Outbreaks of rapidly growing mycobacteria have been occasionally described. The article reports an outbreak of cutaneous abscesses due to Mycobacterium chelonae following mesotherapy in Lima, Peru. From December 2004 through January 2005, 35 subjects who had participated in mesotherapy training sessions presented with persistent cutaneous abscesses. Thirteen (37%) of these suspected cases consented to underwent clinical examination. Skin punch-biopsies were collected from suspicious lesions and substances injected during mesotherapy were analyzed. Suspected cases were mainly young women and lesions included subcutaneous nodules, abscesses and ulcers. Mycobacterium chelonae was isolated from four patients and from a procaine vial. In conclusion, it is important to consider mesotherapy as a potential source of rapidly growing mycobacteria infections.

Cat-Scratch Disease (CSD) is a benign lymphadenitis that may progress to severe or recurrent forms, and it is occasionally associated with morbidity. Between January of 1998 and March of 1999, forty-three suspected CSD patients were assessed in the Hospital Cayetano Heredia and the Instituto de Salud del Niño, in Lima, Peru. Twelve patients had a confirmed diagnosis, 8 of whom were women, and the mean age was 10 years old. The majority (53%) of the cases were encountered in the summer. All patients reported having had contact with cats. Fever, malaise, lymphadenopathy and skin lesions were the most frequent clinical features. Twelve patients had indirect immunofluorescence antibody test titers of between 1/50 and 1/800 for Bartonella henselae and Bartonella clarridgeiae. Two lymph node biopsies were histologically compatible with CSD. No positive blood cultures could be obtained. This is the first Peruvian prospective study able to identify B. henselae and B. clarridgeiae in pediatric patients.

Anthrax is a zoonosis produced by Bacillus anthracis, and as an human infection is endemic in several areas in the world, including Peru. More than 95% of the reported naturally acquired infections are cutaneous, and approximately 5% of them can progress to meningoencephalitis. In this study we review the clinical and epidemiological characteristics of the patients with diagnosis of cutaneous anthrax evaluated between 1969 and 2002 at the Hospital Nacional Cayetano Heredia (HNCH) and the Instituto de Medicina Tropical Alexander von Humboldt in Lima, Peru. Seventy one patients were included [49/71 (69%) of them men], with a mean age of 37 years. The diagnoses were classified as definitive (44%) or probable (56%). The most common occupation of the patients was agriculture (39%). The source of infection was found in 63 (88.7%) patients. All the patients had ulcerative lesions, with a central necrosis. Most of the patients (65%) had several lesions, mainly located in the upper limbs (80%). Four patients (5.6%) developed meningoencephalitis, and three of them eventually died. In conclusion, considering its clinical and epidemiological characteristics, cutaneous anthrax must be included in the differential diagnosis of skin ulcers. A patient with clinical suspicion of the disease should receive effective treatment soon, in order to avoid neurological complications which carry a high fatality rate.

During a health survey in a primary school from the district of Carabayllo (North of Lima, Peru), 200 schoolchildren (96 male and 104 female between five to 12 years old) were randomly selected and divided in two groups (as positive or negative group), according to the serologic result of the Toxocara ELISA test from a total population of 646 schoolchildren. All children were analyzed by hematologic tests to determinate the frequency of eosinophilia and leukocytosis. Additionally, all clinical and epidemiological data were also analyzed to determine their association with toxocariasis. From group of children with positive serology, 40% had some type of eosinophilia in contrast to 19% of children with negative serology, and their association was statistically significant (OR = 2.84, p < 0.001). From all signs and symptoms evaluated, only 'dry cough' was more frequent and statistically significant in the positive serology group (OR = 2.79, p < 0.001). Almost all risk factors evaluated were highly frequent and statistically associated to the positive serology. In conclusion, the presence of eosinophilia and the risk factors evaluated in this population were frequently associated to human toxocariasis.

The aim of this study was to assess the seroprevalence of human toxocariasis in three Andean communities from the Northeast of Lima, Peru. A total of 303 subjects including children and adults were studied and blood samples were collected to detect anti-Toxocara antibodies by ELISA-IgG test and by hematological examination; stool samples were collected also for parasitological examination. The overall seroprevalence of toxocariasis observed in the total population was 20.46%, with a significant high proportion in children from one to 10 years old (p = 0.034). Among the subjects with positive serology, 32.26% of them had respiratory disturbances, 22.58% hepatomegaly, 17.74% ocular signs or symptoms, 14.51% abdominal pain, 9.68% neurological involvement, and 4.84% cutaneous signs, but none of these clinical features were associated to a positive serology by multivariate analysis. Furthermore, 79.03% of seropositive subjects also harbored at least one intestinal parasite, which was associated to a positive serology (p < 0.05). The presence of pets within the houses, a previous history of pica or geophagia and the use of public places were also present in this population, but only the latter was associated to the serology (p < 0.05). In conclusion, clinical, serological, and epidemiological evidences for larval Toxocara infection were found in the studied population.

SUMMARY Feral pigeons (Columbia livia) live in close contact with humans and other animals. They can transmit potentially pathogenic and zoonotic agents. The objective of this study was to isolate and detect strains of diarrheagenic Escherichia coli and Campylobacter jejuniof urban feral pigeons from an area of Lima, Peru. Fresh dropping samples from urban parks were collected for microbiological isolation of E. coli strains in selective agar, and Campylobacterby filtration method. Molecular identification of diarrheagenic pathotypes of E.coliand Campylobacter jejuni was performed by PCR. Twenty-two parks were sampled and 16 colonies of Campylobacter spp. were isolated. The 100% of isolates were identified as Campylobacter jejuni. Furthermore, 102 colonies of E. coli were isolated and the 5.88% resulted as Enteropathogenic (EPEC) type and 0.98% as Shiga toxin-producing E. coli (STEC). The urban feral pigeons of Lima in Peru can act as a reservoir or carriers of zoonotic potentially pathogenic enteric agents.

We report here the reappearence of Aedes aegypti in the Rimac district, and summarize the history of this mosquito species in Peru since its first detection in 1852. On March 17 2000 were found Ae. aegypti and Culex quinquefasciatus in Mariscal Castilla town, Flor de Amancaes, San Juan de Amancaes, El Altillo and Santa Rosa in the Rimac district, Lima Province.

Pseudopsis Newman, 1834 is a genus of Pseudopsinae with about 46 species distributed worldwide, five of which have been reported for South America. In this paper P. monica sp. nov. is described and illustrated based on specimens collected in Lima, Peru. A key to South American species of Pseudopsis and a catalog are also provided.

OBJECTIVE To assess the increased diagnostic yield for pulmonary tuberculosis using bronchial washing cultures compared with sputum cultures. METHODS Study conducted with 61 adults in Lima, Peru, from January 2006 to December 2007. The yield of sputum cultures was compared with the yield of acid-fast bacilli smears and cultures of bronchial washing for diagnosing pulmonary tuberculosis in suspected cases of clinical tuberculosis with negative acid fast bacilli sputum smears. RESULTS Twenty seven (95%CI 32;58) of the cases were eventually diagnosed with smear-negative pulmonary tuberculosis. Bronchial washing samples detected 23 (95%CI 72;99) of the smear-negative pulmonary tuberculosis cases compared with 15 (95%CI 37;74) for sputum cultures (p = 0.02). The incremental diagnostic yield of acid fast bacilli smear and culture of bronchial washing specimens over sputum culture was 44% (95%CI 25;65). CONCLUSIONS In function of the epidemiological context and the resources available, bronchoscopy should be deployed as part of a comprehensive work up that optimizes smear-negative pulmonary tuberculosis diagnosis and minimizes risk and costs.

OBJECTIVE The objective of this study was to investigate the association between food assistance program participation and overweight/obesity according to poverty level. METHODS A cross-sectional analysis of data from 46,217 non-pregnant and non-lactating women in Lima, Peru was conducted; these data were obtained from nationally representative surveys from the years 2003, 2004, 2006, and 2008-2010. The dependent variable was overweight/obesity, and the independent variable was food assistance program participation. Poisson regression was used to stratify the data by family socioeconomic level, area of residence (Lima versus the rest of the country; urban versus rural), and survey year (2003-2006 versus 2008-2010). The models were adjusted for age, education level, urbanization, and survey year. RESULTS Food assistance program participation was associated with an increased risk of overweight/obesity in women living in homes without poverty indicators [prevalence ratio (PR) = 1.29; 95% confidence interval (CI) 1.06;1.57]. When stratified by area of residence, similar associations were observed for women living in Lima and urban areas; no associations were found between food assistance program participation and overweight/obesity among women living outside of Lima or in rural areas, regardless of the poverty status. CONCLUSIONS Food assistance program participation was associated with overweight/obesity in non-poor women. Additional studies are required in countries facing both aspects of malnutrition.

Rhinoscleroma is a rare infection in developed countries; although, it is reported with some frequency in poorer regions such as Central Africa, Central and South America, Eastern and Central Europe, Middle East, India and Indonesia. Nowadays, rhinoscleroma may be erroneously diagnosed as mucocutaneos leishmaniasis, leprosy, paracoccidioidomycosis, rhinosporidiasis, late syphilis, neoplasic diseases or other upper airway diseases. From 1996 to 2003, we diagnosed rhinoscleroma in eight patients attended in the Dermatologic and Transmitted Diseases service of "Cayetano Heredia" National Hospital, in Lima, Peru. The patients presented airway structural alterations producing nasopharyngeal, oropharyngeal and, in one patient, laryngeal stenosis. Biopsy samples revealed large vacuolated macrophages (Mikulicz cells) in all patients. Ciprofloxacin 500 mg bid for four to 12 weeks was used in seven patients and oxytetracycline 500 mg qid for six weeks in one patient. After follow-up for six to 12 months the patients did not show active infection or relapse, however, all of them presented some degree of upper airway stenosis. These cases are reported because of the difficulty diagnosing the disease and the success of antibiotic treatment.

Larvas de Leptus sp. (Acari, Prostigmata, Erythraeidae) foram coletadas das membranas intersegmentares dos tergitos e esternitos abdominais de abelhas (Ápis mellifera L.) em Cerro de Pasco, Peru. Não foram relatados danos por este ectoparasito. Fêmeas de Blattisoeius dentriticus (Berlese, 1918) (Acari, Mesostigmata, Ascidae) foram coletadas de colméias de abelhas, junto com Tyrophagus putrescentiae (Schrank, 1781) (Acari, Astigmata, Acaridae), em Lima, Peru.

Desde 1914, com o trabalho inicial de Castellani, pesquisadores de diversos países teem assinado o encontro de toxoplasmose humana, o que foi até recentemente objeto de controvérsias. Como resultado dos trabalhos de Torres, Levaditi & coL, Wolf, Cowen & Paige, Pinkerton & Henderson e Sabin, a questão do parasitismo do homem por Toxoplasma, deve ser encarada considerando-se que teem sido descritos casos duvidosos e casos não duvidosos: a) Duvidosos, são os casos referidos por Castellani (Ceilão, 1914); Fedorovitch (Mar Negro, 1916); Chalmers & Kamar (Sudão, 1920); e Bland (Londres, 1930-31). b) Não duvidosos são os relatados por Janku (Praga, 1923); Torres, (Rio de Janeiro, 1927); Wolf & Cowen (New-York, 1937); Richter (Chicago, 1936: diagnósticado por Wolf & Cowen em 1938); Wolf, Cowen & Paige (New-York, 1939); Hertig (Massachusetts, 1935: diagnósticado por Pinkerton & Weinman em 1940); Pinkerton & Weinman (Lima-Peru, 1940); Sabin (dois casos, Cincinnati, 1941); Pinkerton & Henderson (dois casos, St. Louis, 1941); Paige, Cowen & Wolf (três casos, New-York, 1942); e De Lange (Amsterdam, 1929: diagnósticado por Paige, Cowen & Wolf em 1942). Destes relatos considerados como casos não duvidosos de toxoplasmose humana, 10 representam uma doença congênita (Torres e Paige, Wolf & Cowen) ocorrendo em crianças recemnascidas ou de poucos meses de vida. Como nas vezes em que foi possivel examinar as mães dessas crianças tratava-se de mulheres com aspecto sadio, concluiu-se que a moléstia, embora sumamente grave e mortal, pode apresentar uma forma inaparente. Em dois casos, provas de soro proteção demonstraram essa infecção materna inaparente, pelo encontro de anticorpos neutralizantes para Toxoplasma (Paige, Cowen & Wolf). Nada obstante não ter sido feito um estudo microscópico detalhado em todos os casos, a doença caracteriza-se por uma meningoence falomielite granulomatosa (Wolf & Cowen), associada frequentemente à miocardite e corioretinite. Os sintomas apresentados pelos pacientes, foram: febre, convulsões, distúrbios respiratórios, hidrocefalia, cianose, vômitos, labidade de temperatura, etc., sendo sinal diagnóstico de máxima importância, o encontro de focos de calcificação cerebral profunda e corioretinite em recem nascido (Dyke, Wolf, Cowen, Paige & Caffey) . Em dois casos de toxoplasmose verificados em pacientes de seis e oito anos respectivamente, foi encontrada uma encefalite clinicamente atípica, sendo que um deles sarou (Sabin). Os três casos restantes foram descritos em adultos; num deles as lesões não eram típicas (havia concomitância de infecção por Bartonella bacilliformis) e nos outros dois, a doença assumiu uma "forma exantemática", simulando as febres maculosas (Pinkerton & Henderson), sendo o achado mais importante microscopicamente, uma pneumonia intersticial. Alem da semelhança clínica, tambem os focos de encefalite encontrados nessa "forma exantemática" da toxoplasmose, são semelhantes aos descritos nas...

Fifty specimens of five strains (10 per strain) of Helisoma duryi from Lima (Peru), St. Croix (Virgin Islands), Formosa (Brazil), Cartago (Costa Rica) and St. Vincent (Lesser Antilles), reared in isolation for about 150 days, laid 103 eggs. The numbers of eggs laid by the 10 specimens of each strain were respectively (viable eggs in parenthesis): 44(26), 1 (1), 5(0), 15 (7) and 38 (0). Egg production widely varied between the individuals of each strain, there being in all strains,except St. Vincent, a number of specimens (3 to 9) which did not lay any eggs. After the observation period the isolated specimens, including those that laid no eggs, readily engaged in cross-breeding when mated and brought forfh large numbers of eggs. Self-fertilized F 1s are fully interfertile, producing normal cross-fertilized offspring. Ten specimens of Helisoma trivolvis (strain from Zempoala, Mexico), also reared in isolation for about 120 days, laid 646 eggs, of which 74 were inviable. our data, added to those from a few ´revious studies cited in the text, show that self-fertilization is not so efficient an alternative mode of reproduction in H. duryi as in many other planorbids (it is a little more efficient in H. trivolvis than in H. duryi). Thus, H. duryi benefits much less from functional hermaphroditism which, besides other advantages, enables a single virgin individual to found a new population.
