Trypanosoma cruzi (Schyzotrypanum, Chagas, 1909), and Chagas disease are endemic in captive-reared baboons at the Southwest Foundation for Biomedical Research, San Antonio, Texas. We obtained PCR amplification products from DNA extracted from sucking lice collected from the hair and skin of T. cruzi-infected baboons, with specific nested sets of primers for the protozoan kinetoplast DNA, and nuclear DNA. These products were hybridized to their complementary internal sequences. Selected sequences were cloned and sequencing established the presence of T. cruzi nuclear DNA, and minicircle kDNA. Competitive PCR with a kDNA set of primers determined the quantity of approximately 23.9 ± 18.2 T. cruzi per louse. This finding suggests that the louse may be a vector incidentally contributing to the dissemination of T. cruzi infection in the baboon colony.

En este trabajo se presenta la caracterización de los ensambles de peces de la cuenca del río Suquía. Los peces fueron recolectados, desde 1998 al 2002, en dieciséis sitios ubicados en los principales ríos dentro de la cuenca: arroyo San Francisco, río Cosquín, río Yuspe, río San Antonio y río Suquía. Se estimó la abundancia relativa de cada especie, riqueza, diversidad y dominancia en cada sitio estudiado. Se aplicó un Análisis Discriminante para determinar diferencias en la composición de las especies entre los ríos de la cuenca y antes y después de los principales centros urbanos. Veintiuna especies de peces fueron recolectadas en toda la cuenca, pertenecientes a doce familias y seis órdenes. Las especies mostraron diferentes patrones de variación. El Análisis Discriminante indicó marcadas diferencias de la ictiofauna entre los ríos. Si bien ellos compartieron un número elevado de especies, los patrones de abundancia de las mismas variaron entre sitios. Los resultados también demostraron el impacto negativo que ejerce la ciudad de Córdoba sobre los ensambles de peces. Este trabajo provee nuevos datos a cerca del estado de la ictiofauna del río Suquía y constituye un punto de partida que permitirá la aplicación de programas de monitoreo y evaluación con el fin de conocer la calidad de los recursos acuáticos.

Os títulos de anticorpos no soro pela reação de inibição da hemaglutinação para rubéola, empregando o caulim para adsorção de beta-lipoproteínas bloqueadores inespecíficos, foram determinados em funcionárias do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo (Brasil), no período de 1982-1983 e confrontados com idade, cor, unidade hospitalar, local, cargo, tempo de emprego, antecedentes de rubéola ou comunicante na vigência ou não de gravidez. Participaram do estudo 1.886 funcionárias (88,9% de 2.121) tendo títulos com a distribuição: 9,6% <10,1,3% -10,3,5% -20, 5,8% -40,10,6% -80, 20,7% -160, 27,8% -320,12,6% -640, 7,3% -1.280 e 0,8% -2.560. Houve fraca associação entre títulos e quaisquer das variáveis de confrontação (P @ 0); 87,1% das funcionárias negaram antecedentes de rubéola e destas 73,9% tiveram títulos > 20; 57,6% negaram ser comunicantes e apresentaram títulos ³ 20; em 1,1% que referiram história de rubéola, os títulos foram £20; 97% negaram contacto com rubéola durante a gravidez. Houve somente um caso de malformação congênita após rubéola no primeiro trimestre da gravidez. Das 351 funcionárias sãs, e com títulos ³ 640, em 9,4% demonstrou-se IgM específica. Não foi notada flutuação significativa dos títulos em diferentes amostras em período de observação de até um ano. Conclui-se que a maioria das funcionárias é imune à rubéola (título > 20) independente de quaisquer parâmetros analisados; a presença de IgM específica em algumas funcionárias pode ser compatível com doença subclínica. Este inquérito foi considerado útil na orientação de funcionárias grávidas comunicantes de caso suspeito ou confirmado de rubéola, e para as não-grávidas e não-imunes a indicação da profilaxia pela vacina.

São apresentados resultados de uma pesquisa exploratória da satisfação/insatisfação de 158 pacientes que recorreram a um Hospital Universitário (HUPE) para consulta inicial. As entrevistas foram feitas por alunos do primeiro ano, como parte de projeto de inserção precoce do mesmo com a prática médica, acompanhando pacientes como observadores. Foi utilizado questionário que cobriu os vários aspectos do problema: variáveis sócio-demográficas, arte do cuidado, resolutividade da consulta médica, sugestões de melhoria dos serviços. Tanto alunos/observadores como os usuários conseguiram perceber com precisão os problemas do processo do atendimento (recepção, matrícula, consulta, enfermagem, serviço social, farmácia, exames complementares) mas houve divergência de opinião entre vários aspectos da satisfação/insatisfação entre alunos e usuários, provavelmente devido às origens sociais diferentes e expectativas diversas.

OBJETIVO: Realizar estudo transversal de serviço regionalizado de emergências psiquiátricas inserido em hospital universitário de emergências pela caracterização da clientela e do atendimento. MÉTODOS: Os dados foram colhidos por um protocolo, sendo considerados todos os atendimentos realizados durante dois meses. RESULTADOS: Foram preenchidos 600 protocolos que corresponderam a 96,5% dos atendimentos efetuados no período estudado, referentes a 487 pacientes. A maioria desses era do sexo masculino, sem vínculos conjugais, com baixa escolaridade, profissionalmente inativa e morava com familiares. Os diagnósticos mais freqüentes foram transtorno do uso de substância psicoativa (26,3%), esquizofrenias (15,5%), episódio maníaco (11,8%), depressão maior (10,9%) e transtornos não psicóticos (10,9%), havendo diferenças entre os sexos quanto à proporção de algumas categorias diagnósticas. Após o atendimento inicial, 2/3 recebeu medicação e 1/2 permaneceu em observação, sendo que 1/4 permaneceu mais de 10h no serviço. Cerca de 20% dos atendimentos resultaram em internação integral e 60%, em encaminhamentos para seguimento ambulatorial. Alta por evasão representou apenas 2,0% dos atendimentos. Os usuários repetitivos não diferiram daqueles que tiveram atendimento único quanto a estado civil, vínculo empregatício e condições de moradia, mas apresentaram maior freqüência de internações anteriores e de transtornos psicóticos. CONCLUSÕES: O serviço atendeu pacientes com quadros psiquiátricos graves, em real situação de urgência, sendo observada uma ampliação das funções do serviço de emergências psiquiátricas e sua efetiva inserção na rede pública de serviços de saúde mental.

OBJETIVO: Avaliar, prospectivamente, o desempenho de atividades socialmente esperadas de pacientes psiquiátricos durante o primeiro ano após a alta de uma enfermaria psiquiátrica em hospital geral; e avaliar a expectativa do próprio paciente e a do familiar em relação a esse desempenho. MÉTODOS: O estudo foi realizado em um hospital geral localizado em Ribeirão Preto, SP. Foi aplicada a Escala de Katz ao paciente e ao familiar informante, durante a internação, referindo-se à semana que antecedeu a admissão e um, seis e 12 meses após a alta hospitalar, referindo-se à semana anterior. Concluíram o estudo, 55 pacientes (33 do sexo feminino e 22 do sexo masculino) e seus respectivos familiares informantes dentre as 86 díades que iniciaram o estudo. Os escores das escalas de desempenho e expectativa foram comparados pela análise de variância (ANOVA) e pelo teste t de Student. RESULTADOS: A expectativa não mostrou variação significativa na avaliação do paciente ou do familiar. Nas duas avaliações, a expectativa foi significativamente maior que o desempenho. Entretanto, foi observada melhora significativa no desempenho de atividades socialmente esperadas em relação à pré-internação, desde o primeiro mês após a alta. Essa melhora se manteve ao longo do primeiro ano, tanto na avaliação do paciente como na da família. CONCLUSÕES: Os resultados sugerem que a internação em uma enfermaria psiquiátrica, em hospital geral, além de reduzir os sintomas psiquiátricos, colabora para a melhora no desempenho de atividades socialmente esperadas. Esse nível de desempenho manteve-se constante durante o primeiro ano após a alta, embora variáveis demográficas e clínicas possam influir nesses resultados.

Cryptosporidiosis occurrence was determined in HIV+ patient assisted in the Clinic of Infect-parasitic Diseases in a hospital of Nova Iguaçu, Rio de Janeiro, Brazil, in the period from Juy/1998 to March/1999. In order to research, seventy-five patient, carriers of diarrhea or not, were appraised. The samples of feces were collected and placed in saline solution with formaldehyde (5%). The Modified Ritchie technique was used for the oocysts research, and the smears were stained with Safranine O methylene blue. The results verified 9.33% of positive samples, with higher frequency of cases in patients of the masculine sex from 20 to 50 years old, however without significant difference.

OBJETIVO: Descrever a atividade docente assistencial, cujo objetivo é proporcionar experiência de promoção da saúde bucal coletiva a estudantes concluintes do curso de odontologia. MÉTODOS: Tal experiência fundamenta-se na avaliação do desempenho do aluno, como educador em saúde bucal, à medida que ele tem, entre outras tarefas, a de motivar pacientes internados e seus acompanhantes, na geração de hábitos saudáveis, visando à assistência integral e mais humanizada do paciente hospitalizado. RESULTADOS: Os resultados mostraram que as metas desejadas foram atingidas, visto que a higiene bucal dos pacientes já se tornou uma tarefa incorporada à rotina hospitalar, considerando-se os índices de biofilme dentário: inicial, 1,72; e final, 1,17, respectivamente. Essa diferença, de acordo com a estatística U-Mann-Whitney, mostrou-se significativa, pois o valor de p<0,0001 traduz o alto nível de motivação, alcançado pelo binômio mãe-criança, no que se refere à higiene oral. CONCLUSÕES: Conclui-se que as experiências de ensino-aprendizagem, oriundas das atividades interdisciplinares e multiprofissionais, têm permitido melhor entendimento do processo saúde-doença, por parte do aluno de odontologia. Servem ainda como oportunidade para o seu aprendizado sobre o planejamento e execução de atividades educativo-preventivas, que complementam sua vivência técnico-profissional e despertam a sensibilidade social, tão necessária à sua formação acadêmica.

OBJECTIVE Develop an index to evaluate the maternal and neonatal hospital care of the Brazilian Unified Health System.METHODS This descriptive cross-sectional study of national scope was based on the structure-process-outcome framework proposed by Donabedian and on comprehensive health care. Data from the Hospital Information System and the National Registry of Health Establishments were used. The maternal and neonatal network of Brazilian Unified Health System consisted of 3,400 hospitals that performed at least 12 deliveries in 2009 or whose number of deliveries represented 10.0% or more of the total admissions in 2009. Relevance and reliability were defined as criteria for the selection of variables. Simple and composite indicators and the index of completeness were constructed and evaluated, and the distribution of maternal and neonatal hospital care was assessed in different regions of the country.RESULTS A total of 40 variables were selected, from which 27 single indicators, five composite indicators, and the index of completeness of care were built. Composite indicators were constructed by grouping simple indicators and included the following variables: hospital size, level of complexity, delivery care practice, recommended hospital practice, and epidemiological practice. The index of completeness of care grouped the five variables and classified them in ascending order, thereby yielding five levels of completeness of maternal and neonatal hospital care: very low, low, intermediate, high, and very high. The hospital network was predominantly of small size and low complexity, with inadequate child delivery care and poor development of recommended and epidemiological practices. The index showed that more than 80.0% hospitals had a low index of completeness of care and that most qualified heath care services were concentrated in the more developed regions of the country.CONCLUSIONS The index of completeness proved to be of great value for monitoring the maternal and neonatal hospital care of Brazilian Unified Health System and indicated that the quality of health care was unsatisfactory. However, its application does not replace specific evaluations.

ABSTRACT OBJECTIVE To assess the impact of implementing long-stay beds for patients of low complexity and high dependency in small hospitals on the performance of an emergency referral tertiary hospital. METHODS For this longitudinal study, we identified hospitals in three municipalities of a regional department of health covered by tertiary care that supplied 10 long-stay beds each. Patients were transferred to hospitals in those municipalities based on a specific protocol. The outcome of transferred patients was obtained by daily monitoring. Confounding factors were adjusted by Cox logistic and semiparametric regression. RESULTS Between September 1, 2013 and September 30, 2014, 97 patients were transferred, 72.1% male, with a mean age of 60.5 years (SD = 1.9), for which 108 transfers were performed. Of these patients, 41.7% died, 33.3% were discharged, 15.7% returned to tertiary care, and only 9.3% tertiary remained hospitalized until the end of the analysis period. We estimated the Charlson comorbidity index – 0 (n = 28 [25.9%]), 1 (n = 31 [56.5%]) and ≥ 2 (n = 19 [17.5%]) – the only variable that increased the chance of death or return to the tertiary hospital (Odds Ratio = 2.4; 95%CI 1.3;4.4). The length of stay in long-stay beds was 4,253 patient days, which would represent 607 patients at the tertiary hospital, considering the average hospital stay of seven days. The tertiary hospital increased the number of patients treated in 50.0% for Intensive Care, 66.0% for Neurology and 9.3% in total. Patients stayed in long-stay beds mainly in the first 30 (50.0%) and 60 (75.0%) days. CONCLUSIONS Implementing long-stay beds increased the number of patients treated in tertiary care, both in general and in system bottleneck areas such as Neurology and Intensive Care. The Charlson index of comorbidity is associated with the chance of patient death or return to tertiary care, even when adjusted for possible confounding factors.

Severe leptospirosis affects predominantly males and presents a high susceptibility to hypokalemic acute renal failure. As hypokalemia and hyperkalemia induce severe complications, it is important to evaluate if the initial serum potassium is an independent risk factor for death in leptospirosis. The medical records of 1016 patients hospitalized with the diagnosis of leptospirosis were reviewed. The analysis was restricted to 442, according to the following criteria: male, 18 years or older, information about death or hospital discharge and recorded values of serum potassium, serum creatinine and duration of symptoms at admission. Potassium values lower than 3.5 mEq/L (hypoK), 3.5-5 mEq/L (normoK) and above 5 mEq/L (hyperK) were detected in 180, 245 and 17 patients, respectively. The death rate increased with serum potassium: 11.1% in the hypoK, 14.7% in the normoK and 47.1% in the hyperK group (p = 0.002). In a logistic regression model (normoK as referent), including age, creatinine and duration of symptoms, hypoK was not associated with increased death rate (odds ratio (OR) = 0.80; p > 0.1). On the other hand, hyperK showed a significant association with increased risk of death (OR = 3.95, p = 0.021). In conclusion, in this sample of men with leptospirosis initial serum potassium was positively and independently correlated with the risk of in-hospital death.

The main objective was to compare the in-hospital case-fatality rate of leptospirosis between pediatric (< 19 years) and adult (>19 years) patients, taking into account gender, renal function, duration of symptoms and jaundice. Medical records of 1016 patients were reviewed. Comparative analysis was restricted to 840 patients (100 pediatric, 740 adults) with recorded information on the variables included in the analysis. Among these patients 81.7% were male and 91.5% were icteric. The case-fatality rate of leptospirosis was 14.4%. The odds of death adjusted for gender, jaundice, duration of symptoms, serum urea and serum creatinine were almost four times higher for the adult than for the pediatric group (odds ratio (OR) = 3.94; 95% confidence interval = 1.19-13.03, p = 0.029). Among adults, increased age was also significantly and independently associated with increased risk of death (p < 0.01). Older patients were also more often treated by dialysis. In conclusion, the data suggest that the in-hospital case fatality rate of leptospirosis is higher for adults than for children and adolescents, even after taking into account the effects of several potential risk factors of death. Among adults, older age was also strongly and independently associated with higher risk of death.

The treatment of malignant or benign colorectal pathologies that require more complex management are priorities in tertiary hospitals such as "Hospital das Clínicas" University of São Paulo Medical Center (HCFMUSP). Therefore, benign, uncomplicated orifice conditions are relegated to second place. The number of patients with hemorrhoids, perianal fistulas, fissures, condylomas and pilonidal cysts who seek treatment at the HFMUSP is very great, resulting in over-crowding in the outpatient clinics and a long waiting list for recommended surgical treatment (at times over 18 months). The authors describe the experience of the HCFMUSP over an eight-day period with day-hospital surgery in which 140 patients underwent surgery. Data was prospectively taken on the patients undergoing surgery for benign orifice pathologies including age, sex, diagnosis, surgery performed, immediate and late postoperative complications, and follow-up. 140 patients operated on over eight days were studied. 68 were males (48.75%) with ages ranging from 25 to 62 (mean 35.2 yrs.). Hemorrhoids was the most frequent condition encountered (82 hemorrhoidectomies, 58.6%), followed by perineal fistula (28 fistula repairs, 20.0%). The most common complication was headache secondary to rachianesthesia occurring in 9 patients (6.4%). One patient (0.7%) developed bleeding immediately PO that required reoperation. Mean follow-up was 104 days. Day-surgery characterized by quality care and low morbidity is feasible in tertiary public hospitals, permitting surgery for benign orifice pathologies on many patients within a short period of time.

The hospital pharmacy in large and advanced institutions has evolved from a simple storage and distribution unit into a highly specialized manipulation and dispensation center, responsible for the handling of hundreds of clinical requests, many of them unique and not obtainable from commercial companies. It was therefore quite natural that in many environments, a manufacturing service was gradually established, to cater to both conventional and extraordinary demands of the medical staff. That was the case of Hospital das Clinicas, where multiple categories of drugs are routinely produced inside the pharmacy. However, cost-containment imperatives dictate that such activities be reassessed in the light of their efficiency and essentiality. METHODS: In a prospective study, the output of the Manufacturing Service of the Central Pharmacy during a 12-month period was documented and classified into three types. Group I comprised drugs similar to commercially distributed products, Group II included exclusive formulations for routine consumption, and Group III dealt with special demands related to clinical investigations. RESULTS: Findings for the three categories indicated that these groups represented 34.4%, 45.3%, and 20.3% of total manufacture orders, respectively. Costs of production were assessed and compared with market prices for Group 1 preparations, indicating savings of 63.5%. When applied to the other groups, for which direct equivalent in market value did not exist, these results would suggest total yearly savings of over 5 100 000 US dollars. Even considering that these calculations leave out many components of cost, notably those concerning marketing and distribution, it might still be concluded that at least part of the savings achieved were real. CONCLUSIONS: The observed savings, allied with the convenience and reliability with which the Central Pharmacy performed its obligations, support the contention that internal manufacture of pharmaceutical formulations was a cost-effective alternative in the described setting.
