M. hominis and U. urealyticum are the better-known mycoplasma species pathogenic to the human genitourinary tract, causing mainly urethritis, bacterial vaginosis and pregnancy complications. In HIV-infected patients, the prevalence and role of these species is still not well known. The aim of this work was to determinate the prevalence of these species in this group of male patients (HIV group), in comparison to a group of men with clinical symptoms of urethritis (STD group). M. hominis was isolated from 7.5% patients (8/106) and U. urealyticum from 18.9% patients (20/106) from the HIV group, being among these 62.5% and 85% in significant concentrations, respectively. In the STD group these rates were 0.9% (1/110) for M. hominis and 13.6% (15/110) for U. urealyticum, being 100% and 93.3% in significant concentrations, respectively. We could demonstrate infection rates by these mycoplasma species in the HIV group as high as the one found in the STD one, what may indicate the occurrence of opportunistic infections in our population. This fact is discussed here because in immunosuppressed patients, specially M. hominis has been reported causing severe infections, even systemically.

Ureaplasma urealyticum (UU) and Mycoplasma hominis (MH) have been detected in the urine of women with systemic lupus erythematosus (SLE). We evaluated the presence of these mycoplasma in the endocervix of women presenting SLE. A total of 40 SLE patients (mean age 40.2 years), and 51 healthy women (mean age 30.9 years), were studied. Endocervical swabs were cultured in specific liquid media for MH or UU, detected by a quantitative color assay, and considered positive at >10³ dilutions. Statistical analysis was performed using the two-tailed Fisher test. UU was detected in 52.5 % of patients and in 11.8% of controls (p= 0.000059). MH was detected in 20% of patients and 2% controls (p=0.003905). Both mycoplasmas were detected in 7.3% patients and 0% controls (p<0.000001). The results reported here corroborate the association of the mycoplasma infection and SLE. Thus, these agents may stimulate the production of autoreactive clones.

OBJETIVOS: determinar a freqüência de infecção pelo Mycoplasma hominis e Ureaplasma urealyticum e relacioná-la a variáveis clínicas de mulheres inférteis. MÉTODOS: estudo transversal com 322 pacientes inférteis submetidas à coleta de swab endocervical para pesquisa de Mycoplasma hominis e Ureaplasma urealyticum, de outubro de 2002 a maio de 2004. Todas as pacientes foram submetidas a protocolo básico de investigação clínica e laboratorial da infertilidade. Como controle, utilizou-se série histórica de 51 mulheres não gestantes, previamente pesquisadas quanto aos agentes infecciosos estudados. RESULTADOS: a freqüência de infecção pelo Mycoplasma hominis e Ureaplasma urealyticum foi de 4,9% nas pacientes inférteis e 13,8% no grupo controle. Entre as pacientes inférteis observou-se relação entre a presença dos dois patógenos e alterações no resultado da histerossalpingografia (OR: 3,20; IC 95%: 1,05-9,73), presença de dispareunia (OR: 10,72; IC 95%: 3,21-35,77) e corrimento vaginal (OR: 8,5; IC 95%: 2,83-26,02), além de cultura endocervical positiva para Escherichia coli (OR: 16,09; IC 95%: 4,95-52,25). CONCLUSÃO: a taxa de infecção pelo Mycoplasma hominis e Ureaplasma urealyticum é baixa em pacientes inférteis e está associada a seqüelas reprodutivas tardias.

OBJETIVO: Hay un aumento significativo de pacientes con Secreción Genital Femenina, en el Sector Público del gran Buenos Aires. Fue necesario actualizar la prevalencia de los microorganismos asociados a los efectos de revisar el apoyo necesario de laboratorio y ajustar las medidas de prevención y control. MÉTODOS: Se incorporan a este estudio, la totalidad de los casos atendidos (1997-1998): 84 adolescentes (15 a 19 años) y 784 adultas (20 a 60 años) sintomáticas. El protocolo incluye (secreción vaginal y endocervical) detección de Neisseria gonorrhoeae, Streptococcus agalactiae, Trichomonas vaginalis, Candida spp y vaginosis bacteriana. Aplicando métodos específicos directos y cultivo, Chlamydia trachomatis (detección de antígeno), Ureaplasma urealitycum y Mycoplasma hominis (cultivos) fueron estudiados en parte de la población total. RESULTADOS: El aumento de la demanda de consulta fue continuo desde 1997 y aumentó 2.10 veces del primero al último semestre de 1998. En las mujeres adultas se encontró: vaginosis bacteriana, 23,8%; Candida spp 17,8%; S. agalactiae 5,6%; T. vaginalis 2,4%. En 50,3% no se detecto ninguno. En adolescentes se detectó: vaginosis bacteriana, 17,8%; Candida spp 29,7%; S. agalactiae 3,6%, T. vaginalis 2,4%: En 46,4% de los casos el resultado fue negativo. En el grupo de mujeres adultas sintomáticas, no en la totalidad, se detectó: C. trachomatis (7/400) 1,76%, U. urealyticum (209/340) 61,4% y M. hominis (45/272) 16,5%. CONCLUSIONES: El aumento significativo de consultas se debe a problemas sociales en la población, no al aumento de ninguna patología en especial. Impacta como problema clínico concreto la prevalencia de vaginosis bacteriana y Candida spp. Llama la atención, la nula incidencia de N. gonorrhoeae y la baja circulación de T. Vaginalis y C. trachomatis, en este tipo de población. La prevalencia de U. urealyticum y M. hominis es alta, pero su real participación en la patología genital de adultas debe ser reconsiderada. Las prevalencias establecidas motivan la necesidad de adecuar los manuales de procedimientos apuntando a la aplicación de metodologías simples de alto valor predictivo.

The objective of the present study was to determine the prevalence of certain mycoplasma species, i.e., Mycoplasma hominis, Ureaplasma urealyticum and Mycoplasma penetrans, in urethral swabs from HIV-1 infected patients compared to swabs from a control group. Mycoplasmas were detected by routine culture techniques and by the Polymerase Chain Reaction (PCR) technique, using 16SrRNA generic primers of conserved region and Mycoplasma penetrans specific primers. The positivity rates obtained with the two methods were comparable. Nevertheless, PCR was more sensitive, while the culture techniques allowed the quantification of the isolates. The results showed no significant difference (p < 0.05) in positivity rates between the methods used for mycoplasma detection.

Objetivo: identificar fatores mais freqüentemente associados ao aborto espontâneo recorrente. Casuística: no período de março de 1993 a março de 1997, 175 pacientes foram avaliadas no Ambulatório de Aborto Recorrente do CAISM/UNICAMP. Os critérios de seleção foram: história de três ou mais abortos espontâneos consecutivos em pacientes com idade inferior a 35 anos e/ou dois abortos e idade superior a 35 anos. Métodos: o protocolo de investigação incluiu: cariótipo; histerossalpingografia; dosagem seriada de progesterona e/ou biópsia de endométrio; pesquisa sorológica de infecções: toxoplasmose, listeriose, brucelose, sífilis e citomegalovírus e pesquisa para Mycoplasma hominis e Chlamydia trachomatis na secreção cérvico-uterina. A investigação também incluiu dosagem de hormônios tiroideanos e da glicemia de jejum; pesquisa de fator auto-imune por meio de painel de auto-anticorpos; pesquisa de fator aloimune mediante anticorpos contra antígenos leucocitários humanos (anti-HLA), prova cruzada por microlinfocitotoxicidade e cultura mista unidirecional de linfócitos, em que se comparam as respostas maternas diante das células paternas e de um doador não-relacionado para pesquisa de fator inibidor destas respostas no soro materno. O exame dos parceiros incluiu: exame físico geral e especial, sorologias para sífilis, doença de Chagas, hepatite B e C e síndrome da imunodeficiência adquirida (AIDS), além da prova cruzada por microlinfocitotoxicidade e da cultura mista de linfócitos. Resultados: o fator mais freqüentemente encontrado foi o aloimune (86,3% dos casos), representado por prova cruzada negativa e inibição na cultura mista de linfócitos inferior a 50%. O segundo fator mais freqüentemente encontrado foi a incompetência istmo-cervical (22,8%), seguido pelo fator hormonal (21,2%), representado principalmente pela insuficiência lútea. Algumas pacientes apresentavam mais de um fator concomitantemente. Conclusão: para uma investigação completa dos fatores associados ao aborto espontâneo recorrente faz-se necessária a inclusão do fator aloimune, sem a qual a maior parte dos casos não terá etiologia esclarecida.

Objetivo: determinar a presença de infecção assintomática do líquido amniótico em gestantes, identificar os agentes bacterianos envolvidos na infecção e determinar o perfil de suscetibilidade a antimicrobianos in vitro. Métodos: foram obtidas 81 amostras de líquido amniótico, colhidas por amniocentese, em gestantes sem sinais de trabalho de parto e sem suspeita clínica de infecção, atendidas na Maternidade Escola Assis Chateaubriand, entre agosto/97 e janeiro/99. Pesquisou-se a presença de bactérias aeróbias, anaeróbias estritas/facultativas e micoplasmas genitais. As bactérias anaeróbias foram identificadas pelo sistema ATBÒ (Bio-Mérieux) e os micoplasmas pelo kit Micoplasmas ISTÒ (Bio-Mérieux). Resultados: entre as amostras obtidas, oito (9,8%) apresentaram culturas positivas, sendo que em duas foram identificadas duas espécies bacterianas. Os patógenos isolados foram: Ureaplasma urealyticum (7 casos, 8,6%), Mycoplasma hominis (1 caso, 1,2%) e Peptostreptococcus sp (2 casos 2,4%). O padrão de resistência aos antimicrobianos caracterizou-se pela maior resistência dos micoplasmas à eritromicina (37,5%) e nenhuma resistência às ciclinas. Conclusões: o percentual de infecções assintomáticas foi muito elevado, havendo necessidade de serem realizadas novas pesquisas para avaliar as conseqüências da infecção subclínica nas grávidas e em seus conceptos, que envolvam métodos que identifiquem micoplasmas genitais, já que foram as bactérias mais freqüentemente isoladas.

A review of the tissular parasitism of Trypanosoma cruzi Y strain in Swiss mice was carried out. This strain parasitized preferentially smooth, skeletal and cardiac muscle fibers, with low transitory spleen and liver parasitism, as previously found by some Authors, although differing from other reports. These results can be related to the host genetical constitution and/or the degree of the strain virulence at the time of this study. Furthermore, we discuss that the high macrophagotropism reported for this strain in some instances could be an artificially induced condition resulting from its serial maintenance in mice, either for a longer time and/or by using young animals. The heavy parasitism and inflammation observed in the bladder, pancreas and spermatic duct of some inoculated mice, as well as the testis parasitization, were also noteworthy findings.

We report one case of parasitism by Phagicola sp. (Trematoda, Heterophyidae) in a 31 years-old woman who, in 1987, travelled and stayed several months in the municipality of Cananéia (SP), where she ingested, in various occasions, raw mullet (Mugil sp.). The patient refered mild intestinal pain and laboratory examinations showed eggs of Phagicola sp. in the stools and a slight increase in eosinophil blood levels (8%). After treatment with praziquantel (75 mg/kg per day for three days) all the symptoms and signs disappeared. This is, certainly, the first record of human infection by Phagicola sp. in Brazil and, perhaps, in countries other than the U.S.A. where unclear references to a few human cases were reported in the South-eastern region.

This paper reports the findings of a survey for intestinal parasites among the Xavánte Indians from Central Brazil. A. lumbricoides (25.0%) and hookworms (33.6%) were the two most common helminths; E. histolytica complex (7.8%) and G. lamblia (8.6%) the most common protozoans. The majority (58.5%) of positive individuals hosted only one species of helminth. Egg counts for helminths, and for A. lumbricoides in particular, were found to be not dispersed at random, with a few individuals, all of whom young children, showing very high counts. The prevalence rates of intestinal parasites for the Xavánte are below those reported for other Amerindian populations from Brazil.

Aortic Valve Stenosis (AVS) has been explained as an atherosclerotic process of the valve as they often exhibit inflammatory changes with infiltration of macrophages, T lymphocytes and lipid infiltration. The present study investigated whether the bacteria Chlamydia pneumoniae (CP) and Mycoplasma pneumoniae (MP), detected previously in atherosclerotic plaques, are also present in AVS. Ten valves surgically removed from patients with AVS were analyzed by immunohistochemistry, in situ hybridization, and electron microscopy. The mean and standard deviation of the percentage areas occupied by CP antigens and MP - DNA were respectively 6.21 +/- 5.41 and 2.27 +/- 2.06 in calcified foci; 2.8 +/- 3.33 and 1.78+/- 3.63 in surrounding fibrotic areas, and 0.21 +/- 0.17 and 0.12 +/- 0.13 in less injured parts of the valve. There was higher amount of CP and MP in the calcified foci and in the surrounded fibrosis than in more preserved valvular regions. In conclusion, the fact that there were greater amounts of CP and MP in calcification foci of AVS favors the hypothesis that AS is not an inevitable degenerative process due to aging, but rather that it may be a response to the presence of these bacteria, similarly to the morphology detected in atherosclerosis damage.

The objective was to estimate the prevalence of Blastocystis hominis, to evaluate the effectiveness of different techniques for its diagnosis as well as to estimate the prevalence of other intestinal parasites in the community of Campo Verde, a district of Pitanga. The work was carried out from August to October 2004. Samples of feces from children and adults were collected and submitted to the techniques of direct wet mount, flotation in zinc sulphate solution, tube sedimentation, sedimentation in formalin-ether and staining by Kinyoun and iron hematoxylin methods. From 181 studied individuals, 128 (70.7%) showed protozoa and/or helminths in stool samples. The most prevalent species were Endolimax nana (33.7%); B. hominis (26.5%); Giardia lamblia (18.2%); Entamoeba coli (17.1%); Ascaris lumbricoides (16.6%); Iodamoeba bütschlii (9.4%); and ancylostomatidae (7.7%). B. hominis was only identified by the techniques of direct wet mount, sedimentation in formalin-ether and staining by iron hematoxylin, though the latter was less sensitive than the other methods. The high frequency of B. hominis demonstrated by this study indicates the need to include laboratory techniques that enable identification of the parasite on a routine basis.

Although Mycoplasma haemofelis and "Candidatus Mycoplasma haemominutum" infections have been reported in wild cats from United States, their presence among native and captive wild cats in Brazil is still unknown. A 12 year old healthy male lion (Panthera leo) from the Zoological Garden of Curitiba, Brazil was anesthetized for transportation and dental evaluation. A blood sample was obtained for a complete blood cell count (CBC) and PCR analysis. DNA was extracted and fragments of Mycoplasma haemofelis and "Candidatus Mycoplasma haemominutum" 16S ribosomal RNA gene were amplified in PCR assays. CBC results were within reference intervals. A weak band of 192 pb for "Candidatus Mycoplasma haemominutum" was observed, and no band was amplified from Mycoplasma haemofelis reaction. A weak PCR band associated with normal CBC results and without visible parasitemia or clinical signs may suggest a chronic subclinical infection with "Candidatus Mycoplasma haemominutum". The lack of clinical signs may also represent the low pathogenicity of this organism; however, it is noteworthy that immune suppression caused by management and/or corticoids treatment may induce parasitemia and anemia in this animal. This detection suggests further studies in captive wild cats in Brazilian Zoological Gardens.
