The objective of this multicenter prospective study was to determine the clinical efficacy and toxicity of a polychemotherapeutic third generation regimen, VACOP-B, with or without radiotherapy as front-line therapy in aggressive localized non-Hodgkin's lymphoma. Ninety-three adult patients (47 males and 46 females, median age 45 years) with aggressive localized non-Hodgkin's lymphoma, 43 in stage I and 50 in stage II (non-bulky), were included in the study. Stage I patients received VACOP-B for 6 weeks plus involved field radiotherapy and stage II patients received 12 weeks VACOP-B plus involved field radiotherapy on residual masses. Eighty-six (92.5%) achieved complete remission and 4 (4.3%) partial remission. Three patients (3.2%) were primarily resistant. Ten-year probability of survival, progression-free survival and disease-free survival were 87.3, 79.9 and 83.9%, respectively. Eighty-four patients are surviving at a median observation time of 57 months (range: 6-126). Statistical analysis showed no difference between stages I and II in terms of response, ten-year probability of survival, progression-free survival or disease-free survival. Side effects and toxicity were negligible and were similar in the two patient groups. The results of this prospective study suggest that 6 weeks of VACOP-B treatment plus radiotherapy may be the therapy of choice in stage I aggressive non-Hodgkin's lymphoma. Twelve weeks of VACOP-B treatment with or without radiotherapy was shown to be effective and feasible for stage II. These observations need to be confirmed by a phase III study comparing first and third generation protocols in stage I-II aggressive non-Hodgkin's lymphoma.

A randomized trial of amphotericin B (AB) alone and in combination with oral itraconazole (IZ) is carried out in two groups of 10 mucocutaneous leishmaniasis patients from Bolivia and Peru. AB+IZ combination is no better than AB monotherapy, as far as efficacy and tolerability are concerned. No antagonism was detected.

OBJECTIVE: The influence of age and the presence of secondary sporocysts in the miraxonal attraction exercised by Biomphalaria glabrata on miracidia of Schistosoma mansoni of the BH strain were studied. MATERIAL AND METHOD: A glass apparatus containing two compartments joined by a tube and previously tested in other experiments, was used. Specimens of B. glabrata or its snail conditioned water (SCW) selected before the first oviposition (sexually immature), after the first oviposition (adult), with or without secondary sporocysts, were used to attract the miracidia. RESULTS: It was noted that snails or their SCW containing secondary sporocysts lost the ability to attract miracidia. The sexual maturity of the snail did not influence miraxonal attraction.

OBJETIVO: Inquéritos sorológicos realizados em diversos paÃses mostraram uma maior prevalência da infecção pelo vÃrus da hepatite B (VHB) em dentistas, especialmente entre os cirurgiões, do que na população geral. O estudo realizado objetivou determinar a prevalência e os fatores associados à vacinação contra hepatite B (HB) entre os dentistas e investigar as principais razões alegadas para a não vacinação e vacinação incompleta. MÉTODOS: Foi conduzido um inquérito entre 299 cirurgiões dentistas residentes em Montes Claros, MG, por meio de questionário auto-aplicável. Foi determinada a prevalência de vacinação segundo o número de doses e os fatores associados à não vacinação e à vacinação incompleta através de regressão logÃstica multinomial. RESULTADOS: Dos 299 questionários distribuÃdos, 296 (99%) foram respondidos. Destes, 74,9% tomaram três doses; 14%, duas doses; 2%, uma dose e 10% não foram vacinados. A vacinação completa foi maior entre os que relataram fazer exclusivamente cirurgia e/ou periodontia (89%). A principal razão alegada para a não vacinação ou vacinação incompleta foi a necessitade de maiores informações. A não vacinação foi mais freqüente entre aqueles com mais de 40 anos (OR=8,62; IC 95%: 1,88-39,41) e os que não se reciclaram nos dois anos prévios ao inquérito (OR=2,72; IC 95%: 1,02-7,22). A vacinação incompleta foi maior entre os que não usam luva no trabalho (OR=2,32; IC 95%: 1,08-4,97). CONCLUSÃO: A falta de informação, possivelmente relacionada a menor reciclagem profissional, parece ser um dos principais fatores limitantes da vacinação.

The aim of this cross-sectional study was to determine the hepatitis B vaccination coverage among medical students at a public university in Rio de Janeiro, Brazil, and their compliance with the postvaccination serologic testing recommendations. Of the total of 858 students, 675 (78.7%) participated in the study. Among the participants, 48.9% (95% CI: 45.1% to 52.7%) were vaccinated against hepatitis B (received ≥ 3 doses of the vaccine), 31.6% were not (received 0, 1 or 2 doses), and 19.6% did not know their vaccination status. Hepatitis B vaccination coverage increased from 26.0% among first-year students to 70.6% among sixth-year students while the prevalence of unknown vaccination status decreased from 39.7% among first-year students to 2.4% among sixth-year students. The frequency of unvaccinated students ranged from 23.7% among fifth-year students to 34.4% among first-year students. Only 34.8% of the vaccinated students performed the anti-HBs testing after vaccination. Among these medical students, we found a low adherence to the hepatitis B vaccination and to the postvaccination serologic testing. A comprehensive hepatitis B immunization program should be offered to students at this medical school.

O objetivo desta pesquisa foi estudar a prevalência de infecção pelo virus da hepatite B em 406 portadores do virus da imunodeficiência humana, maiores de dezoito anos de idade, atendidos na rede pública de saúde da cidade de Belém, Pará, assim como analisar possÃveis fatores de risco para a infecção. A prevalência global de infecção pelo virus da hepatite B foi de 51% (IC: 46,1 - 55,8), com 7,9% (IC: 5,3 - 10,5) para o HBsAg, 45,1% (IC: 40,3 - 49,9) para o anti-HBc e 32,3% (IC: 27,5 - 36,8) para o anti-HBs. Após ajuste por regressão logÃstica, os marcadores sorológicos de infecção pelo vÃrus da hepatite B apresentaram associação com as seguintes variáveis: idade, situação conjugal e preferência sexual. A prevalência dos marcadores do vÃrus B nos heterossexuais foi 28,7% e 68,8% nos homossexuais/bissexuais (IC: 3,50 - 9,08; OR: 5,63; p=0,000). Quanto à situação conjugal, a categoria com companheiro fixo/casado apresentou freqüência de 31%, e foi de 58,7% a observada no grupo sem companheiro fixo (IC: 1,29 - 3,63; OR: 2,16; p=0,003). A análise multivariada não mostrou associação do vÃrus B com o uso de drogas ilÃcitas injetáveis.

O objetivo deste estudo foi determinar a freqüência dos marcadores sorológicos para hepatite B e de fatores de risco associados com a infecção pelo vÃrus B em mulheres jovens, residentes em Vitória, ES, onde a vacinação em recém-nascidos e em adolescentes foi iniciada em 1994 e 2000, respectivamente. Estudo populacional, por amostragem, realizado em três regiões de saúde de Vitória em 2006. Foi realizada entrevista e pesquisa de HBsAg, anti-HBc e anti-HBs. De 1.200 mulheres selecionadas, 1.029 (85,7%) participaram do estudo. A mediana de idade foi 23 anos (distância interquartil 20-26 anos) e 93,2% tinham mais de quatro anos de escolaridade. Quarenta e três (4,2%) mulheres (IC95% 2,97%-5,43%)] apresentaram anti-HBc total positivo e 9 (0,9%) (IC95% 0,4%-1,6%)] HBsAg. Houve 466 (45,3%) testes (IC95% 42,2%-48,4%)] anti-HBs positivos dos quais 427 eram anti-HBc e HBsAg negativas. A única variável independentemente associada com anti-HBc (+) foi renda mensal de até 4 salários mÃnimos [OR =2,6 (IC95% 1,06-6,29)]. Os dados mostram baixa prevalência do vÃrus B e de seus fatores de risco mais conhecidos. A prevalência do anti-HBs com anti-HBC e HBsAg negativos reflete a cobertura vacinal do MunicÃpio neste grupo (43,7%). Não foi possÃvel determinar fatores de risco significativos para a aquisição do vÃrus hepatite B nessa população.

INTRODUCTION: Reductions in the prevalence of hepatitis B virus (HBV) infection and carriage, decreases in liver cancer incidence, and changes in patterns of liver dysfunctions are described after hepatitis B vaccination. METHODS: We conducted a population-based seroprevalence study aimed at estimating the HBV prevalence and risk of infection in the rural area of Lábrea following nineteen years of HBV vaccination. RESULTS: Half of the subjects showed total anti-HBc of 52.1% (95% CI 49.6-54.7). The HBsAg prevalence was 6.2% (95% CI 5.1-7.6). Multivariate analysis showed an inverse association between HBV infection and vaccination (OR 0.62; 95% CI 0.44-0.87). HBsAg remained independently associated with past hepatitis (OR 2.44; 95% CI 1.52-3.89) and inversely to vaccination (OR 0.43; 95% CI 0.27-0.69). The prevalence of HBeAg among HBsAg-positive individuals was 20.4% (95% CI 12.8-30.1), with the positive subjects having a median age of 11 years (1-46) p=0.0003. CONCLUSIONS: We demonstrate that HBV infection is still an important public health issue and that HBV vaccination could have had better impact on HBV epidemiology. If we extrapolate these findings to other rural areas in the Brazilian Amazon, we can predict that the sources of chronic infected patients remain a challenge. Future studies are needed regarding clinical aspects, molecular epidemiology, surveillance of acute cases, and risk groups.

A pupunha aparentemente, possui duas estratégias que asseguram a sua polinização cruzada. Uma delas parece ser um fator genético quantitativo que inibe a auto-polinização. Para estudar este fator, duas series de polinização controlada foram feitas em duas diferentes populações juvenis. A primeira população apresentou uma media, de 19,2% de auto-compatibilidade, medida em percentagem de frutos férteis produzidos em relação a todas as flores. Entretanto, o coeficiente de variação foi extremamente alto. A polinização aberta em algumas plantas produziu 22% de frutos férteis, com um coeficiente de variação menor, sugerindo que, o " fruit set" foi, em geral, baixo nutria população, naquela vez. A segunda população, apresentou em media 37,62% de auto-compatibilidade, com um coeficiente de variação menor. Variação entre as árvores foi também detectada e mostrou ser considerável. Além do mais, a auto-compatibilidade variou de 0 a 81%, em distribuição normal. 0 uso de plantas juvenis pode ser a razão da alta variabilidade entre plantas. Apesar da pouca auto-compatibilidade encontrada nestas populações, esta caracterÃstica é recomendada como descritor para bancos de germoplasma e deve ser obtida em plantas adultas para evitar a alta variabilidade juvenil.

OBJECTIVE: To evaluate whether apolipoproteins A-I (Apo A-I) and B (Apo B) have, higher ensitivity (SN), specificity (SP) and positive predictive value (PPV) than lipoproteins (LP), total cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL), very low density lipoprotein (VLDL), and triglycerides (TGL) in assessing the risk of coronary heart disease (CHD). METHODS: This is a transversal case-control study of 241 patients, who were divided into two groups: 1) 145 patients with CHD, and 2) 96 patients without coronary disease. A model of logistic regression to evaluate the relation between the LPs and CHD was developed in which variables with a p-alpha <0.1 were included. RESULTS: Apo A-I levels were higher in the patients without CHD, (OR 2.08, CI 1.20-3.57). There were no statistical differences between the values of Apo A-I and the remaining lipid fractions (Apo A-I: 67%; Apo B: 100%; PPV: TC= 71%; TGC=71%; HDL=71%; LDL=71%). The costs of the tests in Reais were as follows: Apo A-I: R$ 56.60; Apo B-100: R$ 56.60; TC: R$ 9.94; HDL: R$ 21.30; LDL: R$ 28.40; TGL: R$ 14.20. CONCLUSION: Levels of Apo A-I and Apo B have no advantage over conventional lipoproteins in predicting the risk of CHD, despite the statistical association between Apo A-I and CHD; in addition, their costs are higher than those of the conventional lipoproteins.

Anti-human immunodeficiency virus type 1 (HIV-1) "binding antibodies" (antibodies capable of binding to synthetic peptides or proteins) occur throughout HIV-1 infection, are high-titered and highly cross-reactive, as confirmed in this study by analyzing plasma from B and F genotype HIV-1 infected individuals. Plasma from individuals infected with clade F HIV-1 displayed the most frequent cross-reactivity, in high titers, while Bbr plasma showed much higher specificity. Similarly, neutralization of a reference HIV-1 isolate (HIV-1 MN) was more frequently observed by plasma from F than B genotype infected individuals. No significant difference was seen in neutralization susceptibility of primary B, Bbr or F clade HIV-1 by plasma from individuals infected with the classical B (GPGR) or F HIV-1, but Bbr (GWGR) plasma were less likely to neutralize the F genotype primary HIV-1 isolates. The data indicate that both B and F genotype derived vaccines would be equally effective against B and F HIV-1 infection, with a slightly more probable effectiveness for F than B genotype. Although the Bbr variant appears to induce a much more specific humoral immune response, the susceptibility in neutralizing the Brazilian HIV-1 B genotype Bbr variant is similar to that observed with the classical B genotype HIV-1.

Static electric dipole polarizabilities and first hyperpolarizabilites have been calculated for the title molecules and their 3' and 4'-nitro derivatives at ab-initio Hartree- Fock/6-31G(d, p) level. The influence of the pivotal p vacant 3A elements (B, Al or Ga) substitution on the electrical properties of these molecules is detailed. The axial vector components of the first hyperpolarizabilities β(0) of the push-pull 4'-nitro derivatives, -18.2×10-32 esu (B), -21.1215;10-32 esu (Al) and -20.8215;10-32 esu (Ga) are calculated to be as much as fourfold larger then that calculated for the p-nitroaniline, a reference organic molecule for comparison for this type of molecular property.

Weeds compete with field crops mainly for water, light and nutrients, and the degree of competition is affected by the weed density and the intrinsic competitive ability of each plant species in coexistence. The objective of this research was to compare the competitiveness of alexandergrass (Brachiaria plantaginea) or Bengal dayflower (Commelina benghalensis) in coexistence with soybean, cv. M-Soy 8045. A factorial experiment (2 x 5) with two weed species and five competition proportions was carried out in a completely randomized design with four replicates. Proportions were based on a replacement series competition design, always maintaining the total density of four plants per 10 L plastic pots, which corresponded to 60 plants m ². The weed-crop proportions were: 0:4; 1:3; 2:2; 3:1; 4:0; that corresponded to the proportion of 100, 75, 50, 25 and 0% of soybean plants and the opposite for weeds, B. plantaginea or C. benghalensis plants. Leaf area, shoot dry mass of the weeds and soybean and number of soybean trifoliate leaves were evaluated when the soybean reached the phenologic stage of full flowering. B. plantaginea was a better competitor than soybean plants. Otherwise, C. benghalensis revealed a similar competitive ability that of the soybean. In both cases, there were evidences that intraspecific competition was more important.

We determined if the increased vascular responsiveness to endothelin-1 (ET-1) observed in male, but not in female, DOCA-salt rats is associated with differential vascular mRNA expression of ET-1 and/or ET A/ET B receptors or with functional differences in Ca2+ handling mechanisms by vascular myocytes. Uninephrectomized male and female Wistar rats received DOCA and drinking water containing NaCl/KCl. Control rats received vehicle and tap water. Blood pressure and contractile responses of endothelium-denuded aortic rings to agents which induce Ca2+ influx and/or its release from internal stores were measured using standard procedures. Expression of mRNA for ET-1 and ET A/ET B receptors was evaluated by RT-PCR after isolation of total cell RNA from both aorta and mesenteric arteries. Systolic blood pressure was higher in male than in female DOCA rats. Contractions induced by Bay K8644 (which activates Ca2+ influx through voltage-operated L-type channels), and by caffeine, serotonin or ET-1 in Ca2+-free buffer (which reflect Ca2+ release from internal stores) were significantly increased in aortas from male and female DOCA-salt compared to control aortas. DOCA-salt treatment of male, but not female, rats statistically increased vascular mRNA expression of ET-1 and ET B receptors, but decreased the expression of ET A receptors. Molecular up-regulation of vascular ET B receptors, rather than differential changes in smooth muscle Ca2+ handling mechanisms, seems to account for the increased vascular reactivity to ET-1/ET B receptor agonists and higher blood pressure levels observed in male DOCA-salt rats.

Dipyrone administered intravenously (iv) or intracerebroventricularly (icv) delays gastric emptying (GE) in rats. Gamma-aminobutyric acid (GABA) is the most potent inhibitory neurotransmitter of the central nervous system. The objective of the present study was to determine the effect of icv baclofen, a GABA B receptor agonist, on delayed GE induced by dipyrone. Adult male Wistar rats received a saline test meal containing phenol red as a marker. GE was indirectly evaluated by determining the percent of gastric retention (%GR) of the meal 10 min after orogastric administration. In the first experiment, the animals were injected iv with vehicle (Civ) or 80 mg/kg (240 µmol/kg) dipyrone (Dp iv), followed by icv injection of 10 µl vehicle (bac0), or 0.5 (bac0.5), 1 (bac1) or 2 µg (bac2) baclofen. In the second experiment, the animals were injected icv with 5 µl vehicle (Cicv) or an equal volume of a solution containing 4 µmol (1333.2 µg) dipyrone (Dp icv), followed by 5 µl vehicle (bac0) or 1 µg baclofen (bac1). GE was determined 10 min after icv injection. There was no significant difference between control animals from one experiment to another concerning GR values. Baclofen at the doses of 1 and 2 µg significantly reduced mean %GR induced by iv dipyrone (Dp iv bac1 = 35.9% and Dp iv bac2 = 26.9% vs Dp iv bac0 = 51.8%). Similarly, baclofen significantly reduced the effect of dipyrone injected icv (mean %GR: Dp icv bac1 = 30.4% vs Dp icv bac0 = 54.2%). The present results suggest that dipyrone induces delayed GE through a route in the central nervous system that is blocked by the activation of GABA B receptors.
