Background:Diabetes affects approximately 250 million people in the world. Cardiovascular autonomic neuropathy is a common complication of diabetes that leads to severe postural hypotension, exercise intolerance, and increased incidence of silent myocardial infarction.Objective:To determine the variability of heart rate (HR) and systolic blood pressure (SBP) in recently diagnosed diabetic patients.Methods:The study included 30 patients with a diagnosis of type 2 diabetes of less than 2 years and 30 healthy controls. We used a Finapres® device to measure during five minutes beat-to-beat HR and blood pressure in three experimental conditions: supine position, standing position, and rhythmic breathing at 0.1 Hz. The results were analyzed in the time and frequency domains.Results:In the HR analysis, statistically significant differences were found in the time domain, specifically on short-term values such as standard deviation of NN intervals (SDNN), root mean square of successive differences (RMSSD), and number of pairs of successive NNs that differ by more than 50 ms (pNN50). In the BP analysis, there were no significant differences, but there was a sympathetic dominance in all three conditions. The baroreflex sensitivity (BRS) decreased in patients with early diabetes compared with healthy subjects during the standing maneuver.Conclusions:There is a decrease in HR variability in patients with early type 2 diabetes. No changes were observed in the BP analysis in the supine position, but there were changes in BRS with the standing maneuver, probably due to sympathetic hyperactivity.

Lumbar hernia is defined as an abdominal passage through the posterior abdominal wall. Approximately 250 to 300 cases have been described in the literature, being quite infrequent. Untreated lumbar hernia may result in severe complications. The authors report a case of a 60 year old male patient presenting a large bowel obstruction and perfuration secundary to incarceration of descending colon within a lumbar hernia. This was diagnosed by clinical history and computed tomography. The patient was successfully treated surgically.

The aim of the present study was to evaluate the effect of Ginkgo biloba treatment (EGb 761, 200 mg kg-1 day-1) administered from day 0 to 20 of pregnancy on maternal reproductive performance and on the maternal and fetal liver antioxidant systems of streptozotocin-induced diabetic Wistar rats. On day 21 of pregnancy, the adult rats (weighing approximately 250 ± 50 g, minimum number = 13/group) were anesthetized to obtain maternal and fetal liver samples for superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-Px), and total glutathione (GSH-t) determinations. The uterus was weighed with its contents. The diabetic (G3) and treated diabetic (G4) groups of rats presented significant maternal hyperglycemia, reduced term pregnancy rate, impaired maternal reproductive outcome and fetal-placental development, decreased GSH-Px (G3 = G4 = 0.6 ± 0.2) and SOD (G3 = 223.0 ± 84.7; G4 = 146.1 ± 40.8), and decreased fetal CAT activity (G3 = 22.4 ± 10.6; G4 = 34.4 ± 14.1) and GSH-t (G3 = G4 = 0.3 ± 0.2), compared to the non-diabetic groups (G1, untreated control; G2, treated). For G1, maternal GSH-Px = 0.9 ± 0.2 and SOD = 274.1 ± 80.3; fetal CAT = 92.6 ± 82.7 and GSH-t = 0.6 ± 0.5. For G2, G. biloba treatment caused no toxicity and did not modify maternal or fetal-placental data. EGb 761 at the nontoxic dose used (200 mg kg-1 day-1), failed to modify the diabetes-associated increase in maternal glycemia, decrease in pregnancy rate, decrease in antioxidant enzymes, and impaired fetal development when the rats were treated throughout pregnancy (21 days).

Fried foods are widely consumed in Brazil and their quality depends on the oil or fat they are fried. Qualitative (physical chemistry indices) and quantitative measurements (fry-life oil or fat until disposal, oil turnover, type of oil or fat and amount and type of fried foods) and associations were performed. We applied a structured form and collected 60 mL of frying oil or fat in each of the 70 fried food stands of 15 street-fairs in Goiânia, Brazil. All samples were suitable in the quantity of free fat acids (<0.9% oleic acid), one was inadequate to peroxide value (>10 mEq/kg) and 1/3 was unsuitable to polar compounds (<25%). The majority (62%) use temperature up to the allowed (180 ºC). Approximately 250 units of products are fried in at least one day in 42% of the fried food stands. Soybean oil is used in the majority (94%) of fried food stands and the fry-life is of 6 hours (60%) or a day of work/sale. The nonconformity of the content of total polar compounds in fried foods had significant association with frying time and the conformity of acidity had significant relationship with frying time by a chi-square test. All other associations were not significant. A fry-life of oil or fat up to 6 hours can avoid the excess of polar compounds in the frying medium and protect the quality of fred foods.

O objetivo da pesquisa foi efetuar levantamento das condições referentes à distribuição de risco nas formalizações das parcerias p250;blico-privadas no Brasil, a partir do marco legal que instituiu tal forma de implantação de equipamentos sociais no Brasil. Foram avaliados, por pesquisa documental, como os fatores de riscos estão sendo alocados e distribuídos em 15 contratos celebrados, buscando determinar como são mitigados os riscos inerentes entre os parceiros p250;blicos e privados. Os contratos apresentam definição expressiva dos riscos envolvidos e de sua responsabilidade pelos parceiros. As evidências sinalizaram que os parceiros privados têm absorvido, formalmente, a maioria dos fatores de riscos elencados; contudo, há expressas medidas de mitigação dos riscos atribuídos nos contratos ao parceiro privado, pela intervenção em 250;ltima instância do ente p250;blico envolvido.

Os desafios observados na implementação de políticas p250;blicas têm fomentado uma série de debates no Brasil, entre os quais se destacam aqueles relacionados aos modelos teóricos de policy analysis e seus limites em face da visão multicêntrica e fractal das políticas p250;blicas. Partindo de uma política setorial específica (habitacional), este artigo defende um posicionamento crítico em relação aos estudos monodisciplinares. Contudo, não se pode negar a complexidade dessa empreitada. Compreender os avanços da política habitacional da 250;ltima década de forma interdisciplinar e integrada constitui uma tarefa intricada e desafiadora. Buscando avançar nesse campo, este artigo tem por objetivos avaliar os modelos teóricos de análise de políticas p250;blicas e, a partir desses elementos, propor constructos com vistas a subsidiar futuros modelos teóricos de análise da política habitacional no Brasil.

A administração p250;blica no Brasil conta com diretrizes e parâmetros estabelecidos em documentos, a exemplo de leis, políticas ou planos, que se constituem em instrumentos para nortear e qualificar a gestão democrática das políticas p250;blicas. A investigação buscou identificar instrumentos e mecanismos de gestão utilizados no processo decisório pelos Conselhos Municipais de Assistência Social e de Sa250;de em Chapecó (SC). A partir da leitura de atas (2005 a 2009), obtiveram-se como instrumento mais citado os relatórios de prestação de contas, embora esses não evidenciarem articulação com objetivos e metas definidos no plano municipal. Os mecanismos mais utilizados são comissões internas, capacitação, conferências e comunicação com outras organizações. Os conselhos estudados recorrem a instrumentos e mecanismos com potencial para ampliar a participação na gestão p250;blica municipal.

OBJETIVO: Realizar a tradução, adaptação e validação de conte250;do da versão brasileira do Posttraumatic Cognitions Inventory . MÉTODOS: O processo de tradução e adaptação das instruções iniciais e dos itens do PTCI envolveu cinco etapas: (1) tradução; (2) retradução; (3) correção e adaptação semântica; (4) validação do conte250;do por profissionais da área (juízes); e (5) teste da versão final, por meio de uma escala verbal-numérica. Como indicadores de desempenho para a compreensão, foram computados os escores de tendência central (média) e dispersão (desvio padrão) para cada item na etapa 5. Definiu-se escore médio ≥ 3 para compreensão satisfatória. RESULTADOS: As 36 questões e as instruções iniciais foram traduzidas e adaptadas para compor a versão brasileira do Posttraumatic Cognitions Inventory . Quarenta e cinco adultos responderam aos itens do Posttraumatic Cognitions Inventory na etapa teste da versão final, mostrando compreensão adequada do instrumento na escala verbal-numérica (M = 4,13; dp = 0,11). CONCLUSÕES: O Posttraumatic Cognitions Inventory é um instrumento de fácil compreensão e semanticamente válido. Estudos posteriores são necessários para a verificação e adequação da avaliação de suas propriedades psicométricas na população brasileira.

OBJETIVO Avaliar a confiabilidade e a validade do Índice de Qualidade da Dieta Revisado de acordo com as propriedades psicométricas. MÉTODOS Estudo transversal em amostra probabilística do município de São Paulo, SP, com 2.375 indivíduos com idade maior ou igual a 12 anos, de ambos os sexos, incluídos no Inquérito de Sa250;de, realizado em 2003. As informações sobre características da população foram obtidas por meio de questionário. Dados do consumo alimentar foram obtidos pela aplicação do Recordatório de 24 horas, utilizado para calcular o Índice de Qualidade da Dieta Revisado (IQD-R). Foram avaliados (1) validade de conte250;do, comparando os componentes com as recomendações do Guia Alimentar para a População Brasileira; (2) validade de constructo, por meio da análise fatorial pelo método dos componentes principais e verificando se o índice consegue mensurar a qualidade da dieta independente da energia ingerida; (3) validade discriminante; e (4) a confiabilidade do IQD-R pela análise da consistência interna dos itens utilizando o coeficiente alfa de Cronbach. RESULTADOS As correlações entre os escores dos componentes e a energia consumida foram fracas (r #< 0,30). A análise de componentes principais indicou a presença de quatro fatores com autovalores #> 1 que representaram 67% da variância do índice. A validade discriminante do IQD-R foi observada comparando os indivíduos por sexo e hábito de fumar, identificando diferenças estatisticamente significativas entre as médias dos componentes do IQD-R e o escore final. O valor do alfa de Cronbach (α = 0,7) indicou a presença de consistência interna entre os componentes do IQD-R. O componente Gord_AA, seguido dos componentes frutas totais e frutas integrais, apresentaram maior correlação com a pontuação final do índice. CONCLUSÕES O Índice de Qualidade da Dieta Revisado mostrou-se confiável e estruturalmente válido quando utilizado para avaliar e monitorar a qualidade da dieta de brasileiros.

Treatment with indinavir has been shown to result in marked decreases in viral load and increases in CD4 cell counts in HIV-infected individuals. A randomized double-blind study to evaluate the efficacy of indinavir alone (800 mg q8h), zidovidine alone (200 mg q8h) or the combination was performed to evaluate progression to AIDS. 996 antiretroviral therapy-naive patients with CD4 cell counts of 50-250/mm3 were allocated to treatment. During the trial the protocol was amended to add lamivudine to the zidovudine-containing arms. The primary endpoint was time to development of an AIDS-defining illness or death. The study was terminated after a protocol-defined interim analysis demonstrated highly significant reductions in progression to a clinical event in the indinavir-containing arms, compared to the zidovudine arm (p<0.0001). Over a median follow-up of 52 weeks (up to 99 weeks), percent reductions in hazards for the indinavir plus zidovudine and indinavir groups compared to the zidovudine group were 70% and 61%, respectively. Significant reductions in HIV RNA and increases in CD4 cell counts were also seen in the indinavir-containing groups compared to the zidovudine group. Improvement in both CD4 cell count and HIV RNA were associated with reduced risk of disease progression. All three regimens were generally well tolerated.

The dengue virus (DENV), which is frequently involved in large epidemics, and the yellow fever virus (YFV), which is responsible for sporadic sylvatic outbreaks, are considered the most important flaviviruses circulating in Brazil. Because of that, laboratorial diagnosis of acute undifferentiated febrile illness during epidemic periods is frequently directed towards these viruses, which may eventually hinder the detection of other circulating flaviviruses, including the Saint Louis encephalitis virus (SLEV), which is widely dispersed across the Americas. The aim of this study was to conduct a molecular investigation of 11 flaviviruses using 604 serum samples obtained from patients during a large dengue fever outbreak in the state of Mato Grosso (MT) between 2011 and 2012. Simultaneously, 3,433 female Culex spp. collected with Nasci aspirators in the city of Cuiabá, MT, in 2013, and allocated to 409 pools containing 1-10 mosquitoes, were also tested by multiplex semi-nested reverse transcription PCR for the same flaviviruses. SLEV was detected in three patients co-infected with DENV-4 from the cities of Cuiabá and Várzea Grande. One of them was a triple co-infection with DENV-1. None of them mentioned recent travel or access to sylvatic/rural regions, indicating that transmission might have occurred within the metropolitan area. Regarding mosquito samples, one pool containing one Culex quinquefasciatus female was positive for SLEV, with a minimum infection rate (MIR) of 0.29 per 1000 specimens of this species. Phylogenetic analysis indicates both human and mosquito SLEV cluster, with isolates from genotype V-A obtained from animals in the Amazon region, in the state of Pará. This is the first report of SLEV molecular identification in MT.

O objetivo deste estudo foi avaliar o potencial do cajuzinho do cerrado (Anacardium humile) sobre larvas de Aedes aegypti. Os extratos hexânico, etanólico, aquoso e o óleo das folhas foram obtidos do material vegetal coletado em fragmento de cerrado. Estes foram testados nas concentrações 1%; 0,5%, 0,25%, 0,125%, 0,05% e 0,0125% diluídas em dimetil sulfóxido 1%. A contagem das larvas mortas foi realizada após 24 horas. Utilizou-se o método Probit de análise para obtenção das CL50 e respectivos intervalos de confiança. Conclui-se que apenas o óleo extraído de folhas de Anacardium humile causa 100% de mortalidade em larvas de 4º estádio de Aedes aegypti nas concentrações até 0,125%, o que parece indicar que os ingredientes ativos estão na fase mais apolar. O que indica a potencialidade de uso da planta como larvicida de Aedes aegypti, entretanto, novos testes deverão ser conduzidos utilizando outros órgãos vegetais, assim como outros métodos e solventes utilizados na extração.

Introduction Saint Louis encephalitis virus (SLEV) primarily occurs in the Americas and produces disease predominantly in humans. This study investigated the serological presence of SLEV in nonhuman primates and horses from southern Brazil. Methods From June 2004 to December 2005, sera from 133 monkeys (Alouatta caraya, n=43; Sapajus nigritus, n=64; Sapajus cay, n=26) trap-captured at the Paraná River basin region and 23 blood samples from farm horses were obtained and used for the serological detection of a panel of 19 arboviruses. All samples were analyzed in a hemagglutination inhibition (HI) assay; positive monkey samples were confirmed in a mouse neutralization test (MNT). Additionally, all blood samples were inoculated into C6/36 cell culture for viral isolation. Results Positive seroreactivity was only observed for SLEV. A prevalence of SLEV antibodies in sera was detected in Alouatta caraya (11.6%; 5/43), Sapajus nigritus (12.5%; 8/64), and S. cay (30.8%; 8/26) monkeys with the HI assay. Of the monkeys, 2.3% (1/42) of A. caraya, 6.3% 94/64) of S. nigritus, and 15.4% (4/26) of S. cay were positive for SLEV in the MNT. Additionally, SLEV antibodies were detected by HI in 39.1% (9/23) of the horses evaluated in this study. Arboviruses were not isolated from any blood sample. Conclusions These results confirmed the presence of SLEV in nonhuman primates and horses from southern Brazil. These findings most likely represent the first detection of this virus in nonhuman primates beyond the Amazon region. The detection of SLEV in animals within a geographical region distant from the Amazon basin suggests that there may be widespread and undiagnosed dissemination of this disease in Brazil.

Introduction Arboviruses are an important public health problem in Brazil, in especially flaviviruses, including the Saint Louis encephalitis virus (SLEV) and the Rocio virus (ROCV), are especially problematic. These viruses are transmitted to humans or other vertebrates through arthropod bites and may cause diseases with clinical manifestations that range from asymptomatic infection, viral hemorrhagic fever to encephalitis. Methods A serological survey of horses from various regions of Brazil using an enzyme-linked immunosorbent assay (ELISA) with recombinant SLEV domain III peptides and ROCV E protein as antigens. Results Overall, 415 (55.1%) of the 753 horses that were screened were seropositive for flavivirus and, among them, monotypic reactions were observed to SLEV in 93 (12.3%) and to ROCV in 46 (6.1%). These results suggested that these viruses, or other closely related viruses, are infecting horses in Brazil. However, none of the studied horses presented central nervous system infection symptoms. Conclusions Our results suggest that SLEV and ROCV previously circulated among horses in northeast, west-central and southeast Brazil.
