Se presenta un panorama y los interrogantes fundamentales de la etapa de la Web 3.0. Se analizan las características actuales de los sistemas bibliográficos estructurados con el modelo entidad-relación. Se definen los niveles conceptual, lógico y físico en los sistemas informáticos; consecuentemente se presentan las características de los FRBR y se obervan las relaciones entre obra y documento en el modelo conceptual FRBR. Se describen los FRBRoo como una interpretación con una lógica de objetos de los mismos requerimientos funcionales. Finalmente se plantean las tendencias a futuro, tales como pasar de las modelizaciones de entidad-relación a la de objetos, la explicitación con anotación semántica consistente, el mapeo de bases bibliográficas existentes y el desarrollo de ontologías para que los sistemas documentales se integren en la Web Semática

Statement of purpose The purpose of this concurrent session is to present the main findings and recommendations from a five year study evaluating the implementation of Early Warning Systems (EWS) and the Acute Life-threatening Events: Recognition and Treatment (ALERT) course in Northern Ireland. The presentation will provide delegates with an understanding of those factors that enable and constrain successful implementation of EWS and ALERT in practice in order to provide an impetus for change. Methods The research design was a multiple case study approach of four wards in two hospitals in Northern Ireland. It followed the principles of realist evaluation research which allowed empirical data to be gathered to test and refine RRS programme theory [1]. The stages included identifying the programme theories underpinning EWS and ALERT, generating hypotheses, gathering empirical evidence and refining the programme theories. This approach used a variety of mixed methods including individual and focus group interviews, observation and documentary analysis of EWS compliance data and ALERT training records. A within and across case comparison facilitated the development of mid-range theories from the research evidence. Results The official RRS theories developed from the realist synthesis were critically evaluated and compared with the study findings to develop a mid-range theory to explain what works, for whom in what circumstances. The findings of what works suggests that clinical experience, established working relationships, flexible implementation of protocols, ongoing experiential learning, empowerment and pre-emptive management are key to the success of EWS and ALERT implementation. Each concept is presented as ‘context, mechanism and outcome configurations’ to provide an understanding of how the context impacts on individual reasoning or behaviour to produce certain outcomes. Conclusion These findings highlight the combination of factors that can improve the implementation and sustainability of EWS and ALERT and in light of this evidence several recommendations are made to provide policymakers with guidance and direction for future policy development. À semelhança do que aconteceu em muitas instituições públicas, as universidades têm enfrentado pressões crescentes para mudar, tendo de repensar as suas formas de governança e de gestão, dando mais ênfase à implementação de sistemas de gestão do desempenho (SGD). Apesar de existirem vários estudos sobre o desempenho, estes têm ignorado o uso dado à informação recolhida. Além disso, e apesar de terem ocorrido várias reformas na governança destas instituições, existem ainda poucos estudos que relacionam a governança e o desempenho. Assim, esta pesquisa visa explorar a forma como as universidades medem, reportam e gerem o desempenho e como as estruturas de governança se relacionam com estas práticas. Para alcançar o objetivo proposto, um estudo comparativo entre universidades britânicas e portuguesas foi realizado. Os dados foram recolhidos através da utilização de uma metodologia qualitativa, sendo os métodos utilizados a análise documental e entrevistas semi-estruturadas a membros dos órgãos de governo e gestão de cada instituição. A análise dos dados mostrou a inexistência de um sistema completamente integrado de gestão de desempenho (SGD) em ambas as instituições, essencialmente devido à falta de práticas de gestão de desempenho. De facto, apesar de alguns dos entrevistados terem reportado o "uso positivo" de dados sobre o desempenho, alguns relataram o "não uso" desses dados, principalmente em relação ao desempenho individual, e outros o "mau uso" dessa informação, tendo sido reportadas práticas de gaming e deturpação dos resultados. Como forma de ultrapassar alguns destes problemas, verificou-se a co-existência de duas estruturas de governança: uma 'formal', da qual fazem parte todos os órgão de governo, com um valor mais 'simbólico'; e uma estrutura 'paralela', constituída por órgãos mais ágeis, que gerem a universidade no dia a dia. Verificou-se terem sido vários os fatores a afetarem, negativa e positivamente, os SGD em ambas as instituições, tendo sido rotulados de "inibidores" e "determinantes", respetivamente. A pesquisa mostrou que, apesar de as estruturas de governança serem importantes para a implementação e funcionamento de um SGD, há outros fatores que precisam de ser levados em consideração, nomeadamente, o nível de comunicação e o nível de envolvimento dos atores no processo. Estes dois fatores são considerados relevantes para a integração bem sucedida de práticas de medição, reporte e gestão de desempenho. Esta integração, juntamente com outras mudanças que ocorreram em termos de governança, contribuirá certamente para que se passe de um sistema em que se governa o desempenho para um sistema em que se governa para o desempenho.

Modeling of future water systems at the regional scale is a difficult task due to the complexity of current structures (multiple competing water uses, multiple actors, formal and informal rules) both temporally and spatially. Representing this complexity in the modeling process is a challenge that can be addressed by an interdisciplinary and holistic approach. The assessment of the water system of the Crans-Montana-Sierre area (Switzerland) and its evolution until 2050 were tackled by combining glaciological, hydrogeological, and hydrological measurements and modeling with the evaluation of water use through documentary, statistical and interview-based analyses. Four visions of future regional development were co-produced with a group of stakeholders and were then used as a basis for estimating future water demand. The comparison of the available water resource and the water demand at monthly time scale allowed us to conclude that for the four scenarios socioeconomic factors will impact on the future water systems more than climatic factors. An analysis of the sustainability of the current and future water systems based on four visions of regional development allowed us to identify those scenarios that will be more sustainable and that should be adopted by the decision-makers. The results were then presented to the stakeholders through five key messages. The challenges of communicating the results in such a way with stakeholders are discussed at the end of the article.

En el día de hoy nos enfrentamos a una serie de desafíos económicos, geopolíticos y ambientales que apuntan hacia una transformación profunda del mundo tal y como lo conocemos. La arquitectura esta – siempre lo ha estado – imbricada en todos estos problemas. En nuestra actual condición de recursos limitados e injusticia global surge la necesidad de superar la dicotomía entre progreso y tradición, entre innovación y preservación – la urgencia de redefinir incluso cada uno de estos términos. Los tipos de crisis que encaramos ahora no se resolverán con mas tecnología y más crecimiento – se necesita también una profunda reevaluación de nuestros sistemas de valores e incluso de nuestra sensibilidad. En este contexto, la humilde contribución de esta tesis consiste en proponer la idea de un realismo en arquitectura, entendido como una actitud progresista y atenta a lo que ya existe, dispuesta a abrir los ojos al presente, a las condiciones y los problemas reales que nos rodean. Pensando en esta idea, resulta evidente que si bien el realismo carece de una definición clara en el campo de la arquitectura hoy, cuenta sin embargo con numerosos precedentes parciales, entre ellos algunos de los más notorios episodios de la historia de la arquitectura reciente. Al intentar confeccionar una lista provisional de arquitectos realistas en el último siglo, otro hecho se nos presenta: muchos, por no decir todos estos arquitectos, comparten una fijación por la fotografía, bien apropiándose del trabajo de fotógrafos contemporáneos, bien tomando sus propias imágenes como una forma de mirar fuera de sus estudios e incorporar la realidad del medio construido a sus proyectos. Parece entonces lógico pensar que la arquitectura, como disciplina visual, ha acudido a otra disciplina visual en búsqueda de su propia respuesta al problema del realismo – dentro de este campo la fotografía reverbera con la arquitectura especialmente, ya que ambas son consideradas simultáneamente como practicas utilitarias y como parte de las bellas artes. Parece entonces lógico el organizar la investigación como una serie de casos, con la esperanza de que la acumulación de diversas instancias en las que la arquitectura ha acudido a la fotografía en su búsqueda de un realismo arrojara luz sobre las ideas más generales a debate. Por tanto, cada uno de los episodios en este texto se examina en sus propios términos, si bien una serie de interconexiones emergen a medida que el estudio procede con un suave orden cronológico. Hacia el final del texto cuestiones más grandes recobran protagonismo, a medida que las repercusiones políticas de nuestro estudio se hacen más patentes y comenzamos a interrogar las implicaciones contemporáneas y el potencial futuro de nuestra discusión sobre el realismo. ABSTRACT Today we are faced with a series of economic, geopolitical and environmental challenges that outline a deep transformation of the world as we know it. Architecture is – it has always been – ingrained in all of these problems. In our current condition of limited resources and global inequalities there is a necessity to overcome the dichotomy between progress and tradition, between innovation and preservation – an urgency to even redefine these terms altogether. The types of crises that we are facing will not be solved with more technology and more growth – a deep reevaluation of our systems of values and our sensibilities are also needed. In this context, the humble contribution of this text is to put forward the idea of an architectural realism, understood as an attitude that is both progressive and attentive to what is already in place, willing to open its eyes to the present and accept the real conditions and problems around us. In thinking about this prospect, it immediately becomes apparent that even if realism lacks a clear definition in the field of architecture today, there are numerous partial precedents for it, among them some of the most notorious episodes in the recent history of architecture. In crafting a tentative list of realist architects in the last century, another realization takes place: many, not to say all of these architects, have had a fixation with photography, either appropriating the work of contemporary photographers or taking photographs themselves as a way to look out their windows and bring the reality of the built environment into their practices. It seems then logical to think that architecture, as a visual discipline, has looked to other visual disciplines in search for its own take on the problem of realism – within this field, photography specially resonates with architecture, since both are regarded simultaneously as utilitarian practices and belonging to the fine arts. The idea then becomes to organize the research as a series of cases, with the hope that the accumulation of diverse instances in which architecture has approached photography in its realist drive will shed some light on the larger ideas at stake. Therefore, each of the episodes in this text is examined on its own terms, with a series of interconnections slowly emerging as our survey proceeds with a soft chronologic order. Towards the end of the study, larger issues regain relevance as the political repercussions of our inquiry become more pressing and we start to question the contemporary implications and future potentials of our discussion on realism.

Background Patient safety is concerned with preventable harm in healthcare, a subject that became a focus for study in the UK in the late 1990s. How to improve patient safety, presented both a practical and a research challenge in the early 2000s, leading to the eleven publications presented in this thesis. Research question The overarching research question was: What are the key organisational and systems factors that impact on patient safety, and how can these best be researched? Methods Research was conducted in over 40 acute care organisations in the UK and Europe between 2006 and 2013. The approaches included surveys, interviews, documentary analysis and non-participant observation. Two studies were longitudinal. Results The findings reveal the nature and extent of poor systems reliability and its effect on patient safety; the factors underpinning cases of patient harm; the cultural issues impacting on safety and quality; and the importance of a common language for quality and safety across an organisation. Across the publications, nine key organisational and systems factors emerged as important for patient safety improvement. These include leadership stability; data infrastructure; measurement capability; standardisation of clinical systems; and creating an open and fair collective culture where poor safety is challenged. Conclusions and contribution to knowledge The research presented in the publications has provided a more complete understanding of the organisation and systems factors underpinning safer healthcare. Lessons are drawn to inform methods for future research, including: how to define success in patient safety improvement studies; how to take into account external influences during longitudinal studies; and how to confirm meaning in multi-language research. Finally, recommendations for future research include assessing the support required to maintain a patient safety focus during periods of major change or austerity; the skills needed by healthcare leaders; and the implications of poor data infrastructure.
