OBJECTIVO: Evaluar el efecto de factores de riesgo para el desarrollo infantil en niños, de nivel socioeconómico bajo, nacidos en óptimas condiciones biológicas pero expuestos a condiciones sociales adversas. METODOLOGÍA: Lactantes de ambos sexos em número de 788, fueron exhaustivamente estudiados en términos de: alimentación, crecimiento, ambiente físico y psicosocial. Se controlaron prospectivamente hasta los 12 meses de edad, momento en que se evaluó el desarrollo mental (MDI) y motor (PDI) a través de la escala de Bayley. A partir de las variables medidas se identificó 18 factores de riesgo, los que fueron dicotomizados (bajo y alto riesgo). RESULTADOS: Lactancia materna, temperamento del niño, inteligencia materna y estimulación en el hogar se asocian consistentemente con MDI y PDI. Luego de ajustar por covariables, la estimulación en el hogar persiste como factor significativo en la explicación de la variabilidad del MDI y PDI, en tanto que el sexo del niño se consolida como factor condicionante del desarrollo motor. El efecto acumulado de 7 o más factores de riesgo se asocia con una disminución significativa de los puntajes de desarrollo. La combinación de categorías de riesgo de temperamento infantil, inteligencia materna, rol del padre y estimulación en el hogar alcanza mejor predicción del desarrollo infantil que otras combinaciones puestas a prueba en este estudio. CONCLUSIONE: Se concluye que, aún en niños biológicamente indemnes, el desarrollo psicomotor se afecta negativamente ante la presencia de condiciones ambientales adversas que actúan simultáneamente.

Between November 2001 and December 2002, 600 dog fecal samples were collected in main squares and public parks of 13 cities in Chile, from the extreme north to the extreme south of the country. The samples were processed in the laboratory by centrifugal sedimentation and the Harada-Mori methods. T. canis eggs were found in 12 cities. Detection rates ranged from 1.9 to 12.5% with an average of 5.2%. Seven percent of the samples had eggs and 9.5% had rhabditoid and/or filariform larvae of Ancylostomatidae. Strongyloides stercoralis were not found. Squares and public parks in Chile pose a potential risk of exposure to visceral, ocular, and/or cutaneous larva migrans syndromes.

Sera from patients infected with Taenia solium, Hymenolepis nana and Echinococcus granulosus were tested against homologous and heterologous parasite antigens using an ELISA assay, and a high degree of cross-reactivity was verified. To identify polypeptides responsible for this cross reactivity, the Enzyme Linked Immunoelectro Transfer Blot (EITB) was used. Sera from infected patients with T.solium, H.nana, and E.granulosus were assessed against crude, ammonium sulphate precipitated (TSASP), and lentil-lectin purified antigens of T.solium and crude antigens of.H.nana and E.granulosus. Several bands, recognized by sera from patients with T.solium, H.nana, and E.granulosus infections, were common to either two or all three cestodes. Unique reactive bands in H.nana were noted at 49 and 66 K-Da and in E.granulosus at 17-21 K-Da and at 27-32 K-Da. In the crude cysticercosis extract, a specific non glycoprotein band was present at 61-67 K-Da in addiction to specific glycoprotein bands of 50, 42, 24, 21, 18, 14, and 13 K-Da. None of the sera from patients with H.nana or E.granulosus infection cross reacted with these seven glycoprotein bands considered specific for T.solium infection.

A case is reported of a woman who lived in a rural area with a chronic illness that consisted of weight loss and abdominal pain in the epigastrium and upper right quadrant. The initial diagnosis was a mass in the liver, which was later, demonstrated, both by direct and histological examination, to be an abscess caused by Ascaris lumbricoides. Eggs of Ascaris lumbricoides and abundant Charcot-Leyden Crystals were found.

Seroprevalence of HCMV in Costa Rica is greater than 95% in adults; primary infections occur early in life and is the most frequent congenital infection in newborns. The objectives of this study were to determine the genetic variability and genotypes of HCMV gB gene in Costa Rica. Samples were collected from alcoholics, pregnant women, blood donors, AIDS patients, hematology-oncology (HO) children and HCMV isolates from neonates with cytomegalic inclusion disease. A semi-nested PCR system was used to obtain a product of 293-296 bp of the gB gene to be analyzed by Single Stranded Conformational Polymorphism (SSCP) and sequencing to determine the genetic polymorphic pattern and genotypes, respectively. AIDS patients showed the highest polymorphic diversity with 14 different patterns while fifty-six percent of HO children samples showed the same polymorphic pattern, suggesting in this group a possible nosocomial infection. In neonates three genotypes (gB1, gB2 and gB3), were determined while AIDS patients and blood donors only showed one (gB2). Of all samples analyzed only genotypes gB1, 2 and 3 were determined, genotype gB2 was the most frequent (73%) and mixed infections were not detected. The results of the study indicate that SSCP could be an important tool to detect HCMV intra-hospital infections and suggests a need to include additional study populations to better determine the genotype diversity and prevalence.

INTRODUCTION: Information about HIV phenotypes of resistant to available ART and the influence of different risk factors on virological failures (VF) in Costa Rican HIV positive patients prior or during HAART is unknown. MATERIALS AND METHODS: Eighty nine samples, 72 VF and 17 basal (before treatment) were analyzed by examining resistant mutants in reverse transcriptase (RT) and protease (PT) regions using Trugene or LIPA genotyping kits. Sixty eight control patients were selected and relevant information was collected in a questionnaire. RESULTS: Poor adherence, presence of resistant mutations and number of treatment's changes were the only significant factors found (p = 0.006, 0.04 and 0.01 respectively). From 66 sequenced samples, 78%, 50% and 50% showed resistance to NRTI (nucleoside reverse transcriptase inhibitors), NNRT (non-nucleoside reverse transcriptase inhibitors) and PI (protease inhibitors), respectively. The most frequent mutations were M41L, M184V, and T215FY in RT and L62PI, L10FIRV and M36I in PT. DISCUSSION: The most important factor related to treatment response in this study was adherence to treatment. Mutations in RT were related to the treatment failure while the ones found in PT were secondary mutations which have been previously described to influence the selection of primary resistance mutations in these regions. The study reveals the urgency to detect resistant mutations in VF to be considered by physicians for selection of treatment schedule, to analyze basal HIV patients for monitoring of the spread of resistant mutations and the importance to reinforce the adherence in the patients for overall treatment outcome.

Over a two year period, the incidence of hepatitis C virus (HCV) infection was evaluated in 29 hemodialysis patients, aged between 15 and 75 years (mean ± SD: 45 ± 39.5 years), from the University Hospital Hemodyalisis Unit, Maracaibo, Zulia State, Venezuela. Anti-HCV antibodies were determined using a fourth generation ELISA (Innotest HCV Ab IV) kit and positive blood samples were tested using a recombinant assay kit (Inno-LIA HCV Ab III), both kits from Innogenetics N.V., Belgium. The findings indicate a lack of HCV seroconversion in the hemodialysis patients over the study period, confirmed by the recombinant assay. Risk factors for HCV infection were 0.3270 (95% confidence interval: 0.01323-8.080) in patients undergoing hemodialysis. The findings suggest a lack of significant sources for HCV infection due to the preventive measures to avoid its transmission in the hemodialysis unit.

Estudou-se em laboratório a influência de duas diferentes fontes de alimentação sobre oviposição, eclosão dos ovos e tempo de vida de casais de Triatoma matogrossensis. Foram observados 68 casais separados em quatro diferentes grupos, com 20, 12, 20 e 16 casais, respectivamente. Os grupos A1 e A 2 foram alimentados em pombos e os grupos C1 e C 2 em coelhos. Os casais foram mantidos em estufa a 26o ± 2oC e UR de 60-80%, com fotoperíodo de 14 horas. O maior número de ovos postos foi encontrado no grupo A1. Não houve diferença na proporção de ovos eclodidos dos grupos alimentados em coelhos ou pombos. Não foram observadas diferenças de tempo de vida entre machos e fêmeas dos quatro grupos, entretanto o grupo A1 apresentou o maior tempo de sobrevivência e o grupo C2 o menor.

Doze pacientes com idades entre 7 a 12 anos, na forma indeterminada da doença de Chagas, com sorologia e xenodiagnóstico positivos, receberam tratamento específico. Dois pacientes tomaram 7mg/kg de nifurtimox durante 60 e 90 dias e 10 usaram 5-7mg/kg de benznidazol durante 60 dias. A evolução clínica foi verificada através de exame clínico, eletrocardiograma, exame radiológico contrastado do esôfago. Após o tratamento somente uma (8,3%) paciente apresentou todos os exames negativos. Oito deles foram avaliados após oito anos do tratamento e 4 acompanhados durante 20 anos. Sete (58,4%) permaneceram na forma indeterminada e 4 (33,3%) chagásicos progrediram clinicamente para cardiopatia grau II e/ou esofagopatia, apesar do tratamento precoce. São necessários estudos com maior número de crianças na fase indeterminada e acompanhamento a longo prazo para se estabelecer a influência do tratamento específico na evolução da doença de Chagas.

Vibrio cholerae has been sporadically isolated from rivers in Tucumán, Argentina, since the outbreak in 1991. The aim of this study was to determine the environmental reservoir of the bacterium in these rivers, assessing the presence of Vibrio cholerae non-O1 and O1 (the latter both in its viable culturable and non culturable state) and its relationship to environmental physicochemical variables. 18 water samplings were collected in the Salí River (in Canal Norte and Banda) and the Lules River between 2003 and 2005. Physical-chemical measurements (pH, water temperature, electrical conductivity and dissolved oxygen) were examined. Vibrio cholerae was investigated with conventional culture methods and with Direct Immunofluorescence (DFA-VNC) in order to detect viable non culturable organisms. All isolated microorganisms corresponded to Vibrio cholerae non-O1 and non-O139 (Lules 26%, Canal Norte 33% and Banda 41%). The majority was found during spring and summer and correlated with temperature and pH. Non culturable Vibrio cholerae O1 was detected year round in 38 of the 54 water samples analyzed. Application of the Pearson correlation coefficient revealed that there was no relationship between positive immunofluorescence results and environmental physicochemical parameters. Genes coding for somatic antigen O1 were confirmed in all DFA-VNC-positive samples, whereas the virulence-associated ctxA and tcpA genes were confirmed in 24 samples.

O vírus da hepatite C é caracterizado pela significativa heterogeneidade genética e é atualmente classificado em seis genótipos principais e diversos subtipos. A determinação do genótipo do vírus tem importância na prática clínica para orientar o tratamento dos pacientes portadores de hepatite C crônica. A prevalência dos diferentes genótipos e subtipos do vírus da hepatite C não tem sido amplamente estudada em algumas regiões do Brasil. Neste estudo foram analisadas 788 amostras de pacientes portadores de hepatite C crônica atendidos nos Centros de Referência em Hepatites Virais de Belo Horizonte, entre 2002 e 2006. A genotipagem do vírus foi realizada por seqüenciamento direto da região 5’ UTR. Adicionalmente, foi realizada análise filogenética incluindo todas as variantes genotípicas obtidas. Observou-se alta prevalência do genótipo 1 (78,4%; 1b [40,4%], 1a [37,5%] e 1a/b [0,5 %]), seguida pelo genótipo 3a (17,9%) e pelo 2b (3,1%). Foram identificadas três amostras (0,4%) com o genótipo 2a/c e duas amostras (0,2%) com o genótipo 4. A análise filogenética mostrou a segregação esperada das seqüências obtidas junto às seqüências de referência para os genótipos 1, 2, 3 e 4, exceto em duas amostras do genótipo 1a. A alta prevalência do genótipo 1 (78,4%), encontrada na população de Belo Horizonte é semelhante à previamente descrita em outras cidades, como Rio de Janeiro, mas superior à encontrada em São Paulo e no Sul do país. A presença de raras seqüências atípicas da região 5’UTR sugere a presença de variantes do vírus da hepatite C nesta população.

INTRODUCTION: Salmonella sp infections have been reported over recent years in hospitals in Argentina and other countries due to multiresistant strains. The aim of this study was to characterize the extended-spectrum β-lactamases in third-generation cephalosporin-resistant strains of Salmonella enterica serovar Oranienburg. METHODS: We studied 60 strains isolated from children with gastroenteritis and/or extraintestinal complications. The antibiotic susceptibility patterns of the isolates were analyzed and the β-lactamases were characterized using phenotyping and genotyping methods. RESULTS: All the strains were resistant to ampicillin, cefotaxime, cefepime and aztreonam and partially susceptible to ceftazidime, thus corresponding well with the resistance phenotype conferred by CTX-M-type β-lactamases. An isoelectric point enzyme (pI = 7.9) was detected in all of the strains, and this was confirmed by PCR as a member of the CTX-M-2 group. CONCLUSIONS: This is the first report of Salmonella enterica serovar Oranienburg producing β-lactamases of the CTX-M-2 group in a pediatric hospital in Tucumán, Argentina.

Paxillus is a neotropical genus belonging to the subfamily Passalinae widespread from Mexico to Argentina. Brazil is known to harbor five species belonging to this genus with most of them presenting at head a flattened central tubercle lacking a free apex. However, one of these species, Paxillus macrocerus Reyes-Castillo & Fonseca, differs from that pattern by having a long central tubercle with a completely free apex. The present study describes and illustrates another new species, which has been found in the Brazilian Amazon, showing this same characteristic.

OBJETIVO: Descrever as características de pacientes (P) com suspeita clínica de síndrome coronariana aguda (SCA), identificando-se o tratamento médico e a mortalidade hospitalar. MÉTODOS: Avaliamos 860 pacientes com SCA de janeiro a dezembro de 2003. Analisamos características basais, modalidade de apresentação da SCA, medicamentos durante a internação, indicação de tratamento clínico ou de revascularização miocárdica (RM) e mortalidade hospitalar. RESULTADOS: Foram 503 (58,3%) pacientes do sexo masculino, com média de idade de 62,6 anos (±11,9). O diagnóstico na alta hospitalar foi de infarto agudo do miocárdio (IAM) com supradesnivelamento do segmento ST (SST) em 78 (9,1%), IAM sem SST em 238 (27,7%), angina instável (AI) em 516 (60%), manifestação atípica da SCA (síncope ou dispnéia) em dois (0,2%) e dor torácica não cardíaca em 26 (3%). Foram medicados com betabloqueador em 87,9%, AAS em 95,9%, anti-trombínico em 89,9%, nitroglicerina EV em 86,2%, inibidor do receptor de glicoproteína (IGP) IIb/IIIa em 6,4%, clopidogrel em 35,9%, inibidor da enzima conversora da angiotensina (IECA) em 77,9% e estatina em 70,9%. A cinecoronariografia foi realizada em 72 pacientes (92,3%) com IAM com SST e em 452 (59,8%) com SCA sem SST (p<0,0001). Indicação de cirurgia de RM ocorreu em 12,9% e intervenção coronariana percutânea em 26,6%. A mortalidade hospitalar foi de 4,8%, sem diferença entre a proporção de óbitos em pacientes com IAM com SST e SCA sem SST (6,4% versus 4,8%; p=0,578). CONCLUSÃO: Por meio deste registro apresentamos uma descrição de pacientes com SCA, avaliando características demográficas, tratamento médico e mortalidade hospitalar. O conhecimento da nossa realidade deve auxiliar para a maior aderência da classe médica às condutas recomendadas.
