In the absence of intervention, the rate of vertical transmission of HIV can range from 15-45%. With the inclusion of antiretroviral drugs during pregnancy and the choice of delivery route this amounts to less than 2%. However ARV use during pregnancy has generated several questions regarding the adverse effects of the gestational and neonatal outcome. This study aims to analyze the risk factors for vertical transmission of HIV-1 seropositive pregnant women living in Rio Grande and the influence of the use of ARVs in pregnancy outcome. Among the 262 pregnant women studied the rate of vertical transmission of HIV was found to be 3.8%. Regarding the VT, there was a lower risk of transmission when antiretroviral drugs were used and prenatal care was conducted at the referral service. However, the use of ART did not influence the outcome of pregnancy. However, initiation of prenatal care after the first trimester had an influence on low birth weight, as well as performance of less than six visits increased the risk of prematurity. Therefore, the risk factors analyzed in this study appear to be related to the realization of inadequate pre-natal and maternal behavior.

Neste artigo se examina a não adequação dos EUA ao parecer da OMC no contencioso do algodão. Argumenta-se que tal atitude é irracional do ponto de vista do Estado unitário e racional, mas que pode ser explicada por meio de uma perspectiva fragmentada do Estado. Assim, embora a administração desejasse a adequação, cotonicultores e congressistas potencializados por instituições políticas constrangeram as mudanças necessárias. Ao final são projetados cenários para a modificação da política de subsídios agrícolas dos EUA.
O problema administrativo na política de assentamentos do Brasil: o caso da fazenda Cascata na Bahia

O presente artigo é resultado de um estudo que toma como objeto de observação o estado da Bahia e tendo como objetivo contribuir para o aprimoramento da política de reforma agrária no Brasil. Para essa análise foi selecionado o assentamento de Cascata, localizado no município de Aurelino Leal, na região do extremo sul da Bahia. Esse recorte regional revelou-se interessante por dois motivos principais: por representar as primeiras experiências dos programas de assentamentos no estado da Bahia; e por refletir experiências de desenvolvimento socioeconômico sustentável, em forma de cooperativas, envolvendo a maioria dos assentamentos da região estudada.

Pela primeira vez entre nós estuda-se a mortalidade de 1-4 anos pelas divisões político-administrativas do município de São Paulo. Demonstra-se que para êsse estudo, os dados oficiais dos óbitos do município de São Paulo, face às diferentes condições sócio-econômica-sanitárias de cada uma dessas áreas, quando comparadas entre si, não são adequados. Propõe-se a sua substituição pelos "de direito" correspondentes, obtidos pessoalmente pelo autor pelo exame dos atestados de óbito da Capital. Apresentam-se alguns aspectos da mortalidade de 1-4 anos "de direito", segundo os distritos e subdistritos do município de São Paulo, que se revestem de importância epidemiológica e administrativa para os nossos sanitaristas, quando, êstes últimos, através do uso dêsses dados, entendidos como um indicador de saúde, têm de planejar, programar e executar os seus trabalhos de saúde pública na nossa comunidade.

INTRODUCTION : In this study, clinical-laboratory and epidemiological characteristics are described for a group of 700 individuals with HIV (human immunodeficiency virus)/AIDS (acquired immunodeficiency syndrome) in the ART (antiretroviral therapy) era at a teaching hospital that provides a quaternary level of care, with an emphasis on opportunistic infections (OIs), co-infections and immune profile. METHODS : A retrospective cross-sectional study of AIDS cases was conducted from 1998 to 2008 by reviewing medical records from the Base Hospital/FUNFARME (Funda1;ão Faculdade Regional de Medicina), São José do Rio Preto, São Paulo, Brazil. RESULTS: The individuals were 14 to 75 years of age, and 458 were males. Heterosexuals accounted for 31.1% of all patients. Eighty-three percent were on ART, and 33.8% of those presented difficulties with treatment adherence. OIs were analyzed from medical records, and Pneumocystis jiroveci pneumonia was the most prevalent, regardless of the LTCD4+ (TCD4+ Lymphocytes) levels. Individuals whose viral loads were ≥10,000 showed a 90% greater chance of neurotoxoplasmosis. For P. jiroveci pneumonia, neurotoxoplasmosis, esophageal candidiasis, pulmonary tuberculosis and neurocryptococcosis, the chances of infection were higher among patients with LTCD4+ levels below 200 cells/mm3. HIV/hepatitis C virus (HCV) and HIV/hepatitis B virus (HBV) co-infections were significantly associated with death. CONCLUSIONS : OIs remain frequent in the ART era even in populations where the access to medical care is considered satisfactory.

Paleoparasitology in the Old World has mainly concerned the study of latrine sediments and coprolites collected from mummified bodies or archaeological strata, mostly preserved by natural conditions. Human parasites recovered include cestodes, trematodes, and nematodes. The well preserved conditions of helminth eggs allowed paleoepidemiological approaches taking into account the number of eggs found by archaeological stratum. Tentatively, sanitation conditions were assessed for each archaeological period.

Significant decrease in human immunodeficiency virus type 1 (HIV-1) vertical transmission has been observed worldwide in centers where interventions such as antiretroviral therapy (ART), elective cesarean section, and avoidance of breastfeeding have been implemented. This prospective cohort study aimed to assess the determinants of and the temporal trends in HIV-1 vertical transmission in the metropolitan area of Belo Horizonte, Brazil from January 1998 to December 2005. The rate of HIV-1 vertical transmission decreased from 20% in 1998 to 3% in 2005. This decline was associated with increased use of more complex ART regimens during pregnancy. Multivariate analysis restricted to clinical variables demonstrated that non ART, neonatal respiratory distress/sepsis and breastfeeding were independently associated with HIV-1 vertical transmission. When laboratory parameters were included in the model, high maternal viral load and non maternal ART were associated with HIV-1 vertical transmission. The results from this study confirm the impact of ART in the reduction of HIV-1 vertical transmission and indicate the need for improvement in the care and monitoring of mother and infant pairs affected by HIV-1.

In a recent article in the ReaderR17;s Opinion, advantages and disadvantages of the certification processes of interrupted Chagas disease transmission (American trypanosomiasis) by native vector were discussed. Such concept, accepted by those authors for the case of endemic situations with introduced vectors, has been built on a long and laborious process by endemic countries and Subregional Initiatives for Prevention, Control and Treatment of Chagas, with Technical Secretariat of the Pan American Health Organization/World Health Organization, to create a horizon target and goal to concentrate priorities and resource allocation and actions. With varying degrees of sucess, which are not replaceable for a certificate of good practice, has allowed during 23 years to safeguard the effective control of transmission of Trypanosoma cruzi not to hundreds of thousands, but millions of people at risk conditions, truly “the art of the possible.̶1;

Este estudo objetivou verificar o saber e o fazer das enfermeiras que atuam nas unidades municipais de Saúde do Distrito Administrativo 71-Santo Amaro -São Paulo, frente às Terapias Alternativas/Complementares(TA/C). A pesquisa, de abordagem quantitativa com análise qualitativa dos dados, permitiu evidenciar que 89% dos enfermeiros acreditam nas TA/C porém, apenas 22,2% têm conhecimento do respaldo legal e 5,5% têm cursos nesta área; 44,4% aplicam em si mesma e 11,1% aplicam nos pacientes, sete TA/C distintas, classificadas em cinco grupos de HILL. Desvendou-se que há necessidade de buscar novos saberes como opções de assistência à promoção da saúde da população.

Inventories on necrophagous insects carried out in Brazil encompass mostly species from the southeastern and central-western regions of the country. This review aims to produce the first checklist of necrophagous Diptera and Coleoptera species of forensic relevance in northeastern Brazil, an area that concentrates high rates of homicides. We performed a literature survey on scientific articles, theses and dissertations regarding necrophagous insect species in the region, and contacted scientists who develop research on forensic entomology. Fifty-two species of Diptera belonging to eight families with previous record of necrophagy were reported in the region: Sarcophagidae, Calliphoridae, Muscidae, Fanniidae, Piophilidae, Phoridae, Anthomyiidae and Stratiomyidae. Coleopteran species from six families of forensic relevance were registered, although taxonomical identification remained superficial. Bait traps were the most frequent methodology used, followed by collection on animal carcasses. Seven Dipteran species from two families were registered on human cadavers. All species had been previously reported in other Brazilian states and/or other countries, although none has been effectively used in legal procedures in the region. The status of research on forensic entomology in northeastern Brazil is incipient, and the checklist produced here contributes to the knowledge on the local diversity of necrophagous insects.
