Antibodies of IgG and IgM isotopes reacting with Plasmodium falciparum and P. vivax thick-smear antigens were searched for by the indirect fluorescent antibody test (IFAT) in a random sample of 230 blood donors at the transfusion centre of Porto Velho (HEMERON), Rondônia State, western Brazilian Amazon. A high prevalence of IgG seropositivity (32% against P. falciparum, 24% against P. vivax and 37% against either P. falciparum or P. vivax antigens) was detected among them, despite the fact that candidates reporting recent (<12 months) malaria attacks were not elegible. Only a small proportion of them had also detectable IgM antibodies to these antigens. These data suggest an intense, relatively recent exposure to malaria in such an urban population sample. However, parasitaemia (as detected by microscopical examination of Giemsa-stained thick smears) was patent in only one prospective donor. The antibody profile of blood donors was compared with that of healthy subjects of all age groups, living in a close endemic area (Candeias village, 21 km east of Porto Velho). The villagers were classified into two groups according to their history of a recent (<12 months) or a remote (>12 months) past malaria attack due to either P. falciparum or P. vivax. Extensive overlapping was observed when the distribution of antibody titres of healthy subjects from Candeias village with a recent and remote malaria history was compared. In conclusion, subjects with a recent or a remote malaria history could not be distinguished by sorological criteria alone.

RESUMO Diferentes fatores ambientais podem afetar a seletividade de herbicidas às culturas. O objetivo deste experimento foi avaliar os efeitos da época de semeadura e da redução artificial da luminosidade, em diferentes fases do desenvolvimento das plantas, sobre a seletividade de herbicidas e os componentes de produtividade do arroz irrigado. O experimento foi conduzido a campo, na safra 2012/2013, com o cultivar IRGA 424. Os fatores estudados foram: época de semeadura (18/outubro e 09/novembro); herbicidas (testemunha capinada, bispyribac-sodium e penoxsulam) e fases do desenvolvimento da cultura com luminosidade reduzida de 70% [testemunha, plântula (S0-V4), períodos vegetativo, reprodutivo e todo o ciclo]. Foi utilizado o delineamento inteiramente casualizado, em esquema fatorial, com quatro repetições, sendo que os fatores época de semeadura e redução de luminosidade foram dispostos em faixas, visando a viabilizar o experimento. Avaliaram-se: fitotoxicidade visual, colmos e panículas por área, espiguetas por panícula, esterilidade de espiguetas, peso de 1000 sementes e produtividade. Os dados foram submetidos à análise de variância pelo teste F (p≤0,05) e teste de médias de DMS de Fischer (p≤0,05). A época de semeadura influencia na seletividade de bispyribac-sodium, sendo que, quando realizada a semeadura perto do início do período recomendado, em 18/outubro, reduz-se a produtividade do cultivar estudado. A redução artificial de luz em diferentes fases do desenvolvimento não afeta a seletividade dos herbicidas bispyribac-sodium e penoxsulam sobre o arroz irrigado.

Foram apresentadas as alterações hematológicas do sangue periférico e da medula óssea em pacientes expostos cronicamente ao benzeno. Foram descritos a biotransformação metabólica e os possíveis mecanismos envolvidos neste tipo de toxicidade. Os dados hematológicos do sangue periférico são mostrados e avaliados em sua importância, sendo a macrocitose e a linfocitopenia sinais precoces de toxicidade ao benzeno. As alterações da medula óssea observadas são demonstradas pelos métodos complementares citológico e histológico. A anormalidade histológica de maior importância foi a hipocelularidade global devida principalmente ao setor granulocítico. Foi observado também aumento do percentual de eosinófilos, de mastócitos e de atipias no setor megacariocítico. Foram observadas alterações de caráter inflamatório e ressaltada a presença de sinais de dismielopoiese. Foram enfatizadas a necessidade da valorização das alterações hematológicas do sangue periférico e a visão critica e global desse importante problema de saúde pública.

Foi realizado levantamento da opinião dos médicos que compareceram ao "XIV Congresso Brasileiro de Reprodução Humana", sob o ponto de vista ético, a respeito da fecundação artificial. Foram analisados os resultados, chegando-se a selecionar alguns itens comportamentais como preliminarmente aceitos. Propõe-se a realização de estudos mais profundos que objetivam abordar os vários aspectos aceitos ou não pela sociedade.

Legionella sp has been emerging over the last decade as an important cause of pneumonia both hospital and community-acquired. Following an outbreak in a Renal - Transplant Unit stocked serum was tested for antibodies against Legionella pneumophila serogroup 1, and 5 cases of Legionnaires' Disease were reviewed. Two of the cases were nosocomial and three cases were community - acquired. Clinical and laboratorial aspects were similar to those expected for other causes of pneumonia, however jaundice was encountered in two cases. This study suggests that the real incidence of pneumonia caused by Legionella sp is being underestimated and the authors emphasize the importance of considering Legionnaires' Disease when empirically treating community - acquired pneumonia

The study evaluated six Plasmodium falciparum antigen extracts to be used in the IgG and IgM enzyme-linked immunosorbent assays (ELISA), for malaria diagnosis and epidemiological studies. Results obtained with eighteen positive and nine negative control sera indicated that there were statistically significant differences among these antigen extracts (Multifactor ANOVA, p< 0.0001). Urea, sodium deoxycholate and Zwittergent antigen extracts performed better than did the three others, their features being very similar for the detection of IgG antibodies. Urea, alkaline and sodium deoxycholate antigen extracts proved to be better than the others for the detection of IgM antibodies. A straight line relationship was found between the optical densities (or their respective log 10) and the log 10 of antibody dilutions, with a very constant slope. Thus serum titers could be determined by direct titration and by two different equations, needing only one serum dilution. For IgM antibody detections, log 10 expression gave results that better correlated with direct titration (95% Bonferroni). For IgG antibody detections, the titer differences were not significant. The reproducibility of antibody titers and antigen batches was also evaluated, giving satisfactory results.

Os autores relatam caso clínico de paciente com esquistossomose mansônica, tratado com oxamniquine oral em dose única de 15 mg/kg, que apresenta como efeito colateral um bloqueio átrio-ventricular incompleto tipo Mobitz I, com parada sinusal e escape ventricular. Concluem que, apesar de a oxamniquine ser eficaz e segura, pode ser determinante de cardiotoxicidade

Tuberculosis is one of the most frequent opportunistic infections after renal transplantation and occurred in 30 of 1264 patients transplanted between 1976 and 1996 at Hospital São Paulo - UNIFESP and Hospital Dom Silvério, Brazil. The incidence of 2.4% is five times higher than the Brazilian general population. The disease occurred between 50 days to 18 years after the transplant, and had an earlier and worse development in patients receiving azathioprine, prednisone and cyclosporine, with 35% presenting as a disseminated disease, while all patients receiving azathioprine and prednisone had exclusively pulmonary disease. Ninety percent of those patients had fever as the major initial clinical manifestation. Diagnosis was made by biopsy of the lesion (50%), positivity to M. tuberculosis in the sputum (30%) and spinal cerebral fluid analysis (7%). Duration of treatment ranged from 6 to 13 months and hepatotoxicity occurred in 3 patients. The patients who died had a significant greater number of rejection episodes and received higher doses of corticosteroid. In conclusion, the administration of cyclosporine changed the clinical and histopathological pattern of tuberculosis occurring after renal transplantation.

Barra de Guaratiba is a coastal area of the city of Rio de Janeiro where American visceral leishmaniasis (AVL) is endemic. Although control measures including killing of dogs and use of insecticides have been applied at this locality, the canine seroprevalence remains at 25% and during 1995 and 1997 eight autochthonous human cases were notified. In order to evaluate factors related to the increase of the risk for Leishmania (Leishmania) chagasi infection in dogs we have screened 365 dogs by anti-Leishmania immunofluorescent antibody test (IFAT) and captured sandflies in the domestic and peridomestic environment. Some variables related to the infection were assessed by uni- and multivariate analysis. The distance of the residence from the forest border, its altitude and the presence of the opossum Didelphis marsupialis in the backyard, were found predictor factors for L. (L.) chagasi infection in dogs in Barra de Guaratiba. The presence of Lutzomyia longipalpis in the peridomestic environment indicates the possibility of appearence of new human cases. Our data also suggest the presence of a sylvatic enzootic cycle at this locality.

The situation of rabies in America is complex: rabies in dogs has decreased dramatically, but bats are increasingly recognized as natural reservoirs of other rabies variants. Here, bat species known to be rabies-positive with different antigenic variants, are summarized in relation to bat conservation status across Latin America. Rabies virus is widespread in Latin American bat species, 22.5%75 of bat species have been confirmed as rabies-positive. Most bat species found rabies positive are classified by the International Union for Conservation of Nature as “Least Concern”. According to diet type, insectivorous bats had the most species known as rabies reservoirs, while in proportion hematophagous bats were the most important. Research at coarse spatial scales must strive to understand rabies ecology; basic information on distribution and population dynamics of many Latin American and Caribbean bat species is needed; and detailed information on effects of landscape change in driving bat-borne rabies outbreaks remains unassessed. Finally, integrated approaches including public health, ecology, and conservation biology are needed to understand and prevent emergent diseases in bats.

INTRODUÇÃO: No Brasil, a Região Amazônica é endêmica em malária. Em Santa Catarina, a malária foi eliminada na década de 80. A partir daí, ocorreram poucos casos autóctones isolados, e esporádicos. No entanto, em função da existência do vetor em seu território, da existência de extensa área endêmica no Brasil e da grande mobilidade de pessoas em áreas turísticas no estado, existe a probabilidade de reintrodução da doença. MÉTODOS: Utilizou-se os seguintes dados: Banco de Dados do Núcleo de Entomologia da Fundação Nacional de Saúde, Santa Catarina (ACCES,1997-2000); Sistema de Informação de Vigilância Epidemiológica, Secretaria de Vigilância em Saúde (Malária/SC) e Sistema de Informação de Notificação e Agravo(SINAN/SC). Os mesmos foram transportados e analisados, no programa Microsoft Office Excel 2007. RESULTADOS: As coletas foram realizadas em 48 municípios, 159 localidades, sendo identificados 12.310 Culicídeos, 11.546 (93,7%) Anopheles e 764 (6,2%) como outros. Foram identificados três subgêneros e 13 espécies de anofelinos. CONCLUSÕES: Considerando que nos municípios pesquisados, foi identificada a presença de importantes vetores como Anopheles cruzii e Anopheles albitasis e há circulação de pessoas infectadas provenientes de áreas endêmicas, pode-se considerar que os mesmos são áreas receptivas e vulneráveis à malária. Essas espécies são suspeitas de serem responsáveis pela transmissão de malária na região, principalmente nos municípios de Gaspar, Indaial e Rodeio.

The present work evaluated several aspects of the generalized stress response [endocrine (cortisol), metabolic (glucose), hematologic (hematocrit and hemoglobin) and cellular (HSP70)] in the Amazonian warm-water fish matrinxã (Brycon amazonicus ) subjected to an acute cold shock. This species farming has been done in South America, and growth and feed conversion rates have been interesting. However, in subtropical areas of Brazil, where the water temperature can rapidly change, high rates of matrinxã mortality have been associated with abrupt decrease in the water temperature. Thus, we subjected matrinxã to a sudden cold shock by transferring the fish directly to tanks in which the water temperature was 10ºC below the initial conditions (cold shock from 28ºC to 18ºC). After 1h the fish were returned to the original tanks (28ºC). The handling associated with tank transfer was also imposed on control groups (not exposed to cold shock). While exposure to cold shock did not alter the measured physiological conditions within 1h, fish returned to the ambient condition (water at 28º C) significantly increased plasma cortisol and glucose levels. Exposure to cold shock and return to the warm water did not affect HSP70 levels. The increased plasma cortisol and glucose levels after returning the fish to warm water suggest that matrinxã requires cortisol and glucose for adaptation to increased temperature.

O tambaqui é a principal espécie de peixe cultivado na Amazônia Ocidental. Porém durante o processo produtivo, práticas de manejo são necessárias para o monitoramento do crescimento e estado geral da sanidade dos animais. Para isso os animais devem ser anestesiados para maior segurança no trabalho. O eugenol, componente majoritário do óleo de cravo, tem sido bastante utilizado como anestésico alternativo para peixes por ser um produto natural e de baixo custo. Entretanto, estudos tratando de respostas metabólicas em peixes tropicais expostos a diferentes anestésicos são ainda necessários. Dentro desse intuito, o presente trabalho avaliou respostas metabólicas, detectadas por meio de alterações de parâmetros sanguíneos e plasmáticos do tambaqui, exposto ao eugenol em banhos anestésicos simulados. Respostas típicas ao estresse foram detectadas devido ao manuseio imposto aos peixes durante a realização dos banhos anestésicos. O eugenol não reduziu totalmente essas reações ao estresse. Por outro lado, esse anestésico não provocou estresse adicional em exposições curtas de 15 min em concentrações próximas a 20 mg L-1.

OBJECTIVE: To analyze the initial clinical experience of transmyocardial laser revascularization (TMLR) in patients with severe diffuse coronary artery disease. METHODS: Between February, 1998 and February, 1999, 20 patients were submitted to TMLR at the Heart Institute (InCor), University of São Paulo Medical School, Brazil, isolated or in association with conventional coronary artery bypass graft (CABG). All patients had severe diffuse coronary artery disease, with angina functional class III/IV (Canadian Cardiovascular Society score) unresponsive to medical therapy. Fourteen patients were submitted to TMLR as the sole therapy, whereas 6 underwent concomitant CABG. Fifty per cent of the patients had either been previously submitted to a CABG or to a percutaneous transluminal coronary angioplasty (PTCA). Mean age was 60 years, ranging from 45 to 74 years. RESULTS: All patients had three-vessel disease, with normal or mildly impaired left ventricular global function. Follow-up ranged from 1 to 13 months (mean 6.6 months), with no postoperative short or long term mortality. There was significant symptom improvement after the procedure, with 85% of the patients free of angina, and the remaining 15 % of the patients showing improvement in functional class, as well as in exercise tolerance. CONCLUSION: This novel technique can be considered a low risk alternative for a highly selected group of patients not suitable for conventional revascularization procedures.
