Whether the regression of gastric metaplasia in the duodenum can be achieved after eradication of Helicobacter pylori is not clear. The aim of the present study was to investigate the relationship between H. pylori infection and gastric metaplasia in patients with endoscopic diffuse nodular duodenitis. Eighty-six patients with endoscopically confirmed nodular duodenitis and 40 control patients with normal duodenal appearance were investigated. The H. pylori-positive patients with duodenitis received anti-H. pylori triple therapy (20 mg omeprazole plus 250 mg clarithromycin and 400 mg metronidazole, all twice daily) for one week. A control endoscopy was performed 6 months after H. pylori treatment. The H. pylori-negative patients with duodenitis received 20 mg omeprazole once daily for 6 months and a control endoscopy was performed 2 weeks after treatment. The prevalence of H. pylori infection was 58.1%, and the prevalence of gastric metaplasia was 57.0%. Seventy-six patients underwent endoscopy again. No influence on the endoscopic appearance of nodular duodenitis was found after eradication of H. pylori or acid suppression therapy. However, gastric metaplasia significantly decreased and complete regression was achieved in 15/28 patients (53.6%) 6 months after eradication of H. pylori, accompanied by significant improvement of other histological alterations. Only mild chronic inflammation, but not gastric metaplasia, was found in the control group, none with H. pylori infection in the duodenal bulb. Therefore, H. pylori infection is related to the extent of gastric metaplasia in the duodenum, but not to the presence of diffuse nodular duodenitis.

An investigation related to the frequency and pathology of Heterakis gallinarum and pathology of Heterakis isolonche in pheasants from Rio de Janeiro, Brazil was conducted by means of clinical examinations, necropsies, and histopathological analysis in 50 ring-necked pheasants from backyard flocks of 11 localities; also, histological sections of caeca of golden pheasants deposited in the Helminthological Collection of the Oswaldo Cruz Institute (CHIOC) have been considered in the present study. During necropsies, only specimens of H. gallinarum were recovered with a prevalence of 90%, mean intensity of 81.9 and range of infection of 1-413. Gross lesions were characterized by congestion, thickening, petechial haemorrhages of the mucosa, intussusception, and nodules in the cecal wall. Under microscopy, chronic difuse typhlitis, haemosiderosis, granulomas with necrotic center in the submucosa and leiomyomas in the submucosa, muscular and serosa associated with immature H. gallinarum worms were observed. The examination of histological sections previously deposited in the CHIOC, revealed more severe alterations associated with concomitant infections with H. gallinarum and H. isolonche in golden pheasants, and were characterized by several necrotic areas with cholesterol clefts in the submucosa, giant cell granulomas in the submucosa, and serosa centralized by necrosis and worm sections and neoplastic nodules in the muscular and submucosa.

Neste trabalho apresentamos um caso de hiperplasia nodular focal que foi diagnosticado aos seis anos de idade e que está sendo acompanhado até o momento presente. Para o diagnóstico foram imprescindíveis as técnicas de imagem, tendo importância de realce a cintilografia hepatoesplênica e a tomografia computadorizada. Apresentamos, também, revisão da literatura sobre o assunto.

A neurocisticercose é uma doença caracterizada pelo envolvimento do sistema nervoso central pelo estágio larval intermediário do parasita Taenia solium. O processo de degeneração da larva e a reação inflamatória do organismo causam os sintomas clínicos. Relatamos a reativação clínica e radiológica de uma forma nodular calcificada e assintomática há mais de 20 anos. O tratamento antiparasitário mostrou boa resposta.

OBJETIVO: O presente trabalho se propõe a analisar a precisão da punção aspirativa por agulha fina (PAAF) em pacientes avaliados no Departamento de Cirurgia do Hospital Universitário Walter Cantídio (HUWC), portadores de lesões nodulares da tireóide. MÉTODO: A avaliação contemplou 130 pacientes que apresentavam nódulos de tireóide clinicamente palpáveis, com indicação de tratamento cirúrgico, segundo critérios clínicos e citopatológicos. Utilizaram-se seringas descartáveis de 10ml, agulhas descartáveis 25x06, lâminas esmerilhadas para microscopia, frascos para lâmina e álcool a 95%. RESULTADOS: A análise citopatológica mostrou nódulos benignos - 58 (44,6%); indeterminados (lesões foliculares e suspeitos) - 38 (29,2%); malignos - 21 (16,2%) e insatisfatório - 13 (10,0%). Através do exame histopatológico, foram identificadas 45 neoplasias malignas e 85 lesões benignas. Foram observados os seguintes índices na análise da associação entre os dados obtidos com citopatologia e histopatologia: sensibilidade de 74%; especificidade de 98%; valor preditivo positivo de 95,2%; falso-positivo de 1,9%; valor preditivo negativo de 87,9%; falso-negativo de 25,9% e acurácia de 89,8%. Quando foram incluídos os resultados indeterminados (suspeito e lesão folicular) como positivo para neoplasia maligna, na mesma seqüência anterior, os resultados foram: 82,5%; 66,2%; 55,9%; 33,7%; 87,9%; 17,5% e 71,8%. A avaliação dos grupos supracitados mostrou significância estatística (p = 0,00), aplicando-se o teste exato de Fisher. CONCLUSÕES: Os resultados apresentados confirmam a precisão da PAAF na abordagem dos pacientes com lesões nodulares da tireóide, no HUWC-UFC.

Focal Nodular Hyperplasia is a benign lesion of the liver, which usually presents with one or two localizations. We report the uncommon case of a 51-year-old female who presents with right upper quadrant pain that worsened in the previous month, without association with feeding. Four hepatic lesions were evidenced at Computerized tomography, the largest being of 8 cm in diameter, of atypical behavior. She was submitted to hepatic segmentectomy of the segment III. The pathologic diagnosis returned focal nodular hyperplasia mixed hyperplastic and adenomatous sub-type. The patient had a good postoperative evolution and is in ambulatory follow-up.

A tireoidectomia minimamente invasiva vídeo-assistida (TIMIVA) sem infusão de gás é considerada segura com vantagem estética em relação ao procedimento convencional. OBJETIVO: Apresentar os achados de identificação do ramo externo do nervo laríngeo superior (RELS) durante a TIMIVA. FORMA DE ESTUDO: Estudo de coorte transversal. CASUÍSTICA E MÉTODO: Doze pacientes foram submetidos à hemitireoidectomia por doença nodular tireoidiana. Dissecamos o pedículo vascular tireoidiano superior com o auxílio de endoscópio de cinco milímetros de zero grau visando à identificação do RELS em todos os casos. RESULTADOS: Dos 12 casos, identificamos o RELS em 10 (83,3%), sendo que o trajeto era medial aos ramos da artéria tireoidiana superior em 8 (80%) e cruzava anteriormente tais ramos em 2 (20%). CONCLUSÕES: Identificamos o RELS em 83,3% dos casos, com trajeto medial aos ramos da artéria tireoidiana superior em 80% e cruzando anteriormente tais ramos em 20%. A ligadura do pedículo superior da tireóide pode ser feita com o RELS sob visão direta.

O valor da biópsia de congelação (BC) durante a tireoidectomia é controverso. OBJETIVO: Avaliar o papel da BC na conduta frente os nódulos tireoidianos. CASUÍSTICA E MÉTODO: Trabalho prospectivo de pacientes submetidos à cirurgia tireoidiana por doença nodular e com PAAF guiada por USG prévia em 2006. A BC intra-operatória foi classificada em benigna, maligna ou neoplasia folicular. PAAF, BC e exame de "parafina" foram comparados. RESULTADOS: À BC, 54% dos nódulos eram benignos, 30% neoplasia foliculares e 16% malignos. Todos os casos considerados benignos e malignos pela BC foram confirmados pelo exame de "parafina". Classificando as neoplásicas foliculares como "benignas" à BC, pois não possuem critério para indicação de tireoidectomia total, sensibilidade, especificidade, valores preditivos para os testes positivo e negativo e acurácia global foram, respectivamente, 69%, 100%, 100%, 91,5% e 77%. Casos classificados como "benignos" e "malignos" à PAAF foram confirmados pela BC e "parafina." Dentre os 42 casos de "neoplasia folicular" à PAAF, em um caso, a BC concluiu como carcinoma papilífero, em três, como benigno (confirmados pela "parafina"); e, em 38, manteve "padrão folicular", sendo 29 adenomas foliculares e nove carcinomas à "parafina". CONCLUSÕES: A BC somente está indicada nos casos cuja PAAF seja "neoplasia folicular."

In the present study, clinic and serologic aspects of 47 paracoccidioidomycosis patients were reviewed. The clinic-radiologie-laboratorial data of each patient were obtained from the medical chart before, during and after therapy with Amphotericin B. The age of the patients ranged between 13 to 50 years; the ratio male: female was 10:1. The most frequently associated diseases were alcoholism and tabagism; tuberculosis was present in 14.2% of the cases. Most patients came from Botucatu of adjacent towns (central area of the State of São Paulo). Cough with sputum production, dyspnea and anorexia were the most frequent clinic manifestations. All 47 patients, but 5 presented pulmonary involvement which showed the radiologic pattern of interstitial exudate nodular type and fibrocicatritial lesions in 85.7%. There was associated emphysema in 37.7% of the patients. Blood urea, creatinin and kaliemia increased during therapy with Amphotericin B. Clinic-laboratorial follow-up showed electrocardiographic changes in 13, hypertension in 9 and hyperreactivity in Thorn test in 1 patient. Complement fixation was the best serologic test for monitoring patients with paracoccidioidomycosis. Incomplete dosage of Amphotericin B led to therapeutic ineffectivenes.

The hepatic changes observed in liver specimen from either biopsy or necropsy of 47 patients with visceral leishmaniasis permited us to define three different histopathological patterns of involvement: typical, nodular, and fibrogenic. These patterns seem to be representative of different evolutive stages of the hepatic involvement in the disease either towards a more benign evolution or to more chronic stage with fibrosis and "cirrhosis". These histopathological evolutive stages are related to the prognosis of the disease.

São analisadas as telerradiografias de tórax de 159 pacientes portadores de paracoccidioidomicose atendidos no Hospital Evandro Chagas - FIOCRUZ, no período de janeiro de 1960 a dezembro de 1988. Em 24 pacientes (15,09%) houve associação com tuberculose; 1 com pneumoconiose; 1 com aspergilose e 2 com carcinoma. Foram excluídos da análise do padrão radiológico 20 casos: em 8 o diagnóstico de tuberculose foi concomitante e em 12 por apresentarem fibrose pulmonar por tratamento prévio para tuberculose e/ou paracoccidioidomicose. Nos 139 casos restantes, as alterações radiológicas encontradas foram agrupadas observando-se a predominância de lesões nos diferentes sítios pulmonares, alveolares ou intersticiais, de acordo com a classificação de MAGALHÃES (1982)19 modificada: infiltrativa 55 casos (39,6%); misto 28 (20,1%); pneumônico 23 (16,6%); nodular 16 (11,5%); micronodular 10 (7,2%) e fibrótico 7 (5,0%). Em 113 pacientes acompanhados radiologicamente, a regressão do processo pulmonar ocorreu 85 casos (75,2%) em até 6 meses; em 17 (15,0%), entre 7 e 12 meses; em 4 (3,5%), entre 13 e 24 meses e em 7 (6,1%) não houve modificação do padrão radiológico

We report a case of subcutaneous pheohyphomycosis observed in a male patient presenting pulmonary sarcoidosis and submitted to corticosteroid treatment. He presented nodular erythematous-violaceous skin lesions in the dorsum of the right hand. Histopathological examination of the biopsied lesion revealed dematiaceous hyphae and yeast-like cells, with a granulomatous tissual reaction. The isolated fungus was identified as Phoma cava. A review of the literature on fungal infection caused by different Phoma species, is presented. The patient healed after therapy with amphotericin B, followed by itraconazole

Given that chagasic patients in the indeterminate form of this disease, can have abnormal motility of the digestive tract and immunologic abnormalities, we decided to assess the frequency of peptic disease and Helicobacter pylori (Hp) infection in these individuals. Twenty-one individuals, 13 males and 8 females, mean age 37.6 ± 11.1 years, were examined. Biopsies of the duodenum, antrum, lesser and greater gastric curvature and esophagus were performed. The endoscopic findings were of chronic gastritis in 20 (95.2%) patients, duodenal ulcer in 3 (14.3%), gastric and duodenal ulcer in 3 (14.3%), gastric ulcer alone in 1 (4.8%), esophagitis in 5 (23.8%), and duodenitis in 5 (23.8%). The diagnosis of infection by the Hp was done by the urease test and histologic examination. Hp infection was found in 20 (95.2%) individuals: in 20 out of them in the antrum, in 17 in the lesser curvature, and in 17 in the greater curvature. Hp was not found in the esophagus and duodenum. The only individual with no evidence of infection by Hp was also the only one with normal endoscopic and histologic examinations. The histologic examinations confirmed the diagnoses of gastric ulcer as peptic, chronic gastritis in 20 patients, duodenitis in 14, and esophagitis in 9. In this series the patients had a high frequency of peptic disease, which was closely associated with Hp infection
