A otospongiose é uma osteodistrofia do osso temporal caracterizada pela reabsorção e neoformação óssea desordenada. Clinicamente, caracteriza-se por disacusia condutiva, neurossensorial, e/ou mista, progressiva e zumbidos. O início dos sintomas ocorre entre 30 e 40 anos de idade sendo rara sua manifestação na infância. Descrevemos o caso de um paciente de 11 anos de idade, com quadro de hipoacusia unilateral progressiva há 5 anos. O exame otorrinolaringológico revelou mancha rubra de Schwartze em orelha esquerda. A audiometria, imitanciometria e a tomografia computadorizada demonstraram características sugestivas de otospongiose. Realizamos uma revisão dos aspectos clínicos, diagnósticos e da conduta terapêutica da otospongiose na infância.

El artículo estudia el traslado del Congreso anfictiónico de Panamá a Tacubaya y los diversos intentos de instalación de la 'Asamblea americana' en México entre 1826 y 1828. Su propósito es escalonado: primero identifica los significados decisionales relevantes para comprender el fracaso del proyecto y, enseguida, delimita la autonomía analítica del episodio mexicano del Congreso de Bolívar.

This article examines the formative platform of the Congress of Panama of 1826. It seeks to support the hypothesis that the nature and scope of the first test of integration in the Western Hemisphere depended critically on the platform created by Simón Bolívar and other Latin American Independence heroes from the Declaration of Independence of Venezuela in 1810 until the last bilateral agreement of 1826. In that respect, it corroborates the Latin American Identity of the initiative.

There are complex and diverse methodological problems involved in the clinical and epidemiological study of respiratory diseases and their etiological factors. The association of urban growth, industrialization and environmental deterioration with respiratory diseases makes it necessary to pay more attention to this research area with a multidisciplinary approach. Appropriate study designs and statistical techniques to analyze and improve our understanding of the pathological events and their causes must be implemented to reduce the growing morbidity and mortality through better preventive actions and health programs. The objective of the article is to review the most common methodological problems in this research area and to present the most available statistical tools used.

Two patients from the gold mines of Bolivar State, Venezuela, presenting cutaneous leishmaniasis in the genital region, an unusual location, are described. The first patient showed an ulcerated lesion of the glans penis. Leishmanin skin test was positive. A biopsy specimen revealed a granulomatous infiltrate containing Leishmania parasites. In the second patient, Leishmanin skin test was positive, HIV and VDRL were negative. Leishmania parasites were present in a biopsy of an ulcerated lesion in the scrotum, with an indurated base, infiltrative borders with an yellowish exudate. Patients were treated with meglumine antimoniate and the lesions healed.

FUNDAMENTO: A presença de anemia e de disfunção renal confere mau prognóstico em pacientes com insuficiência cardíaca (IC) e fração de ejeção reduzida (ICFER). O impacto em pacientes com IC e fração de ejeção normal (ICFEN) é pouco estudado. OBJETIVOS: Estudar a prevalência e o prognóstico da anemia e da disfunção renal (DR) em pacientes com IC de acordo com o tipo de disfunção ventricular. MÉTODOS: Foram estudados prospectivamente 209 pacientes com IC crônica estável. Pacientes com fração de ejeção <50 % foram considerados como tendo ICFER. Anemia foi definida pelos critérios da OMS como hemoglobina <13 g/dl em homens e <12 g/dl em mulheres. A função renal foi calculada pela fórmula sMDRD (Simplified Modified Diet Renal Disease). Hospitalizações, visitas a emergências e óbitos por causas cardíacas foram considerados como eventos cardíacos. RESULTADOS: Noventa pacientes tinham ICFER e 119, ICFEN. A taxa de filtração glomerular (TFG) foi menor no grupo com ICFER (57,6 ± 66,2 versus 94,8 ± 36,6 ml/min/1,73m²; p=0,01). Não houve diferença na prevalência da anemia nos dois grupos (23,3% versus 18,5%; p=0,34). A prevalência da DR moderada a grave foi maior no grupo com ICFER (32,2% versus 16,8% p=0,01). A DR foi o único fator associado à anemia e associou-se independentemente com eventos cardíacos (HR 2,52; IC 95% = 1,27 - 5,2; p=0,01). CONCLUSÃO: DR foi menos prevalente na ICFEN, enquanto a prevalência de anemia não diferiu entre os dois grupos. A DR foi preditora de eventos cardíacos, independentemente da fração de ejeção.

The differences between Pseudodynerus auratoides (Bertoni, 1918) and Pseudodynerus serratus (Fox, 1902) are established. Lectotypes of Stenodynerus mondaiensis Bertoni, 1918 and Stenodynerus auratoides Bertoni, 1918 are designated. Pseudodynerus penicillatus (Zavattari, 1912) is redescribed and newly raised from synonymy under P. serratus (Fox, 1902).

The plebotomine sand fly fauna of Ecuador was surveyed in two 3-month collecting trips made in 1988 and 1990. A total of 12 provinces were visited, including three (Bolivar, Loja and Morona Santiago) from wich no previous records to phlebotomines existed. Forty-six species were collected, 13 of wich, together with 1 subspecies and 1 genus (Warileya) represented new records for the country. This survey increases the known number of species in Ecuador to 60. The distribuition of Ecuadorian sand flies is discussed in the light of these new findings.

Fluorescent (acridine orange) microscopical examination of capillary centrifuged blood (quantitative buffy coat [QBC®] analysis) and Giemsa stained thick blood smears (GTS) were compared for diagnosis of malaria in blood specimens from adults living in malaria transmission areas of the States of Bolivar and Amazonas in southeastern and south Venezuela, respectively. Of a total of 198 GTS examined, 95 subjects (48%) showed parasitaemia. Among the 95 blood films with a positive GTS, 94 were judged positive by the QBC. However, positive QBC tubes were found in 29 out of 103 blood specimens with a negative GTS. Thus, relative to a GTS standard, the sensitivity and specificity of the QBC-test was 99.2% and 72%, respectively. Young trophozoites of Plasmodium vivax and P. falciparum could not be distinguished with certainty. It is confirmed that the QBC offers many advantages compared with the standard diagnosis of malaria parasites, specifically in the speed of staining and ease of interpretation. However, in places where P. falciparum and P. vivax occur, species and stage differentiation should be confirmed with the GTS.

Calophyllum brasiliense and Mammea americana (Clusiaceae) are two trees from the tropical rain forests of the American continent. A previous screening showed high trypanocidal activity in the extracts of these species. Several mammea-type coumarins, triterpenoids and biflavonoids were isolated from the leaves of C. brasiliense. Mammea A/AA was obtained from the fruit peels of M. americana. These compounds were tested in vitro against epimastigotes and trypomastigotes of Trypanosoma cruzi, the etiologic agent of Chagas disease. The most potent compounds were mammea A/BA, A/BB, A/AA, A/BD and B/BA, with MC100 values in the range of 15 to 90 g/ml. Coumarins with a cyclized ,-dimethylallyl substituent on C-6, such as mammea B/BA, cyclo F + B/BB cyclo F, and isomammeigin, showed MC100 values > 200 g/ml. Several active coumarins were also tested against normal human lymphocytes in vitro, which showed that mammea A/AA and A/BA were not toxic. Other compounds from C. brasiliense, such as the triterpenoids, friedelin, canophyllol, the biflavonoid amentoflavone, and protocatechuic and shikimic acids, were inactive against the epimastigotes. The isopropylidenedioxy derivative of shikimic acid was inactive, and its structure was confirmed by X-ray diffraction. Our results suggest that mammea-type coumarins could be a valuable source of trypanocidal compounds.

We prospectively sampled flavivirus-naïve horses in northern Colombia to detect West Nile virus (WNV) and St. Louis encephalitis virus (SLEV) seroconversion events, which would indicate the current circulation of these viruses. Overall, 331 (34.1%) of the 971 horses screened were positive for past infection with flaviviruses upon initial sampling in July 2006. During the 12-month study from July 2006-June 2007, 33 WNV seroconversions and 14 SLEV seroconversions were detected, most of which occurred in the department of Bolivar. The seroconversion rates of horses in Bolivar for the period of March-June 2007 reached 12.4% for WNV and 6.7% for SLEV. These results comprise the first serologic evidence of SLEV circulation in Colombia. None of the horses sampled developed symptoms of encephalitis within three years of initial sampling. Using seroconversions in sentinel horses, we demonstrated an active circulation of WNV and SLEV in northern Colombia, particularly in the department of Bolivar. The absence of WNV-attributed equine or human disease in Colombia and elsewhere in the Caribbean Basin remains a topic of debate and speculation.
