En enkel Delphi-enkät skickades till drygt 100 beslutsfattare och experter inom Dalarnas byggsektor. Enkätdeltagarna tog ställning till 15 framtidsteser om bebyggelsens energiomställning. Enkätutfallet visar att få deltagare tror att energieffektiviseringsmålen kommer att kunna nås i tid. Staten och fastighetsägarna anses vara de viktigaste aktörerna för att förverkliga framtidsteserna. Lite förtroende finns det för frivilliga beteendeförändringar hos brukarna och stor oenighet råder kring renoveringens betydelse för energiomställningen.

Objective: This investigation aimed to identify and analyze the general and specific competencies of nurses in the primary health care practice of Brazil. Design: The Delphi Technique was used as the method of study. Sample: 2 groups of participants were selected: One contained primary health care nurses (n=52) and the other specialists (n=57), including public health nurses and public or community health faculty. Measurements: 3 questionnaires were developed for the study. The first asked participants to indicate general and specific competencies, which were compiled into a list for each group. A Likert scale of 1-5 was added to these 2 lists in the second and third questionnaires. A consensus criterion of 75% for score 4 or 5 was adopted. Results: In the nurses` group, 17 general and 8 specific competencies reached the consensus criterion; 19 general and 9 specific competencies reached the criterion in the specialists` group. These competencies were classified into 10 domains: professional values, communication, teamwork, management, community-oriented, health promotion, problem solving, health care, and education and basic public health sciences. Conclusions: These competencies reflect Brazilian health policy and constitute a reference for health professional practice and education.

This study, using a Delphi approach, sought the opinion of a self-selected panel of 320 district nurses regarding research priorities for district nursing in Australia. Over three rounds of questionnaires, the 419 research clinical problem areas requiring research as suggested by the panel were each rated in importance by the panel and then ranked through analysis from high to low average rating scores, thereby, whittling down the list to the top 15% (68) research questions and to a final list of the top 10 research priorities overall. Research questions focusing on discharge planning are dominant in these top 10 priorities, with documentation issues the second most common focus. Other foci in the top 10 priorities are staffing, aged care, palliative care, and assessment. The organization-specific top 10 research priorities focus on wound care, funding, education, and communication issues. Additionally, the top 68 priorities, which are either finitely practice-based or contextual-issues research questions, were categorized into 20 themes. The results will hopefully lead to scarce human and financial resources being directed to practice-relevant research programs that will facilitate improved health for district nursing (primarily home-nursing) clients in Australia and elsewhere. Background: There is ample evidence of important symptomatic efficacy of tumour necrosis factor alpha (TNFalpha) inhibition in ankylosing spondylitis (AS). Moreover, studies suggest that anti-TNF could be considered as the first disease controlling antirheumatic treatment (DC-ART) for AS. Objective: To determine precisely which patients with AS are most likely to benefit from anti-TNFalpha treatment because of the cost and possible long term side effects of such treatment. Methods: Assessment in Ankylosing Spondylitis (ASAS) members were asked to use a Delphi technique to name the characteristics of patients with AS for whom they would start DC-ART, in three different clinical presentations (isolated axial involvement, peripheral arthritis, enthesitis). Results: Among the 62 invited ASAS members, more than 50% actively participated in the four phases of definition according to the Delphi technique. For each of the three clinical presentations, a combination of five to six domains was proposed, with an evaluation instrument and a cut off point defining a minimum level of activity for each domain. Conclusion: This study provides a profile for a patient with AS for considering initiation of biological agents that reflects the opinion of the ASAS members, using a Delphi exercise. Further studies are required to assess their relevance and their consistency with clinical practice.

Esta dissertação insere-se no actual contexto de constante procura pela melhoria do desempenho dos sistemas produtivos em que se pretende estudar a utilização dos recursos produtivos numa das principais linhas de produção da fábrica da Delphi no Seixal. Para o efeito, é desenvolvido um modelo de simulação da linha de produção. O estudo inicia com uma revisão bibliográfica de conceitos e metodologias interligados com a simulação de sistemas. A fase seguinte diz respeito à recolha e tratamento de dados que caracterizam a linha de produção que se pretende simular. Posteriormente, é criado um modelo conceptual da linha de produção que serve de base ao modelo de simulação desenvolvido recorrendo ao software de simulação Arena. Após a simulação do modelo desenvolvido, é realizada a validação do modelo através da análise comparativa de algumas medidas de desempenho resultantes da simulação e do sistema real. Por último é realizada a discussão de resultados e são tecidas considerações que suportam os resultados obtidos e apoiam a diferença de resultados verificada.

INTRODUCTION: Although many countries have improved vaccination coverage in recent years, some, including Guinea-Bissau, failed to meet expected targets. This paper tries to understand the main barriers to better vaccination coverage in the context of the GAVI-Alliance (The Global Alliance for Vaccines and Immunisation) cash-based support provided to Guinea-Bissau. METHODS: The analysis is based on a document analysis and a three round Delphi study with a final consensus meeting. RESULTS: Consensus attributed about 25% of the failure to perform better to implementation problems; and about 10% to governance and also 10% to scarce resources. The qualitative analysis validates the importance of implementation issues and upgraded the relevance of the human resources crisis as an important drawback. The recommendations were balanced in their upstream-downstream focus but were blind to health information issues and logistical difficulties. CONCLUSIONS: It is commendable that such a fragile state, with all sorts of barriers, manages to sustain a slow steady growth of its vaccination coverage. Not reaching the targets set reflects the inappropriateness of those targets rather than a lack of commitment of the health workforce. In the unstable context of countries such as Guinea-Bissau, the predictability of the funds from global health initiatives like the GAVI-Alliance seem to make all the difference in achieving small consistent health gains even in the presence of other major bottlenecks.

Aquest article presenta els resultats d'una enquesta Delphi internacional i interdisciplinària sobre el desenvolupament futur de les revistes electròniques. El quadre d'experts constà de 45 científics, editors, bibliotecaris, agents de revistes i consultors. L'enquesta cobria cinc àrees d'interès sobre revistes electròniques: el futur rol de la literatura erudita en suport revista, escenaris per a les revistes del futur; crisi en les publicacions en sèrie; arxiu de revistes electròniques i nous models en l'adquisició i l'accés. L'enquesta Delphi va estudiar els canvis esperats durant els propers cinc a deu anys. Els resultats són analitzats i indiquen que, durant els seus 300 anys d'història, les revistes mai no han afrontat tants de canvis com els que experimenten ara, o els que s'esperen durant els propers cinc a deu anys.

BACKGROUND: Psoriatic arthritis (PsA) substantially impacts the management of psoriatic disease. OBJECTIVE: This study aimed to generate an interdisciplinary national consensus on recommendations of how PsA should be managed. METHODS: Based on a systematic literature search, an interdisciplinary expert group identified important domains and went through 3 rounds of a Delphi exercise, followed by a nominal group discussion to generate specific recommendations. RESULTS: A strong consensus was reached on numerous central messages regarding the impact of PsA, screening procedures, organization of the interaction between dermatologists and rheumatologists, and treatment goals. CONCLUSION: These recommendations can serve as a template for similar initiatives in other countries. At the same time, they highlight the need to take into account the impact of the respective national health care system. © 2015 S. Karger AG, Basel.

A study by the University of Ulster, funded by Health and Social Care Research and Development Division of the Public Health Agency, enabled members of six Allied Health Professions (AHPs) to express opinions on research needs within their areas of expertise.The respondents to 'A Delphi Study to Identify Research Priorities for the Therapy Professions in Northern Ireland', were selected from professionals based in clinical and academic settings in the areas of physiotherapy, occupational therapy, speech and language therapy, podiatry, nutrition and dietetics and orthoptics. The views of a group of key stakeholders in health and social care and a separate panel of service users were also gathered. A copy of this report, and an Executive Summary,�can be downloaded below.

Purpose. Spanish retina specialists were surveyed in order to propose actions to decrease deficiencies in real-life neovascular age macular degeneration treatment (nv-AMD). Methods. One hundred experts, members of the Spanish Vitreoretinal Society (SERV), were invited to complete an online survey of 52 statements about nv-AMD management with a modified Delphi methodology. Four rounds were performed using a 5-point Linkert scale. Recommendations were developed after analyzing the differences between the results and the SERV guidelines recommendations. Results. Eighty-seven specialists completed all the Delphi rounds. Once major potential deficiencies in real-life nv-AMD treatment were identified, 15 recommendations were developed with a high level of agreement. Consensus statements to reduce the burden of the disease included the use of treat and extend regimen and to reduce the amount of diagnostic tests during the loading phase and training technical staff to perform these tests and reduce the time between relapse detection and reinjection, as well as establishing patient referral protocols to outside general ophthalmology clinics. Conclusion. The level of agreement with the final recommendations for nv-AMD treatment among Spanish retinal specialist was high indicating that some actions could be applied in order to reduce the deficiencies in real-life nv-AMD treatment.

O saber, a ciência são universais e vêm se desenvolvendo muito rapidamente e, o enfermeiro, tem conhecimento de sua responsabilidade neste desenvolvimento científico. Com o objetivo de analisar a Técnica Delphi, descrevendo a sua utilização na validação das intervenções de enfermagem aos lesados medulares em reabilitação, foi possível resultados fidedignos, por meio de uma técnica de caráter flexível e, sobretudo, por valorizar a opinião ou o conhecimento de cada um dos enfermeiros que atuam junto aos lesados medulares. Delphi permitiu aos especialistas apontarem as intervenções preconizadas a esta clientela, mas este contexto teve requisitos necessários que identificam o pensar-fazer em enfermagem.

Este trabalho tem como objetivo identificar as competências gerais do médico e do enfermeiro que atuam em atenção primária de saúde em Marília, estado de São Paulo. A técnica Delphi foi utilizada para identificação destas competências. Foram identificadas oito (8) competências comunicação, trabalho em equipe, gerência, orientada à comunidade, valores profissionais, tomada de decisão, resolver problemas e habilidades educacionais. A competência referente à gerência foi a única a não apresentar consenso entre os médicos que não a consideram necessária para atuar m medicina.

1) Etat des connaissances : Actuellement, les indications à la transplantation pulmonaire sont bien décrites et validées. Par contre, lors d'une retransplantation, les résultats restent encore discutés : la retransplantation pulmonaire reste la seule option thérapeutique dans certains cas de rejet du greffon, de syndrome de bronchiolite oblitérante avancée et aussi lors de graves affections respiratoires, principalement de sténose cicatricielle. Malgré cela, son utilité a été mise en doute en raison de la pénurie de donneurs d'organes et des rapports indiquant des résultats insatisfaisants. 2) But de l'étude : Ce travail est une étude d'évaluation méthodologique dans lequel on utilisera un questionnaire DELPHI auprès de 10 soignants engagés dans le programme Vaud-Genève de transplantation pulmonaire afin de déterminer les critères de sélection qui mèneraient à la pratique d'une retransplantation. 3) Objectifs : Ce projet a pour but d'élaborer un consensus local sur les critères et les contre-indications à une retransplantation pulmonaire, dans le cadre du programme Vaud-Genève et du système d'allocation d'organes en vigueur en Suisse. D'où la question : ''Quelles sont les indications potentielles à une retransplantation pulmonaire ?'' 4) Justification : Dans une situation de pénurie explicite de ressources (manque de donneurs), l'utilisation des greffons doit être basée à la fois sur des critères d'équité et d'efficacité médicale. En l'absence de données claires dans la littérature sur les indications à la retransplantation pulmonaire, la recherche d'une attitude pragmatique au sein du programme local est justifiée. 5) Méthode : Cette étude monocentrique multisite est une étude clinique ouverte où les personnes interrogées restent en insu: un questionnaire DELPHI sera envoyé à 10 soignants spécialisés dans le domaine de la transplantation pulmonaire issus du programme Vaud-Genève. La méthode Delphi a pour but de mettre en évidence des convergences d'opinions afin de trouver des consensus grâce à l'interrogation de différents experts par le biais de questionnaires successifs. L'objectif des études Delphi est d'apporter l'éclairage des experts sur des zones d'incertitude en vue d'une aide à la décision. 6) Sélection des sujets : Les ''sujets'' éligibles sont un groupe de 10 experts anonymes spécialisés dans le domaine de la retransplantation pulmonaire du canton de Vaud et de Genève. 6.1 Critères d'inclusion : Personnel médical et paramédical directement impliqué dans le cadre du programme de transplantation Vaud-Genève. 6.2 Critère d'exclusion : Toute personne qui est elle-même directement impliquée dans l'analyse de l'étude. 7) Déroulement de l'étude et investigations prévues: L'étude aura lieu au CHUV et au HUG pour une durée de 2 ans. Les investigations seront effectuées par le biais de questionnaires envoyés par courrier électronique et seront analysées au CHUV.

This paper describes a study that aimed to identify research priorities for the care of infants, children and adolescents at the sole tertiary referral hospital for children in Western Australia. The secondary aim was to stimulate nurses to explore clinical problems that would require further inquiry. Background. Planning for research is an essential stage of research development; involving clinicians in this exercise is likely to foster research partnerships that are pertinent to clinical practice. Nursing research priorities for the paediatric population have not previously been reported in Australia. Design. Delphi study. Method. Over 12 months in 2005-2006, a three-round questionnaire, using the Delphi technique, was sent to a randomly selected sample of registered nurses. This method was used to identify and prioritise nursing research topics relevant to the patient and the family. Content analysis was used to analyse Round I data and descriptive statistics for Round II and III data. Results. In Round I, 280 statements were identified and reduced to 37 research priorities. Analysis of data in subsequent rounds identified the top two priority research areas as (1) identification of strategies to reduce medication incidents (Mean = 6 center dot 47; SD 0 center dot 88) and (2) improvement in pain assessment and management (Mean = 6; SD 1 center dot 38). Additional comments indicated few nurses access the scientific literature or use research findings because of a lack of time or electronic access. Conclusions. Thirty-seven research priorities were identified. The identification of research priorities by nurses provided research direction for the health service and potentially other similar health institutions for children and adolescents in Australia and internationally. Relevance to clinical practice. The nurse participants showed concern about the safety of care and the well-being of children and their families. This study also enabled the identification of potential collaborative research and development of pain management improvement initiatives.
