OBJETIVO: Analisar o perfil epidemiológico da Aids nos municípios brasileiros entre 2002 e 2006, associando tendência e magnitude com indicadores socio- demográficos e características da epidemia local. MÃTODOS: Foi conduzido um estudo ecológico que categorizou os municípios segundo a magnitude e tendência da epidemia para, posteriormente, analisá-los de acordo com os indicadores sociais, formas de transmissão do HIV e ano de registro do primeiro caso. Os dados são provenientes do Sistema Nacional de Vigilância Epidemiológica, Instituto Brasileiro de Geografia e Estatística e Programa das Nações Unidas para o Desenvolvimento no Brasil. Utilizou-se regressão linear para estimar a tendência e estatísticas de qui-quadrado e Anova para o estudo dos indicadores. RESULTADOS: Um total de 4.190 municípios (75,3%) apresentou casos entre 2002 e 2006. Desses, 3.403 (81,2%) possuíam ocorrência de "pequena magnitude" (média = 4,7 casos), 367 (8,8%) "média magnitude" (média = 30,3 casos) e 420 (10,0%) "grande magnitude" (média = 378,7 casos). Os de "pequena magnitude" associaram-se à menor incidência, início da epidemia após 1991, existência de uma ou duas categorias de transmissão, especialmente heterossexual, com ocorrências de casos em um ou dois anos do período e menor índice de desenvolvimento humano (IDH). Os de "grande magnitude" associaram-se às cidades de maior porte e IDH, apresentaram todas as categorias de transmissão, início da epidemia entre 1980/1991 e tendência de redução/estabilização, especialmente por diminuição da transmissão entre usuários de drogas injetáveis. O crescimento da epidemia concentrou-se em cidades de "pequena magnitude", mas sem significância, a ponto de alterar a participação proporcional (8,7%) desses municípios no conjunto de casos no País. CONCLUSÃES: A epidemia de Aids permanece concentrada nos centros urbanos e a interiorização é caracterizada pela ocorrência irregular e de pequena magnitude. Municípios com baixo IDH e com transmissão exclusivamente por relações heterossexuais apresentaram baixa capacidade de crescimento e a redução da epidemia está associada especialmente à diminuição da transmissão entre usuários de drogas injetáveis.

The objective of the present paper was to compare accessibility and utilisation of schistosomiasis diagnostic and treatment services in a small village and the surrounding rural area in northern part of the state of Minas Gerais Brazil. The study included 1,228 individuals: 935 central village residents and 293 rural residents of São Pedro do Jequitinhonha. Schistosoma mansoni infection rates were significantly higher in the central village than in the rural area during a survey in 2007 (44.3% and 23.5%, respectively) and during the 2002 schistosomiasis case-finding campaign (33.1% and 26.5%, respectively) (p < 0.001). However, during the 2002-2006 period, only 23.7% of the villagers and 27% of the rural residents obtained tests on their own from health centres, hospitals and private clinics in various nearby towns. In 2007, 63% of the villagers and 70.5% of the rural residents reported never having received treatment for schistosomiasis. This paper reveals considerable variation in the accessibility and utilisation of schistosomiasis-related health services between the central village and the rural area. A combination of low utilisation rates between 2002-2006 and persistently high S. mansoni infection rates suggest that the schistosomiasis control program must be more rapidly incorporated into the primary health services.

Leptospirosis is a zoonotic disease affecting most mammals and is distributed throughout the world. Several species of domestic and wild animals may act as reservoirs for this disease. The purpose of this study was to assess the exposure of free-ranging wild carnivores, horses and domestic dogs on a private reserve located in the northern Pantanal (Brazil) and the surrounding areas to Leptospira spp from 2002-2006, 75 free-ranging wild carnivores were captured in the Pantanal and serum samples were collected. In addition, samples from 103 domestic dogs and 23 horses in the region were collected. Serum samples were tested for the presence of Leptospira antibodies using the microscopic agglutination test. Thirty-two wild carnivores (42.7%) were considered positive with titres > 100, and 18 domestic dogs (17.5%) and 20 horses (74.1%) were also found to be positive. Our study showed that horses, dogs and several species of free-ranging wild carnivores have been exposed to Leptospira spp in the Pantanal, suggesting that the peculiar characteristics of this biome, such as high temperatures and an extended period of flooding, may favour bacterial persistence and transmission. In this region, wild carnivores and horses seem to be important hosts for the epidemiology of Leptospira species.

In the last 30 years world population has increased 70% but per capita global fruit consumption is only 20% higher. Even though tropical and temperate fruit have similar contributions to the 50 kg/person/year of US consumption of fresh fruit, in the last 30 years this has been slightly greater for temperate fruit. Within fruit consumption, the largest expansion has been for organic fruit which increased more than 50% in the 2002-2006 period. The largest expansion of area planted in the 1996-2006 has been for kiwi (29%) and blueberries (20%), while apples (-24%) and sour cherries (-13%) have had the largest reductions. Nearly 50% of the total global volume of fruit is produced by 5 countries: China, USA, Brazil, Italy and Spain. The main producer (China) accounts for 23% of the total. While the main exporters are Spain, USA and Italy, the main importers are Germany, Russia and UK. Demands for the industry have evolved towards quality, food safety and traceability. The industry faces higher productions costs (labor, energy, agrichemicals). The retailers are moving towards consolidation while the customers are changing preferences (food for health). In this context there is greater pressure on growers, processors and retailers. Emerging issues are labor supply, climate change, water availability and sustainability. Recent developments in precision agriculture, molecular biology, phenomics, crop modelling and post harvest physiology should increase yields and quality, and reduce costs for temperate fruit production around the world.

This paper presents an overview of the Natural Products Research in Brazil in the last five years (2002-2006), and also discusses how some relevant aspects of the Chemical Biology area could create new research opportunities and challenges for the natural product chemists. In addition, some aspects of the scientific policies and their impact on current projects are discussed.

OBJETIVO: Determinar perfil clínico-nutricional de pacientes obesos submetidos à cirurgia bariátrica, no HC/UFPE. MÃTODOS: Foram avaliados retrospectivamente, 205 pacientes, no período 2002/2006. A análise considerou história clínica para diabetes tipo 2 (DM 2), hipertensão arterial (HA) e síndrome metabólica (SM). O estado nutricional pré-operatório foi avaliado pelo IMC e bioquímica (hemoglobina, hematócrito, albumina, proteínas totais, triglicérides (TG), colesterol associado à lipoproteína de alta (HDLc) e baixa (LDLc) densidade e glicemia de jejum (GJ). Nos períodos pós-operatórios (6, 12, 18, 24 meses) a avaliação nutricional foi feita pelas medidas de peso, perda ponderal, percentual de perda de peso (%PP), IMC e bioquímica incluindo ferro, ferritina e transferrina. RESULTADOS: 71,2% eram do sexo feminino, idade de 38,4 ± 9,96 anos, 129,66±27,40 Kg e IMC 48,6 ± 8,9 Kg/m², no pré-operatório. Receberam o diagnóstico de SM 26,8%, HA 52,7% e DM 2 11,7%. A bioquímica revelou TG, LDLc, GJ elevados, estando normais os demais parâmetros. Evolução antropométrica demonstrou perda ponderal progressiva, atingindo aos 24 meses IMC 31,7±5,82 Kg/m² (p< 0,001) e maior %PP (36,05%). Valores de TG, LDLc e GJ atingiram a normalidade a partir do 6° mês pós-operatório: 104,4mg/dL(p=0,018), 95,5mg/dL(p=0,263) e 84,8g/dL(p=0,004), respectivamente; transferrina apresentou valores reduzidos aos 6 meses. Prevalência maior dos sintomas ocorreu no 6° mês: alopécia (19%), vômitos (18%), intolerância alimentar (12,2%). CONCLUSÃO: A Cirurgia bariátrica foi um procedimento eficaz para promover perda ponderal e sua manutenção por dois anos, assim como melhora de parâmetros bioquímicos e co-morbidades, com sintomas clínico-nutricionais reduzidos e/ou evitados por monitorização nutricional.

Giardia intestinalis infection is prevalent throughout the world and widely distributed in developing countries. In general, children display serious consequences to their state of health, including slow height-weight development; therefore, the main aim of this study was to determine the association between Giardia infection and the nutritional status of children who participate in the program of complementary feeding (Mejoramiento Alimentario y Nutricional de Antioquia (MANA) - Instituto Colombiano de Bienestar Familiar (ICBF)). A cross-sectional study examining the association of giardiasis with nutritional status was conducted. A total of 2035 children aged eight months to six years-old were studied. Data were collected using structured questionnaires, anthropometric measurements and laboratory analysis of blood and stool samples. Analysis of the results showed that 27.6% of children were infected with G. intestinalis, while 8.1% and 1.9% were mildly and significantly underweight, respectively, and 14.1% presented stunting. Giardiasis was statistically identified as a strong predictor of stunting in this study population.

The aim of the present study was to evaluate the data on human exposure to potential rabies virus transmitters in Olinda, State of Pernambuco, Brazil. Data from 7,062 patients who underwent antirabies prophylactic treatment in Olinda between 2002 and 2006 were analyzed. As expected, dogs and cats were involved in most of the cases; i.e. 82.3 and 16.3%, respectively. Attacks by nonhuman primates, bats and other species (unspecified) were also reported. Among the 7,062 patients who underwent antirabies treatment, 582 patients abandoned the treatment, either by indication from the health unit (195) or by their own decision (387). In conclusion, this study has indicated that prophylaxis for human rabies in this urban area will require a multifaceted approach, including health education, post-exposure prophylaxis, systematic vaccination for dogs and cats, and possibly selective control over wild animals such as hematophagous bats.

The presence of Lutzomyia (Helcocyrtomyia) hartmanni, as a vector of Leishmania colombiensis and L. columbiana (Verrucarum group), recently incriminated in the transmission of leishmaniasis, and L. pia (Verrucarum group) are reported for the first time in a periurban area of Medellín city. There is thus a risk of leishmaniasis transmission in this town.

Although once associated only with rural areas, the American leishmaniasis vectors now appear to be associated also with urban and suburban areas of the Neotropics. Following the appearance of the first autochthonous visceral and cutaneous leishmaniasis cases in the urban area of the city of Sincelejo, Colombia, a preliminary entomological survey of the sand fly species composition was performed using Shannon and CDC light traps. A total of 486 sand flies representing six Lutzomyia species were collected. L. evansi, L. panamensis and L. gomezi, known vectors of Leishmania spp. were the predominant sand fly species around dwellings. The finding of these species in relation to the appearance of the first cases of leishmaniasis in the city mentioned is discussed.

Objetivou-se, nesta pesquisa, estudar a ocorrência natural de ácaros fitófagos e predadores em diferentes cultivares de pessegueiro, no município de Presidente Prudente-SP, Brasil. O estudo foi realizado no período de dezembro de 2002 a fevereiro de 2006. Amostras quinzenais de 72 folhas foram coletadas ao acaso, de pessegueiros das cultivares Talismã, Doçura 2, Dourado 2, Tropical, Aurora 1 e Aurora 2. Coletou-se um total de 2.594 ácaros, sendo 2.092 fitófagos, 403 predadores e 99 de hábitos alimentares pouco conhecidos, com 35 espécies de ácaros de 16 famílias. Aculus fockeui ocorreu de maneira esporádica, não causando danos visíveis às plantas. A família Phytoseiidae apresentou a maior abundância e o maior número de indivíduos. O predador Euseius citrifolius foi o mais abundante. Não houve preferência dos ácaros nas cultivares de pessegueiro avaliadas.

Introduction: Tuberculosis is a common opportunistic infection in renal transplant patients. Objective: To obtain a clinical and laboratory description of transplant patients diagnosed with tuberculosis and their response to treatment during a period ranging from 2005 to 2013 at the Pablo Tob&#243;n Uribe Hospital. Methods: Retrospective and descriptive study. Results: In 641 renal transplants, tuberculosis was confirmed in 12 cases. Of these, 25% had a history of acute rejection, and 50% had creatinine levels greater than 1.5 mg/dl prior to infection. The disease typically presented as pulmonary (50%) and disseminated (33.3%). The first phase of treatment consisted of 3 months of HZRE (isoniazid, pyrazinamide, rifampicin and ethambutol) in 75% of the cases and HZME (isoniazid, pyrazinamide, moxifloxacin and ethambutol) in 25% of the cases. During the second phase of the treatment, 75% of the cases received isoniazid and rifampicin, and 25% of the cases received isoniazid and ethambutol. The length of treatment varied between 6 and 18 months. In 41.7% of patients, hepatotoxicity was associated with the beginning of anti-tuberculosis therapy. During a year-long follow-up, renal function remained stable, and the mortality rate was 16.7%. Conclusion: Tuberculosis in the renal transplant population studied caused diverse nonspecific symptoms. Pulmonary and disseminated tuberculosis were the most frequent forms and required prolonged treatment. Antituberculosis medications had a high toxicity and mortality. This infection must be considered when patients present with a febrile syndrome of unknown origin, especially during the first year after renal transplant.
