OBJECTIVE To analyze the methodology used for assessing the spatial distribution of specialized cardiac care units. METHODS A modeling and simulation method was adopted for the practical application of cardiac care service in the state of Santa Catarina, Southern Brazil, using the p-median model. As the state is divided into 21 health care regions, a methodology which suggests an arrangement of eight intermediate cardiac care units was analyzed, comparing the results obtained using data from 1996 and 2012. RESULTS Results obtained using data from 2012 indicated significant changes in the state, particularly in relation to the increased population density in the coastal regions. The current study provided a satisfactory response, indicated by the homogeneity of the results regarding the location of the intermediate cardiac care units and their respective regional administrations, thereby decreasing the average distance traveled by users to health care units, located in higher population density areas. The validity of the model was corroborated through the analysis of the allocation of the median vertices proposed in 1996 and 2012. CONCLUSIONS The current spatial distribution of specialized cardiac care units is more homogeneous and reflects the demographic changes that have occurred in the state over the last 17 years. The comparison between the two simulations and the current configuration showed the validity of the proposed model as an aid in decision making for system expansion.

OBJECTIVE To analyze the clinical and laboratory characteristics of HIV-infected individuals upon admission to a reference health care center.METHODS This cross-sectional study was conducted between 1999 and 2010 on 527 individuals with confirmed serological diagnosis of HIV infection who were enrolled in an outpatient health care service in Santarém, PA, Northern Brazil. Data were collected from medical records and included the reason for HIV testing, clinical status, and count of peripheral CD4+ T lymphocytes upon enrollment. The data were divided into three groups, according to the patient’s year of admission – P1 (1999-2002), P2 (2003-2006), and P3 (2007-2010) – for comparative analysis of the variables of interest.RESULTS In the study group, 62.0% of the patients were assigned to the P3 group. The reason for undergoing HIV testing differed between genders. In the male population, most tests were conducted because of the presence of symptoms suggesting infection. Among women, tests were the result of knowledge of the partner’s seropositive status in groups P1 and P2. Higher proportion of women undergoing testing because of symptoms of HIV/AIDS infection abolished the difference between genders in the most recent period. A higher percentage of patients enrolling at a more advanced stage of the disease was observed in P3.CONCLUSIONS Despite the increased awareness of the number of HIV/AIDS cases, these patients have identified their serological status late and were admitted to health care units with active disease. The HIV/AIDS epidemic in Pará presents specificities in its progression that indicate the complex characteristics of the epidemic in the Northern region of Brazil and across the country.

OBJECTIVE To analyze if the distribution of specialized care services for HIV/AIDS is associated with AIDS rates. METHODS Ecological study, for which the distribution of 10 specialized care services in the Ceara state, Northeastern Brazil, was obtained, and the mean rates of the disease were estimated per mesoregion. We evaluated 7,896 individuals who had been diagnosed with AIDS, were aged 13 years or older, lived in Ceara, and had been informed of their condition between 2001 and 2011. Maps were constructed to verify the relationship between the distribution of AIDS cases and institutionalized support networks in the 2001-2006 and 2007-2011 periods. BoxMap and LisaMap were used for data analysis. The Voronoi diagram was applied for the distribution of the studied services. RESULTS Specialized care services concentrated in AIDS clusters in the metropolitan area. The Noroeste Cearense and west of the Sertoes Cearenses had high AIDS rates, but a low number of specialized care services over time. Two of these services were implemented where clusters of the disease exist in the second period. The application of the Voronoi diagram showed that the specialized care services located outside the metropolitan area covered a large territory. We identified one polygon that had no services. CONCLUSIONS The scenario of AIDS cases spread away from major urban areas demands the creation of social support services in areas other than the capital and the metropolitan area of the state; this can reduce access barriers to these institutions. It is necessary to create specialized care services for HIV/AIDS in the Noroeste Cearense and north of Jaguaribe.

Some patients under antiretroviral therapy (ART) do not reach immune recovery when the viral load becomes undetectable. This is called discordant immunologic and virologic responses. Its prevalence varies between 8% and 24%. This study describes its prevalence and the characteristics of the affected subjects in the outpatient clinic of a Brazilian specialized-care center. Of 934 patients on ART, 536 had undetectable viral loads. Prevalence was 51/536 or 9% (95% confidence interval: 6.6% to 11.4%). Median age at the beginning of ART was 37 years (interquartile range - IQR: 31 to 45). Male gender and mixed race predominated (76.5% and 47.1% respectively). AIDS-defining illnesses were absent at the beginning of ART in 60.8%. Fifty-one percent were taking protease inhibitors, 43.2% Efavirenz and 5.8% both. Median time on ART was 36 months (IQR: 17-81 months). Irregular treatment was recorded for 21.6%. ART had been modified for 63% prior to the study, and 15.7% had used monotherapy or double therapy. Median CD4 count was 255 cells/mm³ (IQR: 200-284). Median viral load before ART was 4.7 log10 copies/mL (IQR: 4.5-5.2). Discordant responders were not different from AIDS patients in general, but there was a high frequency of multiple schedules of treatment.

Introduction: This study describes the frequency of late diagnosis and HIV among children attending a pediatric AIDS clinic. Methods: Cross-sectional study in children exposed to HIV from 2005-2008. A questionnaire was given that included questions on demographics and clinical information. Results: Two-hundred twenty-one (97.8%) children were exposed to HIV during pregnancy/childbirth. A total of 193 (87.3%) children had late enrolment in the service and late access to HIV serology. The frequency of HIV was 21.3% (95% confidence interval [CI] 15.9%-26.7%). Protective factors were earlier diagnosis [odds ratio (OR)=0.17 (0.08-0.37)] and receiving complete prophylaxis [OR=0.29 (0.09-0.97)]; being born by vaginal delivery was a risk factor [OR=4.45 (1.47-13.47)]. Conclusions: There was a high frequency of late diagnosis in this patient cohort. Earlier diagnosis is an important measure for controlling HIV among children.

In this study we present a new record of a plant-animal interaction: the mutualistic relationship between the specialist plant-ant Myrcidris epicharis Ward, 1990 (Pseudomyrmecinae) and its myrmecophyte host Myrcia madida McVaugh (Myrtaceae). We observed more than 50 individuals of M. madida occupied by M. epicharis in islands and margins of the Juruena River, in Cotriguaçu, Mato Grosso, Brazil (Meridional Amazon). We discuss a possible distribution of this symbiotic interaction throughout all the riparian forest of the Amazon River basin and its consequence to coevolution of the system.

OBJECTIVE To construct statements of nursing diagnoses related to nursing practice for individuals with diabetes in Specialized Care, on the basis of the Database of Nursing Practice Terms related to diabetes, in the International Classification for Nursing Practice (ICNP®) and in the Theory of Basic Human Needs and to validate them with specialist nurses in the area. METHOD Methodological research, structured into sequential stages of construction, cross-mapping, validation and categorization of nursing diagnoses. RESULTS A list was indicated of 115 statements of diagnostic, including positive, negative and improvement statements; 59 nursing diagnoses present in and 56 nursing diagnoses absent from the ICNP® Version 2011. 66 diagnoses with CVI ≥ 0.50 were validated, being categorized on the basis of human needs. CONCLUSION It was observed that the use of the ICNP® 2011 favored the specifications of the concepts of professional practice in care with individuals with diabetes.

Abstract OBJECTIVE To assess the nursing workload (NW) in Semi-intensive Therapy Unit, specialized in the care of children with Craniofacial anomalies and associated syndromes; to compare the amount of workforce required according to the Nursing Activities Score (NAS) and the COFEN Resolution 293/04. METHOD Cross-sectional study, whose sample was composed of 72 patients. Nursing workload was assessed through retrospective application of the NAS. RESULTS the NAS mean was 49.5%. Nursing workload for the last day of hospitalization was lower in patients being discharged to home (p<0.001) and higher on the first compared to last day of hospitalization (p< 0.001). The number of professionals required according to NAS was superior to the COFEN Resolution 293/04, being 17 and 14, respectively. CONCLUSION the nursing workload corresponded to approximately 50% of the working time of nursing professional and was influenced by day and outcome of hospitalization. The amount of professionals was greater than that determined by the existing legislation.

Diesel oil is a compound derived from petroleum, consisting primarily of hydrocarbons. Poor conditions in transportation and storage of this product can contribute significantly to accidental spills causing serious ecological problems in soil and water and affecting the diversity of the microbial environment. The cloning and sequencing of the 16S rRNA gene is one of the molecular techniques that allows estimation and comparison of the microbial diversity in different environmental samples. The aim of this work was to estimate the diversity of microorganisms from the Bacteria domain in a consortium specialized in diesel oil degradation through partial sequencing of the 16S rRNA gene. After the extraction of DNA metagenomics, the material was amplified by PCR reaction using specific oligonucleotide primers for the 16S rRNA gene. The PCR products were cloned into a pGEM-T-Easy vector (Promega), and Escherichia coli was used as the host cell for recombinant DNAs. The partial clone sequencing was obtained using universal oligonucleotide primers from the vector. The genetic library obtained generated 431 clones. All the sequenced clones presented similarity to phylum Proteobacteria, with Gammaproteobacteria the most present group (49.8 % of the clones), followed by Alphaproteobacteira (44.8 %) and Betaproteobacteria (5.4 %). The Pseudomonas genus was the most abundant in the metagenomic library, followed by the Parvibaculum and the Sphingobium genus, respectively. After partial sequencing of the 16S rRNA, the diversity of the bacterial consortium was estimated using DOTUR software. When comparing these sequences to the database from the National Center for Biotechnology Information (NCBI), a strong correlation was found between the data generated by the software used and the data deposited in NCBI.

This article evaluates social implications of the "SIGA" Health Care Information System (HIS) in a public health care organization in the city of São Paulo. The evaluation was performed by means of an in-depth case study with patients and staff of a public health care organization, using qualitative and quantitative data. On the one hand, the system had consequences perceived as positive such as improved convenience and democratization of specialized treatment for patients and improvements in work organization. On the other hand, negative outcomes were reported, like difficulties faced by employees due to little familiarity with IT and an increase in the time needed to schedule appointments. Results show the ambiguity of the implications of HIS in developing countries, emphasizing the need for a more nuanced view of the evaluation of failures and successes and the importance of social contextual factors.

O papel da comunicação e da informação no ambiente organizacional, sem dúvida, tem despertado o desenvolvimento de diferentes enfoques conceituais e teóricos. Todavia, a não-aplicabilidade de processos comunicativos relevantes no ambiente das organizações, principalmente em situações complexas, leva-nos a refletir sobre um novo conceito de comunicação organizacional. Neste artigo, analisamos fundamentos teóricos da comunicação de Habermas e de Cohn e, principalmente, algumas contribuições do teórico da complexidade, Dominique Genelot, para o embasamento da comunicação organizacional como um elemento estratégico na realização dos objetivos e da intencionalidade das organizações, na tentativa de superar os limites dos enfoques reducionistas do conceito tradicional de comunicação empresarial, influenciado principalmente pela visão instrumental da comunicação sustentada pela teoria da informação e pelos corpora teóricos que se inspiraram em tal conceito e que com base nele se desenvolveram.

Se describen los resultados de un estudio epidemiológico del estado de la salud bucal en escolares de nivel primario (6 y 12 años) matriculados en establecimientos municipales, provinciales y privados de la Ciudad de Córdoba, Provincia de Córdoba, Argentina. En los niños de 6 años de escuelas municipales, la prevalencia de caries en elementos temporarios fue de moderada a alta, resultando baja para los escolares provinciales y privados (ceo-d = 4,44 , 2,31 y 1,27, respectivamente). Tanto en escolares municipales como provinciales, los componentes de mayor peso en el ceo-d fueron dientes cariados y con extracción indicada, en tanto que en los privados correspondió a dientes obturados. La proporción de niños sin caries activa fue de 13,6% en las escuelas municipales, 52,2% en las provinciales y 76,9% en las privadas. En los escolares de 12 años, la prevalencia de caries en dentición permanente resultó baja y comparable en los tres grupos de escuelas, registrándose un índice CPO-D que osciló entre 1,85 (municipales) y 2,59 (privados). Para ambas edades, las necesidades de tratamiento odontológico fueron elevadas en las tres poblaciones escolares. Se concluye que los niveles de salud dental alcanzados por los escolares municipales y provinciales a la edad de 12 años deben atribuirse principalmente al impacto producido por la aplicación de medidas preventivas, cuya eficiencia podría ser aumentada si se orientaran a atender edades más tempranas, sobre todo en las poblaciones de mayor riesgo.

Ante el creciente deterioro ambiental y sus posibles consecuencias en la salud de la población resulta una prioridad indiscutible el diseño e instrumentación de políticas que controlen las actividades económicas bajo el criterio de protección a la salud humana y al ambiente mismo. Es preciso que dichas políticas consideren la factibilidad económica de las alternativas de protección existentes. Sin embargo, por la cantidad de intereses dentro del área ambiental, otros factores como el social y el político deben también ser considerados. La evaluación económica ha sido vista como un promisorio fundamento para la toma de decisiones en esta materia. Los autores analizan la capacidad de esta herramienta para organizar en forma sistemática y comparable los costos y los beneficios de alternativas para la solución de problemas ambientales. Se resumen las principales características de los estudios de costo-beneficio y costo-efectividad, las formas de evaluación económica del ambiente, y las particularidades de esta área para la aplicación de dichas técnicas de análisis. Se señalan los límites encontrados en estas herramientas para cuantificar los costos no monetarios de los riesgos ambientales y de los consecuentes daños a la salud, tales como el dolor, el sufrimiento ó la incapacidad de personas económicamente inactivas, constituyendo estos aspectos el reto metodológico de la evaluación económica en el área. Se reflexiona sobre la importancia de ampliar los insumos informativos para la toma de decisiones en materia de salud ambiental en aspectos como la distribución de los costos y los beneficios entre los distintos grupos sociales. Por último se señala tanto la creciente politización del tema ambiental, como la posibilidad técnica de manipulación de estas herramientas de análisis. Partiendo de estos elementos se señala la necesidad, por parte de los evaluadores, de ser conscientes sobre las implicaciones políticas de sus estudios, así como de la importancia de su vinculación con los tomadores de decisión con el objetivo de lograr pertinencia en el ejercicio académico.
