Abstract OBJECTIVE To map the sub processes related to turnover of nursing staff and to investigate and measure the nursing turnover cost. METHOD This is a descriptive-exploratory study, classified as case study, conducted in a teaching hospital in the southeastern, Brazil, in the period from May to November 2013. The population was composed by the nursing staff, using Nursing Turnover Cost Calculation Methodology. RESULTS The total cost of turnover was R$314.605,62, and ranged from R$2.221,42 to R$3.073,23 per employee. The costs of pre-hire totaled R$101.004,60 (32,1%), and the hiring process consumed R$92.743,60 (91.8%) The costs of post-hire totaled R$213.601,02 (67,9%), for the sub process decreased productivity, R$199.982,40 (93.6%). CONCLUSION The study identified the importance of managing the cost of staff turnover and the financial impact of the cost of the employee termination, which represented three times the average salary of the nursing staff.

O tema da justiça no contexto da gestão de uma equipe de vendas, embora importante para medir o relacionamento e a efetividade dessas equipes, é pouco ou nada explorado no Brasil. O principal propósito deste trabalho é entender a relação entre a avaliação de justiça em sua constituição tetradimensional (informacional, procedimental, distributiva e interpessoal) e a motivação, a lealdade e a intenção de turnover de equipes de vendas. O estudo de campo foi realizado por meio de um survey eletrônico com uma amostra final de 194 respondentes, todos profissionais de vendas. Uma contribuição acadêmica do estudo foi propor uma nova escala para mensuração da justiça percebida. Outra contribuição foi possibilitar o aprofundamento da compreensão da multidimensionalidade da percepção de justiça, lealdade, motivação e intenção de turnover. Nesse sentido, evidenciou-se a influência da justiça percebida na ligação psicológica da equipe de vendas com a empresa.

The var genes of Plasmodium falciparum code for the antigenically variant erythrocyte membrane proteins 1 (PfEMP1), a major factor for cytoadherence and immune escape of the parasite. Herein, we analyzed the var gene transcript turnover in two ongoing, non-symptomatic infections at sequential time points during two weeks. The number of different circulating genomes was estimated by microsatellite analyses. In both infections, we observed a rapid turnover of plasmodial genotypes and var transcripts. The rapidly changing repertoire of var transcripts could have been caused either by swift elimination of circulating var-transcribing parasites stemming from different or identical genetic backgrounds, or by accelerated switching of var gene transcription itself.

This is an exploratory, cross-sectional study of quantitative design that aimed to identify the communication strategies used and reported by the nursing staff in the care of aphasic patients after a stroke. The techniques used were the participant observation and interviews with 27 subjects of the nursing staff of neurological units in a general hospital. The most frequently mentioned strategies were gestures (100%), verbal communication (33.3%), written communication (29.6%) and the touch (18.5 %). Among the observed strategies, the gestures reached 40.7% and the touch was present in all situations, given its instrumental character essential to care. The findings show lack of knowledge of nonverbal, proxemics , kinesics and tacesics communication. No significant differences were observed among the professional categories depending on the length of experience with respect to the strategies reported by members of the nursing staff in the care for aphasic patients.

OBJECTIVE To evaluate intervening factors in patient safety, focusing on hospital nursing staff. METHOD The study is descriptive, with qualitative approach, excerpt from a larger study with analytical nature. It was undertaken in a public hospital in Fortaleza, CE, Brazil, between January and June 2013, with semi-structured interviews to 70 nurses, using Thematic Content Analysis. RESULTS The principal intervening factors in patient safety related to hospital nursing staff were staff dimensioning and workload, professional qualification and training, team work, being contracted to the institution, turnover and lack of job security, and bad practice/disruptive behaviors. These aspects severely interfere with the establishment of a safety culture in the hospital analyzed. CONCLUSION It is necessary for managers to invest in nursing staff, so that these workers may be valued as fundamental in the promotion of patient safety, making it possible to develop competences for taking decisions with focus on the improvement of quality care.

AbstractOBJECTIVEEvaluate pre- and intraoperative practices adopted by medical and nursing teams for the prevention of surgical infections.METHODA prospective study carried out in the period of April to May 2013, in a surgical center of a university hospital in Belo Horizonte, Minas Gerais.RESULTS18 surgeries were followed and 214 surgical gloves were analyzed, of which 23 (10.7%) had postoperative glove perforation detected, with 52.2% being perceived by users. Hair removal was performed on 27.7% of patients in the operating room, with the use of blades in 80% of the cases. Antibiotic prophylaxis was administered to 81.8% of patients up to 60 minutes prior to surgical incision. An average of nine professionals were present during surgery and the surgery room door remained open in 94.4% of the procedures.CONCLUSIONPartial adhesion to the recommended measures was identified, reaffirming a need for greater attention to these critical steps/actions in order to prevent surgical site infection.

O objetivo deste estudo foi avaliar a influência da glutamina no turnover do carbono do tecido adiposo de leitões. Quarenta leitões foram desmamados aos 21 dias e distribuídos, aleatoriamente, em dois tratamentos: T1, dieta de plantas C3, sem suplementação de glutamina; e T2, dieta de plantas C3, suplementada com 1% de glutamina. Nos dias 0, 1, 3, 5, 8, 11, 15, 20, 29 e 46 pós-desmame, dois leitões por tratamento foram abatidos; amostras de tecido adiposo foram coletadas e analisadas quanto à composição isotópica, e foi mensurada a substituição do carbono em função do tempo. Na primeira semana pós-desmame, não houve alteração dos valores isotópicos, pois os animais nessa fase usam suas reservas corporais de gordura. Após esse período, o tecido adiposo começou a incorporar o sinal isotópico da ração, o que indica início da deposição de gordura corporal. Nos animais que se alimentaram com suplemento de glutamina, a incorporação do carbono da ração no tecido adiposo foi mais rápida. As reservas de gordura corporal, adquiridas durante o aleitamento, exercem importante papel na manutenção dos animais por aproximadamente uma semana pós-desmame, e a glutamina auxilia na retomada de deposição de gordura corporal após esse período.

OBJETIVO: Investigar a influência de anticoncepcionais hormonais (ACH) orais em indicadores bioquímicos relacionados à utilização metabólica e distribuição de zinco e ao turnover ósseo em mulheres adultas jovens.MÉTODOS: Estudo transversal. Amostras de sangue e urina de não usuárias (-ACH; controle; n=69) e usuárias há pelo menos três meses de contraceptivos hormonais orais (+ACH; n=62) foram coletadas em condições padronizadas. Foram analisados os indicadores de homeostase de zinco e de turnoverósseo em soro ou plasma (zinco total e nas frações de albumina e α2-macroglobulina, albumina e atividade de fosfatase alcalina total e de origem óssea), em eritrócitos (zinco e metalotioneína) e em urina (zinco, cálcio e hidroxiprolina). Ingestões habituais de zinco e cálcio foram avaliadas por questionário de frequência de consumo.RESULTADOS: A ingestão alimentar de zinco foi semelhante nos grupos e, em média, acima do recomendado, enquanto que a ingestão de cálcio foi similarmente subadequada em +ACH e -ACH. Comparadas às controles, as +ACH apresentaram menores concentrações de zinco em soro, total e ligado à α2-macroglobulina (11 e 28,5%, respectivamente, p<0,001); albumina em soro (13%, p<0,001); atividade de fosfatase alcalina em plasma, total e de origem óssea (13 e 18%, respectivamente, p<0,05); metalotioneína em eritrócitos (13%, p<0,01) e zinco urinário (34%, p<0,05).CONCLUSÕES: O uso de ACH reduz o zinco sérico, altera a distribuição de zinco nas principais proteínas ligantes do soro com possíveis efeitos na captação tecidual, aumenta a retenção de zinco no organismo e reduz o turnover ósseo. O uso prolongado de ACH poderia levar a menor pico de massa óssea e/ou prejudicar a manutenção de massa óssea em mulheres jovens, principalmente com ingestão marginal de cálcio. Os efeitos de ACH verificados foram mais evidentes nas mulheres <25 anos de idade e nas nulíparas, as quais merecem especial atenção em estudos posteriores.

As variações nos parâmetros hematológicos são utilizadas com intuito de avaliar o grau de treinamento ou estado clínico do animal. A avaliação hematológica de eqüinos em repouso tem sido objeto de estudo, a fim de estabelecer uma relação com treinamento ou capacidade atlética. Objetivou-se avaliar o perfil hematológico de eqüinos submetidos à prova de Team Penning, correlacionando o sexo e freqüência da atividade física. Mediante punção da veia jugular externa coletaram-se dois mL de sangue de 29 eqüinos, 18 machos e 11 fêmeas, em repouso (Momento I) e após o exercício (Momento II). As amostras de sangue foram processadas em analisador hematológico automático veterinário (ABC VET - Horiba ABX Diagnostics). Os animais foram divididos em Grupos A, B, C e D, de acordo com o número de participações na prova. Observou-se que os valores de volume globular, hemoglobina, hemácias, leucócitos, neutrófilos em bastonetes e segmentados, e monócitos aumentaram após o exercício físico, ao contrário do número de linfócitos e eosinófilos, que reduziram. Não existiram diferenças significativas (p<0,05) entre machos e fêmeas ao confrontar as relações antes/depois. Além disso, evidenciou-se que o valor da relação MI/MII para volume globular, hemoglobina e número de hemácias variou de acordo com a freqüência do exercício. Conclui-se que a prova de Team Peninng ocasiona alterações hematológicas em eqüinos, com interferência da freqüência do exercício, independente do sexo.

This study examined if leucine, arginine or glycine supplementation in adult obese patients (body mass index of 33 ± 4 kg/m²) consuming a Brazilian low energy and protein diet (4.2 MJ/day and 0.6 g protein/kg) affects protein and amino acid metabolism. After four weeks adaptation to this diet, each subject received supplements of these amino acids (equivalent to 0.2 g protein kg-1 day-1) in random order. On the seventh day of each amino acid supplementation, a single-dose 15N-glycine study was carried out. There were no significant differences in protein flux, synthesis or breakdown. The protein flux (grams of nitrogen, gN/9 h) was 55 ± 24 during the nonsupplemented diet intake and 39 ± 10, 44 ± 22 and 58 ± 35 during the leucine-, glycine- and arginine-supplemented diet intake, respectively; protein synthesis (gN/9 h) was 57 ± 24, 36 ± 10, 41 ± 22 and 56 ± 36, respectively; protein breakdown (gN/9 h) was 51 ± 24, 34 ± 10, 32 ± 28 and 53 ± 35, respectively; kinetic balance (gN/9 h) was 3.2 ± 1.8, 4.1 ± 1.7, 3.4 ± 2.9 and 3.9 ± 1.6. There was no difference in amino acid profiles due to leucine, arginine or glycine supplementation. The present results suggest that 0.6 g/kg of dietary protein is enough to maintain protein turnover in obese women consuming a reduced energy diet and that leucine, arginine or glycine supplementation does not change kinetic balance or protein synthesis.

We investigated the effects of a saturated fat diet on lipid metabolism and arachidonic acid (AA) turnover in mouse resident peritoneal macrophages. The pro-oxidative effect of this diet was also studied. Female C57BL/6 mice were weaned at 21 days of age and assigned to either the experimental diet containing coconut oil (COCO diet), or the control diet containing soybean oil as fat source (10 mice per group). The fat content of each diet was 15% (w/w). Mice were fed for 6 weeks and then sacrificed. The concentration of total lipids, triglycerides, (LDL + VLDL)-cholesterol, thiobarbituric acid-reactive substances (TBARS) and reduced glutathione were increased in the plasma of mice fed the COCO diet, without changes in phospholipid or total cholesterol concentrations compared to control. The concentrations of total cholesterol, free and esterified cholesterol, triglycerides, and TBARS were increased in the macrophages of COCO-fed mice, while the content of total phospholipids did not change. The phospholipid composition showed an increase of phosphatidylcholine and a decrease of phosphatidylethanolamine. The [³H]-AA distribution in the phospholipid classes showed an increase in phosphatidylcholine and phosphatidylethanolamine. Incorporation of [³H]-cholesterol into the macrophages of COCO-fed mice and into the cholesterol ester fraction was increased. The COCO diet did not affect [³H]-AA uptake but induced an increase in [³H]-AA release. The COCO diet also enhanced AA mobilization induced by lipopolysaccharide. These results indicate that the COCO diet, high in saturated fatty acids, alters the lipid metabolism and AA turnover of peritoneal macrophages in female mice and also produces a significant degree of oxidative stress.
