The '10/90 gap' was first highlighted by the Global Forum for Health Research. It refers to the finding that 90% of worldwide medical research expenditure is targeted at problems affecting only 10% of the world's population. Applying research results from the rich world to the problems of the poor may be a tempting, potentially easy and convenient solution for this gap. This paper had the objective of presenting arguments that such an approach runs the risk of exporting failure. Health interventions that are shown to be effective in the specific context of a Western industrialized setting will not necessarily work in the developing world.

OBJETIVO Analisar a evolução da mortalidade perinatal quanto à dimensão do problema e sua extensão. MÉTODOS Estudo descritivo de tendência temporal com 10.994 óbitos perinatais, de mães residentes em Salvador, BA, com idade gestacional ≥ 22 semanas, idade do recém-nascido até seis dias e 500 g ou mais de peso ao nascer, registrados de 2000 a 2009. Utilizaram-se dados do Sistema de Informações de Nascidos Vivos e do Sistema de Informações sobre Mortalidade do sitio eletrônico do Datasus/Ministério da Saúde. Calcularam-se taxas de mortalidade perinatal e fetal/1.000 nascimentos e neonatal precoce/1.000 nascidos vivos. Aplicaram-se: teste Qui-quadrado de Pearson para diferenças em proporções, teste de sequências ( runs ), cálculo de médias móveis e coeficiente de determinação linear (R 2 ) para análise de tendência. Utilizou-se a classificação de Wigglesworth para causas de morte. RESULTADOS A taxa de mortalidade perinatal mostrou tendência decrescente, sendo reduzida em 42,0% no período (de 33,1 (2000) para 19,2 (2009)), com maior contribuição da taxa neonatal precoce (-56,3%). A mortalidade fetal representou grande proporção (61,9%) da taxa de mortalidade perinatal em 2009. A classificação dos óbitos apontou como causas mais frequentes de óbito perinatal: asfixia intraparto (8,8/1.000), imaturidade (7,1/1.000) e malformações congênitas (1,3/1.000). CONCLUSÕES Mesmo em declínio, a taxa de mortalidade perinatal continua elevada e o predomínio recente da mortalidade fetal indica mudança no perfil de causas e impacto nas ações de prevenção. A consulta pré-natal de qualidade com controle de riscos e melhoria da assistência ao parto pode reduzir a ocorrência de causas evitáveis.

Multiple exposures to parasitic agents are considered an important factor in the genesis of the most severe forms of the diseases they cause. Capillaria hepatica-induced septal fibrosis of the liver in rats usually runs without signs of portal hypertension or hepatic failure. After determining the hepatic profile of 15 animals during the course of a single infection, we submitted 20 rats to multiple Capillaria hepatica infections to determine whether repeated exposures would augment fibrosis production, transforming septal hepatic fibrosis into a true cirrhosis. Ten single-infection rats served as controls. A total of 5 exposures, with 45-day intervals, were made. Histological changes were followed by means of surgical liver biopsies, collected prior to infection and to each re-infection. Functional changes were minimal and transient. Although a slight recrudescence of fibrosis was observed after the first two re-infections and when the single-infected control group was re-infected at the end of the experiment, subsequent re-infections failed to increase the amount of fibrosis. On the contrary, there occurred quantitative and qualitative evidence of collagen degradation and suppression of parasite development. These paradoxical results are in keeping with the hypothesis that a complex immunological modulation participates in the mechanism of hepatic fibrosis induced by Capillaria hepatica infection in rats.

In this study, HIV-1 viral load quantitation determined by Nucleic Acid Sequence Based Amplification (NASBA) was compared with other surrogate disease progression markers (antigen p24, CD4/CD8 cell counts and b-2 microglobulin) in 540 patients followed up at São Paulo, SP, Brazil. HIV-1 RNA detection was statistically associated with the presence of antigen p24, but the viral RNA was also detected in 68% of the antigen p24 negative samples, confirming that NASBA is much more sensitive than the determination of antigen p24. Regarding other surrogate markers, no statistically significant association with the detection of viral RNA was found. The reproducibility of this viral load assay was assessed by 14 runs of the same sample, using different reagents batches. Viral load values in this sample ranged from 5.83 to 6.27 log (CV = 36 %), less than the range (0.5 log) established to the determination of significant viral load changes.

A new computer program has been developed to help the users of force methods for magnetic moment determination. It provides a user-friendly interface for the calculation of corrected magnetic susceptibilities of paramagnetic materials and enables the user to simulate a number of chemical formulations for the sample under study. The program is written in the Perl scripting language and runs on a Unix platform.

Nitrogen content in natural gas was studied in experimental and computational investigations to identify its influence on the emission level of exhaust gases from combustion facilities. Changes in natural gas composition with different N2 concentrations may result from introducing a new source gas into the system. An industrial burner fired at 75 kW, housed in a laboratory-scale furnace, was employed for runs where the natural gas/N2 proportion was varied. The exhaust and in-furnace measurements of temperature and gas concentrations were performed for different combustion scenarios, varying N2 content from 1-10 %v. Results have shown that the contamination of natural gas with nitrogen reduced the peak flame temperature, the concentration of unstable species, the NO X emission level and the heat transfer rate to the furnace walls, resulting from the recombination reactions.

This work aimed to assess the photocatalytic degradation of a model odor compound (dimethyl disulfide, DMDS), found in liquid and gaseous wastes of plants for processing poultry byproducts. The effect of pH and temperature on adsorption and photocatalytic degradation was evaluated through factorial experimental designs. The results suggest the presence of an optimum region for adsorption, at 45.0 ºC and pH 4.0. In the photocatalytic runs an optimum for temperature and pH was also observed. At 45 ºC and pH 4.0 the removal of DMDS was 99% after 60 min of irradiation.

Hydrogenation of (-)-menthone and (+)-isomenthone was studied at 2.7 MPa and 100 ºC. The objective was to produce a liquid menthol mixture rich in (-)-menthol from dementholized peppermint oil. Ni-based catalysts were tested and compared for this reaction: a) 6 and 12% Ni dispersed into a nonstoichiometric magnesium aluminate (Ni-Mg-Al) with spinel structure; b) Ni-Raney catalyst. Both types of catalysts were active for (-)-menthone and (+)-isomenthone hydrogenation. Lower conversion but higher selectivity to (-)-menthol was obtained with Ni-Mg-Al catalysts. However, they rapidly lost their activity. Instead Ni-Raney catalysts kept its original activity even after several hydrogenation runs.

Here we report the validation of a derivatization method that makes use of fluorescamine as a selective reactant for the quantitative analysis of peptide and protein drugs in the dissolution profile from depot formulations. Typical current methods require separation of the nano/microparticles and time-consuming chromatographic runs. In this study we report a method which can be conducted without the need for complete physical separation of the particles or removal of the unreacted probe. This method was used here for the analysis of the release profile of octreotide in a depot formulation, with results in excellent agreement with reported chromatographic assays.

The application of Lipozyme (Termomyces lanuginosus) immobilized in gelatin gel in aliphatic ester synthesis was investigated taking the esterification of hexanoic acid with n-butanol as a model reaction. Conditions were optimized by factorial design and the highest conversion was obtained under the following conditions: molar ratio alcohol: acid of 2:1, reaction time of 48 h and biocatalyst weight of 7.0 g. Under these conditions the esterification yield was around 98 %. The operational stability of the immobilized lipase was assessed and results showed that after 12 batch runs, the enzyme showed no significant loss of activity.

QSAR modeling is a novel computer program developed to generate and validate QSAR or QSPR (quantitative structure- activity or property relationships) models. With QSAR modeling, users can build partial least squares (PLS) regression models, perform variable selection with the ordered predictors selection (OPS) algorithm, and validate models by using y-randomization and leave-N-out cross validation. An additional new feature is outlier detection carried out by simultaneous comparison of sample leverage with the respective Studentized residuals. The program was developed using Java version 6, and runs on any operating system that supports Java Runtime Environment version 6. The use of the program is illustrated. This program is available for download at lqta.iqm.unicamp.br.

O vira-cabeça do fumo (Nicotiana tabacum), causado pelo Groundnut ringspot virus (GRSV), do gênero Tospovirus, tem sido observado em alta incidência em determinadas áreas da região fumageira do agreste do estado de Alagoas. O arranjo espacial da doença foi analisado em duas áreas de plantio (A e B), formadas por quatro parcelas cada, localizadas no município de Arapiraca. As parcelas foram avaliadas semanalmente, sendo efetuado o mapeamento espacial de plantas sadias e com sintomas de vira-cabeça, bem como determinada a incidência da doença, representada pelo número de plantas com sintomas em relação ao total de plantas avaliadas. Pelas análises de "ordinary runs" e autocorrelação espacial, foi constatada a predominância do arranjo aleatório de plantas doentes, embora tenha sido detectada agregação em alguns casos. Embora não se descarte a possibilidade de mudas infetadas terem influenciado no arranjo da doença, a hipótese mais provável é que as infecções ocorreram principalmente pelo inóculo primário transmitido por tripes virulíferos entrando nas parcelas, oriundos de reservatórios externos.

A podridão negra causada por Xanthomonas campestris pv. campestris, e a alternariose causada por Alternaria brassicicola constituem importantes doenças do repolho no estado de Pernambuco. Em virtude do desconhecimento sobre a epidemiologia dessas doenças a nível regional, foi efetuada a análise comparativa das epidemias com ênfase nos aspectos temporais e espaciais, em 1997 e 1998, cada ano em uma área de plantio diferente, localizadas no município de Camocim de São Félix, Agreste de Pernambuco. Em cada área, a intervalos semanais, foram avaliadas 1.920 plantas quanto à incidência e severidade das duas doenças, bem como efetuado o mapeamento espacial de plantas assintomáticas e sintomáticas. Os valores iniciais (y o) e máximos (y max) de incidência e severidade, as taxas estimadas de progresso da doença (k) e as áreas abaixo das curvas de progresso das doenças (AACPD), apresentaram pequena variação entre as doenças, sendo as diferenças mais evidentes entre os anos de plantio. As maiores intensidades das doenças foram observadas em 1998, uma vez que próximo à área de plantio havia grande quantidade de plantas e restos culturais infetados, o que não ocorreu em 1997. Pelas análises de "ordinary runs", ajuste à distribuição beta-binomial e autocorrelação espacial, na maioria das situações, foi constatado um arranjo aleatório de plantas doentes nas avaliações iniciais, evoluindo posteriormente para um arranjo agregado, indicando que as doenças podem ter sido originadas de aloinfecções pela chegada de inóculo externo, seguidas de autoinfecções decorrentes da disseminação planta-a-planta.

The objective of the present work is the experimental determination of pressure drop coefficients (loss coefficients) for elliptic and circular sections in one, two and three-row arrangements of plate fin and tube heat exchangers. The experiments permitted to correlate the dimensionless loss coefficient with the flow Reynolds number in the rectangular channel formed by the plate fins. The experimental technique consisted of the measurement of the longitudinal pressure distribution along the flow channel, for several values of air mass flow rate. The total number of data runs, each one characterized by the flow Reynolds number, was 216. The present geometry is used in compact heat exchangers for air conditioning systems, heaters, radiators, and others. Also, it is verified the influence of the utilization of elliptic tubes, instead of circular ones, in the pressure drop. The measurements were performed for Reynolds numbers ranging from 200 to 1900.

We report a fast (less than 3 h) and cost-effective melting temperature assay method for the detection of single-nucleotide polymorphisms in the MBL2 gene. The protocol, which is based on the Corbett Rotor Gene real time PCR platform and SYBR Green I chemistry, yielded, in the cohorts studied, sensitive (100%) and specific (100%) PCR amplification without the use of costly fluorophore-labeled probes or post-PCR manipulation. At the end of the PCR, the dissociation protocol included a slow heating from 60º to 95ºC in 0.2ºC steps, with an 8-s interval between steps. Melting curve profiles were obtained using the dissociation software of the Rotor Gene-3000 apparatus. Samples were analyzed in duplicate and in different PCR runs to test the reproducibility of this technique. No supplementary data handling is required to determine the MBL2 genotype. MBL2 genotyping performed on a cohort of 164 HIV-1-positive Brazilian children and 150 healthy controls, matched for age and sex and ethnic origin, yielded reproducible results confirmed by direct sequencing of the amplicon performed in blind. The three MBL2 variants (Arg52Cys, Gly54Asp, Gly57Glu) were grouped together and called allele 0, while the combination of three wild-type alleles was called allele A. The frequency of the A/A homozygotes was significantly higher among healthy controls (0.68) than in HIV-infected children (0.55; P = 0.0234) and the frequency of MBL2 0/0 homozygotes was higher among HIV-1-infected children than healthy controls (P = 0.0296). The 0 allele was significantly more frequent among the 164 HIV-1-infected children (0.29) than among the 150 healthy controls (0.18; P = 0.0032). Our data confirm the association between the presence of the mutated MBL2 allele (allele 0) and HIV-1 infection in perinatally exposed children. Our results are in agreement with the literature data which indicate that the presence of the allele 0 confers a relative risk of 1.37 for HIV-1 infection through vertical transmission.
