Background: Sudden cardiac death (SCD) is a sudden unexpected event, from a cardiac cause, that occurs in less than one hour after the symptoms onset, in a person without any previous condition that would seem fatal or who was seen without any symptoms 24 hours before found dead. Although it is a relatively frequent event, there are only few reliable data in underdeveloped countries. Objective: We aimed to describe the features of SCD in Ribeirão Preto, Brazil (600,000 residents) according to Coroners’ Office autopsy reports. Methods: We retrospectively reviewed 4501 autopsy reports between 2006 and 2010, to identify cases of SCD. Specific cause of death as well as demographic information, date, location and time of the event, comorbidities and whether cardiopulmonary resuscitation (CPR) was attempted were collected. Results: We identified 899 cases of SCD (20%); the rate was 30/100000 residents per year. The vast majority of cases of SCD involved a coronary artery disease (CAD) (64%) and occurred in men (67%), between the 6th and the 7th decades of life. Most events occurred during the morning in the home setting (53.3%) and CPR was attempted in almost half of victims (49.7%). The most prevalent comorbidity was systemic hypertension (57.3%). Chagas’ disease was present in 49 cases (5.5%). Conclusion: The majority of victims of SCD were men, in their sixties and seventies and the main cause of death was CAD. Chagas’ disease, an important public health problem in Latin America, was found in about 5.5% of the cases.

Se estudió la variación morfológica entre ocho accesiones de las tres principales especies de curubas cultivadas y silvestres del Valle del Cauca, Passiflora tripartita var. mollissima (curuba de Castilla), Passiflora tarminiana (curuba india) y Passiflora mixta (curuba de monte) y 11 de sus híbridos, usando el análisis de componentes principales y el método de clasificación del vecino más próximo en 105 caracteres cualitativos y cuantitativos. Siete componentes principales explican 84% de la varianza total. Las clasificaciones sobre los caracteres cualitativos y cuantitativos muestran una clara agrupación por especie. Dentro de P. mixta, se distinguen un tipo poco pubescente, representativo de las plantas silvestres más comunes, y un tipo muy pubescente, con caracteres de domesticación como frutos amarillentos de pulpa colorida, y con una mayor variación morfológica. La clasificación de estos últimos individuos sugiere una introgresión entre P. mixta y Passiflora tripartita var. mollissima. Los híbridos ocupan una posición intermedia entre las especies parentales, pero se diferencian claramente según la dirección del cruzamiento, revelando un efecto maternal sobre la herencia de los caracteres vegetativos y florales. Los primeros análisis del fruto muestran características intermedias en cuanto a forma, color y tolerancia a la antracnosis.

O objetivo deste estudo retrospectivo foi avaliar a recuperação funcional de cães paraplégicos sem percepção à dor profunda (PDP) com doença do disco intervertebral (DDIV) toracolombar submetidos à hemilaminectomia dorsolateral. Foram incluídos somente cães com DDIV entre os segmentos da medula espinhal T3 e L3, que estavam paraplégicos sem PDP submetidos à cirurgia descompressiva. Foi observada recuperação funcional satisfatória em 73,3% dos cães (n=11), sendo um, aos cinco dias, sete entre 15 e 30 dias e três acima de 30 dias do procedimento cirúrgico. A duração da perda da PDP antes da cirurgia em cinco cães recuperados foi entre 12 e 48 horas e, em seis cães, acima de 48 horas. Cães paraplégicos sem PDP em decorrência da DDIV toracolombar podem apresentar recuperação funcional satisfatória quando submetidos ao tratamento cirúrgico mesmo sem percepção a dor profunda com tempo superior a 48 horas. Futuras pesquisas serão necessárias para avaliar a eficiência do tratamento cirúrgico, principalmente para aqueles que perderam a PDP acima de 48 horas.

Basado en una reciente investigacion etnografica entre los Embera-Chami de la région plurietnica del Valle del Cauca (Colombia) el examen de escenarios de encuentros interénicos encuadrados por la legislacion multicultural permite vislumbrar la paradojica interdependencia entre la institucionalizacion de un concepto culturalista singularizante de "la" cultura y la homogeneizacion politica de los grupos etnicos. La "estatizacion" de las relaciones inter-étnicas, concomitante a la competicion por la visibilidad, entre grupos de igual estatus en la legislacion nacional e internacional aparecen como dimensiones emergentes del campo interétnico, mientras que se confirma el papel performativo de la ritualizacion en la circulacion y la apropriacion de discursos y practicas. Los escenarios descritos que corresponden a múltiples situaciones semejantes evidencian también que la apropiación multidireccional y uso contextual de conceptos genéricos provenientes del espacio académico, nacional e internacional - como "médico tradicional", "derecho ancestral", "reforzamiento cultural", o "chamanismo" - construyen campos de paradojas. Permiten a los actores étnicos de construir y visibilizar una singularidad cultural aislacionista conforme al concepto oficializado de "cultura" pero al mismo tiempo empujan a una mayor apertura de las "zonas de contacto" interétnicas donde circulan una mayor variedad de prácticas y discursos que antes de la institucionalización del multiculturalismo.

OBJETIVO: Analisar as taxas e algumas características da mortalidade por suicídio entre indígenas e não indígenas no Amazonas. MÉTODO: Estudo de coorte retrospectiva, em que os dados de óbito foram obtidos no Sistema de Informações sobre Mortalidade e os populacionais no Instituto Brasileiro de Geografia e Estatística. Foram utilizados todos os registros de suicídio do período de 2006-2010 e a interpolação geométrica anual para a estimação das subpopulações. RESULTADOS: Ocorreram 688 suicídios no Amazonas, dos quais 19,0% em indígenas. A taxa ajustada de mortalidade por suicídio (TAMS) nos indígenas, de 18,4/100 mil, foi 4,4 vezes superior a dos não indígenas. A TAMS em indígenas aumentou 1,6 vez em 2010 em relação a 2006. Nos municípios de Tabatinga e São Gabriel da Cachoeira, as TAMS foram muito altas, 75,8 e 41,9/100 mil, respectivamente. CONCLUSÕES: Evidenciou-se o comportamento desigual das taxas de mortalidade por suicídio entre indígenas e não indígenas, expondo não só sua importância local, como também sua invisibilidade como problema de saúde pública, principalmente entre jovens 15 e 24 anos.

The morphologically similar taxa Anopheles calderoni, Anopheles punctimacula, Anopheles malefactor and Anopheles guarao are commonly misidentified. Isofamilies collected in Valle de Cauca, Colombia, showed morphological characters most similar to An. calderoni, a species which has never previously been reported in Colombia. Although discontinuity of the postsubcostal pale spots on the costa (C) and first radial (R1) wing veins is purportedly diagnostic for An. calderoni, the degree of overlap of the distal postsubcostal spot on C and R1 were variable in Colombian specimens (0.003-0.024). In addition, in 98.2% of larvae, seta 1-X was located off the saddle and seta 3-C had 4-7 branches in 86.7% of specimens examined. Correlation of DNA sequences of the second internal transcribed spacer and mtDNA cytochrome c oxidase subunit I gene (COI) barcodes (658 bp of the COI gene) generated from Colombian progeny material and wild-caught mosquitoes from Ecuador with those from the Peruvian type series of An. calderoni confirmed new country records. DNA barcodes generated for the closely related taxa, An. malefactor and An. punctimacula are also presented for the first time. Examination of museum specimens at the University of the Valle, Colombia, revealed the presence of An. calderoni in inland localities across Colombia and at elevations up to 1113 m.

Group A human rotaviruses (HuRVA) are causative agents of acute gastroenteritis. Six viral structural proteins (VPs) and six nonstructural proteins (NSPs) are produced in RV-infected cells. NSP4 is a diarrhoea-inducing viral enterotoxin and NSP4 gene analysis revealed at least 15 (E1-E15) genotypes. This study analysed the NSP4 genetic diversity of HuRVA G2P[4] strains collected in the state of São Paulo (SP) from 1994 and 2006-2010 using reverse transcription-polymerase chain reaction, sequencing and phylogenetic analysis. Forty (97.6%) G2P[4] strains displayed genotype E2; one strain (2.4%) displayed genotype E1. These results are consistent with the proposed linkage between VP4/VP7 (G2P[4]) and the NSP4 (E2) genotype of HuRVA. NSP4 phylogenetic analysis showed distinct clusters, with grouping of most strains by their genotype and collection year, and most strains from SP were clustered together with strains from other Brazilian states. A deduced amino acid sequence alignment for E2 showed many variations in the C-terminal region, including the VP4-binding domain. Considering the ability of NSP4 to generate host immunity, monitoring NSP4 variations, along with those in the VP4 or VP7 protein, is important for evaluating the circulation and pathogenesis of RV. Finally, the presence of one G2P[4]E1 strain reinforces the idea that new genotype combinations emerge through reassortment and independent segregation.

Brazil's security agenda during Lula's administration was not homogeneous through the two mandates: the first tenure (2002-2006) revealed a reactive approach towards security topics, while the second one (2006-2010) was more assertive. More specifically, the shift occurred in terms of both its geographical scope - once it incorporated global issues in a more systematic way -, and instruments through which the security agenda was exercised, given the multilateral initiative of Unasur's CDS

Para dar mayor credibilidad a la información publicada en los informes de responsabilidad social corporativa se asigna a estos de un proceso de aseguramiento por parte de un experto externo. Esta investigación se basa en 1.021 observaciones de 12 países de Latinoamérica durante los años 2006-2010. A la vista de los resultados, podemos concluir que Latinoamérica es una región donde cada vez hay un mayor compromiso con la sostenibilidad. Además, los resultados señalan que la decisión de otorgar mayor credibilidad a las políticas sostenibles por medio del aseguramiento depende del tamaño de la empresa, del sector al que pertenece y del país donde se encuentre.

OBJECTIVE To analyze the variation of infant mortality as per condition of life in the urban setting.METHODS Ecological study performed with data regarding registered deaths of children under the age of one who resided in Aracaju, SE, Northeastern Brazil, from 2001 to 2010. Infant mortality inequalities were assessed based on the spatial distribution of the Living Conditions Index for each neighborhood, classified into four strata. The average mortality rates of 2001-2005 and 2006-2010 were compared using the Student’s t-test.RESULTS Average infant mortality rates decreased from 25.3 during 2001-2005 to 17.7 deaths per 1,000 live births in 2006-2010. Despite the decrease in the rates in all the strata during that decade, inequality of infant mortality risks increased in neighborhoods with worse living conditions compared with that in areas with better living conditions.CONCLUSIONS Infant mortality rates in Aracaju showed a decline, but with important differences among neighborhoods. The assessment based on a living condition perspective can explain the differences in the risks of infant mortality rates in urban areas, highlighting health inequalities in infant mortality as a multidimensional issue.

Introduction Several cases of acute Chagas disease (ACD) have been reported in the Peruvian Amazon basin. Methods The objective was to describe and investigate 6 ACD cases in children from indigenous Amazon communities in the province of Datem del Marañón in Loreto department (2006-2010). Results The mean age was 3.6 years. All patients had fever, 4/6 hepatomegaly, 2/6 splenomegaly, and 5/6 had trypomastigotes of Trypanosoma cruzi on thick smears. The fatality rate was 33.3%. Rhodnius pictipes and Rhodnius robustus adults were found inside the homes and in the peri-domiciles. Conclusions All cases reported were isolated cases. We report a new focus of ACD in indigenous populations.

Introduction Açucena Municipality, Rio Doce Valley, State of Minas Gerais, Brazil temporarily (2001-2005) interrupted epidemiological surveillance for Chagas disease. The objective of this work was to evaluate the Chagas Disease Control Program (CDCP) in Açucena and to offer suggestions for improving local epidemiological surveillance. Methods This study was conducted in three phases: I) a serological investigation of schoolchildren aged 5 to 15 years using an enzyme-linked immunosorbent assay (ELISA) test performed on blood collected on filter paper followed by ELISA, indirect immunofluorescence (IIF) and indirect hemaglutination (IHA) on venous blood for borderline cases and those in the gray zone of reactivity; II) vector evaluation using the data obtained by local health agents during 2006-2010; and III) examination by ELISA, IIF and IHA of serum samples from the inhabitants of houses where infected Triatoma vitticeps was found and evaluation of their knowledge about Chagas disease. Results Five individuals had inconclusive results in the ELISA screening but were seronegative for Chagas disease. The triatomine evaluation revealed the presence of three species: Triatoma vitticeps, Panstrongylus megistus and Panstrongylus diasi. Triatoma vitticeps was the most prevalent and widespread, with a higher (67%) index of Trypanosoma cruzi flagellates and evidence of colonization. Most of the inhabitants of the infested houses recognized triatomines and had basic knowledge about Chagas disease. Conclusions Although T. vitticeps is not clearly associated with Chagas disease transmission, these results highlight the importance of maintaining CDCP in endemic areas and the need for greater emphasis on epidemiological surveillance, especially in areas with important vectorial changes or that have been modified by human intervention.

OBJETIVO: Calcular los índices infestación por triatominos en comunidades indígenas en Colombia. MÉTODOS: Se realizó estudio descriptivo en 19 comunidades indígenas del municipio de Valledupar Departamento de Cesar, Colombia. Durante junio a diciembre de 2007 se recolectaron triatominos por búsqueda activa en las viviendas de los indígenas. Los insectos luego fueron identificados por las claves de Lent & Wygodzinsky. Se desarrolló estudio del proceso infectivo en modelo animal y análisis enzimático de cepas de Trypanosoma cruzi, detectadas en heces de triatominos. RESULTADOS: Rhodnius prolixus presentó índice de densidad en las viviendas de 154,7%, Triatoma dimidiata de 102,45%, Triatoma maculata de 109,25% y Panstrogylus geniculatus de 0,3%. El índice promedio de infestación de las cuatro especies fue de 40,54% y, el de infección con T. cruzi de 9,4%. De cinco hemocultivos positivos para T. cruzi, tres se caracterizaron por isoenzimas, clasificándose en T. cruzi grupo I. El estudio de las biopsias reveló pocas características patológicas durante el proceso de infección con las cepas de T. cruzi aisladas de triatominos domiciliados. CONCLUSIÓN: Los altos índices de infestación por triatominos en las viviendas y el índice de infección por T. cruzi, evidencian la transmisión activa de la enfermedad de Chagas, situación que amerita la aplicación de medidas de control vectorial y el estudio seroepidemilógico de la población en riesgo. La identificación de las cepas de T. cruzi como grupo I concuerda con otros estudios realizados en esta región colombiana.

O déficit habitacional no Brasil é um problema crônico, apesar dos diversos programas implementados a fim de solucionar essa questão. Este estudo objetiva analisar os elementos impulsionadores dos financiamentos habitacionais e seus impactos, em termos regionais, no estado do Rio Grande do Sul, no período de 2006 a 2010, por meio da combinação de três procedimentos: o método estrutural-diferencial, a técnica de krigagem e a regressão espacial. Os resultados indicam que as faixas de crédito que mais cresceram foram as de valor intermediário, que os municípios de menor nível de renda foram os mais beneficiados com o crédito e que houve um deslocamento dos valores dos imóveis para as regiões menos valorizadas, reduzindo as desigualdades regionais. Ou seja, o programa "Minha Casa, Minha Vida" intensificou o volume de crédito, mas não alterou os aspectos estruturais dos financiamentos.

Giardia intestinalis infection is prevalent throughout the world and widely distributed in developing countries. In general, children display serious consequences to their state of health, including slow height-weight development; therefore, the main aim of this study was to determine the association between Giardia infection and the nutritional status of children who participate in the program of complementary feeding (Mejoramiento Alimentario y Nutricional de Antioquia (MANA) - Instituto Colombiano de Bienestar Familiar (ICBF)). A cross-sectional study examining the association of giardiasis with nutritional status was conducted. A total of 2035 children aged eight months to six years-old were studied. Data were collected using structured questionnaires, anthropometric measurements and laboratory analysis of blood and stool samples. Analysis of the results showed that 27.6% of children were infected with G. intestinalis, while 8.1% and 1.9% were mildly and significantly underweight, respectively, and 14.1% presented stunting. Giardiasis was statistically identified as a strong predictor of stunting in this study population.
