2, 3A, 3B, and 3C proteins. The purpose of this study was to identify the EBV genotype prevalent in 53 samples of scrapings from the lateral border of the tongue of HIV-1 seropositive patients, with and without OHL, and to correlate the genotypes with presence of clinical or subclinical OHL with the clinic data collected. EBV-1 and EBV-2 were identified through PCR and Nested-PCR based on sequence differences of the EBNA-2 gene. EBV-1 was identified in the 31 samples (15 without OHL, 7 with clinical OHL and 9 with subclinical OHL), EBV-2 in 12 samples (10 without OHL, 1 with clinical and 1 subclinical OHL), and a mixed infection in 10 samples (2 without OHL, 3 with clinical and 5 with subclinical OHL). The presence of EBV-1 was higher in women, but a significant statistical result relating one the EBV genotypes to the development of OHL was not found. We conclude that the oral epithelium in HIV-1 seropositive patients can be infected by EBV-1, EBV-2 or by a mixed viral population.

Patients infected with the human immunodeficiency virus (HIV) are at higher risk of developing Epstein-Barr Virus (EBV)-associated lymphomas. The usefulness of monitoring EBV in peripheral blood mononuclear cells (PBMCs) of patients infected with HIV has not been established. The aim of this study was to evaluate the EBV viral load in PBMCs, the frequency of viral genotypes, and the presence of the 30-bp deletion in the BNLF-1 gene. DNA samples from 156 patients attending the HIV/AIDS Day Clinic at Botucatu School of Medicine, Sao Paulo State University were evaluated. The EBV viral load was detectable by real time PCR in 123/156 (78.8%) cases and was higher in patients not receiving antiretroviral treatment or under therapeutic failure than in patients under successful highly active antiretroviral therapy (HAART) (P=0.0076). Overall, the profile of patients with high EBV viral load included elevated HIV viremia (P=0.0005), longer time of HIV diagnosis (P=0.0026), and increased levels of T CD8 + lymphocytes (P=0.0159). The successful amplification of the EBNA-2 gene by nested-PCR was achieved in 95 of 123 (77.2%) cases, of which 75.8% were EBV-1, 9.5% EBV-2, and 14.7% were co-infected with both EBV-1 and -2. The analysis of the BNLF-1 gene was possible in 99 of 123 (80.5%) cases, of which 50.5% had the 30-bp deletion. Two genetic events contribute to the development of endemic Burkitt lymphoma (BL) infection of B lymphocytes with Epstein-Barr virus (EBV) and the activation of the protooncogene c-myc through chromosomal translocation. The viral genes EBV nuclear antigen 2 (EBNA2) and latent membrane protein 1 (LMP1) are essential for transformation of primary human B cells by EBV in vitro; however, these genes are not expressed in BL cells in vivo. To address the question whether c-myc activation might abrogate the requirement of the EBNA2 and LMP1 function, we have introduced an activated c-myc gene into an EBV-transformed cell line in which EBNA2 was rendered estrogen-dependent through fusion with the hormone binding domain of the estrogen receptor. The c-myc gene was placed under the control of regulatory elements of the immunoglobulin kappa locus composed a matrix attachment region, the intron enhancer, and the 3' enhancer. We show here that transfection of a c-myc expression plasmid followed by selection for high MYC expression is capable of inducing continuous proliferation of these cells in the absence of functional EBNA2 and LMP1. c-myc-induced hormone-independent proliferation was associated with a dramatic change in the growth behavior as well as cell surface marker expression of these cells. The typical lymphoblastoid morphology and phenotype of EBV-transformed cells completely changed into that of BL cells in vivo. We conclude that the phenotype of BL cells reflects the expression pattern of viral and cellular genes rather than its germinal center origin.

Primary Hodgkin`s lymphoma (HL) of the stomach is an extremely rare entity. Most cases of gastric involvement by HL are observed in the setting of disseminated disease. The nonspecific nature of the symptoms and endoscopic findings, which include a large malignant-looking ulcer and mass or wall thickening, together with the considerable histological overlap between HLs and some non-HLs or undifferentiated carcinoma, make the surgical resection diagnosis extremely difficult. An accurate diagnosis is important as treatment and outcome differ significantly for these neoplasms. In small endoscopic gastric biopsies and even in postoperative specimens, the precise histological diagnosis of HL is particularly challenging. Here, the authors report 5 cases of 2 women and 3 men aged 22 to 68, with gastric involvement by classic HLs-3 primary gastric HLs and 2 as part of widespread disease. All 5 patients presented with digestive symptoms. At endoscopy, the lesions presented as ulcerated and elevated lesions, with or without mucosal thickening. Four patients were misdiagnosed in the preoperative biopsy or in the gastrectomy specimen. Association with Epstein-Barr virus (EBV) was detected in 4 cases, with a predominance of subtype A EBV. These cases illustrate the significant difficulties, both clinical and pathological, in achieving the diagnosis of HL involving the stomach in immunocompetent patients.

Twenty-four whole blood and serum samples were drawn from an eight year-old heart transplant child during a 36 months follow-up. EBV serology was positive for VCA-IgM and IgG, and negative for EBNA-IgG at the age of five years old when the child presented with signs and symptoms suggestive of acute infectious mononucleosis. After 14 months, serological parameters were: positive VCA-IgG, EBNA-IgG and negative VCA-IgM. This serological pattern has been maintained since then even during episodes suggestive of EBV reactivation. PCR amplified a specific DNA fragment from the EBV gp220 (detection limit of 100 viral copies). All twenty-four whole blood samples yielded positive results by PCR, while 12 out of 24 serum samples were positive. We aimed at analyzing whether detection of EBV-DNA in serum samples by PCR was associated with overt disease as stated by the need of antiviral treatment and hospitalization. Statistical analysis showed agreement between the two parameters evidenced by the Kappa test (value 0.750; p < 0.001). We concluded that detection of EBV-DNA in serum samples of immunosuppressed patients might be used as a laboratory marker of active EBV disease when a Real-Time PCR or another quantitative method is not available.

Epstein-Barr virus (EBV) is associated with several types of cancers including Hodgkin's lymphoma (HL) and nasopharyngeal carcinoma (NPC). EBV-encoded latent membrane protein 1 (LMP1), a multifunctional oncoprotein, is a powerful activator of the transcription factor NF-κB, a property that is essential for EBV-transformed lymphoblastoid cell survival. Previous studies reported LMP1 sequence variations and induction of higher NF-κB activation levels compared to the prototype B95-8 LMP1 by some variants. Here we used biopsies of EBV-associated cancers and blood of individuals included in the Swiss HIV Cohort Study (SHCS) to analyze LMP1 genetic diversity and impact of sequence variations on LMP1-mediated NF-κB activation potential. We found that a number of variants mediate higher NF-κB activation levels when compared to B95-8 LMP1 and mapped three single polymorphisms responsible for this phenotype: F106Y, I124V and F144I. F106Y was present in all LMP1 isolated in this study and its effect was variant dependent, suggesting that it was modulated by other polymorphisms. The two polymorphisms I124V and F144I were present in distinct phylogenetic groups and were linked with other specific polymorphisms nearby, I152L and D150A/L151I, respectively. The two sets of polymorphisms, I124V/I152L and F144I/D150A/L151I, which were markers of increased NF-κB activation in vitro, were not associated with EBV-associated HL in the SHCS. Taken together these results highlighted the importance of single polymorphisms for the modulation of LMP1 signaling activity and demonstrated that several groups of LMP1 variants, through distinct mutational paths, mediated enhanced NF-κB activation levels compared to B95-8 LMP1.

Epstein-Barr virus (EBV) has been consistently associated with multiple sclerosis (MS), but whether this virus is a trigger of MS remains undetermined. Recently, EBV-infected B cells recognized by activated CD8_ T cells have been detected in the meninges of autopsied MS patients. In addition, a strong EBV-specific CD8_ T cell response in the blood of patients with MS of recent onset was reported. Here, to further explore the putative relationship between MS and EBV, we assessed the EBV-specific cellular and humoral immune responses in the blood and the cerebrospinal fluid (CSF) of patients with early MS or other neurological diseases, separated into inflammatory (IOND) and non-inflammatory (NIOND) groups. The MS non-associated neurotropic herpesvirus cytomegalovirus (CMV) served as a control. Fifty-eight study subjects were enrolled, including 44 patients (13 with early MS (onset of MS less than one year prior to the assay), 15 with IOND and 16 with NIOND) in the immunological arm of the study. The cellular immune response was investigated using a functional CFSE cytotoxic T lymphocyte (CTL) assay performed with short-term cultured EBV- or CMVspecific effector T cells from the CSF and the blood. The humoral immune response specific for these two viruses was also examined in both the blood and the CSF. The recruitment of a given virusspecific antibody in the CSF as compared to the blood was expressed as antibody indexes (AI). We found that, in the CSF of early MS patients, there was an enrichment in EBV-, but not CMV-specific, CD8_ CTL as compared to the CSF of IOND (P_ 0.003) and NIOND patients (P_0.0009), as well as compared to paired blood samples (P_0.005). Additionally, relative viral capsid antigen (VCA)-, but not EBV encoded nuclear antigen 1 (EBNA1)- or CMV-specific, AI were increased in the CSF of early MS as compared to IOND (P_0.002) or NIOND patients (P_0.008) and correlated with the EBVspecific CD8_ CTL responses in the CSF (rs_0.54, P_0.001). Fourteen additional patients were enrolled in the virological arm of the study: using semi-nested PCR, EBV-encoded nuclear RNA1 (EBER1)-a transcript expressed during all stages of EBV infection-was detected in the CSF of 2/4 early MS, but only 1/6 IOND and 0/4 NIOND patients. Altogether, our data suggest that a reactivation of EBV, but not CMV, is taking place in the central nervous system of patients with MS of recent onset. These data significantly strengthen the link between EBV and MS and may indicate a triggering role of EBV in this disease. It was hypothesized that the EBV-specific CD8(+) T cell response may be dysregulated in multiple sclerosis (MS) patients, possibly leading to a suboptimal control of this virus. To examine the CD8(+) T cell response in greater detail, we analyzed the HLA-A2-, HLA-B7-, and HLA-B8-restricted EBV- and CMV-specific CD8(+) T cell responses in a high number of MS patients and control subjects using tetramers. Content in cytolytic granules, as well as cytotoxic activity, of EBV- and CMV-specific CD8(+) T cells was assessed. We found that MS patients had a lower or a higher prevalence of HLA-A2 and HLA-B7, respectively. Using HLA class I tetramers in HLA-B7(+) MS patients, there was a higher prevalence of MS patients with HLA-B*0702/EBV(RPP)-specific CD8(+) T cells ex vivo. However, the magnitude of the HLA-B*0702/EBV(RPP)-specific and HLA-B*0702/CMV(TPR)-specific CD8(+) T cell response (i.e., the percentage of tetramer(+) CD8(+) T cells in a study subject harboring CD8(+) T cells specific for the given epitope) was lower in MS patients. No differences were found using other tetramers. After stimulation with the HLA-B*0702/EBV(RPP) peptide, the production of IL-2, perforin, and granzyme B and the cytotoxicity of HLA-B*0702/EBV(RPP)-specific CD8(+) T cells were decreased. Altogether, our findings suggest that the HLA-B*0702-restricted viral (in particular the EBV one)-specific CD8(+) T cell response is dysregulated in MS patients. This observation is particularly interesting knowing that the HLA-B7 allele is more frequently expressed in MS patients and considering that EBV is associated with MS.

Epstein-Barr virus (EBV) has been associated with multiple sclerosis (MS), however, most studies examining the relationship between the virus and the disease have been based on serologies, and if EBV is linked to MS, CD8+ T cells are likely to be involved as they are important both in MS pathogenesis and in controlling viruses. We hypothesized that valuable information on the link between MS and EBV would be ascertained from the study of frequency and activation levels of EBV-specific CD8+ T cells in different categories of MS patients and control subjects. We investigated EBV-specific cellular immune responses using proliferation and enzyme linked immunospot assays, and humoral immune responses by analysis of anti-EBV antibodies, in a cohort of 164 subjects, including 108 patients with different stages of MS, 35 with other neurological diseases and 21 healthy control subjects. Additionally, the cohort were all tested against cytomegalovirus (CMV), another neurotropic herpes virus not convincingly associated with MS, nor thought to be deleterious to the disease. We corrected all data for age using linear regression analysis over the total cohorts of EBV- and CMV-infected subjects. In the whole cohort, the rate of EBV and CMV infections were 99% and 51%, respectively. The frequency of IFN-gamma secreting EBV-specific CD8+ T cells in patients with clinically isolated syndrome (CIS) was significantly higher than that found in patients with relapsing-remitting MS (RR-MS), secondary-progressive MS, primary-progressive MS, patients with other neurological diseases and healthy controls. The shorter the interval between MS onset and our assays, the more intense was the EBV-specific CD8+ T-cell response. Confirming the above results, we found that EBV-specific CD8+ T-cell responses decreased in 12/13 patients with CIS followed prospectively for 1.0 +/- 0.2 years. In contrast, there was no difference between categories for EBV-specific CD4+ T cell, or for CMV-specific CD4+ and CD8+ T-cell responses. Anti-EBV-encoded nuclear antigen-1 (EBNA-1)-specific antibodies correlated with EBV-specific CD8+ T cells in patients with CIS and RR-MS. However, whereas EBV-specific CD8+ T cells were increased the most in early MS, EBNA-1-specific antibodies were increased in early as well as in progressive forms of MS. Our data show high levels of CD8+ T-cell activation against EBV--but not CMV--early in the course of MS, which support the hypothesis that EBV might be associated with the onset of this disease.

To find Epstein-Barr virus (EBV) strains with genetic variations of EBV latent membrane protein 1 (EBV-LMP1) from nasopharyngeal carcinoma (NPC), the full-length DNA of LMP1 genes from 21 NPC biopsies obtained in Hunan province in southern China was amplified and sequenced. Our sequences were compared to those previously reported by the Clustal V method. Results showed that all 21 sequences displayed two amino acid changes most frequently in LMP1 of CD4+ T cell epitopes at codons 144 (F®I, 21/21) and 212 (G®S, 19/21) or (G®N, 2/21). We also show that type A EBV strain is prevalent in the cases of NPC from Hunan province with a 30-bp 18/21 deletion, and we highlight that this deletion resulted in loss of one of the CD4+ T cell-restricted epitopes. The other 3 sequences without this deletion all had a change at codon 344 (G®D). Furthermore, in the major epitope sequence of CD8+ T cells restricted by HLA-A2, all 21 sequences showed changes at codons 126 (L®F) and 129 (M®I). Our study discovered that one of the 21 sequence variations harbored a new change at codon 131 (W®C), and 5/21 specimens showed another novel change at codon 115 (G®A) in the major epitope sequence of CD8+ T cells restricted by HLA-A2. Our study suggests that these sequence variations of NPC-derived LMP1 may lead to a potential escape from host cell immune recognition, protecting latent EBV infection and causing an increase in tumorigenicity.

A utilização de dois critérios de seleção na pré-desmama, ganho médio diário do nascimento à desmama (GMD) e dias para ganhar 160 kg do nascimento à desmama (D160), foi estudada, analisando-se informações de 16.592 animais, provenientes do controle de desenvolvimento ponderal da Associação Brasileira dos Criadores de Zebu, nascidos no período de 1978 a 1994. Foram incluídos no modelo o efeito fixo de grupo de contemporâneos e os efeitos aleatórios genético aditivo de animal e materno, de ambiente permanente materno e o erro. A covariância entre os efeitos direto e materno foi considerada igual a zero. As estimativas dos componentes de variância e herdabilidade foram obtidas pelo método da máxima verossimilhança restrita e os valores genéticos preditos (VGs), por modelos animais uni-característica. As estimativas de herdabilidade foram: 0,12; 0,05; 0,10 e 0,05 para GMD (efeito direto), GMD (efeito materno), D160 (efeito direto) e D160 (efeito materno), respectivamente. Foram estimadas a correlação genética entre GMD e D160 (efeito direto e materno) e a correlação de classificação (Spearman) entre os valores genéticos para as categorias de touros, vacas e bezerros. As estimativas de correlação genética entre GMD e D160 foram 0,86 e 0,88, para o efeito direto e materno, respectivamente. As estimativas de correlação de ;rank;, também foram altas, entretanto, nenhuma foi igual a um, resultando em alterações na classificação dos animais. A relação entre as médias aritmética (A) e harmônica (H) e o desvio-padrão (S) do GMD ajustado para efeitos ambientais e maternos (GMDc) foi verificada utilizando-se um modelo restrito, sem intercepto, mediante as regressões linear e quadrática do S do GMDc sobre a diferença entre a média aritmética e média harmônica (A-H). Os resultados evidenciaram que, semelhantemente a H, o critério D160 apresentou a propriedade de discriminar touros com progênie mais uniforme.
