The central goal of this paper is thinking about the Brazilian military power and its linking to the international ambitions of the country in the 21st century. After a comparative analysis to other BRICs and with a historical one about Brazil's strategic irrelevance, we aim to establish what the minimum military capacity Brazil would need in order to meet the country's latest international interests. Similarly, it will be discussed if the National Strategy of Defense, approved in 2008, and the recent strategic agreements signed with France represent one more step toward this minimum military capacity.

This paper reports two cases of afibrinogenemia with normal platelet count following Crotalus durissus terrificus, snake bite Both patients presented high output acute renal failure and case two also had increased blood levels of CPK and LDH compatible with the diagnosis of rhabdomyolysis. Case one was given an unknown amount of antivenom and was treated with epsilonaminocaproic acid and a fresh whole blood transfusion and showed recovery of the coagulation disturbance 40 hours following these measures. Case two was given an adequate amount of crotalide antivenom and the coagulation tests performed 12 hours later showed a normal partial thromboplastin time and fibrinogen 86 mg/100ml. Case one presented no haemorrhagic disturbances. Case two presented persistent bleeding following venopuncture and after removal of impetigo crust in the legs. Acute renal failure was treated conservatively and both patients were discharged from the hospital with recovery of the renal function.

A retrospective survey of 473 cases of snake bite admitted to a Brazilian teaching hospital from 1984 to 1990 revealed 91 cases of bite without envenoming and/or caused by non-venomous snakes. In 17 of these cases the snake was identified, and one patient was bitten by a snake-like reptile (Amphisbaena mertensii). In 43 cases diagnosis was made on clinical grounds (fang marks in the absence of signs of envenoming). The other 30 cases were of patients who complained of being bitten but who did not show any sign of envenoming or fang mark. Most cases occurred in men (66;73%), in the 10-19 years age group (26;29%), in the lower limbs (51/74;69%), between 6 A. M. and 2 P.M. (49;61%) and in the month of April (16; 18%). One patient bitten by Philodryas olfersii developed severe local pain, swelling and redness at the site of the bite, with normal clotting time. The patient bitten by Drymarcon corais was misdiagnosed as being bitten by a snake of the genus Bothrops, was given the specific antivenom, and developed anaphylaxis. One patient bitten by Sibynomorphus mikanii presented prolonged clotting time, and was also given antivenom as a case of Bothrops bite. Correct identification of venomous snakes by physicians is necessary to provide correct treatment to victims of snake bite, avoiding unnecessary distress to the patient, and overprescription of antivenom, which may eventually cause severe untoward effects.

A 5-year-old girl was bitten in her left eye by a lance-headed viper identified as Bothrops moojeni, measuring 115 cm of length. There was severe facial swelling and left exophthalmus, and enucleation of the eye was necessary. The patient apparently had mild systemic envenoming, but local inflammatory signs and histological evidence of necrosis suggest that both the mechanical trauma and the local action of the venom had a role in the genesis of the eye lesion. It is arguable if the loss of the eye could be prevented even if the antivenom was administered earlier.

A 5-year-old boy bitten by a specimen of Philodryas patagoniensis, a colubrid snake currently classified as nonvenomous, developed signs of local envenoming characterized by swelling and warmth on the bitten limb. This is the first time that local envenoming following Philodryas patagoniensis bite is recognized. Based on the clinical findings and misidentification of the snake, the patient was treated as a victim of Bothrops bite, having received unnecessarily the specific antivenom. Educational efforts to make doctors and health workers capable to identify correctly venomous snakes are necessary, to avoid inappropriate indication of antivenom and decrease the risk of its potentially harmful untoward effects. Examination of the bite site can be useful to the differential diagnosis between pit viper and colubrid bites.

A retrospective survey done from 1987 till 1990 revealed that 23 patients bitten by pigs sought medical help at a teaching hospital in Uberlândia, in southeastern Brazil. Most cases (21) were from Uberlândia. The cases were evenly distributed by month and by year; most of them (14/16; 87.5%) occurred between 7. OOa.m. and 7.00 p. m. The male to female ratio was 6.7:1. Age ranged from 6 to 73 (mean 38.95 ± SD 22.06, median 36). The bites were more common on the upper limbs, particularly on the forearms. In 11(47.8%) cases the injury was described as deep. In most cases where information was available the injury was related to capture, transport or immobilisation ofthe pigfor slaughter. The following medical procedures were performed: local cleansing in 19(82.6%) cases, rabies vaccine (12; 52.2%), antirabies serum (2; 8.7%), suturing (6; 26.1%) and tetanus vaccine (12; 52.2%). There was no case of infection at the bite site, neither of rabies or tetanus. By our data, the annual incidence of pig bite in Uberlândia can be estimated to be about 1.5/100.000.

The case of a man bitten by a South American rattlesnake (Crotalus durissus) and who developed an abscess at the site of the bite is reported. Abcesses are a rare complication of this type of envenoming, possibly due to the lack of a strong cytotoxic action of Crotalus durissus venom.

Os autores iniciam com êste, uma série de trabalhos sôbre a Dermatozoonose provocada pela picada de Culicoides, em Salvador, Bahia, Brasil. No presente, tratam das espécies de Culicoides encontradas em Salvador, baseados na coleta de 2.947 exemplares, durante os anos de 1959 a 1963. Encontraram as 4 espécies seguintes; C. (O.) paraensis (Goeldi, 1905) C. (O.) limonensis Ortiz & Leon, 1955 C. (C.) insignis Lutz, 1913 C. (C.) flavivenula Costa Lima, 1937. Não reencontraram o C. (C.) maruim, Lutz, 1913 assinalado por fox (1948) e WIRTH & BLANTON (1956) para Salvador. Dessas espécies o C. (C.) paraensis se mostrou a predominante, abrangendo 98% de exemplares coletados. Apresentam a distribuição das espécies por bairros e respectivas quantidades coletadas. Finalizando, dão as principais características morfológicas das espécies, ilustrando essas com desenhos do material por êles estudado.

Neste segundo trabalho da série "Dermatozoonose pela picada de Culicoides em Salvador, Bahia", os Autores apresentam dados sôbre a bionomia das espécies encontradas em Salvador, em condições naturais. Salientam que devido a localização geográfica e das condições de clima e umidade a cidade de Salvador apresenta condições ótimas para a proliferação do díptero. Cêrca de 98% dos espécimens de Culicoides coletados em tôda a cidade foi representado pelo Culicoides (O.) paraensis (Goeldi, 1903), sendo, por êsse motivo, levado em maior consideração. O C. paraensis distribui-se por tôda a cidade, descrevendo sua densidade ao nível dos mangues, onde predomina o C. flavivenula. Inicia sua atividade entre 5 e 6 horas, tendo 3 picos de maior ocorrência durante o dia: 7 horas, 12 horas e predominantemente às 15 as 17 horas. Desaparece entre 18 e 19 horas. O C. limonensis só ocorre entre 6 e 8 horas, junto ao C. paraensis. o C. paraensis incide durante o ano inteiro, tendo uma variação estacional nítida. Sua densidade máxima corresponde aos meses chuvosos e mais frios. Não foi encontrado o criadouro natural das larvas, porém os autores julgam que se criem em ambientes de alta umidade, ou em águas coletadas em tronco de árvores, lixeiras e vasilhames diversos abandonados em terrenos baldios. em relaççao aos hábitos hematófagos, salienta-se que o C. paraensis os tem acentuadamente antropófilos. Foi observado que certos indivíduos possuem uma atratividade especial para o culicoides. Em adição, a espécie tem preferências para sugar os membros inferiores. Para a explicação d~este fato, os autores julgam que o vôo rasteiro, a proteção contra aluz e correntes aéreas, e a maior exposição das pernas do hospedeiro, favoreçam uma característica específica que o C. paraensis possua para picar os membros inferiores.

Nesta terceira contribuiçãos os Autores apresentam os aspectos Epídemiológicos da Dermatozoonose pela picada de Culicoides em Salvador. Salientam que embora a densidade de insetos outros de hábitos antropófilos seja elevada na cidade, as seguintes evidências os conduziram a responsabilizar os Culicoides: conincidência do aparecimento de casos de Dermatozoonose após um período de maior densidade de Culicoides; maior número de casos, desde que a densidade de Culicoides aumentou nos últimos anos; proveni~encia de maior número de casos dos bairros onde há maior infestação de Culicoides. A Dermatozoonose é acentuadamente mais freqüente no sexo feminino. Houve maior número de casos entre os negros, talvez devido a maior freqüencia de negros que procuram tratamento no Hospital das Clínicas. Não há predominância acentuada para determinado grupo etário. Num levantamento que fizeram sôbre a incomodidade do Culicoides observaram que 81% de 593 residências visitadas em diferentes bairros, são incomodadas, sendo o inverno a época de maior incômodo. As horas de maior incômodo, coincidem com a ocorrência horária máxima do Culicoides. Observaram que as medidas usadas pela população para combate ao inseto são inadequadas pois, em 56% das residências não se obtém qualquer resultado. Considerando que nesses último cinco anos a densidade de Culicoides aumentou inexplicàvelmente em Salvador, julgam que os seguintes fatôres participara para que êsse fenômeno ocorresse: a extinção do Serviço de Profilaxia da Febre Amarela em 1956, o qual, indiretamente, por meio de sua "polícia de fócos" combatendo o Aedes aegypti, controlava os Culicoides; o crescimento da cidade, aumentando o número de fossas, já que não existe um sistema de esgotos adequado; e a deficiência do Serviço de Limpeza Pública da Cidade, ocasionando o acúmulo de lixo nos quintais, terrenos baldios e mesmo em logradouros públicos. Essas condições permitiram a existência de uma extensa rêde de "focos potenciais" para a proliferação dos Culicoides que agora infestam a cidade.

A observação de 211 pacientes com reação intensa à picada do Culicoides, que procuraram tratamento na Clínica dermatológica do Hospital das Clínicas da Universidade da Bahia, durante os anos de 1959 e 1962, permitiu o estudo clínico dessa Dermatozoonose, cujos dados são aqui apresentados. A lesão parece ser de natureza alérgica e devido ao aspecto polimorfo pelo qual se apresenta, essa Dermatose pode lembrar o Prorigo, a Escabiose, as Lesões liquenoide; quando a manifestação é mais intensa torna-se uma verdadeira eczematização; quando há infecção secundária, lembra o impetigo folicular. O estudo histológico da lesão revelou ser ela a de uma inflamação crônica, com vascularites e preivascularites dermo-epidérmica, provàvelmente de natureza alérgica. Para que haja a formação da lesão, são necessários: a substância inoculada pelo inseto e o componente alérgico do indivíduo. Não se conhece a natureza da substância inoculada pelo inseto e as seguintes hipóteses são apresentadas para explicá-la: substâncias enzimáticas ou a histamina existentes nas glândulas salivares do Culicoides. Após a picada do Culicoisdes forma-se no local uma pequena área eritematosa que logo após se transforma em pápula; as pápulas podem desaparecer ou transformarem-se em vesículas; estas ao se romperem dilaceram a superfície cutânea, descamam-na ou pode advir uma infecção secundária e transformam-se em pústulas.

Lutzomyia furcata transmitted Leishmania chagasi to a hamster 10 days after being experimentally fed on an infected spleen. An individual female Psychodopygus carrerai carrerai that had fed on a hamster lesion caused by Leishmania mexicana amazonensis transmitted this parasite 6 days later to another hamster. Transmission electron microscopy of this fly's head revealed a small number of degenerate promastigotes in the foregut, but only a few were attached.

A clinical-serological follow-up was carried out in a canine population in endemic foci of Leishmania braziliensis spread in northwestern Argentina. Each dog was studied in at least two visits, 309±15 days (X±SE) apart. Some initially healthy dogs (n=52) developed seroconversion or lesions. The clinical evolution of the disease in dogs resembles in many aspects the human disease. Similarities include the long duration of most ulcers with occasional healing or appearance of new ones and the late appearance of erosive snout lesions in some animals. Yearly incidence rates of 22.7% for seroconversion and of 13.5% for disease were calculated as indicators of the force of infection by this parasite upon the canine population.

In Chagas disease serious cardiac dysfunction can appear. We specifically studied the cardiac function by evaluating: ventricle contractile force and norepinephrine response, affinity and density of beta-adrenergic receptors, dynamic properties of myocardial membranes, and electrocardiography. Albino swiss mice (n = 250) were infected with 55 trypomastigotes, Tulahuen strain and studied at 35, 75, and 180 days post-infection, that correspond to the acute, indeterminate, and chronic phase respectively. Cardiac beta-adrenergic receptors' affinity, myocardial contractility, and norepinephrine response progressively decreased from the acute to the chronic phase of the disease (p < 0.01). The density (expressed as fmol/mg.prot) of the receptors was similar to non-infected mice (71.96 ± 0.36) in both the acute (78.24 ± 1.67) and indeterminate phases (77.28 ± 0.91), but lower in the chronic disease (53.32 ± 0.71). Electrocardiographic abnormalities began in the acute phase and were found in 65% of the infected-mice during the indeterminate and chronic phases. Membrane contents of triglycerides, cholesterol, and anisotropy were similar in all groups. A quadratic correlation between the affinity to beta-adrenergic receptors and cardiac contractile force was obtained. In conclusion the changes in cardiac beta-adrenergic receptors suggests a correlation between the modified beta-adrenergic receptors affinity and the cardiac contractile force.

The apicomplexan parasite Toxoplasma gondii is unusual in being able to infect almost any cell from almost any warm-blooded animal it encounters. This extraordinary host-range contrasts with its far more particular cousins such as the various species of the malaria parasite Plasmodium where each species of parasite has a single genus or even species of host that it can infect. Genetic and genomic studies have revealed a key role for a number of gene families in how Toxoplasma invades a host cell, modulates gene expression of that cell and successfully evades the resulting immune response. In this review, I will explore the hypothesis that a combination of sexual recombination and expansion of host range may be the major driving forces in the evolution of some of these gene families and the specific genes they encompass. These ideas stem from results and thoughts published by several labs in the last few years but especially recent papers on the role of different forms of rhoptry proteins in the relative virulence of F1 Toxoplasma progeny in a particular host species (mice).
