OBJETIVO: Caracterizar las condiciones de trabajo, salud y riesgos ocupacionales de recuperadores ambientales del área rural de Medellín. MÉTODOS: Estudio transversal con cien recuperadores ambientales informales en cinco corregimientos de Medellín en 2008. La fuente de información fue primaria: una encuesta estructurada sobre condiciones de trabajo, salud, riesgos ocupacionales y morbilidad sentida, que permitió caracterizar los riesgos de la población mediante estadística descriptiva y un acercamiento a las relaciones y asociaciones de los riesgos con la morbilidad en la población. RESULTADOS: Ciento cuatro personas ejercían el reciclaje, y 100 de estos se incluyeron en el estudio: 34 personas agremiadas en organizaciones de base comunitaria y 66 personas que ejercían la labor independientemente. La afiliación a seguridad social, suplencia de necesidades básicas, satisfacción en la labor, recibir capacitaciones y suministro periódico de elementos para su trabajo presentaron diferencias según grupo (p < 0,05). Los riesgos biológicos, relacionados con postura corporal inadecuada, riesgos físicos y químicos, tuvieran frecuencia particular de exposición según grupo. El conocimiento y uso de las medidas de protección para los riesgos de la labor difirieron estadísticamente entre los grupos (p < 0,05). CONCLUSÕES: Los recuperadores son potencial apoyo para la gestión integral de residuos en áreas rurales de Medellín, aunque su carácter informal los expone a condiciones de trabajo que repercuten directamente en su salud y la de sus familias. El reconocimiento social del reciclaje va de la mano con la mejora de las condiciones laborales de las personas dedicadas a esta actividad. Los recuperadores ambientales, aun agremiados, tienen condiciones laborales y de salud precarias. La trasformación de estas condiciones debería ser una de las prioridades del Estado.

Background:Cardiovascular research publications seem to be increasing in Latin America overall.Objective:To analyze trends in cardiovascular publications and their citations from countries in Latin America between 1999 and 2008, and to compare them with those from the rest of the countries.Methods:We retrieved references of cardiovascular publications between 1999 and 2008 and their five-year post-publication citations from the Web of Knowledge database. For countries in Latin America, we calculated the total number of publications and their citation indices (total citations divided by number of publications) by year. We analyzed trends on publications and citation indices over time using Poisson regression models. The analysis was repeated for Latin America as a region, and compared with that for the rest of the countries grouped according to economic development.Results:Brazil (n = 6,132) had the highest number of publications in1999-2008, followed by Argentina (n = 1,686), Mexico (n = 1,368) and Chile (n = 874). Most countries showed an increase in publications over time, leaded by Guatemala (36.5% annually [95%CI: 16.7%-59.7%]), Colombia (22.1% [16.3%-28.2%]), Costa Rica (18.1% [8.1%-28.9%]) and Brazil (17.9% [16.9%-19.1%]). However, trends on citation indices varied widely (from -33.8% to 28.4%). From 1999 to 2008, cardiovascular publications of Latin America increased by 12.9% (12.1%-13.5%) annually. However, the citation indices of Latin America increased 1.5% (1.3%-1.7%) annually, a lower increase than those of all other country groups analyzed.Conclusions:Although the number of cardiovascular publications of Latin America increased from 1999 to 2008, trends on citation indices suggest they may have had a relatively low impact on the research field, stressing the importance of considering quality and dissemination on local research policies.

Based on the results of comparative analyses of 1,039 specimens of several progenies of Anopheles nuneztovarifrom three localities in Colombia, eight costal wing spot patterns were observed. Patterns I and III were the most frequent: 77.96% and 11.36%, respectively. Using the diagnostic characters ratio of the length of the basal dark area of hind tarsomere II/length of hind tarsomere II, ratio of the length of the humeral pale spot/length of the pre-humeral dark spot, and the ratio of the length of the subcostal pale spot/length of the distal sector dark spot (DS-III2/Ta-III2, HP/PHD, SCP/DSD) approximately 5% of the adult females were misidentified as a species of Nyssorhynchus, different from An. nuneztovari. Approximately 5% of the specimens showed DS-III2/Ta-III2 ratio less than 0.25 (range 0.21 - 0.24), and among them 3.34% shared a HP/PHD ratio less than 1.50. Consequently, 1.52% of An. nuneztovari individuals can be misidentified as Anopheles oswaldoi. In those specimens with the DS-III2/Ta-III2 ratios higher than 0.25, 34.45% displayed SCP/DSD values greater than 0.50 and of these, 3.65% displayed HP/PHD values greater than 1.8. This combination of characters could lead one to misidentify samples of An. nuneztovari as Anopheles rangeli. Similarly, 2.43% of the females could be identified erroneously as either Anopheles aquasalis or Anopheles benarrochi. Individuals with a HP/PHD ratio greater than 2.0, could be misidentified as Anopheles trinkae, Anopheles strodei or Anopheles evansae. A distinct combination of diagnostic characters for An. nuneztovari from Colombia is proposed.

Leprosy in Colombia is in the post-elimination phase; nevertheless, there are regions of this country where the incidence is still around 3-4/100,000. Early detection of leprosy patients is a priority for achieving control and elimination of leprosy; however, the clinical exam is not very sensitive and thus, the majority of patients are diagnosed only when they demonstrate lesions, and damage to the nerves and skin has already occurred. The goal of the present study was to identify Mycobacterium leprae infection and immune responses in household contacts (HHC) of leprosy patients from three prevalent regions of Colombia. Clinical examination, the Mitsuda test, evaluation of IgM anti-PGL-I in the serum, the bacillar index (BI), and polymerase chain reaction (PCR) from nasal swabs (NS) were performed for 402 HHC of 104 leprosy patients during a cross-sectional survey. Positive titers for IgM anti-PGL1 were found for 54 HHC, and PCR-positive NS for 22. The Mitsuda reaction was negative for 38 HHC, although three were positive for IgM anti-PGL-1 titers. The data document that leprosy transmission among HHC is still occurring in a non-endemic country.

The bionomics of sand flies (Diptera: Phlebotominae) was studied monthly for two consecutive years in alluvial gallery forests in the department of Casanare, Northeastern Colombia. A total of 2,365 specimens and 10 species were captured using CDC light traps and Shannon traps, and from diurnal resting places, and human landing collections. Lutzomyia fairtigi Martins (55%), Lutzomyia micropyga (Mangabeira) (20.9%), and Lutzomyia antunesi (Coutinho) (13.5%) were the predominant species in the region. Lutzomyia flaviscutellata and Lutzomyia panamensis, potential vectors of Leishmania in Colombia and neighboring countries, were also collected, but in low numbers. Lu. fairtigi is an endemic species to Colombia, and minimal data are available on its biology and distribution. The present study provides additional information about Lu. fairtigi, such as the diurnal activity displayed by females on cloudy days, the greater density during the rainy season (April to October), marked anthropophilia, and the presence of flagellates in the midgut of one female.

We evaluated the Plasmodium vivax polymorphism by studying the Pvmsp-3 gene's polymorphic region by PCR-RFLP in 55 samples from patients living in Tierralta, Colombia. Three different sizes of the Pvmsp-3 gene were found, type A (1,900 bp), type B (1,500 bp) and type C (1,100 bp); most of the samples were type A (96.4 %). The Pvmsp-3 gene exhibited high polymorphism. Seven restriction patterns were found when using Alu I, and nine were found with Hha I; 12 different alleles were obtained when these patterns were combined. The findings suggest that this gene could be used in Colombia as a molecular epidemiologic marker for genotyping P. vivax.

With the aim of determining the prevalence of asymptomatic Plasmodium spp. infection by thick smear and PCR and its association with demographic and epidemiological characteristics in the village of Nuevo Tay, Tierralta, Córdoba, Colombia, a cross-sectional population study was carried out, using random probabilistic sampling. Venous blood samples were taken from 212 people on day 0 for thick smear and PCR. Clinical follow-up and thick smears were carried out on days 14 and 28. The prevalence of Plasmodium spp. infection was 17.9% (38/212; 95% CI: 12.5-23.3%) and the prevalence of asymptomatic Plasmodiumspp. infection was 14.6% (31/212; 95% CI: 9.6-19.6%). Plasmodium vivax was found more frequently (20/31; 64.5%) than Plasmodium falciparum (9/31; 29%) and mixed infections (2/31; 6.5%). A significantly higher prevalence of asymptomatic infection was found in men (19.30%) than in women (9.18%) (prevalence ratio: 2.10; 95% CI: 1.01-4.34%; p = 0.02). People who developed symptoms had a significantly higher parasitemia on day 0 than those who remained asymptomatic, of 1,881.5 ± 3,759 versus 79 ± 106.9 (p = 0.008). PCR detected 50% more infections than the thick smears. The presence of asymptomatic Plasmodium spp. infection highlights the importance of carrying out active searches amongst asymptomatic populations residing in endemic areas.

Malaria is a serious health problem in the states of Córdoba and Antioquia, Northwestern Colombia, where 64.4% of total Colombian cases were reported in 2007. Because little entomological information is available in this region, the aim of this work was to identify the Anopheles species composition and natural infectivity of mosquitoes distributed in seven localities with highest malaria transmission. A total of 1,768 Anopheles mosquitoes were collected using human landing catches from March 2007-July 2008. Ten species were identified; overall, Anopheles nuneztovari s.l. was the most widespread (62%) and showed the highest average human biting rates. There were six other species of the Nyssorhynchus subgenus: Anopheles albimanus (11.6%), Anopheles darlingi (9.8%), Anopheles braziliensis (6.6%), Anopheles triannulatus s.l. (3.5%), Anopheles albitarsis s.l. and Anopheles oswaldoi s.l. at < 1%; and three of the Anopheles subgenus: Anopheles punctimacula, Anopheles pseudopunctipennis s.l. and Anopheles neomaculipalpusat < 1% each. Two species from Córdoba, An. nuneztovari and An. darlingi, were found to be naturally infected by Plasmodium vivax VK247, as determined by ELISA and confirmed by nested PCR. All species were active indoors and outdoors. These results provide basic information for targeted vector control strategies in these localities.

Sycorax utriensis n. sp. is described and illustrated from a male specimen collected with a CDC light trap in the Parque Nacional Natural Ensenada de Utría, on the Pacific Coast of Colombia. Five species of Sycorax are now known from Colombia, S. andicola Young, 1979, S. colombiensis Young, 1979, S. fairchildi Young, 1979, S. trispinosa Young, 1979 and S. utriensis n. sp.

En este trabajo se presenta información sobre los escarabajos coprófagos del Área Natural Única Los Estoraques (ANUE), Departamento de Norte de Santander, Colombia. Los muestreos fueron realizados durante los meses de febrero a diciembre del año 2002 en cuatro sitios con diferente grado de intervención, suministrando la captura de 10,538 individuos. Los sitios muestreados correspondieron a bosque seco premontano y bosque montañoso húmedo con diferentes grados de intervención. 16 especies de Scarabaeinae conformaron el 89% de las ejemplares colectados, el 11% restante (3 especies) fue conformado por Hibosóridos. La totalidad de las especies se completo en el quinto muestreo. Se registró diferencia significativa en la riqueza y abundancia entre los sitios (P <0,05), funcionando las zonas de bosque montañoso humedo como un solo ensamblaje. Las abundancias de las especies no muestran correlación con la pluviosidad. Se reportan por primera vez siete especies para el departamento y se amplia el rango altitudinal conocido en Colombia para cuatro.

La uchuva, Physalis peruviana L., crece como planta silvestre en las zonas tropicales altas de América, estando el centro de origen y diversificación en los Andes Suramericanos, principalmente de Colombia, Perú y Ecuador. Se realizó la caracterización morfológica de 46 accesiones de uchuva provenientes del Banco de Germoplasma de la nación Colombiana, a cargo de La Corporación Colombiana de Investigación Agropecuarias, CORPOICA, en el Centro de Investigación La Selva, ubicado en la vereda Llanogrande del municipio de Rionegro (Antioquia, Colombia). Los genotipos de uchuva se sembraron utilizando un diseño látice siete por siete simple desbalanceado duplicado. Las accesiones se ubicaron en parcelas constituidas por cinco plantas, de las cuales se evaluaron las tres plantas centrales de las dos replicaciones y cinco estructuras por planta. Se empleó un listado de 69 descriptores, 40 de ellos cualitativos y 29 cuantitativos, 56 de los cuales (81,16%) fueron útiles en la diferenciación de las accesiones. Para las variables cualitativas se estimaron los coeficientes de disimilaridad de Gower, que fluctuaron desde 0 a 0,20; y para las variables cuantitativas se estimaron los valores de distancia Euclediana, que fluctuaron entre 0,25 y 1,22.

OBJETIVO: Describir la experiencia en la implementación de un Sistema de Registro de Trauma (SRT) en dos hospitales en Cali, Colombia. MÉTODOS: El SRT incluye información prehospitalaria, hospitalaria y estatus de egreso del paciente. Cada hospital tiene una estrategia para la captura electrónica de datos. Se presenta un análisis descriptivo exploratorio durante un piloto de tres meses. RESULTADOS: Se han registrado 3293 pacientes, 1626(49.4%) del Hospital Público y 1613(50.6%) en el Privado. 67.2% fueron hombres; edad promedio 30,5±20 años, 30,5% menores de 18 años. Mortalidad global 3,52 %. Causa más frecuente de consulta fueron las caídas (33,7%); 11.6% fueron heridas por arma de fuego, la mortalidad en este grupo fue del 44.7%. CONCLUSIÓN: Se determinaron las necesidades para la implementación del SRT y los mecanismos para darle continuidad. El registro se convierte en una fuente de información para el desarrollo de la investigación. Se identificaron las causas de consulta, morbilidad y muerte por trauma que permitirá una mejor planeación de los servicios de urgencias y del sistema regional de trauma con el fin de optimizar y de reducir los costos de atención. A partir de este sistema de información de trauma se podrán plantear los ajustes indispensables para rediseñar el sistema de trauma y emergencias del suroccidente colombiano.
