OBJECTIVE To know the structure and functioning of healthcare services from the perspective of women who have suffered rape. METHOD A qualitative study conducted with 11 women who experienced rape, monitored in a maternity in the state of Alagoas, Brazil. Data were systematically based on content analysis. RESULTS It allowed for understanding the path taken by women in search of support from health services, as well as the limitations and capabilities of these services. CONCLUSION The assistance received in healthcare services leans towards a revictimization process of women who already carry trauma from the rape. It is necessary to reflect about care practices aimed at sexually victimized women.

Studies were conducted to estimate parameters and relationships associated with sub-processes in soil seed banks of oilseed rape in Gorgan, Iran. After one month of burial, seed viability decreased to 39%, with a slope of 2.03% per day, and subsequently decreased with a lower slope of 0.01 until 365 days following burial in the soil. Germinability remained at its highest value in autumn and winter and decreased from spring to the last month of summer. Non-dormant seeds of volunteer oilseed rape did not germinate at temperatures lower than 3.8 ºC and a water potential of -1.4 MPa ºd. The hydrothermal values were 36.2 and 42.9 MPa ºd for sub- and supra-optimal temperatures, respectively. Quantification of seed emergence as influenced by burial depth was performed satisfactorily (R² = 0.98 and RMSE = 5.03). The parameters and relationships estimated here can be used for modelling soil seed bank dynamics or establishing a new model for the environment.

In 1970 the population of Brazil with 94,508,554 inhabitants was extreme youth, since 42.67% was composed of children under 14 years old. In that year the proportion of female was 50.2%. The population density increased from 1.17 inhabitants /km² in 1872 to 11.18 in 1970, and in this last year the range was 1.03 in the North region and 43.90 in the South-East region. The urban population increased from 31.24% in 1940 to 55.98% in 1970 and for the first time the rural population was smaller than the urban population. In 1950 concerning with marital status 39% of the population 15 years old and over was single and 54% married. In 1970 this rate was respectively 35.4% and 56.6%. The population economically inactive increased from 49.17% in 1940 to 52.24% in 1970. The literacy ratio increased from 43% in 1940, to 48% in 1950 and 68.04% in 1970. The crude birth rate was 43/1000 live births in 1950 and fell to 37.7/1000 in 1970. The fertility rate decreased from 179.3/1000 women (15-49 years old} to 156.7/1000 in 1960/70. The crude death rate decreased from 20.60/1000 inhabitants in 1940/50 to 9.4/1000 in 1960/70. The infant mortality rate still remains high: 171/1000 live births in 1940/50 and 170/1000 in 1971. Concerning with the size of the cities, 8 in 1940 had 100,000 or more inhabitants and in 1970 this number increased to 94 cities. The population growth increased from 2.38% in 1940/50, to 2.99% in 1950/60 and 2.83% in 1960./70. Brazil is the first country in population size in Latin America and the eighth in the world. Concerning his area, Brazil is the fifth country in size.

Em casuística de 20.850 nascidos vivos não gemelares ocorridos em 31 maternidades do Município de São Paulo, SP, Brasil (parte da casuística total do Estudo Antropométrico do Recém-nascido Brasileiro), procurou-se identificar fatores de risco para o baixo peso ao nascer (peso < 2.500 g), tendo sido estipulados a priori, para estudo, os fatores escolaridade materna, estado marital, idade materna, paridade, peso pré-gestacional, tabagismo na gravidez e assistência pré-natal. A partir de análise multivariada pela técnica de modelos log-lineares, foram identificados quatro fatores de risco significativos: "ausência de assistência pré-natal", "peso pré-gestacional < 50 kg", "tabagismo na gestação" e "idade materna < 20 anos". O risco relativo associado a "ausência de pré-natal" foi de 2,2 nas mães de escolaridade inferior ao ginásio completo e de 3,4 nas de escolaridade igual ou superior àquele nível. Os riscos associados às características maternas de peso, tabagismo e idade foram respectivamente 1,9, 1,7 e 1,4 e independentes da escolaridade materna. Considerando-se além da magnitude dos riscos detectados a freqüência com que os fatores de risco se apresentaram na população, constatou-se que o peso pré-gestacional insuficiente, o tabagismo na gestação e a ausência de assistência pré-natal, particularmente em mães de "baixa escolaridade", são fatores cujo controle exerceria importante decréscimo na incidência de recém-nascidos de baixo peso. Assim sendo, tais fatores deveriam ser cuidadosamente considerados em programas de intervenção.

Propõe-se identificar determinadas características de natureza biológica, demográfica, social e institucional com base nas informações registradas nas fichas de prontuário de 2.588 mulheres atendidas com complicações de aborto em um hospital de Santo André, zona urbana e industrializada da Grande São Paulo (Brasil), no período de janeiro de 1978 a dezembro de 1982. Os dados revelaram uma duplicação no número de mulheres com complicações de aborto no período estudado, elevando-se gradualmente de 302 em 1978 para 672 em 1982. O número de partos atendidos no mesmo hospital, não obstante, cresceu em apenas 14% no mesmo período, obtendo-se a média de relação aborto/parto: 1: 3,6 e 1: 1,8 em 1978 e 1982, respectivamente. A maioria da população estudada (60%) possuía ocupação assalariada não especializada. A idade (16,4% das mulheres estudadas eram menores de 20 anos) apresentou relação estatisticamente significativa com o estado marital, cor, ocupação e resultados gestacionais. Houve correlação significativa entre idade na menarca, idade na primeira relação sexual e idade na primeira gravidez, no grupo de mulheres primigestas.

São analisados o conhecimento e a utilização de métodos anticoncepcionais por adolescentes. Foram levantados os dados a partir de prontuários médicos e de entrevistas relativos a 78 adolescentes puérperas (parto ou aborto), atendidas em um serviço de obstetrícia do Município de Cotia, SP (Brasil), no período de 1/5/86 a 31/7/86. Do total de adolescentes estudadas, 61,5% tinham algum tipo de conhecimento sobre métodos anticoncepcionais, conhecimento este influenciado por fatores tais como idade, escolaridade, paridade e estado marital. As fontes de informação mais procuradas foram os amigos, os parentes e os parceiros, nesta ordem; as menos procuradas foram os profissionais de saúde. Somente uma em cada dez adolescentes usava algum tipo de anticoncepcional, sendo os mais prevalentes a pílula, o método Ogino-Knauss, preservativos e o coito interrompido. Em 100% dos casos de utilização destes métodos houve indicação por parte de pessoas do grupo social das adolescentes, sendo os anticoncepcionais adquiridos no comércio, sem nenhum controle de saúde.

Population ageing is currently a phenomenon not only in developed countries but also in third world countries. In this paper the features of a population's ageing and the process of epidemiological transition are discussed along with the worldwide changes in age-structure. Population statistics in Brazil and the characteristics of the elderly population are presented and analysed in the light of recent changes. The Brazilian elderly population is also discussed, particularly the issues relating to the social cost of the aged population, its urban and rural distribution, the elderly by sex, marital status and level of schooling, and emphasis is given to the imbalance of the sexes and the consequences of it for women.

The goal this follow-up study was to relate the mother's marital satisfaction to family health status in a low SES. The random sample was made up of 30 families with children under 7 years old: 15 considered as sick (Group A) and 15 as healthy (Group B). Both group had similar demographic characteristics (age of father and mother, persons per family group and age of children) and SES. Results showed that mothers were those mainly in charge of their family groups. Mothers of Group A were significantly less understanding and more dissatisfied than those of Group B ( p < .05 and p < .01). Mothers of Group A had significantly more arguments with their partners than those of Group B (p < .006). Health care was learned less from the child's own mother in group A than in B (p < .05). Health was considered by mothers of Group A as something that "must be taken care of" more than by those of Group B (p < .01). The behaviours of mothers in choosing one of the health systems was similar in both groups. Dissatisfied mothers were associated more with sick family members during the 6 month follow-up. It is suggested that the satisfaction of the mother is a factor that needs further investigation because health is managed by mothers is the large majority of families.

OBJECTIVE: The aim of the study was to identify the variables that predict the revolving door phenomenon in psychiatric hospital at the moment of a second admission. METHODS: The sample consisted of 3,093 patients who have been followed during 5 to 24 years after their first hospital admission due to schizophrenia, and affective or psychotic disorders. Those who had had four or more admissions during the study period were considered as revolving door patients. Logistic regression analyses were used to assess the impact of gender, age, marital status, urban conditions, diagnosis, mean period of stay on the first admission, interval between the first and second admissions on the patterns of hospitalization. RESULTS: The variables with the highest predictive power for readmission were the interval between first and second admissions, and the length of stay in the first admission. CONCLUSIONS: These data may help public health planners in providing optimal care to a small group of patients with more effective utilization of the available services.

OBJECTIVE: To identify risk factors for low birth weight (LBW) among live births by vaginal delivery and to determine if the disappearance of the association between LBW and socioeconomic factors was due to confounding by cesarean section. METHODS: Data were obtained from two population-based cohorts of singleton live births in Ribeirão Preto, Southeastern Brazil. The first one comprised 4,698 newborns from June 1978 to May 1979 and the second included 1,399 infants born from May to August 1994. The risks for LBW were tested in a logistic model, including the interaction of the year of survey and all independent variables under analysis. RESULTS: The incidence of LBW among vaginal deliveries increased from 7.8% in 1978--79 to 10% in 1994. The risk was higher for: female or preterm infants; newborns of non-cohabiting mothers; newborns whose mothers had fewer prenatal visits or few years of education; first-born infants; and those who had smoking mothers. The interaction of the year of survey with gestational age indicated that the risk of LBW among preterm infants fell from 17.75 to 8.71 in 15 years. The mean birth weight decreased more significantly among newborns from qualified families, who also had the highest increase in preterm birth and non-cohabitation. CONCLUSIONS: LBW among vaginal deliveries increased mainly due to a rise in the proportion of preterm births and non-cohabiting mothers. The association between cesarean section and LBW tended to cover up socioeconomic differences in the likelihood of LBW. When vaginal deliveries were analyzed independently, these socioeconomic differences come up again.

OBJECTIVE: To evaluate the hypothesis that work burden, the simultaneous engagement in paid work and unpaid family housework, is a potential risk factor for psychiatric symptoms among women. METHODS: A cross-sectional study was carried out with 460 women randomly selected from a poor area of the city of Salvador, Brazil. Women between 18 to 70 years old, who reported having a paid occupation or were involved in unpaid domestic activities for their families, were eligible. Work burden-related variables were defined as: a) double work shift, i.e., simultaneous engagement in a paid job plus unpaid housework; and b) daily working time. Psychiatric symptoms were collected through a validated questionnaire, the QMPA. RESULTS: Positive, statistically significant associations between high (>7 symptoms) QMPA scores and either double work shift (prevalence ratio -- PR=2.04, 95% confidence interval -- CI: 1.16, 2.29) or more than 10 hours of daily work time (PR=2.29, 95% CI: 1.96, 3.43) were found after adjustment for age, marital status and number of pre-school children. CONCLUSIONS: Major correlates of high QMPA scores are work burden variables. Being married or having pre-school children are also associated with high QMPA scores only when associated with work burden.

OBJETIVO: Verificar a associação entre fatores epidemiológicos e infecção genital pelo papilomavírus humano (HPV). MÉTODOS: Realizou-se estudo transversal com 975 mulheres atendidas em um serviço público de rastreamento para o câncer cervical, em Porto Alegre, Brasil. As mulheres foram consideradas infectadas pelo HPV quando apresentaram o teste de DNA positivo para esse vírus, tanto pelo método de captura híbrida II (CH II) como pelo método de reação em cadeia da polimerase (PCR). Mulheres infectadas pelo HPV foram comparadas com mulheres não infectadas oriundas da mesma população. RESULTADOS: Foram estudadas 975 mulheres. A prevalência observada de HPV (pela combinação dos métodos de DNA) foi de 27%. Quando a análise de cada método de DNA foi feito isoladamente, a prevalência de HPV-DNA foi de 15% para a CH II e de 16% para PCR. Regressão logística múltipla incondicional foi utilizada na identificação dos fatores associados à infecção pelo HPV. Foi encontrada associação positiva com as seguintes variáveis: anos de escolaridade (11 anos: OR=2,05; IC95%=1,31; 3,20; referência: até oito anos de escolaridade); ser casada (OR=1,69; IC95%=0,78; 2,00; referência: ser solteira); parceiros sexuais ao longo da vida (dois parceiros: OR=1,67; IC95%=1,01; 2,77; quatro ou mais: OR=2,18; IC95%=1,15; 4,13; referência: um parceiro); idade da primeira relação sexual (15-16 anos: OR=4,05; IC95%=0,89; 18,29; referência: > ou = 22 anos). CONCLUSÕES: Vários fatores parecem estar associados à presença de infecção genital pelo HPV, especialmente aqueles referentes ao comportamento sexual (idade da primeira relação sexual, número de parceiros sexuais ao longo da vida e estado marital) e aqueles relacionados à situação socioeconômica (escolaridade).

OBJECTIVE: To study patterns of alcohol consumption and prevalence of high-risk drinking. METHODS: A household survey was carried out in a sample of 2,302 adults in Salvador, Brazil. Cases of High-Risk Drinking (HRD) were defined as those subjects who referred daily or weekly binge drinking plus episodes of drunkenness and those who reported any use of alcoholic beverages but with frequent drunkenness (at least once a week). RESULTS: Fifty-six per cent of the sample acknowledged drinking alcoholic beverages. Overall consumption was significantly related with gender (male), marital status (single), migration (non-migrant), better educated (college level), and social class (upper). No significant differences were found regarding ethnicity, except for cachaça (Brazilian sugarcane liquor) and other distilled beverages. Overall 12-month prevalence of high-risk drinking was 7%, six times more prevalent among males than females (almost 13% compared to 2.4%). A positive association of HRD prevalence with education and social class was found. No overall relationship was found between ethnicity and HRD. Male gender and higher socioeconomic status were associated with increased odds of HRD. Two-way stratified analyses yielded consistent gender effects throughout all strata of independent variables. CONCLUSIONS: The findings suggest that social and cultural elements determine local patterns of alcohol-drinking behavior. Additional research on long-term and differential effects of gender, ethnicity, and social class on alcohol use and misuse is needed in order to explain their role as sources of social health inequities.

In the context of the design of working hours inequities in health associated with biological, psychological, social, and socioeconomic diversities can be observed. The paper first tries to set up a frame of reference for a discussion of this topic, relating to the Universal Declaration of Human Rights and some recent discussions on equity in health and then goes into some factors that produce inequities in health in the context of the design of working hours, dealing with sex or gender, age and job age, personality traits, marital status, social support, diversities in values, and socio-economic differences; the discussion deals with approaches on how to deal with these differences and inequities.

OBJECTIVE: To analyze the prevalence of cigarette smoking in individuals with severe mental illnesses in a large urban centre of a middle income country. METHODS: Cross-sectional study carried out in São Paulo. The sample (N=192) comprised individuals diagnosed with severe mental illnesses who had contact with public psychiatric care services from September to November 1997 and were aged between 18 and 65 years. Prevalence of daily tobacco smoking in the 12 months previous to the interview and characteristics associated were studied. RESULTS: Out of 192 subjects with severe mental illnesses interviewed, 115 (59.9%; 95% CI: 52.6%; 66.9%) reported smoking cigarettes on a daily basis. Male gender, marital status separated or widowed, irregular use of neuroleptic drugs and history of ten or more psychiatric admissions were independently associated with cigarette smoking. CONCLUSIONS: The prevalence of cigarette smoking in the present sample was higher than that found in the general Brazilian population. Mental care services should implement non-smoking policies and mental health providers need to help patients with severe mental illness who want to quit smoking.
