Objective Conduct a systematic review to investigate whether healthy elderly have deficits in the decision-making process when compared to the young. Methods We performed a systematic search on SciELO, Lilacs, PsycINFO, Scopus and PubMed database with keywords decision making and aging (according to the description of Mesh terms) at least 10 years. Results We found nine studies from different countries, who investigated 441 young and 377 elderly. All studies used the IOWA Gambling Task as a way of benchmarking the process of decision making. The analysis showed that 78% of the articles did not have significant differences between groups. However, 100% of the studies that assessed learning did find relevant differences. Furthermore, studies that observed the behavior of individuals in the face of losses and gains, 60% of articles showed that the elderly has more disadvantageous choices throughout the task. Conclusion: The consulted literature showed no consensus on the existence of differences in performance of the decision-making process between old and young, but it is observed that the elderly has deficits in learning and a tendency to fewer advantageous choices.

The main objective of the present study was to evaluate the diagnostic value (clinical application) of brain measures and cognitive function. Alzheimer and multiinfarct patients (N = 30) and normal subjects over the age of 50 (N = 40) were submitted to a medical, neurological and cognitive investigation. The cognitive tests applied were Mini-Mental, word span, digit span, logical memory, spatial recognition span, Boston naming test, praxis, and calculation tests. The brain ratios calculated were the ventricle-brain, bifrontal, bicaudate, third ventricle, and suprasellar cistern measures. These data were obtained from a brain computer tomography scan, and the cutoff values from receiver operating characteristic curves. We analyzed the diagnostic parameters provided by these ratios and compared them to those obtained by cognitive evaluation. The sensitivity and specificity of cognitive tests were higher than brain measures, although dementia patients presented higher ratios, showing poorer cognitive performances than normal individuals. Normal controls over the age of 70 presented higher measures than younger groups, but similar cognitive performance. We found diffuse losses of tissue from the central nervous system related to distribution of cerebrospinal fluid in dementia patients. The likelihood of case identification by functional impairment was higher than when changes of the structure of the central nervous system were used. Cognitive evaluation still seems to be the best method to screen individuals from the community, especially for developing countries, where the cost of brain imaging precludes its use for screening and initial assessment of dementia.

It is well known that the kidney plays an important role in the development of cardiovascular diseases such as hypertension. The normal aging process leads to changes in kidney morphology, hemodynamics and function, which increase the incidence of cardiovascular events in the elderly population. These disturbances are influenced by several factors, including gender. In general, females are protected by the effects of estrogens on the cardiorenal system. Several studies have demonstrated the beneficial effects of estrogens on renal function in the elderly; however, the relationships between androgens and kidney health during one’s lifetime are not well understood. Sex steroids have many complex actions, and the decline in their levels during aging clearly influences kidney function, decreases the renal reserve and facilitates the development of cardiovascular disorders. Therefore, in this review, we discuss the cellular, biochemical, and molecular mechanisms by which sex hormones may influence renal function during the aging process.
Differential effects of aging on spatial contrast sensitivity to linear and polar sine-wave gratings

Changes in visual function beyond high-contrast acuity are known to take place during normal aging. We determined whether sensitivity to linear sine-wave gratings and to an elementary stimulus preferentially processed in extrastriate areas could be distinctively affected by aging. We measured spatial contrast sensitivity twice for concentric polar (Bessel) and vertical linear gratings of 0.6, 2.5, 5, and 20 cycles per degree (cpd) in two age groups (20-30 and 60-70 years). All participants were free of identifiable ocular disease and had normal or corrected-to-normal visual acuity. Participants were more sensitive to Cartesian than to polar gratings in all frequencies tested, and the younger adult group was more sensitive to all stimuli tested. Significant differences between sensitivities of the two groups were found for linear (only 20 cpd; P<0.01) and polar gratings (all frequencies tested; P<0.01). The young adult group was significantly more sensitive to linear than to circular gratings in the 20 cpd frequency. The older adult group was significantly more sensitive to linear than to circular gratings in all spatial frequencies, except in the 20 cpd frequency. The results suggest that sensitivity to the two kinds of stimuli is affected differently by aging. We suggest that neural changes in the aging brain are important determinants of this difference and discuss the results according to current models of human aging.

Normal aging is accompanied by renal functional and morphological deterioration and dietetic manipulation has been used to delay this age-related decline. We examined the effects of chronic administration of diets containing 5% lipid-enriched diet (LD, w/w) on renal function of rats at different ages. Three types of LD were tested: canola oil, fish oil and butter. Mean systemic tail-cuff blood pressure and glycemia remained within the normal range whatever the age and the diet of the animals. Proteinuria began to rise from the 8th month in the groups ingesting LD, while in the control group it increased significantly (above 10 mg/24 h) only after the 10th month. With age, a significant and progressive decline in glomerular filtration rate (GFR) and renal plasma flow was observed in the LD groups but after 6 months of lipid supplementation, the decline in these parameters was more marked in the butter and fish oil groups. By the 18th month, the lowest GFR level was observed in the group ingesting the butter diet (2.93 ± 0.22 vs 5.01 ± 0.21 ml min-1 kg-1 in control, P<0.05). Net acid excretion, evaluated in 9- and 18-month-old rats, was stimulated in the fish oil group when compared both to control and to the other two LD groups. These results suggest that even low levels of LD in a chronic nutritional regimen can modify the age-related changes in renal function and that the impact of different types of lipid-supplemented diets on renal function depends on the kind of lipid present in the diet.

The multidrug resistance P-glycoprotein is a transmembrane efflux pump expressed by lymphocytes and is involved in their cytolytic activity. In the present study, we investigated the age-related changes of P-glycoprotein function in normal peripheral blood lymphocytes. Blood samples from 90 normal volunteers (age range, 0 to 86 years) were analyzed. P-glycoprotein function was assessed by the flow cytometric rhodamine 123 assay. P-glycoprotein function was highest in cord blood and progressively declined with age in peripheral blood T CD4+ and CD8+ cells. In contrast, P-glycoprotein function did not vary with age in CD19+ B or CD16+CD56+ natural killer cells. These data suggest that the decline in P-glycoprotein function in T CD4+ and CD8+ lymphocytes as a function of age may contribute to the decrease in T cell cytolytic activity with aging.

Thromboelastography (TEG®) provides a functional evaluation of coagulation. It has characteristics of an ideal coagulation test for trauma, but is not frequently used, partially due to lack of both standardized techniques and normal values. We determined normal values for our population, compared them to those of the manufacturer and evaluated the effect of gender, age, blood type, and ethnicity. The technique was standardized using citrated blood, kaolin and was performed on a Haemoscope 5000 device. Volunteers were interviewed and excluded if pregnant, on anticoagulants or having a bleeding disorder. The TEG® parameters analyzed were R, K, α, MA, LY30, and coagulation index. All volunteers outside the manufacturer’s normal range underwent extensive coagulation investigations. Reference ranges for 95% for 118 healthy volunteers were R: 3.8-9.8 min, K: 0.7-3.4 min, α: 47.8-77.7 degrees, MA: 49.7-72.7 mm, LY30: -2.3-5.77%, coagulation index: -5.1-3.6. Most values were significantly different from those of the manufacturer, which would have diagnosed coagulopathy in 10 volunteers, for whom additional investigation revealed no disease (81% specificity). Healthy women were significantly more hypercoagulable than men. Aging was not associated with hypercoagulability and East Asian ethnicity was not with hypocoagulability. In our population, the manufacturer’s normal values for citrated blood-kaolin had a specificity of 81% and would incorrectly identify 8.5% of the healthy volunteers as coagulopathic. This study supports the manufacturer’s recommendation that each institution should determine its own normal values before adopting TEG®, a procedure which may be impractical. Consideration should be given to a multi-institutional study to establish wide standard values for TEG®.

The testicular feminized (Tfm) mouse carries a nonfunctional androgen receptor (AR) and reduced circulating testosterone levels. We used Tfm and castrated mice to determine whether testosterone modulates markers of aging in cardiomyocytes via its classic AR-dependent pathway or conversion to estradiol. Male littermates and Tfm mice were divided into 6 experimental groups. Castrated littermates (group 1) and sham-operated Tfm mice (group 2, N = 8 each) received testosterone. Sham-operated Tfm mice received testosterone in combination with the aromatase inhibitor anastrazole (group 3, N = 7). Castrated littermates (group 4) and sham-operated untreated Tfm mice (group 5) were used as controls (N = 8 and 7, respectively). An additional control group (group 6) consisted of age-matched non-castrated littermates (N = 8). Cardiomyocytes were isolated from the left ventricle, telomere length was measured by quantitative PCR and expression of p16INK4α, retinoblastoma (Rb) and p53 proteins was detected by Western blot 3 months after treatment. Compared with group 6, telomere length was short (P < 0.01) and expression of p16INK4α, Rb and p53 proteins was significantly (P < 0.05) up-regulated in groups 4 and 5. These changes were improved to nearly normal levels in groups 1 and 2 (telomere length = 0.78 ± 0.05 and 0.80 ± 0.08; p16INK4α = 0.13 ± 0.03 and 0.15 ± 0.04; Rb = 0.45 ± 0.05 and 0.39 ± 0.06; p53 = 0.16 ± 0.04 and 0.13 ± 0.03), but did not differ between these two groups. These improvements were partly inhibited in group 3 compared with group 2 (telomere length = 0.65 ± 0.08 vs 0.80 ± 0.08, P = 0.021; p16INK4α = 0.28 ± 0.05 vs 0.15 ± 0.04, P = 0.047; Rb = 0.60 ± 0.06 vs 0.39 ± 0.06, P < 0.01; p53 = 0.34 ± 0.06 vs 0.13 ± 0.03, P = 0.004). In conclusion, testosterone deficiency contributes to cardiomyocyte aging. Physiological testosterone can delay cardiomyocyte aging via an AR-independent pathway and in part by conversion to estradiol.

Estudos atuais apontam para o fato que o indivíduo idoso pode apresentar uma desordem do processamento auditivo mesmo sem haver uma lesão estrutural que a explique diretamente. OBJETIVO: Caracterizar o desempenho de idosos com sensibilidade auditiva normal no Teste de Padrão de Freqüência (TPF) e de Padrão de Duração (TPD). FORMA DE ESTUDO: Clínico prospectivo. MÉTODO: A amostra contou com 25 idosos com audição normal e sem história de comprometimentos centrais. Foram realizados os Testes de Padrão de Duração e Teste de Padrão de Freqüência a 50dBNS, sendo solicitada resposta através de nomeação. RESULTADOS: Não foram encontradas diferenças nos dois testes, segundo a variável lado da orelha. A porcentagem média de acertos obtida no Teste de Padrão de Duração foi de 67,5% e no Teste de Padrão de Freqüência foi de 49,2%. A correlação entre os testes TPD e TPF e a idade dos pacientes mostrou-se inversamente proporcional, ou seja, quanto maior a idade pior o desempenho nos testes, sendo que a maior correlação foi encontrada no Teste de Padrão de Duração. CONCLUSÃO: Não foram observadas diferenças segundo a variável lado da orelha e os indivíduos idosos com sensibilidade auditiva normal apresentam uma porcentagem média de acertos de 47,2% no Teste de Padrão de Freqüência de 67,5% no Teste de Padrão Duração.

As causas de tontura são de difícil diagnóstico. Atualmente dispomos de uma variedade de exames complementares, contudo, nenhum avalia bem a função vestibular. Os testes mais utilizados para este fim são as provas eletronistagmográficas e posturográficas. OBJETIVO: O objetivo do trabalho foi o de analisar os resultados da estabilometria de pacientes com queixa de tontura que apresentaram eletronistagmografia normal e compará-los aos de um grupo controle. FORMA DE ESTUDO: Prospectivo, do tipo seccional transversal. MATERIAL E MÉTODO: Realizou-se no SME do Serviço de Otorrinolaringologia do HUCFF/UFRJ. No total foram avaliados 22 pacientes (15 mulheres e sete homens) com idade média de 47,6±9 anos. O grupo controle foi de 25 sujeitos saudáveis (18 mulheres e sete homens) com idade média de 46,8±7 anos. Todos os indivíduos foram submetidos à estabilometria com os olhos abertos, em seguida por olhos fechados, durante 30 segundos cada. RESULTADOS: O grupo de pacientes apresentou resultados estatisticamente significativos em todos os parâmetros estabilimétricos avaliados quando comparados ao grupo controle. Na comparação dos resultados com os olhos fechados e abertos, apenas a velocidade média ântero-posterior no grupo controle foi significativa. CONCLUSÃO: Conclui-se que o grupo de pacientes apresentou resultados estatisticamente significativos em relação ao grupo controle em todos os parâmetros estabilométricos analisados, demonstrando, assim, que o grupo de pacientes com queixa de tontura apresentou maior instabilidade na posição ortostática do que o grupo de sujeitos saudáveis.

Pacientes com zumbido e audiometria normal constituem um grupo importante, pois seus achados não sofrem influência da perda auditiva. Apesar disso, este grupo é pouco estudado e não sabemos se suas características clínicas e repercussões são semelhantes às dos indivíduos com perda auditiva. OBJETIVOS: Comparar as características clínicas do zumbido e sua interferência nas atividades diárias em pacientes com e sem perda auditiva. FORMA DE ESTUDO: coorte histórica. MATERIAL E MÉTODO: Entre 744 pacientes atendidos no Grupo de Pesquisa em Zumbido do HCFMUSP, avaliou-se retrospectivamente com um corte transversal os 55 indivíduos com audiometria tonal normal. O grupo controle correspondeu a 198 pacientes com zumbido e perda auditiva atendidos sob o mesmo protocolo. Analisou-se os dados dos pacientes, as características clínicas do zumbido e sua repercussão na vida do paciente. RESULTADOS: A idade média no grupo de estudo (43,1 ± 13,4 anos) foi significantemente menor do que a do grupo controle (49,9 ± 14,5 anos). Em ambos os grupos houve predomínio do sexo feminino e o zumbido foi predominantemente bilateral, único e constante, porém sem diferença entre os grupos. A interferência na concentração e no equilíbrio emocional foi significantemente menor no grupo de estudo (25,5% e 36,4%) do que no controle (46% e 61,6%), porém sem diferença quanto à interferência no sono e na atividade social. CONCLUSÕES: O grupo de pacientes com zumbido e audição normal apresentou características clínicas semelhantes em relação ao grupo com perda auditiva. Entretanto, a faixa etária acometida e a interferência sobre a concentração e o equilíbrio emocional foram significantemente menores nestes.

A audiometria de alta freqüência é capaz de detectar precocemente alterações em sensibilidade advindas de processos como o envelhecimento. Seu uso é limitado, o que recomenda estudos para esclarecer seu desempenho, especialmente entre adultos de mais idade. OBJETIVO: Comparar os limiares para as freqüências de 250Hz a 16kHz, entre adultos jovens e mais velhos normoacúsicos, com e sem queixa audiológica. CASUÍSTICA E MÉTODO: A sensibilidade a tons puros de 250Hz a 16kHz foi avaliada com audiômetro AC-40, em 64 adultos, igualmente distribuídos: jovens (25 a 35 anos) e mais velhos (45 a 55 anos) de ambos os gêneros, com forma de estudo de coorte transversal. RESULTADOS: Os adultos mais velhos apresentaram limiares mais elevados em todas as freqüências, mais significativamente nas mais altas (8 a 16kHz), quando comparados com os adultos jovens. Homens apresentaram limiares mais elevados do que mulheres entre 3 e 10kHz. CONCLUSÃO: O processo de envelhecimento auditivo, envolvendo perda de sensibilidade auditiva para altas freqüências, pode ser detectado em idades anteriores às tipicamente pesquisadas, uma vez que a audiometria de alta freqüência demonstrou ser instrumento importante para distinguir a sensibilidade auditiva entre adultos jovens e mais velhos, quando audiologicamente normais.

Os potenciais de média latência são potenciais auditivos que ocorrem entre 10ms a 80ms, formados por ondas polifásicas positivas e negativas denominadas de N0, P0, Na, Pa, Nb e Pb. O logon é um estímulo acústico que permite eliciar respostas de média latência ativando regiões específicas da cóclea. Sua vantagem sobre o clique é a possibilidade avaliar as áreas de baixa freqüência (abaixo de 1kHz). OBJETIVO: O objetivo deste trabalho foi verificar a resposta eletrofisiológica das MLR estimulados por logon nas freqüências de 500, 1000 e 2000 Hz. CASUÍSTICA E MÉTODO: Forma de estudo prospectivo e descritivo de uma amostra de 14 voluntárias do sexo feminino, normais dos pontos de vista otológico e audiológico convencional. O estímulo foi monoaural e ipsilateral à derivação de captação dos potenciais (Cz/A1-2). RESULTADOS: O complexo NaPa foi o mais facilmente identificado e esteve presente em 100% dos exames realizados a 2000 Hz e em 96,4% a 500 e 1000 Hz. CONCLUSÕES: As MLR podem se eliciadas pelo logon nas freqüências de 500, 1000 e 2000 Hz; o complexo NaPa foi o evento prevalente das MLR e o estímulo logon a 2000 Hz eliciou maior número de respostas do que as outras freqüências.

A resolução temporal é essencial na percepção acústica da fala, podendo estar alterada nos distúrbios auditivos gerando prejuízos no desenvolvimento da linguagem. OBJETIVO: Comparar a resolução temporal de crianças com audição normal, perda auditiva condutiva e distúrbios do processamento auditivo. CASUÍSTICA E MÉTODO: A amostra foi de 31 crianças de 07 a 10 anos, divididas em três grupos: G1: 12 com audição normal, G2: sete com perda auditiva condutiva e G3: 12 com distúrbio do processamento auditivo. Os procedimentos de seleção foram: questionário aos responsáveis, avaliação audiológica e do processamento auditivo. O procedimento de pesquisa foi o teste de detecção de intervalos no silêncio realizado a 50 dB NS acima da média de 500, 1000 e 2000Hz na condição binaural em 500, 1000, 2000 e 4000Hz. Na análise dos dados foi utilizado o Teste de Wilcoxon, com nível de significância de 1%. RESULTADO: Observou-se que houve diferença entre os G1 e G2 e entre os G1 e G3 em todas as freqüências. Por outro lado, esta diferença não foi observada entre os G2 e G3. CONCLUSÃO A perda auditiva condutiva e o distúrbio do processamento auditivo têm influência no limiar de detecção de intervalos.

OBJECTIVE: To describe the characteristics and associated factors of the smoking habit among older adults. METHODS: A population-based study was carried out comprising 1,606 (92.2%) older adults (>60 years old) living in the Bambuí town, Southeastern Brazil in 1997. Data was obtained by means of interview and socio-demographic factors, health status, physical functioning, use of healthcare services and medication were considered. The multiple multinomial logistic regression was used to assess independent associations between smoking habits (current and former smokers) and the exploratory variables. RESULTS: The prevalence of current and past smoking was 31.4% and 40.2% among men, and 10.3% and 11.2% among women, respectively (p<0.001). Among current smokers, men consumed a larger number of cigarettes per day and started the habit earlier than women. Among men, current smoking presented independent and negative association with age (>80 years) and schooling (>8 years) and positive association with poor health perception and not being married. Among women, independent and negative associations with current smoking were observed for age (75-79 and >80 years) and schooling (4-7 and >8 years). CONCLUSIONS: Smoking was a public health concern among older adults in the studied community, particularly for men. Yet, in a low schooling population, a slightly higher level was a protective factor against smoking for both men and women. Programs for reducing smoking in the elderly population should take these findings into consideration.
