Resumo:
Setecentos e oitenta pacientes com recidivas clínicas de infecções conjuntivais ou queixas persistentes de prurido, ardor, dor, lacrimejamento e hiperemia, foram submetidos ao raspado conjuntival com citologia e pesquisa de inclusões. Destes 247(31,7%) apresentaram inclusões citoplasmáticas; 235 pacientes foram, também, submetidos a coloração com anticorpo monoclonal fluorescente (imunofluorescência direta - DFA) com 90 (38,3%) casos positivos. Discute-se também, a importância do quadro clinico e dos métodos laboratoriais (coloração com anticorpo monoclonal fluorescente e pesquisa de inclusões citoplasmáticas) no diagnóstico. Os pacientes fazem parte de clientela de clínica particular e tem nível sócio-econômico de médio a alto.

OBJETIVO: Avaliar os resultados da tomografia computadorizada por múltiplos detectores na avaliação dos resultados tardios de pacientes submetidos ao implante de endopróteses com sirolimus. MÉTODOS: Selecionamos 30 pacientes, previamente submetidos ao implante de stents com sirolimus com sucesso e com mais de seis meses de evolução. Todos foram submetidos à angiografia invasiva e ao ultra-som intravascular após a angiotomografia, feita com a injeção de 1,5 ml/kg de peso de meio de contraste iodado. RESULTADOS: A média dos diâmetros proximais de referência foi 3,01 ± 0,31 mm pela tomografia e 3,14 ± 0,31 mm pela angiografia (p = 0,04). Ao eliminarmos a artéria circunflexa da análise, a discrepância entre os dois exames deixou de ser significante -(tomografia= 3,01 ± 0,32 mm, angiografia= 3,10 ± 0,30 mm, p = 0,65). A média dos diâmetros distais de referência foi 2,86 ± 0,30 mm pela tomografia e 2,92 ± 0,32 pela angiografia (p = 0,25). A média do calibre mínimo no interior da endoprótese foi 2,85 ± 0,25 mm pela tomografia e 2,85 ± 0,29 mm angiografia (p = 0,27). A área de secção transversal mínima intra-stent foi 7,19 ± 1,47 mm² pela tomografia e 6,90 ± 1,52 mm² pelo ultra-som intracoronariano (p = 0,36), mas a correlação entre estas medidas era fraca (r= 0,33). CONCLUSÃO: A tomografia possibilita a avaliação qualitativa das endopróteses, a estimativa correta do diâmetro de referência proximal e distal dos vasos-alvo, além do calibre mínimo intra-stent. Sua correlação com as medidas feitas pelo ultra-som intracoronário, porém é menos intensa.

Heart rate variability is a relevant predictor of cardiovascular risk in humans. A significant genetic influence on heart rate variability is suggested, although the genes involved are ill-defined. The Mas-protooncogene encodes a G-protein-coupled receptor with seven transmembrane domains highly expressed in testis and brain. Since this receptor is supposed to interact with the signaling of angiotensin II, which is an important regulator of cardiovascular homeostasis, heart rate and blood pressure were analyzed in Mas-deficient mice. Using a femoral catheter the blood pressure of mice was measured for a period of 30 min and 250 data values per second were recorded. The mean values and range of heart rate and blood pressure were then calculated. Neither heart rate nor blood pressure were significantly different between knockout mice and controls. However, high resolution recording of these parameters and analysis of the data by non-linear dynamics revealed significant alterations in cardiovascular variability in Mas-deficient animals. In particular, females showed a strong reduction of heart rate variability. Furthermore, the data showed an increased sympathetic tone in knockout animals of both genders. The marked alterations detected in Mas-deficient mice of both genders suggest that the Mas-protooncogene is an important determinant of heart rate and blood pressure variability.

It is well known that the responses to vasoactive kinin peptides are mediated through the activation of two receptors termed bradykinin receptor B1 (B1R) and B2 (B2R). The physiologically prominent B2R subtype has certainly been the subject of more intensive efforts in structure-function studies and physiological investigations. However, the B1R activated by a class of kinin metabolites has emerged as an important subject of investigation within the study of the kallikrein-kinin system (KKS). Its inducible character under stress and tissue injury is therefore a field of major interest. Although the KKS has been associated with cardiovascular regulation since its discovery at the beginning of the last century, less is known about the B1R and B2R regulation in cardiovascular diseases like hypertension, myocardial infarction (MI) and their complications. This mini-review will summarize our findings on B1R and B2R regulation after induction of MI using a rat model. We will develop the hypothesis that differences in the expression of these receptors may be associated with a dual pathway of the KKS in the complex mechanisms of myocardial remodeling.

Pheochromocytomas are rare chromaffin cell tumors that nevertheless must be excluded in large numbers of patients who develop sustained or episodic hypertension as well as in many others with suggestive symptoms or with a familial history of pheochromocytoma. Diagnosis of pheochromocytoma depends importantly on biochemical evidence of excess catecholamine production by a tumor. Imperfect sensitivity and specificity of commonly available biochemical tests and the low incidence of the tumor among the tested population mean that considerable time and effort can be expended in confirming or ruling out pheochromocytoma in patients where the tumor is suspected. Measurements of plasma free metanephrines provide a superior test compared to other available tests for diagnosis of pheochromocytoma. In particular, the high sensitivity of plasma free metanephrines means that a normal test result reliably excludes all but the smallest of pheochromocytomas so that no other tests are necessary. Measurements of plasma free metanephrines, when systematically combined with other diagnostic procedures outlined in this review, provide a more efficient, reliable and cost-effective approach for diagnosis of pheochromocytoma than offered by previously available approaches.
