In August 1983,85 inhabitants of the municipality of Humaitá, Amazonas State, Brazil were studied to determine the prevalence of antigens to HLA-A, -B, -C and DR. Thirty-eight were sick with malaria due to Plasmodium falciparum. All subjects were examined for splenomegaly, blood parasitaemia and antibodies to malaria. They constituted three groups: 1) 25 subjects native to the Amazon region who had never had malaria; 2) 38 Amazonian subjects who had malaria in the past or currently had an infection; 3) 22 patients with malaria who had acquired the infection in the Amazon Region but came from other regions of Brazil. Blood was taken from each person, the lymphocytes were separated and typed by the test of microlymphocytotoxicity. There was a high frequency of antigens that could not be identified in the groups studied which suggests the existence of a homozygote or phenotype not identified in the population. There was a high frequency of the phenotype Ag(W24) (44.7%) in group 2 when compared with group 1 (32%) or group 3 (9%). Also the individuals in group 2 showed an elevated frequency of antigen DR(4)80%) when compared with group 1 (36.6%) or group 3 (16.6%). These observations suggest the possibility of a genetic susceptibility to malaria among Amazonian residents and indicate a necessity for more extensive studies of the frequency of HLA antigens among inhabitants of this endemic malarial zone.

A total of 207 patients with malaria caused by Plasmodium falciparum were submitted to 5 different treatment schedules with clindamycin from 1981 to 1984: A - 89 patients were treated intravenously and orally, or intramuscularly and orally with 20 mg/kg/day divided into two daily applications for 5 to 7 days; B-40 patients were treated orally with 20 mg/kg/day divided into two daily doses for 5 to 7 days; C-27 patients were treated with 20 mg/kg/day intravenously or orally divided into two daily applications for 3 days; D-16 patients were treated orally and/or intravenously with a single daily dose of 20 to 40 mg/kg/day for 5 to 7 days; E-35 patients were treated orally with 5 mg/kg/day divided into two doses for 5 days. Patients were examined daily during treatment and reexamined on the 7th, 24th, 21st, 28th and 35th day both clinically and parasitologically (blood test). Eighty three (40.1%) had moderate or severe malaria, and 97 (46.8%) had shown resistance to chloroquine or to the combination ofsulfadoxin and pyrimethamine. The proportion of cured patients was higher than 95% among patients submitted to schedules A and B. Side effects were only occasional and of low intensity. Three deaths occurred (1.4%), two of them involving patients whose signs and symptoms were already very severe when treatment was started. Thus, clindamycin proved to be very useful in the treatment of patients with malaria caused by Plasmodium falciparum and we recommend schedule A for moderate and severe cases and Bfor initial cases.

FUNDAMENTO: A inflamação vem sendo implicada na fisiopatologia de uma série de doenças cardiovasculares. A proteína C-reativa (PCR) titulada é um marcador de inflamação de fácil obtenção na sala de emergência. OBJETIVO: Estudar o valor prognóstico da PCR em pacientes admitidos por insuficiência cardíaca (IC) descompensada. MÉTODOS: Coorte prospectiva de 119 pacientes com IC descompensada, atendidos na sala de emergência, com média de idade de 74 ± 11 anos, dos quais 76 (64%) eram do sexo masculino. Todos estavam em classe funcional III ou IV da New York Heart Association. A dosagem da PCR foi realizada por ocasião da admissão na sala de emergência, pelo método de nefelometria. Os pacientes foram acompanhados, após a alta hospitalar, por um tempo médio de 12 ± 9,7 meses e o desfecho analisado foi a mortalidade cardiovascular. RESULTADOS: Houve 44 (36,9%) óbitos, todos por causa cardiovascular. Indivíduos com PCR > 3 mg/dl apresentaram maior mortalidade que indivíduos com valores inferiores a esse (p=0,018). A análise multivariada pelo modelo proporcional de Cox destacou como fator independente para prognóstico mais importante a PCR (razão de chances de 0,0916 [intervalo de confiança de 95% = 0,0341 a 0,1490] para aumentos de uma unidade na PCR). CONCLUSÃO: A PCR é um preditor independente de mortalidade cardiovascular em pacientes com IC descompensada, indicando que a inflamação representa componente importante na fisiopatologia da doença.

A community-based random survey was conducted in a southern Brazilian Amazonian county aiming to investigate hepatitis C virus (HCV) infection prevalence and the association of demographic variables and lifestyle behaviours. Seven hundred eighty individuals were serologically screened with a third generation enzyme-linked immunosorbent assay to detect anti-HCV antibodies between 1994/1995. Positive samples were retested for confirmation with a line immunoassay (LIA, Inno-LIA HCV Ab III). Most of these subjects were low income and came from southern Brazilian states (65.8). Two point four percent (IC 95% 1.2%- 4.6%) of the subjects had LIA-confirmed anti-HCV antibodies reactivity. The age-specific prevalence of HCV antibodies slightly increased with age, with the highest prevalence after the age of 40 years. The results of multivariate analysis indicate a strong association between HCV antibodies and previous surgery and history of intravenous drug use. There were no apparent association with gender, hepatitis B virus markers, blood transfusion, and sexual activity. Mean time living in Amazon did not differ between confirmed and negative anti-HCV individuals. The present data point out an intermediate endemicity of HCV infection among this immigrant community to the Amazon region and that few HCV infected participants presented known risk factors.

A population survey of the giant conifer aphid Cinara spp. was carried out in two areas of Pinus elliottii using yellow pan traps, during two years, from July 1997 to June 1999. During the collection period, the predominant species was Cinara pinivora (Wilson, 1919), representing 99% of the collected species. A few specimens of Cinara maritimae (Dufour, 1833) and only one winged female of Cinara fresai Blanchard, 1939 were collected. The population of C. pinivora was statistically higher in the area with 4.5 year-old trees than in the 1.5 year-old ones. The highest population peaks were registered in July 1997 and in August 1998. An unexpected high number of winged parthenogenetic females was collected in December 1998. C. fresai Blanchard, 1939 is recorded for the first time for Brazil.

Despite the importance of human taeniasis/bovine cysticercosis for public and animal health and the economy, its actual epidemiological status in Brazil is not well-studied. We sought to assess the epidemiological profile of this zoonosis in the rural area of Viçosa County, Minas Gerais, Brazil. The prevalence of bovine cysticercosis was 0.42%, whereas no case of human taeniasis was diagnosed. Factors favoring the persistence of zoonosis were identified. These included the supply of untreated water to animals, animals raised for slaughter without sanitary supervision, poor mechanization of raising techniques, and the use of untreated water for human consumption. Bovine meat for human consumption acquired in the city or from the farm itself was characterized as a risk factor for bovine cysticercosis (Odds Ratio (OR) =16.77; p<0.05). Nevertheless, the families on the investigated farms did apply several appropriate measures to control this disease, such as the virtual lack of open sewers and the consumption of well-cooked meat. The presence of bovine cysticercosis cases, and the factors favoring its persistence, point to the need for constant epidemiological and sanitary surveillance in this county.
