A identificação precoce do sexo em peixes é um procedimento importante para o desenvolvimento de estratégias que facilitem o manejo no plantel de reprodutores. A endoscopia pode ser utilizada para essa identificação, embora em peixes seu uso ainda seja restrito. Diante disso, o objetivo deste trabalho foi validar o uso do endoscópio rígido via poro urogenital para determinação do sexo do catfish de canal (Ictalurus punctatus). O equipamento utilizado foi um endoscópio Multipurpose Rigid 30º com 2,7 mm de diâmetro e 18 cm de comprimento. Foram examinados 60 peixes com peso variando entre 86 e 1.032 gramas nos quais foi introduzido o endoscópio via poro genital e, logo em seguida, eutanaziados para confirmação do sexo. Amostras de tecido gonadal foram coletadas para avaliação histológica do estádio de maturação e confirmação do sexo. Os resultados mostraram que dos 60 peixes examinados, 57 tiveram confirmação positiva do sexo via endoscopia, o que corresponde a 95% da amostra. A utilização da endoscopia para determinação do sexo em I. punctatus via poro genital é viável.

A patient with miliary tuberculosis and a chronic urogenital focus is described, who had a borderline renal function at diagnosis and developed overt renal failure upon daily treatment with rifampin (RMP), isoniazid (INH) and ethambutol (EMB). This is the first Brazilian report of BMP induced renal damage. A renal biopsy taken on the third day of oliguria showed recent tubular necrosis with acute interstitial inflammation and granuloma formation. The aspect of the granulomatous lesion hightly suggested drug etiology because of the lack of palisading, high incidence of neutrophils and absence of facid-fast bacilli. This is the first presentation of an acute granulomatous interstitial nephritis probably due to RMP. Furthermore the pathogenesis of the renal damage caused by tuberculosis and RMP are discussed.

Lymphatic filariasis (LF) causes a wide range of clinical signs and symptoms, including urogenital manifestations. Transmission control and disability/morbidity management/control are the two pillars of the overall elimination strategy for LF. Lymph scrotum is an unusual urological clinical presentation of LF with important medical, psychological, social and economic repercussions. A retrospective case series study was conducted on outpatients attended at the National Reference Service for Filariasis, in an endemic area for filariasis (Recife, Brazil), between 2000 and 2007. Over this period, 6,361 patients were attended and seven cases with lymph scrotum were identified. Mean patient age was 45 years (range, 26 to 64 years). Mean disease duration was 8.5 years (range, two to 15 years). All patients had evidence of filarial infection from at least one laboratory test (parasitological, antigen investigation or "filarial dance sign" on ultrasound). Six patients presented histories of urological surgery. The authors highlight the importance of the association between filarial infection and the inadequate surgical and clinical management of hydrocele in an endemic area, as risk factors for lymph scrotum. Thus, filarial infection should be routinely investigated in all individuals presenting urological morbidity within endemic areas, in order to identify likely links in the transmission chain.

Actinomycosis is a rare, chronic, suppurative, granulomatous infection caused by a group of gram-positive anaerobic bacteria belonging to the natural flora of the oral cavity and gastrointestinal and urogenital tracts. It may involve several organs. This case study refers to pulmonary actinomycosis with chest wall involvement and cord compression in a 29-year-old male who presented with fever, cough, hemoptysis, neck pain, and paresis and plegia of the lower limbs of 5-month duration.

INTRODUCTION: Chlamydia infection is associated with debilitating human diseases including trachoma, pneumonia, coronary heart disease and urogenital diseases. Serotypes of C. trachomatis show a fair correlation with the group of diseases they cause, and their distribution follows a well-described geographic pattern. Serotype A, a trachoma-associated strain, is known for its limited dissemination in the Middle East and Northern Africa. However, knowledge on the spread of bacteria from the genus Chlamydia as well as the distribution of serotypes in Brazil is quite limited. METHODS: Blood samples of 1,710 individuals from ten human population groups in the Amazon region of Brazil were examined for antibodies to Chlamydia using indirect immunofluorescence and microimmunofluorescence assays. RESULTS: The prevalence of antibodies to Chlamydia ranged from 23.9% (Wayana-Apalai) to 90.7% (Awa-Guaja) with a mean prevalence of 50.2%. Seroreactivity was detected to C. pneumoniae and to all serotypes of C. trachomatis tested; furthermore, we report clear evidence of the as-yet-undescribed occurrence of serotype A of C. trachomatis. CONCLUSIONS: Specific seroreactivity not only accounts for the large extent of dissemination of C. trachomatis in the Amazon region of Brazil but also shows an expanded area of occurrence of serotype A outside the epidemiological settings previously described. Furthermore, these data suggest possible routes of Chlamydia introduction into the Amazon region from the massive human migration that occurred during the 1,700s.

A ocorrência do papiloma vírus humano (HPV) é um problema de saúde pública, pois tem sido associado ao câncer. O objetivo da pesquisa foi identificar a ocorrência de papilomavírus humano na cérvice uterina de mulheres da região ocidental da Amazônia Brasileira. O estudo foi realizado na capital de Rondônia, Porto Velho. Foram identificados os tipos de HPV e resultados moleculares foram correlacionados com aqueles os testes colpocitológicos de amostras provenientes de 334 mulheres que realizaram exames preventivos no Sistema Único de Saúde. Obteve-se o material genético viral do papilomavírus humano (DNA-HPV) e o fragmento de 450 pb da região conservada do gene L1 amplificado e submetido à análise do polimorfismo dos fragmentos de restrição (RFLP). Das 334 amostras analisadas, 31% foram confirmados com a presença de material viral (DNA-HPV). Confirmou-se a existência dos tipos: HPVS-16, 18, 33, 53 e 58, que identificam o grupo de alto risco oncogênico com 72% (74/103) de ocorrência, bem como os HPVS-11, 42 e 44 pertencentes ao grupo de baixo risco oncogênico com 28% de ocorrência. Os perfis recorrentes durante o desenvolvimento da análise foram do HPV-16 e -18 com 17% e 16%, respectivamente. Os resultados da pesquisa indicam que mais de 80% das amostras analisadas e que continham material viral não apresentavam nenhuma alteração celular no teste citológico, o que reforça a necessidade de se difundir o uso das técnicas moleculares em diagnósticos convencionais.

Trypanosoma cruzi is usually transmitted by contact with the excreta of infected Triatominae; among non-vectorial infections, direct transmission through coitus has been proposed. We investigated this possibility by instilling, through the external meatus of the vagina and the penis of previously anesthetized NMRI albino mice, blood of mice infected with strains isolated from Didelphis marsupialis (opossum, strain CO57), Rattus rattus (rat, strain CO22) and human (strain EP). Some animals were allowed to copulate the same day of the instillation. In other experiments, the strains were inoculated in the scrotum. To determine the effect of immunosuppression, some mice were treated with cyclophosphamide 30 days post-instillation. Controls were instilled orally and ocularly. Vaginal instillation with strain CO22 produced systemic infection with tropism to the heart, skeletal muscle, skin, duodenum, pancreas, ovary and sternum. Scrotal inoculation with strain EP likewise invaded liver, spleen, lung, lymph nodes and urogenital organs; while strain CO57 invaded skeletal and cardiac muscle, pancreas, testis, and vas deferens. Penile infection with strain CO22 was detected by xenodiagnosis. Immunosuppression did not increase parasitemia of vaginally infected mice or controls. Mating did not produce infection. Our results show that contact of blood trypomastigotes of T. cruzi with genital mucosa can produce blood and tissue infections. These results are discussed in relation to reports of frequent experimental tropism of T. cruzi toward urogenital organs.

Trichomonas vaginalis is a parasite of the human urogenital tract that causes trichomonosis, the most prevalent non-viral sexually transmitted disease. Ectonucleoside triphosphate diphosphohydrolase (NTPDase) family members, which hydrolyse extracellular ATP and ADP and ecto-5′-nucleotidase, which hydrolyses AMP, have been characterised in T. vaginalis. For trichomonad culture, the growth medium is supplemented with 10% serum, which is an important source of nutrients, such as adenosine. Here, we investigated the ATP metabolism of T. vaginalis trophozoites from long-term cultures and clinical isolates under limited bovine serum conditions (1% serum). The specific enzymatic activities were expressed as nmol inorganic phosphate (Pi) released/min/mg protein, the gene expression patterns were determined by reverse transcriptase-polymerase chain reaction, the extracellular adenine nucleotide hydrolysis was analysed by high performance liquid chromatography and the cell cycle analysis was assessed by flow cytometry. Serum limitation led to the profound activation of NTPDase and ecto-5'-nucleotidase activities. Furthermore, the levels of NTPDase A and B transcripts increased and extracellular ATP metabolism was activated, which led to enhanced ATP hydrolysis and the formation of ADP and AMP. Moreover, the cell cycle was arrested at the G0/G1 stage, which suggested adenosine uptake. Our data suggest that under conditions of serum limitation, NTPDase and ecto-5'-nucleotidase play a role in providing the adenosine required for T. vaginalis growth and that this process contributes to the establishment of parasitism.

Trichomonas vaginalis and Tritrichomonas foetus are parasitic, flagellated protists that inhabit the urogenital tract of humans and bovines, respectively. T. vaginalis causes the most prevalent non-viral sexually transmitted disease worldwide and has been associated with an increased risk for human immunodeficiency virus-1 infection in humans. Infections by T. foetus cause significant losses to the beef industry worldwide due to infertility and spontaneous abortion in cows. Several studies have shown a close association between trichomonads and the epithelium of the urogenital tract. However, little is known concerning the interaction of trichomonads with cells from deeper tissues, such as fibroblasts and muscle cells. Published parasite-host cell interaction studies have reported contradictory results regarding the ability of T. foetus and T. vaginalis to interact with and damage cells of different tissues. In this study, parasite-host cell interactions were examined by culturing primary human fibroblasts obtained from abdominal biopsies performed during plastic surgeries with trichomonads. In addition, mouse 3T3 fibroblasts, primary chick embryo myogenic cells and L6 muscle cells were also used as models of target cells. The parasite-host cell cultures were processed for scanning and transmission electron microscopy and were tested for cell viability and cell death. JC-1 staining, which measures mitochondrial membrane potential, was used to determine whether the parasites induced target cell damage. Terminal deoxynucleotidyltransferase-mediated dUTP nick end labelling staining was used as an indicator of chromatin damage. The colorimetric crystal violet assay was performed to ana-lyse the cytotoxicity induced by the parasite. The results showed that T. foetus and T. vaginalis adhered to and were cytotoxic to both fibroblasts and muscle cells, indicating that trichomonas infection of the connective and muscle tissues is likely to occur; such infections could cause serious risks to the infected host.

Trichomonas vaginalis and Tritrichomonas foetus are parasitic protists of the human and bovine urogenital tracts, respectively. Several studies have described the cytotoxic effects of trichomonads on urogenital tract epithelial cells. However, little is known about the host cell response against trichomonads. The aim of this study was to determine whether T. foetus and T. vaginalis stimulated the release of the cytokine interleukin (IL)-10 from cultured bovine epithelial cells. To characterise the inflammatory response induced by these parasites, primary cultures of bovine oviduct epithelial cells were exposed to either T. vaginalis or T. foetus. Within 12 h after parasite challenge, supernatants were collected and cytokine production was analysed. Large amounts of IL-10 were detected in the supernatants of cultures that had been stimulated with T. foetus. Interestingly, T. vaginalis induced only a small increase in the release of IL-10 upon exposure to the same bovine cells. Thus, the inflammatory response of the host cell is species-specific. Only T. foetus and not T. vaginalis induced the release of IL-10 by bovine oviduct epithelial cells.

Trichomonas vaginalis is a flagellate protozoan that parasitises the urogenital human tract and causes trichomoniasis. During the infection, the acquisition of nutrients, such as iron and purine and pyrimidine nucleosides, is essential for the survival of the parasite. The enzymes for purinergic signalling, including adenosine deaminase (ADA), which degrades adenosine to inosine, have been characterised in T. vaginalis. In the evaluation of the ADA profile in different T. vaginalisisolates treated with different iron sources or with limited iron availability, a decrease in activity and an increase in ADA gene expression after iron limitation by 2,2-bipyridyl and ferrozine chelators were observed. This supported the hypothesis that iron can modulate the activity of the enzymes involved in purinergic signalling. Under bovine serum limitation conditions, no significant differences were observed. The results obtained in this study allow for the assessment of important aspects of ADA and contribute to a better understanding of the purinergic system in T. vaginalis and the role of iron in establishing infection and parasite survival.

OBJETIVO: As fístulas vesicovaginais e ureterovaginais são complicações incomuns, secundárias a doenças ou a cirurgias pélvicas. O sucesso terapêutico dessas fístulas depende de adequada avaliação pré-operatória para o diagnóstico e visualização do seu trajeto. Este trabalho tem o objetivo de demonstrar o potencial da urorressonância no diagnóstico das fístulas urogenitais e na visualização dos seus trajetos. MATERIAIS E MÉTODOS: Foram analisados, retrospectivamente, os prontuários médicos e as imagens radiológicas e de urorressonância magnética de sete pacientes do sexo feminino com diagnóstico de fístula urogenital. Para a urorressonância foram realizadas seqüências 3D-HASTE com saturação de gordura. RESULTADOS: Seis pacientes apresentavam fístula vesicovaginal e uma paciente tinha diagnóstico de fístula ureterovaginal à direita. Com a utilização da urorressonância magnética, foi possível demonstrar o trajeto da fístula em seis das sete pacientes (85,7%), sem a necessidade de cateterização vesical ou da injeção de contraste. CONCLUSÃO: Este estudo demonstra o potencial e a aplicabilidade da urorressonância na avaliação dessas fístulas.

OBJETIVO: Avaliar o impacto da prática de atividade física na qualidade de vida de mulheres de meia idade. MÉTODOS: Estudo de base populacional e corte transversal, que incluiu uma amostra estratificada de 370 mulheres de meia idade entre 40 a 65 anos, recrutadas a partir de uma população de 20.801 mulheres atendidas no período de um ano nas redes básicas de saúde, inseridas nos quatro distritos (Norte, Sul, Leste e Oeste) que compõem o sistema de saúde da cidade de Natal, Rio Grande do Norte, de junho a setembro de 2011. O cálculo da amostra teve por base um nível de confiança de 95%, com poder do teste de 80%, erro de estimativa de 5% e considerou-se a proporção de pacientes classificadas com qualidade de vida adequada (indicador >26) da amostra piloto. Os dados foram coletados enquanto as mulheres aguardavam na sala de espera para a consulta de rotina. Para avaliar a qualidade de vida geral, utilizou-se a versão abreviada do WHOQOL (WHOQOL-Bref-WHO Quality of Life - BREF), e sua relação com os sintomas do climatério foi avaliada por meio do Menopause Rating Scale (MRS). O nível de atividade física foi avaliado pelo questionário International Physical Activity Questionnaire (IPAQ), versão curta, semana usual. Para obter-se a classificação dos níveis de atividade física, utilizaram-se três categorias: sedentária, moderadamente ativa e muito ativa. A análise estatística foi realizada utilizando o programa estatístico Minitab, versão 16. RESULTADOS: A média de idade das mulheres foi de 49,8 anos (±8.1), foram predominantemente caucasianas (72,7%), casadas (61,6%), não fumantes (93,5%) e com o Ensino Médio completo (47,8%). Considerando os domínios presentes no WHOQOL-Bref para avaliar qualidade de vida, os escores foram significativamente diferentes entre os grupos de mulheres sedentárias, moderadamente ativas e muito ativas (p<0,01). Em relação à atividade física e aos sintomas do climatério, foram observadas diferenças significativas para todos os domínios: psicológico (p<0,01), somático-vegetativo (p<0,01) e urogenital (p<0,01). CONCLUSÕES: A prática de atividade física melhora significativamente a qualidade de vida das mulheres de meia idade.

PURPOSE: To evaluate changes to the pelvic floor of primiparous women with different delivery modes, using three-dimensional ultrasound. METHODS: A prospective cross-sectional study on 35 primiparae divided into groups according to the delivery mode: elective cesarean delivery (n=10), vaginal delivery (n=16), and forceps delivery (n=9). Three-dimensional ultrasound on the pelvic floor was performed on the second postpartum day with the patient in a resting position. A convex volumetric transducer (RAB4-8L) was used, in contact with the large labia, with the patient in the gynecological position. Biometric measurements of the urogenital hiatus were taken in the axial plane on images in the rendering mode, in order to assess the area, anteroposterior and transverse diameters, average thickness, and avulsion of the levator ani muscle. Differences between groups were evaluated by determining the mean differences and their respective 95% confidence intervals. The proportions of levator ani muscle avulsion were compared between elective cesarean section and vaginal birth using Fisher's exact test. RESULTS: The mean areas of the urogenital hiatus in the cases of vaginal and forceps deliveries were 17.0 and 20.1 cm², respectively, versus 12.4 cm² in the Control Group (elective cesarean). Avulsion of the levator ani muscle was observed in women who underwent vaginal delivery (3/25), however there was no statistically significant difference between cesarean section and vaginal delivery groups (p=0.5). CONCLUSION: Transperineal three-dimensional ultrasound was useful for assessing the pelvic floor of primiparous women, by allowing pelvic morphological changes to be differentiated according to the delivery mode.

OBJETIVO: Avaliar a prevalência da Síndrome Climatérica (SC) em mulheres de São Luís, Maranhão, uma das regiões menos desenvolvidas do Nordeste brasileiro.MÉTODOS: Estudo descritivo e exploratório de corte transversal, tipo inquérito populacional domiciliar. Foram selecionadas 1.210 mulheres climatéricas de 45 a 60 anos de idade em São Luís, Maranhão, Brasil. As entrevistas foram aplicadas por meio de questionários, no período de abril a julho de 2008, contendo variáveis sociodemográficas, status menopausal e intensidade dos sintomas climatéricos (índices circulatório e psicológico), sendo que para estes últimos foi realizada a sua associação com os períodos pré e pós-menopausal. A técnica de Análise Correspondência Múltipla (ACM) foi utilizada para avaliar a inter-relação entre os sintomas climatéricos.RESULTADOS: A maioria das pacientes tinha entre 55 a 60 anos (35,3%), era de cor parda (37,9%), com 9 a 11 anos de escolaridade (39,8%), com parceiro (56%), católica (73,9%) e de classe social C (51,1%). A prevalência da SC foi de 85,9%, destacando-se fogachos (56,4%) e sudorese (50,4%) como os sintomas vasomotores mais prevalentes. Os sintomas psicológicos mais frequentes foram nervosismo (45%) e irritabilidade (44,8%). Houve predomínio do ressecamento vaginal (62,7%) como a queixa urogenital mais prevalente. A intensidade dos sintomas vasomotores e psicológicos foi significativamente mais elevada nas fases de peri e pós-menopausa (p<0,05).CONCLUSÃO: A prevalência da SC foi elevada em mulheres dessa cidade do Nordeste do Brasil.
