Existem barreiras para a efetiva proteção auditiva entre músicos. OBJETIVO: Verificar a aceitação de protetor auditivo pelos componentes de banda instrumental e vocal. MATERIAL E MÉTODO: Estudo prospectivo realizado com a Banda Municipal de Indaial, em 2005. O grupo de estudo consistiu de 34 componentes. Os níveis de pressão sonora foram mensurados durante um ensaio. Os sujeitos responderam questionários e realizaram audiometria tonal. Os limiares tonais dos componentes da banda foram comparados a um grupo controle. Ministrado palestra e distribuído protetores auditivos por 3 meses. RESULTADOS: Os níveis de pressão sonora variaram de 96,4 dB(A) a 106,9 dB(A). As maiores queixas foram: incômodo a sons 58,8% e zumbido 47%. Ao compararmos a mediana dos limiares auditivos dos músicos com o grupo controle observou-se diferença significativa à direita nas freqüências de 4 e 6 kHz, e à esquerda nas freqüências de 3, 4 e 6 kHz. 77,1% referiram que a música pode ocasionar prejuízo auditivo. 56,2% referiram não ter gostado do protetor, 43,7% referiram ter gostado. CONCLUSÃO: Os sujeitos têm a informação sobre o risco, mas não há prevenção em relação aos efeitos auditivos, sugerindo a necessidade de campanhas periódicas e legislação específica aos profissionais ligados à música.

A ossificação da cóclea, decorrente principalmente de meningite, impede a inserção completa do implante coclear convencional. Os implantes com 2 feixes de eletrodos mais curtos do que o convencional foram desenvolvidos especialmente para cócleas ossificadas. Porém, durante essa cirurgia há um grande risco de lesão da artéria carótida interna (ACI). Portanto, a medida da profundidade das cocleostomias para inserir os dois feixes de eletrodos aumentaria a segurança desse procedimento. OBJETIVOS: 1) Obter as distâncias entre as cocleostomias e a ACI em ossos temporais de cadáver; 2) Confeccionar instrumento que possa ser usado na cirurgia de implante coclear com 2 feixes de eletrodos. FORMA DE ESTUDO: Experimental prospectivo. MATERIAL E MÉTODO: Em 21 ossos de cadáveres foi realizada: 1) mastoidectomia cavidade aberta; 2) cocleostomias nos giros basal e médio da cóclea; 3) identificação da ACI; 4) medida da distância entre as cocleostomias e a artéria. RESULTADOS: A medida média ± desvio padrão obtida para o túnel superior foi 8,2 ± 1,1mm e para o túnel inferior foi 8,1± 1,3mm. A menor distância encontrada foi 6,5mm para o túnel superior e 6,0mm para o túnel inferior. CONCLUSÃO: Apesar dos parâmetros calculados concluímos que a melhor medida para ser considerada na confecção do instrumento cirúrgico serão as mínimas medidas obtidas em cada um dos giros cocleares, pois é a maneira mais segura para evitar a lesão da ACI, que pode ser fatal.

The article analyses how Brazilian state actions and policies regarding peace operations during the Presidency of Lula da Silva relate to the country's positions and attitudes towards United Nations peacekeeping. It argues that the inconsistencies identified on the Brazilian positions reflect the lack of a clear strategic horizon guiding the country's participation in UN peacekeeping, which consequentially hinders the country emergence as a great power.

OBJECTIVE To analyze gender differences in the incidence and determinants of disability regarding instrumental activities of daily living among older adults. METHODS The data were extracted from the Saúde, Bem-Estar e Envelhecimento (SABE – Health, Wellbeing and Ageing) study. In 2000, 1,034 older adults without difficulty in regarding instrumental activities of daily living were selected. The following characteristics were evaluated at the baseline: sociodemographic and behavioral variables, health status, falls, fractures, hospitalizations, depressive symptoms, cognition, strength, mobility, balance and perception of vision and hearing. Instrumental activities of daily living such as shopping and managing own money and medication, using transportation and using the telephone were reassessed in 2006, with incident cases of disability considered as the outcome. RESULTS The incidence density of disability in instrumental activities of daily living was 44.7/1,000 person/years for women and 25.2/1,000 person/years for men. The incidence rate ratio between women and men was 1.77 (95%CI 1.75;1.80). After controlling for socioeconomic status and clinical conditions, the incidence rate ratio was 1.81 (95%CI 1.77;1.84), demonstrating that women with chronic disease and greater social vulnerability have a greater incidence density of disability in instrumental activities of daily living. The following were determinants of the incidence of disability: age ≥ 80 and worse perception of hearing in both genders; stroke in men; and being aged 70 to 79 in women. Better cognitive performance was a protective factor in both genders and better balance was a protective factor in women. CONCLUSIONS The higher incidence density of disability in older women remained even after controlling for adverse social and clinical conditions. In addition to age, poorer cognitive performance and conditions that adversely affect communication disable both genders. Acute events, such as a stroke, disables elderly men more, whereas early deficits regarding balance disable women more.

ABSTRACT INTRODUCTION: Human metapneumovirus (hMPV) is an emergent human respiratory pathogen. This study aimed to evaluate the performance of direct immunofluorescence (DIF) to detect hMPV in a clinical laboratory setting. METHODS: Nasopharyngeal aspirate samples (448) of children and adults with respiratory illness were used to detect hMPV by using DIF and real time quantitative reverse transcription-polymerase chain reaction (qRT-PCR) assays. RESULTS: In all, 36 (8%) samples were positive by DIF and 94 (21%) were positive by qRT-PCR. Direct immunofluorescence specificity was 99% and sensitivity was 38%. CONCLUSIONS: DIF is not very sensitive under clinical laboratory settings.

No presente trabalho são apresentados valores de concentração para os elementos minerais Ca, Fe, Κ, Mn, Na, Se e Zn em alimentos convencionais e não convencionais da região Amazônica, obtidos por análise por ativação neutrônica instrumental. As concentrações dos elementos minerais nos alimentos analisados, variaram, sendo que o potássio se sobressaiu com teores expressivos na jaca (caroço e polpa) e graviola. Dentre os minerais, o selênio foi o que apresentou menor concentração nos alimentos analisados. Entretanto, notadamente importantes foram as concentrações verificadas na graviola, pupunha fruto e palmito. De acordo com os resultados obtidos sugere-se a indicação dos mesmos na alimentação diária dos amazonenses e espera-se contribuir na elaboração de tabelas de composição de alimentos regional e nacional.

Passage of malaria infected blood through a two-layered column composed of acid-washed glass beads and CF 11 cellulose removes white cells from parasitized blood. However, because use of glass beads and CF 11 cellulose requires filtration of infected blood separately through these two resins and the addition of ADP, the procedure is time-consuming and may be inapropriate for use in the field, especially when large numbers of blood samples are to be treated. Our modification of this process yields parasitized cells free of contaminating leukocytes, and because of its operational simplicity, large numbers of blood samples can be processed. Our procedure also compares well with those using expensive commercial Sepacell resins in its ability to separate leukocytes from whole blood. As a test of usefulness in molecular biologic investigations, the parasites obtained from the blood of malaria-infected patients using the modified procedure yield genomic DNA whose single copy gene, the circumsporozite gene, efficiently amplifies by polymerase chain reaction.

Vancomycin-resistant enterococci (VRE) have recently emerged as a nosocomial pathogen and present an increasing threat to the treatment of severely ill patients in intensive-care hospital settings. We outline results of a study of the epidemiology of VRE transmission in ICUs and define a reproductive number R0; the number of secondary colonization cases induced by a single VRE-colonized patient in a VRE-free ICU, for VRE transmission. For VRE to become endemic requires R0 >1. We estimate that in the absence of infection control measures R0 lies in the range 3-4 in defined ICU settings. Once infection control measures are included R0=0.6, suggesting that admission of VRE-colonized patients can stabilize endemic VRE.

Concerns have been raised that universal availability of antiretroviral agents in resource-limited settings might lead to the emergence and spread of resistant strains. We present the largest survey on human immunodeficiency virus type 1 (HIV-1) resistance among treatment-naïve and experienced patients followed in small, relatively underprivileged cities in Brazil with universal availability to standard of care antiretroviral combinations. Samples were collected between 2004 and 2006 from 95 patients followed in the cities of Saquarema and Santo Antonio de Pádua, state of Rio de Janeiro. A proviral fragment encompassing protease and reverse transcriptase (RT) regions was generated and drug susceptibility level was inferred. Among 50 strains from drug-naïve subjects, one (2%) had intermediate-level resistance to RT inhibitors. Among 38 patients on therapy as of sampling, 28 (73.7%) had plasma viral load (PVL) below detection limit (26 of whom without evidence of resistance mutations) and 11 (28.9%) harbored strains with reduced susceptibility. Only two strains harbored both protease and RT inhibitor mutations. Among seven patients who were off-treatment as of sampling, two (28.5%) harbored strains with reduced susceptibility to RT inhibitors. The relatively high frequency of undetectable PVL among patients on treatment and the overall low prevalence of resistance-associated mutations are reassuring. Continued surveillance, however, is necessary.

Experimental animal models offer possibilities of physiology knowledge, pathogenesis of disease and action of drugs that are directly related to quality nursing care. This integrative review describes the current state of the instrumental and ethical aspects of experimental research with animal models, including the main recommendations of ethics committees that focus on animal welfare and raises questions about the impact of their findings in nursing care. Data show that, in Brazil, the progress in ethics for the use of animals for scientific purposes was consolidated with Law No. 11.794/2008 establishing ethical procedures, attending health, genetic and experimental parameters. The application of ethics in handling of animals for scientific and educational purposes and obtaining consistent and quality data brings unquestionable contributions to the nurse, as they offer subsidies to relate pathophysiological mechanisms and the clinical aspect on the patient.

ABSTRACT Objective analyze how studies have approached the results obtained from the application of the Nursing Activities Score (NAS) based on Donabedian’s model of healthcare organization and delivery. Method CINAHL and PubMed databases were searched for papers published between 2003 and March 2015. Results 36 articles that met the inclusion criteria were reviewed and double-coded by three independent coders and analyzed based on the three elements of Donabedian’s health care quality framework: structure, process and outcome. The most frequently addressed, but not always tested, variables were those that fell into the structure category. Conclusion variables that fell into the process category were used less frequently. Beside NAS, the most frequently used variables in the outcome category were mortality and length of stay. However, no study used a quality framework for healthcare or NAS to evaluate costs, and it is recommended that further research should explore this approach.

A técnica analítica nuclear de ativação neutrônica é multielementar, precisa, exata e, além de outras características, tem sido indicada como técnica primária na determinação elementar principalmente de solo. Entretanto, ao ser aplicada, não está isenta de interferências inerentes à técnica, como a ocorrência de reações de interferências primárias, cuja contribuição depende das características do fluxo de nêutrons do reator, do local de irradiação, etc. Essas reações ocorrem durante a irradiação, quando a amostra é submetida ao fluxo de nêutrons no reator no qual há contribuição de nêutrons de diversas energias. Uma interferência que pode ser significativa é a determinação de Mn em presença de Fe, pois a formação extra do 56Mn poderá não permitir afirmar se a concentração de Mn determinada deve-se à interferência do Fe ou não. Para verificar se essa interferência é significativa, ao analisar amostras com elevada concentração de Fe no reator TRIGA MARK I IPR-R1 do CDTN/CNEN, foi realizado este estudo, visando determinar o Mn em solo do Quadrilátero Ferrífero, Minas Gerais, Brasil, no qual há ocorrência de minérios com teores da ordem de 40 a 60 % em Fe. Os resultados mostraram que na reação de 56Fe com nêutrons rápidos a formação de 56Mn é pouco significativa, não havendo necessidade de aplicar a correção da interferência.

Simple experiments are proposed for measuring molecular absorption of chromate and dichromate ions using an atomic absorption spectrometer. The experiments can help undergraduate students in instrumental analysis courses understand important aspects involving conceptual and instrumental similarities and differences between frequently used analytical techniques. Hollow cathode lamps were selected with wavelengths in the region of molecular absorption of chromate and dichromate. Calibration curves were obtained and the linear dynamic range was evaluated. Results were compared with those obtained in a molecular absorption spectrometer. The molar absorptivities obtained were also compared.

In general, laboratory activities are costly in terms of time, space, and money. As such, the ability to provide realistically simulated laboratory data that enables students to practice data analysis techniques as a complementary activity would be expected to reduce these costs while opening up very interesting possibilities. In the present work, a novel methodology is presented for design of analytical chemistry instrumental analysis exercises that can be automatically personalized for each student and the results evaluated immediately. The proposed system provides each student with a different set of experimental data generated randomly while satisfying a set of constraints, rather than using data obtained from actual laboratory work. This allows the instructor to provide students with a set of practical problems to complement their regular laboratory work along with the corresponding feedback provided by the system's automatic evaluation process. To this end, the Goodle Grading Management System (GMS), an innovative web-based educational tool for automating the collection and assessment of practical exercises for engineering and scientific courses, was developed. The proposed methodology takes full advantage of the Goodle GMS fusion code architecture. The design of a particular exercise is provided ad hoc by the instructor and requires basic Matlab knowledge. The system has been employed with satisfactory results in several university courses. To demonstrate the automatic evaluation process, three exercises are presented in detail. The first exercise involves a linear regression analysis of data and the calculation of the quality parameters of an instrumental analysis method. The second and third exercises address two different comparison tests, a comparison test of the mean and a t-paired test.
