Foram examinados exsudatos peritoneais e órgãos (cérebro, coração, pulmão e músculo estriado) de 53 camundongos infectados experimentalmente pelo Toxoplasma gondii, sendo 21 na fase aguda e 32 na crônica. Camundongos albinos, machos, de cerca de 25 g e 2 meses de idade foram inoculados, por via intraperitoneal, com 0,5 ml de exsudato peritoneal (taquizoitas) ou macerado de cérebro (cistos) de camundongos previamente infectados. O exame a fresco foi feito no exsudato peritoneal, entre 3 e 12 dias após inoculação e no cérebro, após 10 dias. Foram realizadas inoculações de macerados de órgãos em novos camundongos (repiques) para a recuperação do parasita no exsudato ou no cérebro. Na infecção aguda as positividades foram, ao exame a fresco: exsudato peritoneal 19/19, pulmão 12/14, músculo 6/9, coração 4/9 e cérebro 1/3. Após inoculação: exsudato peritoneal 5/5, cérebro 2/2, coração 19/19, pulmão 13/13 e músculo 14/17. Após estes últimos resultados foram registrados 9 novos órgãos positivos. A positividade final (igual à recuperação do parasita) foi: exsudato peritoneal 19/19 (100%), coração 15/17 (88,5%), músculo 12/14 (85,7%), pulmão 14/14 (100%) e cérebro 2/3 (66,6%). Na infecção crônica, que transcorreu entre 10 e 495 dias, as positividades foram, ao exame a fresco: cérebro 28/32, coração 0/4 e músculo 0/4. Após repique: cérebro 6/6, coração 14/29 e músculo 16/26. Neste exame foi revelado um novo camundongo positivo elevando para 29 o total de camundongos positivos ou 90,6%. O resultado final foi: cérebro 28/32 (87,5%), músculo 16/28 (57,15%) e coração 14/31 (45,1%). No fim da pesquisa, aos 495 dias, o cérebro apresentava grandes cistos ao exame a fresco e coração e músculo mostravam-se positivos através da inoculação. Conclusões: 1º) nos camundongos o toxoplasma persistiu por 495 dias no cérebro, coração e músculo estriado; 2º) o exame a fresco do pulmão pode substituir ou confirmar o do exsudato peritoneal; 3º) a inoculação de órgãos é necessária pois pode revelar novos casos positivos; 4º) a atividade dos cistos foi demonstrada pelo aumento gradual do seu tamanho e pela recuperação do toxoplasma no cérebro, coração e músculo, após o longo tempo de infecção.

A total of 110 strains of Streptococcus suis, isolated from diseased pigs in Brazil were serotyped and analyzed for virulence. Serotyping of the strains resulted in the following classification: 42 strains of serotype 2 (38.2%), 10 strains of serotype 14 (9.1%), seven strains of serotype 9 (6.4%), three strains each of serotype 7 and 11 (2.7%), two strains each of serotype 1 and 8 (1.8%) and one strain each of serotypes ½, 3, 5, 6 and 10 (0.9%). Cross reactions among serotypes 1, 14 and 7 were observed in 21 strains (19.1%). Only 41.9% of the strains were lethal for mice using the pathogenicity test.

INTRODUCTION: Toxoplasma gondii and Neospora caninum are related Apicomplexa parasites responsible for systemic diseases in many species of animals, including dogs. METHODS: This study aimed to determine the occurrence of T. gondii and N. caninum infections in 50 dogs with neurological signs that were admitted to the Veterinary Hospital of Universidade Estadual Paulista, City of Botucatu, Brazil. All animals were screened for antibodies using an immunofluorescent antibody test for both parasites. Tissues of positive animals were bioassayed in mice (T. gondii) and gerbils (N. caninum), and DNA was analyzed using the polymerase chain reaction (PCR). Positive samples for T. gondii by PCR were typed using restriction fragment length polymorphism-PCR for 11 markers: SAG1, SAG2 (5′-3′-SAG2 and alt.SAG2), SAG3, Btub, GRA6, L358, c22-8, c29-6, PK1 and Apico, and CS3 marker for virulence analysis. RESULTS: Specific antibodies were detected in 11/50 (22%; 95% confidence interval (CI95%), 12.8-35.3%) animals for T. gondii and 7/50 (14%; CI95%, 7.02-26.3%) for N. caninum. In the bioassay and PCR, 7/11 (63.6%; CI95%, 34.9-84.8%) samples were positive for T. gondii and 3/7 (42.9%; CI95%I, 15.7-75.5%) samples were positive for N. caninum. Three different genotypes were identified, but only 1 was unique. CONCLUSIONS: These data confirm the presence of T. gondii and N. caninum in dogs from Brazil, indicating the importance of this host as a sentinel of T. gondii for human beings, and the genotypic variation of this parasite in Brazil.

Introduction Few Latin American studies have assessed the prevalence of hepatitis C virus (HCV) infection in elderly individuals, in whom the highest rates are expected. We aimed to investigate the prevalence of and factors associated with HCV infection in elderly residents in the municipality of Tubar7;o, Santa Catarina. Methods This cross-sectional study included 820 individuals (aged ≥ 60 years) who were selected by simple random sampling. The presence of anti-HCV antibodies was tested by chemiluminescence, and HCV RNA detection was performed for the anti-HCV-reactive subjects. Those individuals who were anti-HCV reactive but had undetectable HCV RNA levels were tested using a third-generation recombinant immunoblot assay. The variables were compared using the chi-squared test or Fisher's exact test, and those variables with p < 0.05 were included in the logistic regression model. Results The mean patient age was 68.6 years (SD 7.0 years); 39% were men, and 92% were Caucasian. Eighteen subjects were anti-HCV positive. Among these individuals, 4 were characterized as false-positives, leaving 14 (1.7%) individuals with confirmed infections for analysis. HCV infection was associated with an age older than 65 years, households with 3 or more residents and the previous transfusion of blood products. In the logistic regression analysis, the following variables were independently associated with HCV infection: households with 3 or more residents (OR 7.9, 95% CI 1.7–35.9, p = 0.008) and previous blood transfusion (OR 6.2, 95% CI 2.1–18.6, p = 0.001). Conclusions The HCV prevalence in the elderly population in the municipality of Tubar7;o was higher than that found in previous studies of blood donors in the same region. Although exposure to contaminated blood products remained important, other transmission routes, such as household transmission, could play a role in HCV infection.

INTRODUCTION: Toxoplasma gondii infection is widely prevalent in humans and other animals worldwide. Information on the prevalence of T. gondii infection is scarce in some regions of Brazil, including riverside communities along the Amazon River basin. M METHODS: The prevalence of T. gondii in 231 people, aged 1-85 years, who were living in four riverside communities along the Purus River, Lábrea, State of Amazonas, Brazil, was determined. Antibodies against T. gondii were assayed using a commercial enzyme-linked immunosorbent assay (ELISA) kit. The hearts and brains of 50 chickens, which were raised free-range in the communities, were pooled according to the community of origin and bioassayed in mice. The isolates were genotyped using polymorphisms at 12 nuclear markers (SAG1, 5' and 3'-SAG2, alt.SAG2, SAG3, BTUB, GRA6, c22-8, c29-2, L358, PK1, Apico and CS3). RESULTS: The overall seroprevalence of T. gondii was 56.7% (131/231). IgG antibodies were presented by 117 (89.3%) and IgM by 14 (10.7%) of the 131 positive individuals. No association between age group and gender with prevalence was observed (chi-square test, p > 0.05); however, the comparison between localities showed that the seroprevalence of T. gondii was significantly lower among the individuals living in the Boca do Ituxi (p < 0.05) community. Five isolates of T. gondii were obtained in the mouse bioassay, and genotyping revealed two complete genotypes that had not been described previously and three mixed isolates. CONCLUSIONS: These results support previous findings that T. gondii population genetics are highly diverse in Brazil and that T. gondii infection is active in these riverside communities.

PURPOSE: The aim of this study was to evaluate the degree of metastatic bone pain palliation and medullar toxicity associated with samarium-153-EDTMP treatment. METHODS: Seventy-three patients with metastatic bone pain having previously undergone therapy with samarium-153-EDTMP (1 mCi/kg) were retrospectively evaluated. Routine follow-up included pain evaluation and blood counts for 2 months after treatment. Pain was evaluated using a subjective scale (from 0 to 10) before and for 8 weeks after the treatment. Blood counts were obtained before treatment and once a week for 2 months during follow-up. Dosimetry, based upon the urinary excretion of the isotope, was estimated in 41 individuals, and the resulting radiation absorbed doses were correlated with hematological data. RESULTS: Reduction in pain scores of 75% to 100% was obtained in 36 patients (49%), with a decrease of 50% to 75%, 25% to 50%, and 0% to 25% in, respectively, 20 (27%), 10 (14%), and 7 (10%) patients. There was no significant relationship between the pain response and location of the primary tumor (breast or prostate cancer). Mild to moderate myelosuppression was noted in 75.3% of patients, usually with hematological recovery at 8 weeks. The mean bone marrow dose was 347 ± 65 cGy, and only a weak correlation was found between absorbed dose and myelosuppression (Pearson coefficient = .4). CONCLUSIONS: Samarium-153-EDTMP is a valuable method for metastatic bone pain palliation. A mild to moderate and transitory myelosuppression is the main toxicity observed after samarium therapy, showing a weak correlation with dosimetric measures.

Abundância, riqueza, horário de vôo, raz7;o sexual de Sphingidae s7;o estudadas no Munic7;pio de Itacoatiara, Estado do Amazonas, em área perturbada de terra-firme na Amazônia brasileira, durante treze meses consecutivos (40 noites), entre julho/1990 - julho/1991. Obteve-se 61 espécies, incluindo todo material coletado, com a predominância de Dilophonotini (29 espécies), seguida por Sphingini (14), Philampelini (7), Smerinthini (6) e Macroglossini (5). A comunidade de Sphingidae é comparada com dados de outras localidades da regi7;o Neotropical (Américas do Sul e Central).

Esse experimento foi elaborado em parceria com o CENAQUA (Centro Nacional de Quelônios do Amazonas) e a Pró-Fauna (localizada em Iguape - SP) objetivando o estudo da validade comercial e a aceitabilidade da carne de tartaruga-da-amazônia criada em cativeiro. Foi avaliado o efeito do tempo de estocagem de amostras de carne de 14 tartarugas (7 fêmeas e 7 machos) nos dias 01, 30, 60 e 120, estocadas a -10 ºC, nos valores de pH e de Bases Voláteis Totais (BVT), cor, textura, odor e prova de cocção. Observou-se diferença significativa entre os valores de pH em relação ao tempo de estocagem nos dias 01 (5,81 e 5,81), 30 (6,04 e 6,05), 60 (6,22 e 6,23) e 120 (6,30 e 6,30), respectivamente para a carne das fêmeas e machos. Entre os sexos não houve diferença significativa. Quanto a BVT o tempo de estocagem influenciou significativamente entre os dias 01 (11,34 e 11,74 g/100g), 30 (13,92 e 13,90 g/100g) e 120 (17,62 e 17,82 g/100g); entre os dias 30 (13,92 e 13,90 g/100g) e 60 (14,02 e 14,06 g/100g) não observou-se diferença significativa, respectivamente para a carne de fêmeas e machos. Entre os sexos não ocorreu diferenças significativas. A cor, textura, odor e a prova de cocção não apresentaram alteração ao longo do tempo de estocagem. A carne de tartaruga obteve media de aceitabilidade global de 7,04. De acordo com os resultados obtidos concluímos que, pelas provas de pH e BVT, a carne de tartaruga pode ter uma validade comercial de 120 dias, quando embaladas a vácuo e estocadas a -10 ºC, e que a os provadores gostaram muito desse tipo de carne.

OBJECTIVE: To compare single-photon-emission computed tomography (SPECT) imaging scans using 201Tl and 99mTc-MIBI in detection of viable myocardium, in regions compromised by infarction. METHODS: Thirty-two (59.3±9.8 years old and 87% male) myocardial infarction patients were studied. All had Q waves on the ECG and left ventricle ejection fraction of <50%. They underwent coronary and left ventricle angiographies and SPECT before (including 201Tl reinjection) and after coronary artery bypass surgery (CABG). Improvement in perfusion observed after surgery was considered the gold standard for myocardial viability. RESULTS: Among 102 studied regions of the heart, there were 40 (39.2%) areas of transient perfusion defects in the conventional protocol with 201Tl and 52 (51.0%) after reinjection. Therefore, 12/62 (19.4%) more viable regions were identified by reinjection. Using 99mTc-MIBI, only 14 (13.7%) regions with transient defects were identified, all of which were seen also in 201Tl protocols. After surgery, 49 of a total of 93 regions analyzed (52.7%) were viable. Sensitivity, specificity, accuracy, positive and negative prediction values were, respectively, 201Tl SPECT scans - 65.3%, 90.9%, 77.4%, 88.9% and 70.2%, reinjection protocol with 201Tl scans - 81.6%, 81.8%, 81.7%, 83.3% and 80.0%; 99mTc-MIBI SPECT scans - 20.4%, 90.9%, 53.8%, 71.4% and 50.6%. Logistic regression demonstrated that the reinjection protocol with 201Tl was the best predictor of viability (P<0.001). CONCLUSION: Our data suggest the election of 201Tl for viability studies, especially when using the reinjection protocol.

OBJECTIVE: Aortopulmonary window (APW) is an uncommon congenital malformation. Its clinical presentation is dependent on the size of the defect and on the associated lesions. We evaluated our experience with this anomaly and compared it with 296 cases reported in the literature. METHODS: Retrospective study of 18 patients diagnosed as having APW (age range from 13 days to 31 years, 13 (72.2%) females), divided into two groups: Group A (GA): 10 patients with isolated APW, and Group B (GB): 8 patients with associated lesions. RESULTS: Heart failure occurred in 14 patients, and cyanosis in 3: 2 from GB (tetralogy of Fallot - TF, and double outlet right ventricle - DORV), and one from GA with pulmonary hypertension. In 5 patients from GA the diagnosis of mitral regurgitation was made based on a systolic murmur and LV hypertrophy on the EKG. In GB, clinical findings were determined by the associated defect. Diagnosis was established by echocardiography in 11 (61.2%) of the patients. In 3 patients, a wrong diagnosis of mitral regurgitation was made, in 1 a patent ductus arteriosus was diagnosed and in 3 others, the diagnosis of APW was masked by other important associated defects (2 cases of DORV and 1 case of TF). The diagnosis was made by catheterization in 3 (16.6%) patients, by surgery in 3 (16.6%) and by necropsy in 1 (5.5%). Corrective surgery was performed in 14 (77.7%) patients, with one immediate death and good long-term follow-up in the remaining patients. CONCLUSION: APW can be confused with other defects. Clinical findings, associated with an adequate echocardiogram can provide the information for the correct diagnosis.

FUNDAMENTO: A caracterização de uma enzima conversora de angiotensina (ECA) no líquido pericárdico humano é relevante diante do seu papel na liberação de angiotensina II e, portanto, do papel do pericárdio na homeostase cardivascular. OBJETIVO: Isolar e caracterizar uma ECA do líquido pericárdico humano. Comparar as atividades conversoras de angiotensina I do fluido pericárdico e do soro de pacientes submetidos à cirurgia cardiovascular. MÉTODOS: A enzima do líquido pericárdico humano foi purificada por meio de etapas cromatográficas e caracterizada por eletroforese em gel de poliacrilamida (SDS-PAGE), hidrólise de angiotensina I, bradicinina, Hip-His-Leu e substratos sintéticos com supressão interna de fluorescência. Lisinopril foi usado como inibidor. A atividade de ECA foi determinada em amostras de sangue e líquido pericárdico de 23 pacientes submetidos à cirurgia cardiovascular. RESULTADOS: A ECA purificada (MM = 140 kDa) libera angiotensina II, hidrolisa a bradicinina e o substrato Hip-His-Leu. Os parâmetros cinéticos k cat,(s-1) e k cat/Km (µM-1. s-1) foram respectivamente: Hip-His-Leu (1,14 e 7 x 10 -4), Abz-YRK(Dnp)P-OH (2,60 e 0,77), Abz-LFK(Dnp)-OH (2,77 e 0,36) e Abz-SDK(Dnp)P-OH (1,92 e 0,19). As atividades conversoras de angiotensina I (média ± DP) do líquido pericárdico e no soro foram, respectivamente, 3,16 ± 0,90 mU x mg -1x min-1 e 0,33 ± 0,11 mU x mg -1x min-1 . A diferença foi significativa entre os dois fluidos. CONCLUSÃO: Uma ECA com grande similaridade com a enzima somática foi isolada do fluido pericárdico humano. A atividade conversora de angiotensina I é maior no líquido pericárdico quando comparada com a atividade do soro. Esses dados constituem importante evidência do papel do líquido pericárdico no metabolismo de peptídeos ativos.

The activity pattern of the small sand lizard, Liolaemus occipitalis Boulenger, 1885, was investigated in the coastal sand dunes at Quintão beach (Palmares do Sul, southern, Brazil), between September 1998 and August 1999. The results showed that L. occipitalis is active all along the year, but with variations in its daily and seasonal activity patterns associated to climatic changes in the habitat. Lizard activity pattern was distributed as follows: under the sand, burrowed (73%), under vegetation (14%), dislocation (7%) and basking (6%). Mean habitat temperatures (air and substrate) were significantly different. The results indicate that L. occipitalis is a thigmothermic and heliothermic species that regulates its body temperature through behavioral mechanisms, and that thermoregulation is mainly associated with substrate.

Adenosine deaminase (ADA) activities in sera, lymphocytes and granulocytes in patients with cutaneous leishmaniasis were investigated and compared with control groups. Fifty patients and 50 healthy individuals were studied. The clinical diagnosis was parasitologically confirmed by culture and Giemsa stain. ADA activities were measured by colorimetric method. Serum ADA activities 37.80 ± 11.90, 18.28 ± 6.08 IU/L (p<0.0001), lymphocyte specific ADA activities 14.90 ± 7.42, 8.38 ± 7.42 U/mg protein (p = 0.04), granulocyte specific ADA activities 1.15 ± 0.73 , 1.09 ± 0.67 U/mg protein ( p>0.05) were found in patients and control groups, respectively. ADA activity increases in some infectious diseases were cell mediated immune mechanisms are dominant. In cutaneous leishmaniasis, lymphokine-mediated macrophage activity is the main effector mechanism. Increase in serum and lymphocyte ADA activities in patients with cutaneous leishmaniasis may be dependent on and reflects the increase in phagocytic activity of macrophages and maturation of T-lymphocytes.

Chagas disease in the chronic phase may develop into cardiac and/or digestive forms. The pathogenesis of the disease is not yet clear and studies have been carried out to elucidate the role of parasite persistence in affected organs. The aim of this study was to detect and quantify Trypanosoma cruzi in paraffin-embedded tissue samples from chronic patients using NPCR (nested polymerase chain reaction) and QPCR (quantitative polymerase chain reaction) methods. These results were correlated to anatomopathological alterations in the heart and gastrointestinal tract (GIT). Of the 23 patients studied, 18 presented the cardiac form and five presented the cardiodigestive form of Chagas disease. DNA samples were randomly isolated from formalin-fixed paraffin-embedded sections of heart and GIT tissue of 23 necropsies and were analyzed through NPCR amplification. T. cruzi DNA was detected by NPCR in 48/56 (85.7%) heart and 35/42 (83.3%) GIT samples from patients with the cardiac form. For patients with the cardiodigestive form, NPCR was positive in 12/14 (85.7%) heart and in 14/14 (100%) GIT samples. QPCR, with an efficiency of 97.6%, was performed in 13 samples (11 from cardiac and 2 from cardiodigestive form) identified previously as positive by NPCR. The number of T. cruzi copies was compared to heart weight and no statistical significance was observed. Additionally, we compared the number of copies in different tissues (both heart and GIT) in six samples from the cardiac form and two samples from the cardiodigestive form. The parasite load observed was proportionally higher in heart tissues from patients with the cardiac form. These results show that the presence of the parasite in tissues is essential to Chagas disease pathogenesis.

Streptococcus pyogenes is responsible for a variety of infectious diseases and immunological complications. In this study, 91 isolates of S. pyogenes recovered from oropharynx secretions were submitted to antimicrobial susceptibility testing, emm typing and pulsed-field gel electrophoresis (PFGE) analysis. All isolates were susceptible to ceftriaxone, levofloxacin, penicillin G and vancomycin. Resistance to erythromycin and clindamycin was 15.4%, which is higher than previous reports from this area, while 20.9% of the isolates were not susceptible to tetracycline. The macrolide resistance phenotypes were cMLSB (10) and iMLSB (4). The ermB gene was predominant, followed by the ermA gene. Thirty-two emm types and subtypes were found, but five (emm1, emm4, emm12, emm22, emm81) were detected in 48% of the isolates. Three new emm subtypes were identified (emm1.74, emm58.14, emm76.7). There was a strong association between emm type and PFGE clustering. A variety of PFGE profiles as well as emm types were found among tetracycline and erythromycin-resistant isolates, demonstrating that antimicrobial resistant strains do not result from the expansion of one or a few clones. This study provides epidemiological data that contribute to the development of suitable strategies for the prevention and treatment of such infections in a poorly studied area.
