Resumo:
El presente artículo compara el ranking Minsal de 40 patologías Auge-GES con el determinado por impacto financiero. Se elaboró una base de datos homogénea, se estandarizaron datos y se aplicaron correlaciones Pearson y Spearman. Se concluye que el criterio de impacto financiero cambia efectivamente las prioridades y modifica la congruencia social de la política de Estado.

Com o fenômeno da globalização, a contabilidade tem passado por modificações que se refletiram, inicialmente, no âmbito empresarial e só alguns anos mais tarde na administração pública. Há uma tendência cada vez maior e crescente de pressionar os governantes para ampliarem o campo de suas responsabilidades financeiras a partir da contabilidade voltada para a gestão das contas públicas. A responsabilidade fiscal dos governos é fundamental para fomentar o crescimento econômico e o desenvolvimento dos municípios. O presente estudo visa demonstrar em que medida os índices de análise econômico-financeira podem ser empregados na administração pública a fim de se estabelecer um ranking na gestão dos municípios catarinenses. Para atingir tal finalidade, utilizou-se de conceitos e técnicas de análise de balanço, os quais foram tratados com a aplicação da análise de componentes principais. Os resultados indicam a importância do emprego da análise de balanços na área pública como ferramenta a ser utilizada no cotidiano da administração governamental.

Nos últimos 10 anos, o governo federal tem coordenado e articulado ações visando a implementação de projetos com ênfase em iniciativas voltadas para o uso das tecnologias de informação e comunicação (TICs) em suas relações com os cidadãos, com o setor privado e com a sociedade em geral. Considerando um forte movimento em prol de inserir o cidadão no centro desse processo, este artigo busca, a partir de uma pesquisa quantitativa/qualitativa, verificar em qual estágio de evolução encontram-se os sites governamentais dos tribunais de contas brasileiros. A partir dessa análise, foi possível mapear pontos a serem melhorados bem como estabelecer um ranking dos sites, tendo em vista o direito de acesso, a disponibilização de informações, a participação e o acompanhamento na execução das políticas públicas.

A Lei de Acesso à Informação representa um passo fundamental rumo à consolidação da democracia brasileira. Entre seus preceitos, está a obrigação da disposição de informações na rede sobre a estrutura, funcionamento e prestação de contas dos órgãos/entidades da administração direta e indireta e de todos aqueles que recebem subvenção pública. Considerando sua relevância no contexto atual, este artigo busca, a partir de uma pesquisa mista, analisar como a referida lei está sendo implementada pelas prefeituras da região Sul do país. A partir dessa análise, foi possível mapear os efeitos iniciais da aplicação da lei durante o seu primeiro ano de vigência bem como estabelecer um ranking dos sites, tendo em vista o cumprimento das disposições nela estabelecidas.

OBJECTIVE: To evaluate the potential advantages and limitations of the use of the Brazilian hospital admission authorization forms database and the probabilistic record linkage methodology for the validation of reported utilization of hospital care services in household surveys. METHODS: A total of 2,288 households interviews were conducted in the county of Duque de Caxias, Brazil. Information on the occurrence of at least one hospital admission in the year preceding the interview was obtained from a total of 10,733 household members. The 130 records of household members who reported at least one hospital admission in a public hospital were linked to a hospital database with 801,587 records, using an automatic probabilistic approach combined with an extensive clerical review. RESULTS: Seventy-four (57%) of the 130 household members were identified in the hospital database. Yet only 60 subjects (46%) showed a record of hospitalization in the hospital database in the study period. Hospital admissions due to a surgery procedure were significantly more likely to have been identified in the hospital database. The low level of concordance seen in the study can be explained by the following factors: errors in the linkage process; a telescoping effect; and an incomplete record in the hospital database. CONCLUSIONS: The use of hospital administrative databases and probabilistic linkage methodology may represent a methodological alternative for the validation of reported utilization of health care services, but some strategies should be employed in order to minimize the problems related to the use of this methodology in non-ideal conditions. Ideally, a single identifier, such as a personal health insurance number, and the universal coverage of the database would be desirable.

This study aimed to describe the probabilistic structure of the annual series of extreme daily rainfall (Preabs), available from the weather station of Ubatuba, State of São Paulo, Brazil (1935-2009), by using the general distribution of extreme value (GEV). The autocorrelation function, the Mann-Kendall test, and the wavelet analysis were used in order to evaluate the presence of serial correlations, trends, and periodical components. Considering the results obtained using these three statistical methods, it was possible to assume the hypothesis that this temporal series is free from persistence, trends, and periodicals components. Based on quantitative and qualitative adhesion tests, it was found that the GEV may be used in order to quantify the probabilities of the Preabs data. The best results of GEV were obtained when the parameters of this function were estimated using the method of maximum likelihood. The method of L-moments has also shown satisfactory results.

O objetivo do artigo é analisar a estratégia logística das empresas brasileiras a partir da mensuração do impacto de diferentes características do negócio sobre as principais decisões de produção e distribuição física de produtos acabados. Foi conduzida uma pesquisa de campo em empresas listadas no Ranking Exame para confrontar as quatro políticas propostas por Pagh e Cooper (1998), derivadas das combinações entre centralizar/descentralizar os estoques e produzir para estoque/produzir contra-pedido, com diversas características do produto, da operação e da demanda. Técnicas de análise multivariada, como as análises de regressão logística simples e multinomial, foram empregadas, permitindo o desenvolvimento de diversos modelos quantitativos. Seus resultados apontam para avanços acadêmicos e gerenciais, como a quantificação e hierarquização de relações empíricas descritas na literatura e o direcionamento da atenção gerencial para as variáveis relevantes à decisão. Os modelos também permitem avaliar a adequação dessas políticas para diferentes valores do tempo de entrega, da variabilidade das vendas e do grau de obsolescência dos produtos.

Este trabalho analisou a influência do país sobre o desempenho das firmas. A análise empírica tomou por base uma amostra de 83.641 observações de 10.927 firmas em 37 países e 224 indústrias, em um período de 10 anos. Um modelo linear hierárquico de três níveis com classificação cruzada permitiu identificar que a importância relativa dos efeitos país, indústria e interação indústria-país é semelhante e, em conjunto, aproximadamente da mesma magnitude do efeito firma. Além de estimar a importância do efeito país, o trabalho analisou a infl uência individual de cada país sobre o desempenho das fi rmas, o que permitiu a elaboração de um ranking de países com base na lucratividade das fi rmas.

INTRODUCTION: Self-reported weight and height were compared with direct measurements in order to evaluate the agreement between the two sources. METHOD: Data were obtained from a cross-sectional study on health status from a probabilistic sample of 1,183 employees of a bank, in Rio de Janeiro State, Brazil. Direct measurements were made of 322 employees. Differences between the two sources were evaluated using mean differences, limits of agreement and intraclass correlation coefficient (ICC). RESULTS AND CONCLUSIONS: Men and women tended to underestimate their weight while differences between self-reported and measured height were insignificant. Body mass index (BMI) mean differences were smaller than those observed for weight. ICC was over 0.98 for weight and 0.95 for BMI, expressing close agreement. Combining a graphical method with ICC may be useful in pilot studies to detect populational groups capable of providing reliable information on weight and height, thus minimizing resources needed for field work.

OBJECTIVE: To identify popular beliefs regarding the treatment of senile cataract in patients enrolled in the community health programs on eye rehabilitation. METHODS: A cross-sectional survey was carried out using an interview questionnaire that was applied to 776 subjects drawn from a non-probabilistic sample in five cities of the state of São Paulo. The sample was made up of 47.2% males and 52.8% females, aged 50 to 96 years (average age 71.6 years). RESULTS: Of the total of subjects studied, 41.9% had never attended school, and 78.5% were no longer in the employment market. Most (85.1%) credited the sight restoration to cataract surgery. Among those unconvinced, 47.4% asserted that sight restoration depended only on God's will. A greater proportion of women than men (p 0.0000) believed in the association of cataract and menopause, maternity, and menstrual periods and they admitted using herbal and rose teas for treating cataract. CONCLUSIONS: Misbeliefs related to the causes and treatment of senile cataract were identified, most probably of sociocultural basis, indicating the need of education on the subject.

OBJECTIVES: To estimate the prevalence of occupational injuries and identify their risk factors among students in two municipalities. METHODS: A cross-sectional survey was conducted in public schools of the municipalities of Santo Antonio do Pinhal and Monteiro Lobato, Brazil. A stratified probabilistic sample was drawn from public middle and high schools of the study municipalities. A total of 781 students aged 11 to 19 years participated in the study. Students attending middle and high school answered a comprehensive questionnaire on living and working conditions, as well as aspects of work injuries, and health conditions. Multiple logistic regression models were fitted to estimate risk factors of previous and present occupational injuries. RESULTS: Of 781 students, 604 previously had or currently have jobs and 47% reported previous injuries. Among current workers (n=555), 38% reported injuries on their current job. Risk factors for work injuries with statistically significant odds ratio >2.0 included attending evening school, working as a housekeeper, waiter or brickmaker, and with potentially dangerous machines. CONCLUSIONS: The study results reinforce the need of restricting adolescent work and support communities to implement social promotion programs.

OBJECTIVE: To determine the prevalence and severity of occlusal problems in populations at the ages of deciduous and permanent dentition and to carry out a meta-analysis to estimate the weighted odds ratio for occlusal problems comparing both groups. METHODS: Data of a probabilistic sample (n=985) of schoolchildren aged 5 and 12 from an epidemiological study in the municipality of São Paulo, Brazil, were analyzed using univariate logistic regression (MLR). Results of cross-sectional study data published in the last 70 years were examined in the meta-analysis. RESULTS: The prevalence of occlusal problems increased from 49.0% (95% CI =47.4%-50.6%) in the deciduous dentition to 71.3% (95% CI =70.3%-72.3%) in the permanent dentition (p<0.001). Dentition was the only variable significantly associated to the severity of malocclusion (OR=1.87; 95% CI =1.43-2.45; p<0.001). The variables sex, type of school and ethnic group were not significant. The meta-analysis showed that a weighted OR of 1.95 (1.91; 1.98) when compared the second dentition period with deciduous and mixed dentition. CONCLUSIONS: In planning oral health services, some activities are indicated to reduce the proportion of moderate/severe malocclusion to levels that are socially more acceptable and economically sustainable.

OBJECTIVE: To determine health care costs and economic burden of epidemiological changes in diseases related to tobacco consumption. METHODS: A time-series analysis in Mexico (1994-2005) was carried out on seven health interventions: chronic obstructive pulmonary diseases, lung cancer with and without surgical intervention, asthma in smokers and non-smokers, full treatment course with nicotine gum, and full treatment course with nicotine patch. According with Box-Jenkins methodology, probabilistic models were developed to forecast the expected changes in the epidemiologic profile and the expected changes in health care services required for selected interventions. Health care costs were estimated following the instrumentation methods and validated with consensus technique. RESULTS: A comparison of the economic impact in 2006 vs. 2008 showed 20-90% increase in expected cases depending on the disease (p<0.05), and 25-93% increase in financial requirements (p<0.01). The study data suggest that changes in the demand for health services for patients with respiratory diseases related to tobacco consumption will continue showing an increasing trend. CONCLUSIONS: In economic terms, the growing number of cases expected during the study period indicates a process of internal competition and adds an element of intrinsic competition in the management of preventive and curative interventions. The study results support the assumption that if preventive programs remain unchanged, the increasing demands for curative health care may cause great financial and management challenges to the health care system of middle-income countries like Mexico.

OBJECTIVE: To assess rates of offering and uptake of HIV testing and their predictors among women who attended prenatal care. METHODS: A population-based cross-sectional study was conducted among postpartum women (N=2,234) who attended at least one prenatal care visit in 12 cities. Independent and probabilistic samples were selected in the cities studied. Sociodemographic data, information about prenatal care and access to HIV prevention interventions during the current pregnancy were collected. Bivariate and multivariate analyses were carried out to assess independent effects of the covariates on offering and uptake of HIV testing. Data collection took place between November 1999 and April 2000. RESULTS: Overall, 77.5% of the women reported undergoing HIV testing during the current pregnancy. Offering of HIV testing was positively associated with: previous knowledge about prevention of mother-to-child transmission of HIV; higher number of prenatal care visits; higher level of education and being white. HIV testing acceptance rate was 92.5%. CONCLUSIONS: The study results indicate that dissemination of information about prevention of mother-to-child transmission among women may contribute to increasing HIV testing coverage during pregnancy. Non-white women with lower level of education should be prioritized. Strategies to increase attendance of vulnerable women to prenatal care and to raise awareness among health care workers are of utmost importance.

OBJECTIVE: To determine the best cut-offs of body mass index for identifying alterations of blood lipids and glucose in adolescents. METHODS: A probabilistic sample including 577 adolescent students aged 12-19 years in 2003 (210 males and 367 females) from state public schools in the city of Niterói, Southeastern Brazil, was studied. The Receiver Operating Characteristic curve was used to identify the best age-adjusted BMI cut-off for predicting high levels of serum total cholesterol (>150mg/dL), LDL-C (>100mg/dL), serum triglycerides (>100mg/dL), plasma glucose (>100mg/dL) and low levels of HDL-C (< 45mg/dL). Four references were used to calculate sensitivity and specificity of BMI cut-offs: one Brazilian, one international and two American. RESULTS: The most prevalent metabolic alterations (>50%) were: high total cholesterol and low HDL-C. BMI predicted high levels of triglycerides in males, high LDL-C in females, and high total cholesterol and the occurrence of three or more metabolic alterations in both males and females (areas under the curve range: 0.59 to 0.67), with low sensitivity (57%-66%) and low specificity (58%-66%). The best BMI cut-offs for this sample (20.3 kg/m² to 21.0 kg/m²) were lower than those proposed in the references studied. CONCLUSIONS: Although BMI values lower than the International cut-offs were better predictor of some metabolic abnormalities in Brazilian adolescents, overall BMI is not a good predictor of these abnormalities in this population.
