De 890 "manchas" provenientes de insetos dos gêneros Panstrongylus, Rhodnius e Triatoma, positivas para sangue humano, 7 deram resultados presumptivamente positivos por hemaglutinação passiva para HBsAg. Deste total só um caso foi confirmado como positivo, por RIE (radioimunoensaio), correspondendo a uma "mancha" obtida de Triatoma sordida (ninfa de 5.º estádio).

Foram examinados 135 soros de pacientes com hanseníase para identificação dos subtipos ad e ay de HB sAg, tendo-se encontrado somente 3 casos positivos para o antígeno Austrália, todos eles do subtipo ad (2,2%).

De 3.200 "manchas" provenientes de insetos dos gêneros Triatoma e Panstrogylus, positivas para sangue humano, 12 deram resultados presuntivamente positivos, por imunodifusão, para HBsAg. Deste total, só 7 casos foram confirmados como positivos, por radioimunoensaio, correspondendo todos eles a "manchas" obtidas de ninfas de Triatoma infestans.

Serum samples from 356 HBsAg positive asymptomatic carriers, which were titrated by reverse passive hemagglutination, were analysed for the presence of HBV-DNA, HBsAg and IgM anti-HBc. The samples were divided in three classes, according to the titers of HBsAg and IgM anti-HBc and the distribution of HBV-DNA and HBsAg among these classes was studied. In the high titer class of HBsAg, 65% of samples have one or both markers against only 19% in the low titer class. From the total of 356 samples, 121 gave positive results for IgM anti-HBc (33.9%). From these, 38.9% of HBV-DNA and 47.9% of HBeAg were observed, whereas in samples with absence of IgM anti-HBc, 18.3% and 16.6% were respectively found. A higher frequency of agreement between all these markers was found in the class of high titers of HBsAg; however, HBV-DNA was detected in the low titer class of HBsAg and little or no IgM anti-HBc, showing potential blood infectivity even in HBsAg positive borderline samples.

Em 29833 doadores pesquisados encontramos prevalência de 1,52% para o HBsAg e de 11% para o anti-HBc. A co-positividade anti-HBc/anti-HBs em 2783 doadores HBsAg negativos/anti-HBc positivos foi de 81,9%. A prevalência para o HBsAg é baixa nos doadores de Campinas, enquanto o anti-HBc apresenta-se com prevalência elevada quando comparado a outros países. A pesquisa do anti-HCV, em doadores de sangue de Campinas, mostrou prevalência de 2,6% para este marcador, que é bem maior que as observadas nos EUA e Europa. Cerca de 36% dos doadores anti-HCV positivos são anti-HBc reagentes, permitindo inferir, que estas duas viroses acometem simultânea ou sequencialmente os doadores de sangue brasileiros.

This paper reports an unusual pattern of serological HBV markers and the presence of HBsAg/anti-HBs immune complexes in serum samples from two patients with fulminant hepatitis from the Brazilian Western Amazon Basin. The diagnosis was made by both serologic tests and demonstration of antigen/antibody complexes by transmission electron microscopy. Concurrent Delta virus superinfection is also discussed.

Were analyzed 648 serum samples from laboratory staff in Goiânia, Goiás aiming detection of three serological markers of HBV: HBsAg, anti-HBsAg and anti-HBcAg. The HBsAg and anti-HBcAg positive samples were also analyzed for HBeAg, anti-HBeAg and anti-HBcAgIgM markers. HBV infection rate of 24.1% was observed and, from them, 0.7% were positive for HBsAg. Viral DNA was detected by PCR in two HBsAg positive samples. A vaccination index of 74.5% and a global index of 89.9% of serological response to vaccination were observed. The direct work with biological fluids as well as cleaning workers represented significant risks for acquisition of HBV infection. The data from the present study showed an increase of the vaccination index among laboratory staff but the rates of HBV infection did not change through the years in the region.

Conjugates of goat anti-HBs IgG and horseradish peroxidase (HRP) prepared by two different methods, one using NaIO4 and the other SPDP, were compared. Anti-HBs antibodies obtained from goat, rabbit and guinea-pig were tested as capture serum. The ELISA showed a sensitivity similar to RIA and a level of antigen captation ranging from 4.37 to 8.75 nanograms/ml was obtained when rabbit or guinea-pig captures were used combined with both NaIO4 or SPDP conjugates.

HBsAg positive serum samples (896) from five brazilian regions were analysed for HBsAg subtypes. The presence of five different subtypes (ayw2, ayw3, ayw4, adw2 and adw4) was detected. In Northern region subtypes adw4 (41.2%) and adw2 (37.2%) were predominant. In the North East only subtype adw2 was encountered. In Central West, South-East and South, subtypes ayw2, ayw3, adw2 and adw4 were present, with predominance of adw2 in Central West and South East (84.3% and 69.4% respectively) whereas in the South the predominant subtype was ayw3 (41.9%) followed by ayw2 (36.4%). Subtypes ayw1, ayr and adr were not found among the samples studied. These results show the difference in the incidence of HBsAg subtypes in the different regions of Brazil and their significance in relation to the colonization and migrations in this country.

To increase blood safety Brazil introduced screening for anti-HBc among blood donors in 1993. There was a decrease in the hepatitis B virus (HBV) transmission, but this measure identified a great number of HBsAg-negative, anti-HBc-positive donors. Surveillance policy determines that contacts of HBV carriers should be screened to HBV markers, but there is no recommendation about how to guide contacts of HBsAg-negative, anti-HBc-positive donors. Aiming to evaluate whether the contacts of this group are at greater risk for HBV infection, a cross-sectional study was performed to compare prevalence of HBV infection between contacts of HBsAg-positive blood donors (group I) and contacts of HBsAg-negative, anti-HBc-positive donors (group II). Contacts were submitted to a questionnaire and blood tests for HBV markers. In group I (n = 143), 53 (37.1%) were anti-HBc-positive and 11 (7.7%) were HBsAg-positive. In group II (n = 111), there were 9 and 0.9%, respectively. HBV exposure was associated with group I, sexual activity, blood transfusion, being one of the donor's parents, and living for more than ten years with the donor. Regarding the families as sample units, it was more common to find at least one member with HBV markers (p < 0.05) among the families of group I compared to group II. Contacts of HBsAg-negative, anti-HBc-positive individuals presented a much lower risk of having already been exposed to HBV and there is no need to screen them for HBV in low to moderate prevalence populations.

Foi realizado estudo epidemiológico com vistas a determinar a prevalência de marcadores sorológicos de hepatite B na população de um pequeno município, de características rurais, do Estado de São Paulo. Observou-se prevalência total de marcadores igual a 7,74%, com valores de HBsAg, anti-HBs e anti-HBc, respectivamente iguais a 0,10%, 1,69% e 7,64%. Ressalta-se a importância da determinação do anti-HBc em estudos epidemiológicos, bem como discute-se a relevância de se comparar a reduzida circulação viral, observada na área, com as elevadas prevalências verificadas em outras regiões, buscando assim levantar hipóteses acerca de mecanismos alternativos de transmissão.

Hepatitis B virus (HBV) serological markers were investigated in 40 incident cases of hepatocellular carcinoma (HCC) and in two age and sex matched control groups, comprising 40 patients with other cancers and 80 healthy individuals, resident in Bahia, Brazil. Serologic tests were done by radioimmunoassay. The study observed high proportion of seropositivity to HBsAg (42.5%) and of those presenting HBsAg or antiHBc (65.0%) among HCC cases, higher in men than women and in those aged 17 to 30 years old. HBsAg seropositivity among HCC patients was greater than in the control group with other cancers (7.5%) and in healthy controls (2.5%), corresponding to odds ratio estimates of 15.0 (95% CI 3.29, 68.30) and 33.0 (95% CI 9.13, 119.28), both statistically significant. HBeAg was not observed and antiHBe was present in 41.2% of cases, suggesting the absence of viral replication, possibly with viral DNA intergration into the hepatocyte genome. The presence of cirrhosis was associated with HBsAg seropositivity among HCC cases. A history of chronic alcoholism is shown to be more frequently related to those cases with cirrhosis. This study highlights the relevant association between HCC and HBV in Northeast Brazil, particularly for young individuals, and the high risk of development of HCC for HBsAg carriers.

OBJETIVO: Estudar aspectos da epidemiologia da hepatite B em pessoas submetidas à coleta de sangue em unidades de saúde. MÉTODOS: Indivíduos dos quais se coletou sangue em unidades de saúde de Ribeirão Preto, independentemente do motivo, foram solicitados a fornecer uma quantidade adicional de material, obtida no momento da coleta e submetida à detecção de marcadores de hepatite B. Simultaneamente, por meio de questionário padronizado, foram obtidas informações de possíveis fatores de risco para a doença. Os dados foram analisados por meio de um modelo de regressão logística. RESULTADOS: As prevalências de HBsAg e de anti-HBcAg foram de 0,3% e 13,9%, respectivamente. Os fatores de risco associados à infecção foram: idade, residência na cidade há menos de um ano, antecedente de hepatite, exposição prévia a casas de correção e homo/bissexualismo masculino. CONCLUSÕES: Devido a dificuldades crescentes de obtenção de sangue de indivíduos sadios, essa pode ser uma alternativa para estudos que objetivem fornecer informações sobre a circulação de agentes infecciosos na população. Embora não se possa generalizar os dados obtidos pela metodologia usada, ela traz conhecimento referente à circulação do vírus de hepatite B.

OBJETIVO: En Venezuela las trabajadoras sexuales reciben un control sanitario para la sífilis y el virus de inmunodeficiencia humana (VIH). Sin embargo, otras importantes infecciones de transmisión sexual no son evaluadas. Así, se realizó este estudio con el objetivo de determinar el nivel socio-cultural de un grupo de trabajadores sexuales y su relación con la sero-presencia de marcadores de Hepatitis C y Hepatitis B, en adición a la evaluación de rutina. MÉTODOS: Se evaluaron 212 trabajadoras sexuales, que acudieron al control sanitario en el servicio de infecciones de transmisión sexual, de la ciudad de Los Teques, Venezuela. Fueron entrevistadas en cuanto a edad, nivel educativo, uso de anticonceptivos y del condón. Se les tomó una muestra de sangre para determinar sífilis, antígeno de superficie de hepatitis B (HBsAg) y la presencia de anticuerpos contra el core de hepatitis B (anti-HBc), virus de hepatitis C (anti-HC) y VIH. Los datos fueron evaluados estadísticamente por Chi-cuadrado y correlación de Pearson. RESULTADOS: La prevalencia fue de 2,4% para sífilis, 0,5% para anti-HC, 3,8% para HBsAg y 13,8% para anti-HBc. Un aumento en la prevalencia de marcadores de hepatitis B se correlacionó con un bajo nivel educativo (p<0,05) e incremento en la edad (p<0,05). No se encontró ningún caso positivo de VIH. La encuesta reveló que el 38,5% de las trabajadoras sexuales nunca utilizan el condón y un 25,6% de ellas no utiliza ningún tipo de método anticonceptivo. CONCLUSIONES: Es necesario implementar planes de inmunización de hepatitis B en este grupo de mujeres, así como campañas de educación sobre la importancia del uso del condón para disminuir la probabilidad de contraer infecciones de transmisión sexual.
