Thirty one infective endocarditis (IE) fatal cases whose diagnosis was first obtained at autopsy were studied. The clinical data of these patients (Group 1) showed significant differences compared to other 141 IE cases (Group 2). The average age of 53 years in Group 1 patients was 18 years higher than that of Group 2. The Group 1 patients had a low frequency of IE predisposing heart disease. Both patient groups presented fever (about 87%), but a significant low frequency of cardiac murmur (25.8%) was observed in Group 1 patients and echocardiography tests were performed in only 16.1%, suggesting that IE diagnosis was not suspected. Likewise, although most Group 1 patients appeared with severe acute illness, they did not present the classic IE clinical presentation. Blood cultures were performed in only 64.5% of the Group 1 patients. However, bacteria were isolated in 70% of these blood cultures and Staphylococcus aureus was isolated in 71.4%. The bacteria attacked mitral and aortic valves. Complications such as embolizations and cardiac failure occurred in almost half of the cases and they also presented with infections of the lungs, urinary tract, and central nervous system. Medical procedures were performed in practically all fatal cases whose diagnosis was first obtained at autopsy. Sepsis occurred in about half of the patients and it was followed by shock in more than 25%. This form of IE must be suspected in mature and in old febrile hospitalized patients having infection predisposing diseases, embolization, and suffering medical procedures.

Cytomegalovirus (CMV) is a genus in the family Herpesviridae that has been associated with gastrointestinal syndromes. In this work we looked for a possible association of CMV infection with colorectal cancer and ulcerative colitis (UC). Blood and enteric tissue samples of 14 patients with colorectal cancer and of 21 with UC were subjected to a nested-PCR that amplifies part of the gB gene of CMV and also to immunohistochemistry using a specific monoclonal antibody to IE 76kDa protein of CMV. CMV was detected by nested-PCR in the blood and/or the enteric tissue of nine (64.3%) colorectal cancer and 16 (76.2%) ulcerative colitis patients. In the immunohistochemistry it was observed that 12 (12/21, 57.1%) positive enteric tissue samples of patients with UC and none from patients with colorectal cancer (0/14) were positive to CMV. The positivity of CMV infections in the UC patient group (12/21, 57.1%) showed by both techniques, was significantly higher (p = 0.015) than that observed for colorectal cancer patients (2/14, 14.3%). These results suggest an association of ulcerative colitis with CMV infection of the enteric tissue.

A natureza dos antigenos do T. cruzi, bem como dos fatores do hospedeiro que contribuem para a cardiopatia chagásica tem sido intensamente investigada nestes últimos anos. Nesse contexto, a caracterização funcional das populações de linfócitos T reativos na fase crônica da doença é particularmente relevante. No presente trabalho, pretende-se analisar a resposta proliferativa de células mononucleares de sangue periférico de pacientes acometidos com a forma cardíaca da doença de Chagas. Os estudos se restrigem à cruzipaina, a cisteíno-protease majoritária do T. cruzi, uma glicoproteína altamente irnunogênica em pacientes chagásicos. Utilizando o índice de estimulação (IE) das culturas de células mononucleares como critério de avaliação de reatividade celular, analisamos 24 individuos: doadores normais (n = 8), cardiopatas não-chagásicos (n = 8) e cardiopatas chagásicos crônicos (n = 8) sem outras associações mórbidas. Pela análise de variância observou-se que os IE dos pacientes chagásicos são significativamente mais altos do que o valor observado nos demais grupos (p = 0,0001) enquanto o teste de comparações múltiplas de Tukey revelou que a média do IE dos individuos normais e cardiopatas não-chagásicos não difere significativamente entre si. Nossos estudos indicam que a resposta dos linfócitos T, face à cruzipaina, está exclusivamente associada ã doença de Chagas. A análise do repertório de epitópos T da cruzipaina e do padrão funcional de reatividade (Th1/Th2) de linfócitos T de sangue periférico estã sendo presentemente conduzida. Em vista da abundante expressão de cruzipaina presente em amastigotas, é possível que linfócitos T anticruzipaina participem das reações inflamatórias associadas com a cardiopatia chagásica. A caracterização destas subpopulações poderá oferecer possíveis subsídios para a identificação de marcadores de cardiopatia chagásica.

We describe the case of a 41-year-old man with congenital heart disease and infective endocarditis (IE), who presented multiple vegetations attached to the pulmonary, mitral, and aortic valves. Three valve replacements were performed, but the patient developed an abscess at the mitral-aortic intervalvular fibrosa and died due to sepsis. We briefly discuss the indications for surgery in IE, emphasizing its role in the treatment of uncontrolled infection.

Com o objetivo de avaliar o efeito de duas espécies amazônicas em doenças relacionadas aos processos de oxidação, determinou-se a capacidade antioxidante (método Oxygen Radical Absorbance Capacity), o teor de polifenóis totais (método Folin-Ciocalteu - PT), bem como os efeitos farmacológicos in vitro (efeito antiproliferativo) e in vivo (antinociceptivo, antiinflamatório, antiulcerogênico) dos extratos hidroalcoólicos (65:35; v/v; etanol:água) das folhas de Byrsonima crassifolia (BC) e Inga edulis (IE). Os extratos de BC e IE apresentaram elevada capacidade antioxidante (1.422 e 694 µmol de Trolox Equivalente g-1 de folha seca - FS, respectivamente) e um valor relativamente alto de PT (35,93 e 24,50 mg Equivalente ácido gálico g-1 FS, respectivamente). Essa atividade antioxidante não teve relação direta com o teor de compostos fenólicos dos extratos, sugerindo a contribuição de outros grupos químicos nessa atividade. Em cultura de células tumorais humanas (nove linhagens), os extratos não apresentaram atividade antiproliferativa significante, com efeito citotóxico somente na concentração mais elevada. Em modelo de nocicepção induzida pelo calor (placa quente), o extrato de IE apresentou efeito antinociceptivo (P < 0,05) após 30 (250 e 500 mg kg-1) e 60 min (125 e 500 mg kg-1) de sua administração oral. Nos modelos de inflamação houve somente redução do edema para IE na concentração de 500 mg kg-1. Os extratos das duas espécies reduziram as lesões ulcerativas produzidas por etanol em até 84% (P < 0,05), sugerindo uma possível ligação com a atividade antioxidante observada e indicando a necessidade de estudos para a elucidação do mecanismo de ação envolvido.

OBJECTIVE: To analyze the epidemiology, diagnosis, clinical aspects causes and evolution of infectious endocarditis. METHODS: The patients analyzed were treated at the University Hospital of the Faculdade de Medicina of Ribeirão Preto-USP and had a diagnosis of infectious endocarditis defined by Duke's criteria, which classifies infectious endocarditis as native, prosthetic valve or that occurring in intravenous drug users. RESULTS: One hundred and eighty episodes of infectious endocarditis in 168 patients were observed. Echocardiograms in 132 (73.3%) provided a diagnosis of infectious endocarditis in 111 (84%) patients; mitral valves were affected in 55 (30.5%), tricuspid valves in 30 (16.6%) and the aortic valve in 28 (15.5%) patients. Hemocultures were performed in 148 (93.8%) episodes of IE. The most commonly isolated infectious organisms were Staphylococcus aureus in 46 (27.2%) patients and Streptococcus viridans in 27 (15.9%). Complications occurred in 116 (64.4%) patients and 73 (40.5%) of the patients died. CONCLUSION: The general profile of the observed infectious endocarditis was similar to that reported in studies performed in other countries and included users of intravenous drugs. The high degree of mortality observed is not compatible with progress in diagnosis and treatment of infectious endocarditis and is probably due to the absence of diagnostic suspicion. The high frequency of fatal cases of septicemia (45.1% of deaths) in the patients studied indicates that unnoticed cases of infectious endocarditis had only been diagnosed at necropsy.

OBJETIVO: Testar a hipótese de que existe correlação entre o diâmetro máximo diastólico do forame oval e o índice de excursão do septum primum em fetos normais. MÉTODOS: Foram examinados, por ecocardiografia, 102 fetos normais de gestantes com idades gestacionais entre 20 e 40 semanas. O diâmetro do forame oval e a excursão máxima do septum primum foram medidos num corte de 4 câmaras. Na análise dos dados utilizou-se o coeficiente de correlação de Pearson. RESULTADOS: A média do forame oval foi de 5,06 ± 1,29 mm; a da excursão máxima do septum primum de 5,42 ± 1,41 mm; a do átrio esquerdo 11,47 ± 2,76 mm e a do índice de excursão 0,48 ± 0,09. A relação média FO/IE foi de 11,35 ± 3,94 mm. Não houve correlação FO/IE (r = -0,03) e observou-se correlação fraca do forame oval com o átrio esquerdo (r = 0,031) e com a excursão do septum primum (r = 0,21). CONCLUSÃO: A mobilidade do septum primum não depende do diâmetro do forame oval em fetos normais, sugerindo que as modificações da sua excursão diastólica não decorram do grau de abertura interatrial.
Healthcare-Associated Infective Endocarditis: a Case Series in a Referral Hospital from 2006 to 2011

Background: Healthcare-associated infective endocarditis (HCA-IE), a severe complication of medical care, shows a growing incidence in literature. Objective: To evaluate epidemiology, etiology, risk factors for acquisition, complications, surgical treatment, and outcome of HCA-IE. Methods: Observational prospective case series study (2006-2011) in a public hospital in Rio de Janeiro. Results: Fifty-three patients with HCA-IE from a total of 151 cases of infective endocarditis (IE) were included. There were 26 (49%) males (mean age of 47 ± 18.7 years), 27 (51%) females (mean age of 42 ± 20.1 years). IE was acute in 37 (70%) cases and subacute in 16 (30%) cases. The mitral valve was affected in 19 (36%) patients and the aortic valve in 12 (36%); prosthetic valves were affected in 23 (43%) patients and native valves in 30 (57%). Deep intravenous access was used in 43 (81%) cases. Negative blood cultures were observed in 11 (21%) patients, Enterococcus faecalis in 10 (19%), Staphylococcus aureus in 9 (17%), and Candida sp. in 7 (13%). Fever was present in 49 (92%) patients, splenomegaly in 12 (23%), new regurgitation murmur in 31 (58%), and elevated C-reactive protein in 44/53 (83%). Echocardiograms showed major criteria in 46 (87%) patients, and 34 (64%) patients were submitted to cardiac surgery. Overall mortality was 17/53 (32%). Conclusion: In Brazil HCA-IE affected young subjects. Patients with prosthetic and native valves were affected in a similar proportion, and non-cardiac surgery was an infrequent predisposing factor, whereas intravenous access was a common one. S. aureus was significantly frequent in native valve HCA-IE, and overall mortality was high.

The black flies of the genus Inaequalium present a Neotropical distribution, with Panama at the northern limit, and the Argentinian pampas at the southern, but do not occur in the Central Amazon. This study offers a cladistic analysis establishing a hypothesis of relationships between the species of Inaequalium. A total of 37 characters have been considered in order to establish the hypothetic phylogenetic relationships. Cerqueirellum (Py-Daniel, 1983) was considered as outgroup. Data were analyzed using Henning 86 version 1.5. Wich the ie* command and implicit enumeration a unique possible cladogram was obtained in Inaequalium with 52 steps, and a CI of 0.76 and RI of 0.81. Two well-defined clades was obtained in the resulting cladogram, the "botulibranchium" species-group, includes I. travassosi, I. souzalopesi, I. botulibranchium and I. petropoliense, and the "inaequale" species-group, includes I. rappae, I. nahimi, I. inaequale, I. leopoldense, I. subnigrum, I. diversibranchium, I. mariavulcanoae, I. nogueirai, I. beaupertuyi, I. clavibranchium and I. subclavibranchium.

Treatment of cancer using gene therapy is based on adding a property to the cell leading to its elimination. One possibility is the use of suicide genes that code for enzymes that transform a pro-drug into a cytotoxic product. The most extensively used is the herpes simplex virus thymidine kinase (TK) gene, followed by administration of the antiviral drug ganciclovir (GCV). The choice of the promoter to drive the transcription of a transgene is one of the determinants of a given transfer vector usefulness, as different promoters show different efficiencies depending on the target cell type. In the experiments presented here, we report the construction of a recombinant adenovirus carrying TK gene (Ad-TK) driven by three strong promoters (P CMV IE, SV40 and EN1) and its effectiveness in two cell types. Human HeLa and mouse CCR2 tumor cells were transduced with Ad-TK and efficiently killed after addition of GCV. We could detect two sizes of transcripts of TK gene, one derived from the close together P CMV IE/SV40 promoters and the other from the 1.5 Kb downstream EN1 promoter. The relative amounts of these transcripts were different in each cell type thus indicating a higher flexibility of this system.

Malaria during pregnancy can be severe in non-immune women, but in areas of stable transmission, where women are semi-immune and often asymptomatic during infection, malaria is an insidious cause of disease and death for mothers and their offspring. Sequelae, such as severe anaemia and hypertension in the mother and low birth weight and infant mortality in the offspring, are often not recognised as consequences of infection. Pregnancy malaria, caused by Plasmodium falciparum, is mediated by infected erythrocytes (IEs) that bind to chondroitin sulphate A and are sequestered in the placenta. These parasites have a unique adhesion phenotype and distinct antigenicity, which indicates that novel targets may be required for development of an effective vaccine. Women become resistant to malaria as they acquire antibodies against placental IE, which leads to higher haemoglobin levels and heavier babies. Proteins exported from the placental parasites have been identified, including both variant and conserved antigens, and some of these are in preclinical development for vaccines. A vaccine that prevents P. falciparum malaria in pregnant mothers is feasible and would potentially save hundreds of thousands of lives each year.

Because an enriched environment (EE) enhances T-cell activity and T-lymphocytes contribute to immunopathogenesis during heterologous dengue virus (DENV) infections, we hypothesised that an EE increases dengue severity. To compare single serotype (SS) and antibody-enhanced disease (AED) infections regimens, serial intraperitoneal were performed with DENV3 (genotype III) infected brain homogenate or anti-DENV2 hyperimmune serum followed 24 h later by DENV3 (genotype III) infected brain homogenate. Compared AED for which significant differences were detected between the EE and impoverished environmental (IE) groups (Kaplan-Meyer log-rank test, p = 0.0025), no significant differences were detected between the SS experimental groups (Kaplan-Meyer log-rank test, p = 0.089). Survival curves from EE and IE animals infected with the AED regimen were extended after corticoid injection and this effect was greater in the EE than in the IE group (Kaplan-Meyer log-rank test, p = 0.0162). Under the AED regimen the EE group showed more intense clinical signs than the IE group. Dyspnoea, tremor, hunched posture, ruffled fur, immobility, pre-terminal paralysis, shock and death were associated with dominant T-lymphocytic hyperplasia and presence of viral antigens in the liver and lungs. We propose that the increased expansion of these memory T-cells and serotype cross-reactive antibodies facilitates the infection of these cells by DENV and that these events correlate with disease severity in an EE.

Os estrumes empregados como fontes de fósforo podem ter eficiência fertilizante diferente da dos adubos comerciais solúveis. Tal eficiência pode variar com a origem do estrume e com as proporções das diferentes frações químicas do P total contido. A eficiência de estrumes de frangos de corte e de bovinos de leite, relativamente ao superfosfato triplo-ST, e a correlação desta eficiência com frações de fósforo contidas nestes materiais foram estimadas em experimento de casa de vegetação. Os tratamentos aplicados foram: um controle, sem P; as doses 6,6, 13,2 e 19,7 mg kg-1 de P de ST, e uma dose única de 15,3 mg kg-1 de P de 11 amostras de cada um dos dois tipos de estrumes. Utilizou-se um Podzólico Vermelho-Escuro e a resposta aos tratamentos foi determinada com o cultivo de milheto (Pennisetum americanum) durante 27 dias, a partir da adubação fosfatada, período em que foram colhidos três cortes de plantas. As frações de fósforo dos estrumes foram determinadas por extrações seqüenciais. O índice de eficiência (IE), estimado pelo método de equivalentes em ST, dos estrumes de frango foi de 0,84 ± 0,071, valor menor do que o dos estrumes de bovinos de leite, de 0,94 ± 0,095. Não ocorreu diferença de IE entre as amostras de estrumes de mesma espécie animal. Considerando todas as amostras, o IE apresentou correlação positiva com as proporções de P lábil, extraídas por resina trocadora de ânions e NaHCO3 0,5 mol L-1, e de P inorgânico total e correlação negativa com as proporções de P extraídas por NaOH 0,1 mol L-1 e HCl 0,5 mol L-1 e de P orgânico total. Não ocorreu correlação entre o IE e o teor total de P dos estrumes.

Muitos trabalhos mostram a importância da biomassa microbiana do solo (BMS), principalmente como fonte/dreno de C e de N em plantações florestais; contudo, são escassos os trabalhos relacionados ao fósforo microbiano (PBM), sobretudo aqueles relativos aos métodos de determinação do PBM nesses ecossistemas. O presente trabalho foi realizado com o objetivo de avaliar métodos de determinação do PBM em solo com diferentes coberturas vegetais. O trabalho consistiu da análise de amostras de Latossolo Vermelho-Amarelo distrófico muito argiloso (LVAd) localizado no município de Viçosa (MG), coletadas nas profundidades de 0 a 5 e 5 a 10 cm, em áreas com as seguintes coberturas vegetais: pínus (Pinus taeda), eucalipto (Eucalyptus grandis) e floresta nativa. Para determinação do P microbiano, foram empregados os métodos fumigação-extração (FE), irradiação com micro-ondas-extração (IE) e irradiação com micro-ondas-extração com membrana de troca aniônica (EMTA). Em termos gerais, menores teores de PBM foram obtidos com o método irradiação-extração. Considerando a cobertura vegetal, foi detectada diferença significativa entre os três métodos sob floresta de eucalipto e floresta nativa, principalmente na camada superficial. Sob pínus, apenas o método IE diferiu dos demais, na camada subsuperficial. Menores coeficientes de variação (CV) foram obtidos com o FE, retratando maior precisão do método. Entretanto, o método IE mostrou-se, em termos operacionais, o mais adequado à determinação do PBM quando se tem maior número de amostras. Com relação às coberturas vegetais, a grande variabilidade observada nos CVs obtidos para cada cobertura, nos três métodos testados, inviabiliza a escolha de um único método que apresente maior precisão na avaliação do PBM.

ABSTRACT In recent years, geotechnologies as remote and proximal sensing and attributes derived from digital terrain elevation models indicated to be very useful for the description of soil variability. However, these information sources are rarely used together. Therefore, a methodology for assessing and specialize soil classes using the information obtained from remote/proximal sensing, GIS and technical knowledge has been applied and evaluated. Two areas of study, in the State of São Paulo, Brazil, totaling approximately 28.000 ha were used for this work. First, in an area (area 1), conventional pedological mapping was done and from the soil classes found patterns were obtained with the following information: a) spectral information (forms of features and absorption intensity of spectral curves with 350 wavelengths -2,500 nm) of soil samples collected at specific points in the area (according to each soil type); b) obtaining equations for determining chemical and physical properties of the soil from the relationship between the results obtained in the laboratory by the conventional method, the levels of chemical and physical attributes with the spectral data; c) supervised classification of Landsat TM 5 images, in order to detect changes in the size of the soil particles (soil texture); d) relationship between classes relief soils and attributes. Subsequently, the obtained patterns were applied in area 2 obtain pedological classification of soils, but in GIS (ArcGIS). Finally, we developed a conventional pedological mapping in area 2 to which was compared with a digital map, ie the one obtained only with pre certain standards. The proposed methodology had a 79 % accuracy in the first categorical level of Soil Classification System, 60 % accuracy in the second category level and became less useful in the categorical level 3 (37 % accuracy).
