O complexo da saúde refere-se a toda a base produtiva envolvida na prestação de serviços, promoção e vigilância em saúde, envolve tanto o segmento secundário da economia, a exemplo das indústrias de base química, biotecnológica, mecânica e de materiais, quanto o terciário, referente aos serviços prestados nos estabelecimentos de saúde. O referido complexo mobiliza uma série de tecnologias relevantes para a inserção competitiva na economia global, responde pela geração de emprego, renda e parte significativa do PIB nacional, motivo pelo qual a área da saúde tem tido sua importância crescentemente institucionalizada na agenda de desenvolvimento nacional. Este artigo propõe analisar a introdução desse complexo nas prioridades do Plano Brasil Maior, dado que a saúde responde a diferentes elementos relacionados tanto a uma conjuntura política quanto à escolha dos tomadores de decisão entre variadas opções de políticas públicas, como também aos problemas socioeconômicos observados na referida área. Esses distintos elementos serão estudados à luz do referencial teórico de fluxos múltiplos, utilizando como método de análise a confrontação dos fluxos de problemas, soluções e político. Os principais resultados do estudo apontam que a decisão de incorporar o complexo econômico industrial da saúde nas prioridades do Plano Brasil Maior é fruto de uma abordagem renovada da saúde, tanto no nível cognitivo quanto no institucional, na qual a mesma é entendida simultaneamente como um direito do cidadão, um bem econômico e social assim como um espaço de acumulação de capital.

OBJECTIVE: To estimate the basic reproduction number (R0) of dengue fever including both imported and autochthonous cases. METHODS: The study was conducted based on epidemiological data of the 2003 dengue epidemic in Brasília, Brazil. The basic reproduction number is estimated from the epidemic curve, fitting linearly the increase of initial cases. Aiming at simulating an epidemic with both autochthonous and imported cases, a "susceptible-infectious-resistant" compartmental model was designed, in which the imported cases were considered as an external forcing. The ratio between R0 of imported versus autochthonous cases was used as an estimator of real R0. RESULTS: The comparison of both reproduction numbers (only autochthonous versus all cases) showed that considering all cases as autochthonous yielded a R0 above one, although the real R0 was below one. The same results were seen when the method was applied on simulated epidemics with fixed R0. This method was also compared to some previous proposed methods by other authors and showed that the latter underestimated R0 values. CONCLUSIONS: It was shown that the inclusion of both imported and autochthonous cases is crucial for the modeling of the epidemic dynamics, and thus provides critical information for decision makers in charge of prevention and control of this disease.

The purpose of this work was to acquire an overview of the infectious cycle of HAdV-41 in permissive HEK 293 cells and compare it to that observed with the prototype of the genus, Human adenovirus C HAdV-2. HEK 293 cells were infected with each virus separately and were harvested every 12 h for seven days. Infection kinetics were analysed using confocal and electronic microscopy. The results show that, when properly cultivated, HAdV-41 was not fastidious. It had a longer multiplication cycle, which resulted in the release of complete viral particles and viral stocks reached high titres. After 60 h of infection, the export of viral proteins from the infected cell to the extracellular milieu was observed, with a pattern similar to that previously described for HAdV-2 penton-base trafficking after 30 h of infection. HAdV-41 had a non-lytic cycle and the infection spread from the first infected cell to its neighbours. The release process of the viral particles is unknown. The results observed for HAdV-41 infection in HEK 293 cells show how different this virus is from the prototype HAdV-2 and provides information for the development of this vector for use in gene therapy.

RotaTeq® (Merck & Company, Inc, Whitehouse Station, NJ, USA) is an oral pentavalent rotavirus vaccine (RV5) that has shown high and consistent efficacy in preventing rotavirus gastroenteritis (RGE) in randomised clinical trials previously conducted in industrialised countries with high medical care resources. To date, the efficacy and effectiveness data for RV5 are available in some Latin American countries, but not Brazil. In this analysis, we projected the effectiveness of RV5 in terms of the percentage reduction in RGE-related hospitalisations among children less than five years of age in four regions of Brazil, using a previously validated mathematical model. The model inputs included hospital-based rotavirus surveillance data from Goiânia, Porto Alegre, Salvador and São Paulo from 2005-2006, which provided the proportions of rotavirus attributable to serotypes G1, G2, G3, G4 and G9, and published rotavirus serotype-specific efficacy from the Rotavirus Efficacy and Safety Trial. The model projected an overall percentage reduction of 93% in RGE-related hospitalisations, with an estimated annual reduction in RGE-related hospitalisations between 42,991-77,383 in the four combined regions of Brazil. These results suggest that RV5 could substantially prevent RGE-related hospitalisations in Brazil.
